Making connecting rod is done by casting. In the casting process there are many common phenomena that cannot be controlled directly such as temperature distribution, thermal stress, shrinkage and mold deformation. Therefore used ANSYS Mechanical Workbench based on finite element method to predict phenomena in the casting process. Use silica sand molds for gray sand and cast iron molds as permanent mold. The material used for castings is 7075 aluminum alloy. in this simulation analyze the temperature distribution during cooling and thermal stress, after the simulation results show that the cooling speed of gray cast iron molds to reach a minimum temperature of 30oC is higher than the sand mold that is equal to 36.64 seconds for model 1 and 34.64 seconds for the model 2 compared to the sand mold of 44.766 seconds for model 1 and 45.333 seconds for the 2 second model and the thermal stress on the gray cast iron material is greater than the sand mold which is the highest maximum thermal stress on the model 1 gray cast iron mold of 1.5541e + 015 pa and the sand mold model 2 is 6.0414e + 014 p

Co-author Nils Henninger is a doctoral student in the Millennium PhD Program (MPP) in the Graduate School of Biomedical Sciences (GSBS) at UMass Medical School.BACKGROUND AND PURPOSE: Leukoaraiosis is a common finding among patients with ischemic stroke and has been associated with poor stroke outcomes. Our aim was to ascertain whether the severity of pre-existing leukoaraiosis is associated with outcome in patients with acute ischemic stroke who are treated with endovascular stroke therapy. MATERIALS AND METHODS: We retrospectively analyzed data from 129 consecutive, prospectively enrolled patients with stroke undergoing endovascular stroke therapy at a single tertiary care center between January 2006 and August 2013. Leukoaraiosis was assessed as supratentorial white matter hypoattenuation on admission head CT and graded as 0-2 (absent-to-moderate) versus 3-4 (severe) according to the van Swieten scale. We dichotomized the 90-day mRS into good (0-2 or return to baseline) versus poor (3-6) as the primary study outcome. Incremental multivariable logistic regression analyses were performed to identify independent predictors of a poor 90-day outcome. RESULTS: In all multivariable models, severe leukoaraiosis was independently (P < .05) associated with a poor outcome at 90 days (fully adjusted model: OR, 6.37; 95% CI, 1.83-12.18; P = .004). The independent association between leukoaraiosis and a poor outcome remained when the analysis was restricted to patients who were alive at discharge (n = 87, P < .05). Last, among patients who were alive at discharge, those with severe leukoaraiosis had significantly less frequent improvement on the mRS from discharge to 90 days compared with patients with absent-to-moderate leukoaraiosis (P = .034). CONCLUSIONS: The severity of pre-existing leukoaraiosis is independently associated with 90-day functional outcome in patients with stroke who underwent endovascular stroke therapy. These results highlight the need to further explore leukoaraiosis as a promising surrogate marker for poor outcome after endovascular stroke therapy to improve risk assessment, patient selection, and early prognostic accuracy.Millennium Ph

BACKGROUND AND OBJECTIVES: Current guidelines do not address recommendations for mechanical thrombectomy (MT) in the extended time window (>6 hours after time last seen well [TLSW]) for large vessel occlusion (LVO) patients with preexisting modified Rankin Scale (mRS) > 1. In this study, we evaluated the outcomes of MT vs medical management in patients with prestroke disability presenting in the 6- to 24-hour time window with acute LVO. METHODS: We analyzed a multinational cohort (61 sites, 6 countries from 2014 to 2020) of patients with prestroke (or baseline) mRS 2 to 4 and anterior circulation LVO treated 6-24 hours from TLSW. Patients treated in the extended time window with MT vs medical management were compared using multivariable logistic regression and inverse probability of treatment weighting (IPTW). The primary outcome was the return of Rankin (ROR, return to prestroke mRS by 90 days). RESULTS: Of 554 included patients (448 who underwent MT), the median age was 82 years (interquartile range [IQR] 72-87) and the National Institutes of Health Stroke Scale (NIHSS) was 18 (IQR 13-22). In both MV logistic regression and IPTW analysis, MT was associated with higher odds of ROR (adjusted OR [aOR] 3.96, 95% CI 1.78-8.79 and OR 3.10, 95% CI 1.20-7.98, respectively). Among other factors, premorbid mRS 4 was associated with higher odds of ROR (aOR, 3.68, 95% CI 1.97-6.87), while increasing NIHSS (aOR 0.90, 95% CI 0.86-0.94) and decreasing Alberta Stroke Program Early Computed Tomography Scale score (aOR per point 0.86, 95% CI 0.75-0.99) were associated with lower odds of ROR. Age, intravenous thrombolysis, and occlusion location were not associated with ROR. DISCUSSION: In patients with preexisting disability presenting in the 6- to 24-hour time window, MT is associated with a higher probability of returning to baseline function compared with medical management. CLASSIFICATION OF EVIDENCE: This investigation's results provide Class III evidence that in patients with preexisting disability presenting 6-24 hours from the TLSW and acute anterior LVO stroke, there may be a benefit of MT over medical management in returning to baseline function.sponsorship: This study was supported by Medtronic and the Society of Vascular and Interventional Neurology. Factors influencing immediate post-angiographic occlusion outcomes in intracranial aneurysms treated with the woven endobridge device: a multi-center analysis and predictive model from the WorldWideWEB consortium

