Abstract 1122‐000190: Persistent DWI Signal for 18 Months in Ischemic Stroke Patient with Carotid Web
No full textIntroduction: Diagnostic tools for acute ischemic infarcts include the use of DWI sequence on MRI to identify acute infarcts is especially useful since lesions can become hyperintense on this sequence very rapidly (Albers 1998). Over the next 15 days, DWI hyperintensity slowly decreases back to isointense. In some patients, however, there is persistent DWI hyperintensity past 1 month. There are theories that these persistent areas exhibit delayed onset infarct, prolonged ischemia, or perhaps different repair processes (Rivers, et al 2006). To this day, all DWI signals have been known to resolve within a few months even for persistent hyperintensities (Rivers, et al 2006). Carotid webs are a rare form of fibromuscular dysplasia that protrudes from the intimal tissues of carotid arteries. They are shelf‐like projections that grow into the lumen and disrupt normal blood flow (Zhang, et al 2018). These outgrowths are theorized to lead to ischemic strokes due to flow stasis and subsequent embolization of clots that form (Zhang, et al 2018). There is no consensus on the best management of carotid webs, and secondary prevention of recurrent strokes range from medical management to carotid stenting. Methods: This is a case report, and information for the patient was gathered through review of medical records on the EMR. Results: We present a case of ischemic stroke in the right basal ganglia/corona radiata, who presented with left sided weakness. The patient was found to have prediabetes, HTN, and HLD. However, she had recurrence of her symptoms over the next 18 months (figure 1). Repeat MRIs showed persistent DWI hyperintensity that slowly decreased in size and signal intensity over this period but in the same area as the initial infarct. The rest of the work up was only significant for a carotid web in the right internal carotid artery identified on conventional angiography. Ultimately she was managed with medical therapy including aspirin, statin, and antihypertensives. Conclusions: It is unclear whether the carotid web is associated with persistent DWI for such an extended time frame. There is very little research that explores the pathophysiology of ischemic strokes from carotid webs. In addition, there is even less information about the physiology of an evolving infarct that shows persistent DWI signals for such an extended time frame. Further studies that look into carotid webs may help us understand the best long term management in such patients. Future studies that explore the physiology of ischemic strokes that show such persistent DWI signals may elucidate and perhaps expand upon current management options and possibly identify new areas for intervention
The Analysis Of Land Use Changes And The Growth Potential Of Transit Oriented Development (TOD) Around Stasiun Kereta Api Bandar Khalipah, Percut Sei Tuan District, Deli Serdang Regency
No full textThe integration of land use and transportation is imperative for sustainable urban planning, with the objective of creating efficient, safe, and sustainable areas. A key approach that supports this is Transit Oriented Development (TOD), which emphasises the development of transit-based areas with the integration of public transportation modes, the reduction of private vehicles, and the utilisation of high-intensity mixed-use spaces. The present study aims to analyse land use changes and the growth potential of TOD areas in Percut Sei Tuan District, Deli Serdang Regency, focusing on the area around Bandar Khalipah Railway Station. The analysis was conducted using analytical methods, incorporating survey and mapping methods (on-screen digitisation, buffering, and overlay), descriptive methods, and methods to measure the potential growth of existing TODs with TOD indices at the station level. The data analysis was conducted using spatial analysis methods, encompassing land use analysis, land diversity analysis, land mixing analysis, and TOD index analysis at the station level. The results are described in a descriptive manner. The study of land use reveals an increase in residential and commercial areas, and a significant decrease in public areas and green open spaces. The population density indicator demonstrates that the population density indicator has not met the TOD requirement of less than 750 inhabitants/hectare. The land use diversity indicator fulfils the entropy index requirement, indicating a high level of land use diversity. Conversely, the land use mix indicator demonstrates that the ideal proportion between residential and non-residential areas is achieved. The indicators of population, land use diversity, and land use mix, when considered collectively, fall below the Transit Oriented Development (TOD) Index value. Reefer Container, adalah muatan container yang memerlukan penanganan khusus dalam masalah suhu udara (temperature-controlled), terutama proses pendinginan. Oleh karena itu para mualim harus menguasai dasar-dasar mesin pendingin. Pada masa silam Refrigerator merupakan tugas tambahan yang cukup menyulitkan bagi para ahli mesin dan bagi para mualim. Sekarang meskipun para ahli mesin telah mempunyai pengetahuan yang cukup mengenai mesin pendingin, akan tetapi akan sangat penting bagi para mualim untuk menguasainya karena tanggung jawab mengenai muatan di dalam terletak diatas pundaknya. Tanggung jawab seorang mualim harus didasarkan prosedur penanganan muatan dingin yang standar dengan pengetahuan dan kecakapan yang cukup terhadap muatan dan mesin pendingin. Pengetahuan mengenai muatan adalah mutlak begitu juga perawatan mesin dan perlakuan terhadap muatan yang mempunyai karakteristik yang berbeda sehingga tidak terjadi kerusakan atau penurunan kualitas muatan. Refeer Cargo. Muatan dingin dan beku erat hubungannya dengan temperatur dan suhu. Perubahan suhu yang menyebabkan suhu tidak sesuai dengan yang diharuskan pada suatu jenis muatan tertentu mengakibatkan kerusakan muatan dan hal ini harus dihindari untuk mencegah kerugian. Dengan pengetahuan dan pengalaman mengenai jenis muatan dan perawatan mesin juga penerapan prosedur yang standar dapat mengurangi resiko kerusakan muatan.
