Kejadian Hiperglikemik pada Penderita Diabetes Melitus Tipe 2 Rawat Jalan di RSU Herna dan RSU Pusat H. Adam Malik Medan Tahun 2013
DM (diabetes mellitus) type 2 is a chronic disease which occurs when pancreas produces enough insulin while the body cannot effectively use the insulin. The DM type 2 outpatients in RSU Herna and in RSUP H. Adam Malik, Medan, increased from 2010 to 2012. This increase was probably caused by the eating pattern and the noncompliance in taking medicines. The objective of the research was to find out the correlation of eating pattern and compliance in taking medicines with the incident of hyperglycemic in DM type 2 patients in RSU Herna and in RSUP H. Adam Malik, Medan.
The research was a cross sectional study. The samples consisted of 100 DM type 2 patients, taken consecutively. The primary data were about eating pattern and compliance in taking medicines by using questionnaires and analyzed by using logistic regression analysis. The correlation of eating pattern and noncompliance in taking medicines with the incident of hyperglycemic was determined by probability value (p) at the significance level α = 0.05.
The result of the research showed that there was the correlation between calorie intake and the incident of hyperglycemic in DM type 2 patients at RP = 2,81; 95% CI(1,6-4,8), type of food RP = 3,69;95% CI(1,4-9,3), and compliance in drug administrator medicines RP = 3,17; CI(1,8-5,5) which were correlated with the incident of hyperglycemic in DM type 2 patients.
It was concluded that there was the correlation of the number of calories, types of food, and compliance in taking medicines with the incident of hyperglycemic in DM type 2 patients. It is recommended that the management of RSU Herna and RSUP H. Adam Malik increase the promotion strategy about DM type 2 diet and about the compliance in taking medicines.Diabetes Melitus (DM) tipe 2 adalah penyakit kronis yang terjadi ketika pankreas menghasilkan cukup insulin tetapi tubuh tidak dapat secara efektif menggunakan insulin yang dihasilkan. Jumlah penderita DM tipe 2 rawat jalan di RSU Herna dan RSU H. Adam Malik Medan meningkat dari tahun 2010 hingga 2012 dan peningkatan ini diduga disebabkan oleh pola makan dan ketidakpatuhan minum obat. Penelitian ini bertujuan untuk mengetahui hubungan pola makan dan kepatuhan minum obat terhadap kejadian hiperglikemik pada penderita DM tipe 2 di RSU Herna Medan dan RSUP H. Adam Malik Medan.
Penelitian ini adalah penelitian cross sectional. Sampel sebanyak 100 penderita DM tipe 2 diambil secara consecutive. Data primer tentang pola makan dan kepatuhan minum obat dengan menggunakan kuesioner, analisis data dilakukan menggunakan analisis regresi logistik. Hubungan pola makan dan ketidakpatuhan minum obat terhadap kejadian hiperglikemik ditentukan berdasarkan nilai probabilitas (p) pada taraf nyata = 0,05.
Hasil penelitian menunjukkan bahwa terdapat hubungan asupan kalori dengan kejadian hiperglikemik pada penderita DM tipe 2 dengan RP=2,81; CI 95% (1,6-4,8) jenis makanan RP=3,69; CI 95% (1,4-9,3), dan kepatuhan minum obat RP=3,17; CI 95% (1,8-5,5) berhubungan dengan kejadian hiperglikemik pada penderita DM tipe 2.
Berdasarkan hasil analisis disimpulkan bahwa teradapat hubungan jumlah kalori, jenis makanan, dan kepatuhan minum obat terhadap kejadian hiperglikemik pada penderita DM tipe 2. Job satisfaction and organizational commitment can improve
organizational citizenship behavior. The purpose of this study is to find out
influence of job satisfaction and organizational commitment to
organizational citizenship behavior among nurses at Herna Hospital Medan.
Measuring tool of this study was organizational citizenship behavior scale
based on the theory of Organ (1988), job satisfaction scale based on the
theory of Spector (1997), and organizational commitment scale based on the
theory of Mowday, Steers, and Porter (1982). This study was involved 159
nurses at Herna Hospital Medan. The results showed that job satisfaction
and organizational commitment influenced of organizational citizenship
behavior (R=0.477, R2=0.227, F=22.958; p<0.01). It means, job satisfaction
and organizational commitment contributed to organizational citizenship
behavior. Meanwhile, organizational commitment has a greater contribution
to organizational citizenship behavior (r square change=0.225) than job
satisfaction (r square change=0.047). Implications of this study that
management of hospital are expected to evaluate the factors that influence
the job satisfaction among nurses and retain organizational commitment

Indirect DNA Readout by an H-NS Related Protein: Structure of the DNA Complex of the C-Terminal Domain of Ler
Ler, a member of the H-NS protein family, is the master regulator of the LEE pathogenicity island in virulent Escherichia coli strains. Here, we determined the structure of a complex between the DNA-binding domain of Ler (CT-Ler) and a 15-mer DNA duplex. CT-Ler recognizes a preexisting structural pattern in the DNA minor groove formed by two consecutive regions which are narrower and wider, respectively, compared with standard B-DNA. The compressed region, associated with an AT-tract, is sensed by the side chain of Arg90, whose mutation abolishes the capacity of Ler to bind DNA. The expanded groove allows the approach of the loop in which Arg90 is located. This is the first report of an experimental structure of a DNA complex that includes a protein belonging to the H-NS family. The indirect readout mechanism not only explains the capacity of H-NS and other H-NS family members to modulate the expression of a large number of genes but also the origin of the specificity displayed by Ler. Our results point to a general mechanism by which horizontally acquired genes may be specifically recognized by members of the H-NS family

An application/web/game always has micro-interaction, including Instagram. Micro interaction is detailed interaction to make users know everything they are currently doing or has done by making feedback in small animation to have more attachment to the application. In the present paper, the author analyses the micro-interaction in Instagram application based on literature and the micro-interaction effect in helping users use Instagram application using the qualitative method of In-Depth Interview with 6 users. Journal Impact Factors for evaluating scientific performance : use of h-like indicators
This article introduces the Impact Factor squared or IF²-index, an h-like indicator of research performance. This indicator reflects the degree to which large entities such as countries and/or their states participate in top-level research in a field or subfield. The IF²-index uses the Journal Impact Factor (JIF) of research publications instead of the number of citations. This concept is applied to other h-type indexes and their results compared to the IF²-index. These JIF-based indexes are then used to assess the overall performance of cancer research in Australia and its states over 8 years from 1999 to 2006. The IF²-index has three advantages when evaluating larger research units: firstly, it provides a stable value that does not change over time, reflecting the degree to which a research unit participated in top-level research in a given year; secondly, it can be calculated closely approximating the publication date of yearly datasets; and finally, it provides an additional dimension when a full article-based citation analysis is not feasible. As the index reflects the degree of participation in top-level research it may favor larger units when units of different sizes are compared
