    Hospital performance comparison of inpatient fall rates; the impact of risk adjusting for patient-related factors: a multicentre cross-sectional survey.

    BACKGROUND Comparing inpatient fall rates can serve as a benchmark for quality improvement. To improve the comparability of performance between hospitals, adjustments for patient-related fall risk factors that are not modifiable by care are recommended. Thereafter, the remaining variability in risk-adjusted fall rates can be attributed to differences in quality of care provided by a hospital. Research on risk-adjusted fall rates and their impact on hospital comparisons is currently sparse. Therefore, the aims of this study were to develop an inpatient fall risk adjustment model based on patient-related fall risk factors, and to analyse the impact of applying this model on comparisons of inpatient fall rates in acute care hospitals in Switzerland. METHODS Data on inpatient falls in Swiss acute care hospitals were collected on one day in 2017, 2018 and 2019, as part of an annual multicentre cross-sectional survey. After excluding maternity and outpatient wards, all inpatients older than 18 years were included. Two-level logistic regression models were used to construct unadjusted and risk-adjusted caterpillar plots to compare inter-hospital variability in inpatient fall rates. RESULTS One hundred thirty eight hospitals and 35,998 patients were included in the analysis. Risk adjustment showed that the following factors were associated with a higher risk of falling: increasing care dependency (to a great extent care dependent, odds ratio 3.43, 95% confidence interval 2.78-4.23), a fall in the last 12 months (OR 2.14, CI 1.89-2.42), the intake of sedative and or psychotropic medications (OR 1.74, CI 1.54-1.98), mental and behavioural disorders (OR 1.55, CI 1.36-1.77) and higher age (OR 1.01, CI 1.01-1.02). With odds ratios between 1.26 and 0.67, eight further ICD-10 diagnosis groups were included. Female sex (OR 0.78, CI 0.70-0.88) and postoperative patients (OR 0.83, CI 0.73-0.95) were associated with a lower risk of falling. Unadjusted caterpillar plots identified 20 low- and 3 high-performing hospitals. After risk adjustment, 2 low-performing hospitals remained. CONCLUSIONS Risk adjustment of inpatient fall rates could reduce misclassification of hospital performance and enables a fairer basis for decision-making and quality improvement measures. Patient-related fall risk factors such as care dependency, history of falls and cognitive impairment should be routinely assessed

    After the start of the bastion of San Carlos as the first defensive element of Alicante´s fortification, it was essential to undertake the second of the points that was urgently in the city, its provisional defense. Thanks to a letter dated November 10th of 1691, we know that the field master Don Simon Bernet and Sergeant Don Diego de Herrera Davila acknowledged the city and they arranged everything to execute urgently "with the short time allowed and the resources to the most necessary as a paper which has been formed." Simon Bernet presented the proposal to the king. He expressed the town regret by not remedy its situation more than three months after the fatal bombing. Also he mentioned the fear of a new offensive by the French fleet and the inability to defend the city for more than an hour. Everything was evidenced by the recognition of the city by Sergeant Don Diego de Herrera. Unfortunately, only the letter of Bernet and Herrera has come down to us. Thanks to it we can find out the intentions of the project to prevent the landing on the beach of Bavel

    The recently published article by Clemente and Padilla-Racero made incorrect statements regarding Richard Gardner and the mental condition that he identified, parental alienation syndrome. Clemente and Padilla-Racero conducted research on children and concluded, “Gardner‟s ideas about parental alienation syndrome, and in particular the ease of parental manipulation of children, were not empirically verified.” When we reviewed the data from their own research, we arrived at the opposite conclusion, i.e., that over 40% of child subjects, age 6 to 12, were manipulated by adult suggestion to describe a non-event. We conclude that the data of Clemente and Padilla-Racero were consistent with Gardner‟s theory that a parent can influence a child to make false statements about the other parent and to develop false beliefs and ultimately false memories of non-events. That article, which features misinformation and methodological flaws, should be withdrawn from publication