    No full text
The Woven EndoBridge (WEB) device treats wide-necked bifurcation aneurysms, but occlusion rates vary. This study aims to identify factors associated with immediate WEB device occlusion. Data from patients treated with WEB devices across 36 sites were analyzed. Machine learning algorithms and ordinal regression models were developed to predict immediate incomplete occlusion for ruptured and unruptured aneurysms. The study included 1565 patients, with 436 ruptured and 1129 unruptured aneurysms. Immediate complete occlusion was achieved in 38.3% of ruptured and 32.8% of unruptured aneurysms. For ruptured aneurysms, the CatBoost classifier achieved an AUROC of 0.69. Key predictors of incomplete occlusion included pretreatment mRS, aneurysm diameter, and MCA location. Ordinal regression revealed that smoking history (OR: 1.95, p < 0.001), neck diameter (Odds Ratio [OR]: 1.50, p < 0.001), and presence of a branch from the aneurysm (OR: 2.06, p = 0.016) were associated with incomplete, while bifurcation aneurysms (OR: 0.55, p = 0.017) were associated with complete immediate occlusion. For unruptured aneurysms, the CatBoost classifier achieved an AUROC of 0.68. Significant predictors of immediate incomplete occlusion included aneurysm neck width, MCA location, and presence of daughter sac. Ordinal regression revealed that smoking history (OR: 1.29, p = 0.032), neck diameter (OR: 1.24, p < 0.001), and presence of a daughter sac (OR: 1.53, p = 0.005) were associated with incomplete, while bifurcation aneurysms (OR: 0.71, p = 0.02) and posterior circulation location (OR: 0.68, p = 0.01) were associated with complete immediate occlusion. Careful evaluation of patient demographics and specific aneurysm characteristics may help improve the outcomes of intracranial aneurysms treated with WEB device. © 2025. The Author(s)

Eyewitness accounts of 'the Indies' in the Later Medieval West: reading, reception, and re-use (c. 1300-1500)

Despite increased mercantile and missionary contact between the Latin West and India and China between the thirteenth and fifteenth centuries, scholars have often noted that Western Europe's knowledge of India, as judged by geographical texts from the period, changed surprisingly little during this time. This thesis employs some of the methodologies of reception studies in order to investigate the role played by first-hand travel accounts in the construction and change of concepts of the Indies during the fourteenth and fifteenth centuries. It investigates in particular the reception in Italy, France and England of the information about the area known as India or the 'three Indies' presented in the texts produced by two Italian travellers to the East: the Divisament dou monde of the Venetian merchant Marco Polo (c. 1298), and the Relatio of the Franciscan missionary Odorico da Pordenone (1330). The thesis falls into three distinct parts. In the first section, I contextualise the project with a broad survey of the Latin European ideas of India in the late thirteenth and early fourteenth centuries and with an outline of the travellers' journeys and their contexts. The second part of the thesis provides a broad overview of the circumstances of diffusion of the two travel accounts in England, France and Italy over the fourteenth and fifteenth centuries, before conducting a detailed, manuscriptbased investigation of the ways in which the two accounts of India were approached by their early readers. This investigation focuses principally upon the presentation and possible modes of reception of the texts' geographical and ethnographic details and relies heavily on the evidence of presentation, paratext and the traces of reading present in the physical texts of the accounts. The third and final part of the thesis considers the evidence of the reception of elements from first-hand travel accounts in other textual and cartographic productions. Proceeding on the basis of case studies, it demonstrates that first-hand accounts of 'the Indies' were used by the authors and compilers of cosmo graphical texts in this period in a variety of ways. It suggests, however, that the manner and context of the deployment of elements from such accounts often tended to assimilate these with, rather than distinguish them from, the writings of accepted authorities. This section also contrasts the way that details from travel accounts were re-used in texts with the way the same information was handled in the composition of maps. Finally, by analysis of the ways eyewitness accounts of the Indies were re-used in certain ambiguous and comic texts produced in this period, the thesis sheds light on an underexplored aspect of the reception both of eyewitness information and of the genres in which it appeared. The appendices contain tables presenting information relative to the manuscripts discussed that support the arguments presented in section two