Penelitian ini menggunakan metode fishbone dan USG (Urgency, Seriously dan Growth). Dalam menentukan penyebab-penyebab yang menyebabkan kerusakan pada pengunci stern ramp peneliti menggunakan metode fishbone. Dan untuk menentukan prioritas masalah untuk diselesaikan peneliti menggunakan metode USG (Urgency, Seriously dan Growth).
Berdasarkan dari hasil penelitian dapat disimpulkan bahwa upaya-upaya yag dapatdilakukan untuk mencegah terjadinya kerusakan muatan pada reefer container antara lain dengan melaksanakan penanganan reefer container sesuai dengan prosedur.

Reefer Container, is a cargo container that requires special handling in the matter of temperature (temperature-controlled), especially the cooling process. For this reason, the pilgrims must master the basics of cooling machines. In the past, Refrigerator was an additional task that was quite difficult for engineers and for the pilgrims. Now even though engineers have sufficient knowledge of cooling machines, it will be very important for the pilgrims to master it because the responsibility for the load inside lies on his shoulders. The responsibility of a missionary must be based on a standard cold cargo handling procedure with sufficient knowledge and skills for cargo and cooling machines. Knowledge of cargo is absolute as well as engine maintenance and treatment of cargo that have different characteristics so that there is no damage or decrease in the quality of the cargo. Refeer Cargo. Cold and frozen charges are closely related to temperature and temperature. Changes in temperature that cause temperatures not to be in accordance with what is required in a particular type of load cause damage to the load and this must be avoided to prevent losses. With knowledge and experience regarding the types of cargo and engine maintenance, the application of standard procedures can reduce the risk of cargo damage
This research used fishbone and USG method (Urgency, Seriouslu and Growth). In determining the causes causing damage to render stern locking researcher using fishbone method. And to determine the priority of the problem to be solved by researchers using USG method (Urgency, Seriously and Growth).
Based on the results of the study it can be concluded that efforts that can be done to prevent the occurrence of load damage on the reefer container, among others, by carrying out the handling of reefer containers in accordance with the procedure

The Comment reviews the Supreme Court decisions that established the right to court-appointed counsel for the indigent accused and examines the federal constitutional limitations on the collateral use of prior uncounseled DWI convictions. It also critically evaluates state court decisions involving the collateral use of prior uncounseled DWI convictions. The author concludes that, because an uncounseled conviction is inherently unreliable, it should not be used to mandatorily incarcerate a DWI repeat offender

Gaya kepemimpinan merupakan cara atau perilaku yang dipergunakan oleh seorang pemimpin dalam mempengaruhi bawahannya (pengkiut). Gaya kepemimpinan di perpustakaan sangat berpengaruh pada pencapaian tujuan organisasi. Berkaitan dengan itu, maka diperlukan pembahasan mengenai gaya kepemimpinan kepala perpustakaan di Dinas Perpustakaan Umum dan Arsip Daerah Kota Malang. Penelitian dilaksanakan dengan tujuan untuk mendeskripsikan beberapa hal, yang mencakup gaya kepemimpinan kepala perpustakaan, bentuk motivasi yang diberikan kepala kepada pegawai dan dampak kepemimpinan kepala perpustakaan. Penelitian ini menggunakan rancangan penelitian deskriptif kualitatif. Data penelitian berupa paparan pembahasan dalam bentuk verbal mengenai gaya kepemimpinan kepala perpustakaan di Dinas Perpustakaan Umum dan Arsip Daerah Kota Malang. Pengumpulan data dilakukan dengan menggunakan teknik wawancara, observasi dan telaah dokumen. Instrumen yang digunakan untuk mengumpulkan data ialah menggunakan wawancara semi terstruktur dan participant observation (observasi berperan serta).Untuk menjaga keabsahan data, dilakukan kegiatan triangulasi data. Kegiatan analisis data dimulai dari tahap reduksi data, tahap penyajian data, tahap penarikan kesimpulan atau verifikasi. Berdasarkan hasil analisis data tersebut, diperoleh tiga kesimpulan penelitian sebagai berikut. Pertama, mengenai gaya kepemimpinan kepala perpustakaan. Gaya kepemimpinan kepala perpustakaan di Dinas Perpustakaan Umum dan Arsip Daerah Kota Malang ialah gaya kepemimpinan demokratis. Gaya kepemimpinan ini dilihat dari cara kepala perpustakaan dalam memecahkan masalah dalam mengambil keputusan dan sikap kepala perpustakaan dalam menerima masukan dari