    The adhering human Lactobacillus acidophilus strain LA1 inhibits the cell association and cell invasion of enteropathogens in cultured human intestinal Caco-2 cells (M. F. Bernet, D. Brassard, J. R. Neeser, and A. L. Servin, Gut 35:483-489, 1994). Here, we demonstrate that strain LA1 developed its antibacterial activity in conventional or germ-free mouse models orally infected by Salmonella typhimurium. We present evidence that the spent culture supernatant of strain LA1 (LA1-SCS) contained antibacterial components active against S. typhimurium infecting the cultured human intestinal Caco-2 cells. The LA1-SCS antibacterial activity was observed in vitro against a wide range of gram-negative and gram-positive pathogens, such as Staphylococcus aureus, Listeria monocytogenes, S. typhimurium, Shigella flexneri, Klebsiella pneumoniae, Pseudomonas aeruginosa, and Enterobacter cloacae. By contrast, no activity was observed against species of the normal gut flora, such as lactobacilli and bifidobacteria. The LA1-SCS antibacterial activity was insensitive to proteases and independent of lactic acid production

    Measurement of the inclusive φ cross-section in pp collisions at √s=7 TeV

    The cross-section for inclusive φ meson production in pp collisions at a centre-of-mass energy of √s = 7 TeV has been measured with the LHCb detector at the Large Hadron Collider. The differential cross-section is measured as a function of the φ transverse momentum pT and rapidity y in the region 0.6< pT <5.0 GeV/c and 2.44< y <4.06. The cross-section for inclusive φ production in this kinematic range is σ(pp→φX)=1758±19(stat) +43−14(syst)±182(scale) μb, where the first systematic uncertainty depends on the pT and y region and the second is related to the overall scale. Predictions based on the Pythia 6.4 generator underestimate the cross-section

    Molecular analysis of sentinel lymph nodes and search for molecular signatures of the metastatic potential of breast cancer

    Molecular assays are a new and invaluable tool in the assessment of axillary lymph node status and metastatic potential of breast cancer. Many protocols for assessing the sentinel lymph node (SLN) status have been developed based on cytology and/or histology, showing that the rate of detection of metastasis increases with the number of histologic sections examined and with use of immunohistochemical staining in addition to conventional Hematoxylin & Eosin staining. However, full standardization of protocols for this procedure has not been achieved. Further attempts to increase sensitivity and specificity of sentinel node analysis include molecular biology-based techniques such as the real-time polymerase chain reaction (RT-PCR) and, more recently, one step nucleic acid amplification (OSNA). The latter technique, that has sensitivity close to 100% and extremely high specificity along with good reproducibility, allows analysis of the SLN in full with an intraoperative procedure in approximately 30 minutes. This highly standardized method permits to compare results between groups and predicts the probability of involvement of the remaining axillary lymph nodes based on the total tumor load of the SLN(s). Results of multicenter clinical trials suggest that OSNA allows a better personalization of patients' care based on the results of SLN analysis, because it offers criteria to select patient with metastatic SLN who will not receive additional benefit from axillary clearance. Due to the current controversy on the best treatment of the axilla after a positive SLN, the SLN copy number of CK19 mRNA can have a high impact on therapeutic decisions in this group of patients. Breast cancer is a highly heterogeneous group of diseases, characterized by remarkable differences in the histopathological features, response to treatment and clinical outcome. Most of the clinical and translational research efforts during the last decades aimed at identifying markers that would allow to predict the metastatic potential of early breast cancer, and hence to assess accurately its prognosis and to inform the choice of adjuvant systemic treatments. It is now clear that neoplastic transformation, tumor progression and response to treatment are driven and accompanied by the deregulated expression of hundred or thousand genes, whose status cannot be assessed by the currently established histopathological and immunohistochemical approach. The new molecular assays have elicited a great deal of expectations, and for the most part they have been enthusiastically welcomed as potentially offering new chances for a better and more personalized care of the patients. Many, however, are still reluctant to consider these assays ready for use in the clinical practice, and keep waiting for a confirmatory evidence of their utility when the results of ongoing clinical trials will be mature