pegawai.Kedua, bentuk motivasi yang diberikan kepala perpustakaan kepada pegawai Dinas Perpustakaan Umum dan Arsip Daerah Kota Malang ialah insentif non material. Data dalam penelitian berupa teks, baik berupa kata, kalimat, atau wacana yang sesuai dengan rumusan masalah. Adapun sumber data dalam penelitian ini yaitu novel Hati Sinden karya Dwi Rahyuningsih yang diterbitkan oleh Diva Press pada tahun 2011. Instrumen dalam penelitian ini yaitu penulis yang berorientasi pada penelitian tentang sejarah dan budaya Indonesia dalam novel Hati Sinden karya Dwi Rahyuningsih. Teknik pengumpulan data dalam penelitian ini dilakukan melalui dua tahapan yaitu baca dan catat. Teknik analisis data dilakukan melalui tiga tahapan yaitu kategorisasi, deskripsi, dan penyajian data. Hasil penelitian menunjukkan bahwa pertama, dalam novel Hati Sinden karya Dwi Rahyuningsih terdapat representasi sejarah Indonesia yaitu tentang Partai Komunis Indonesia. Kedua, representasi unsur budaya dalam novel Hati Sinden karya Dwi Rahyuningsih yang terdiri dari unsur budaya pada sistem kesenian, sistem mata pencaharian hidup, dan sistem kepercayaan.
Abstract: This research used new historicism perspective with the aim of identifying Indonesian history and culture in the novel of Hati Sinden by Dwi Rahyuningsih. The data in this research is the form of text, either the form of words, sentences, or discourse in accordance with the formulation of problem. The data source in this research is novel of Hati Sinden by Dwi Rahyuningsih, published by Diva Press in 2011. The instrument in this research is the author who oriented towards research on Indonesian history and culture in the novel of Hati Sinden by Dwi Rahyuningsih. The data collection technique in this research was carried out in two stages, namely reading and taking notes. The data analysis technique was carried out in three stages, namely categorization, description, and presentation of the data. The results show that first, in the novel of Hati Sinden by Dwi Rahyuningsih, there is a representation of Indonesian history, namely the Indonesian Communist Party. Dengan menyaksikan keindahan atau keanekaragaman alam Pulau Biawak secara langsung, mereka akan lebih terinspirasi untuk turut serta dalam upaya konservasi. Namun dalam praktiknya, kendala seperti jarak yang jauh dan biaya akses menjadi kendala utama. Untuk mengatasi kendala tersebut, alternatif eduwisata daring melalui ensiklopedia digital dapat menjadi solusi yang efektif. Tujuan penelitian ini adalah mengembangkan eduwisata daring untuk konservasi biologi Pulau Biawak Indramayu melalui ensiklopedia digital. Metode penelitian yang digunakan dalam penelitian ini adalah penelitian ADDIE yang terdiri dari lima tahap yaitu Analyze, Design, Develop, Implementation, dan Evaluation. Penelitian ini akan dilakukan selama satu tahun dengan fokus pengembangan ensiklopedia digital yang menyajikan informasi terkini tentang upaya perlindungan yang tengah dilakukan di Pulau Biawak. Hasil penelitian menunjukkan bahwa struktur ensiklopedia digital yang dikembangkan untuk eduwisata daring biologi konservasi di Pulau Biawak Indramayu memiliki kriteria sangat valid, dan eduwisata daring biologi konservasi di Pulau Biawak Indramayu melalui ensiklopedia digital dapat diimplementasikan secara efektif
Revealing The Impact Of Regional Government Function Budget Patterns On Achievements SDG 11 In Indonesia
No full textThis study aims to analyze the influence and effectiveness of local government budget allocations on nine main functions in achieving SDG 11 in Indonesia, focusing on decent and affordable housing. This study uses a quantitative method with 1,424 observations from 356 provinces/districts/cities during 2019–2023 through propositional sampling techniques, and it was analyzed using individual regression. The study results indicate variations in the influence of budget allocations on achieving SDG 11. The budget for public services, housing and public facilities, tourism, and culture negatively affect SDG 11. In contrast, the budget for the environment and education positively affects SDG 11, while the budget for the economy, health, and social protection does not show an effect. Control variables, such as government age and status, positively affect, while geographical location has no effect. The theoretical implications of this study strengthen the theory that appropriate budget allocation can improve access to decent housing, clean water, and sanitation. Practically, this study guides local governments in formulating more effective budget allocation policies to support social development and the achievement of SDG 11 in a sustainable manner
