    BACKGROUND: In adults, pulse wave velocity (PWV) is regarded as a predictor of cardiovascular disease.(1) However, associations in infants are not well established. One study has linked neonatal aortic PWV, at 1-3 days, with birthweight and maternal blood pressure.(2) AIM: To examine the relationship between infant brachio-femoral PWV and size at birth. METHODS: Baby VIP study recruited 362 newborn babies from the Leeds Teaching Hospitals Trust, including 64 small for gestational age (SGA) (18%). PWV was measured non-invasively from each baby at a follow-up home visit 2-6 weeks after recruitment, using the Vicorder kit. Birthweight and other covariables were collected from the delivery and antenatal medical notes. Individualised birthweight centiles were calculated using the GROW-Centile calculator taking into account maternal weight, height, parity, ethnicity, gestational age and baby's sex.(3) RESULTS: Mean birthweight was 3329 g (standard deviation [sd] 632). Mean infant PWV was 6.7 m/s (sd 1.3). In univariable analysis, SGA babies had, on average, lower PWV by 0.4 m/s (95% confidence interval 0.0, 0.9, P = 0.04). This association persisted after adjusting for pregnancy factors including maternal smoking, pre-eclampsia, gestational diabetes, blood pressure at booking and 36 weeks, and infant factors including type of feeding, baby's age, position and whether asleep or awake at the time of measurement (0.5 m/s lower, 0.1, 0.9, P = 0.02). CONCLUSION: This study has demonstrated the feasibility and acceptability of measuring PWV in early infancy. SGA was associated with a lower PWV. These findings support the evidence linking SGA with cardiovascular indicators, even very early in life

    Flowcytometric assessment of fetomaternal hemorrhage during external cephalic version at term

    External cephalic version (ECV) at term is a safe procedure and reduces the incidence of cesarean sections for breech presentation. One of the known complications, however, is an ECV-related disruption of the placental barrier and a subsequent transfusion of fetal blood into maternal circulation. While the incidence of ECV-related fetomaternal hemorrhage (FMH) has been determined recently in a large trial using a manual Kleihauer-Betke test (KBT), questions remain on the amount of ECV-related FMH. KBT, which detects fetal red blood cells (RBC) on the basis of acidic resistance of fetal hemoglobin (HbF), is known to be a sensitive test, yet prone to procedural errors limiting its accuracy in quantifying FMH. In this study we investigated 50 patients for FMH before and after ECV, using a dual-color flow cytometric test kit with a lower limit of quantification of 0.05% fetal RBC in maternal peripheral blood. Three patients had a quantifiable increase of fetal RBC detected after ECV (0.06%; 0.08%; 0.1%). None of these subtle increments was predictable by ECV-related clinical parameters or translated into fetal compromise. Using a sensitive and accurate flow cytometric test method, our data provide further assurance to mothers on the safety of ECV at term

    From smells to stories: The design and evaluation of the smell memory kit

    The study presented is a research through design of the motivational, story sharing effects of smell, within the context of addiction care. This investigation led to the co-design of the Smell Memory Kit: a kit using eight selected smells as motivational elements to evoke and share autobiographical episodic stories among addiction care clients. A pilot evaluation study with clients (N = 14) of the Kit showed the potential of its effect in evoking and sharing episodic autobiographical stories. Moreover, qualitative analysis of the data links the distinct smells to four qualities of the autobiographical stories (i.e., emotion, clarity, specificity, and age). The research outcomes are promising in depicting the use of smells as sense data to effectively encode practices and experiences which can later be retrieved as simulations through an encounter/exposure to an odour. With regard to design, the present study shows that smells can perfectly function as a design material for storytelling, and more specifically as promising game elements in persuasive game design, motivating clients to elicit and share rich episodic memories.</p

With the objective to study peculiarities of AH with obesity into the research included 60 patients, among them 45 women and 15 men, among all of them examined features of vasodilatation group, measured leptin and endothelin-1(ЕТh-1) levels. Detected AH accompanied by obesity peculiarities of course of in dependence from the gender. Among men leptin and ЕТh-1 levels were more favorable for blood pressure correction. Among women increasing leptin and ЕТh-1 levels accompanied by decreasing of nitric oxide levels is a negative process, what is necessary to take into consideration when prescribing antihypertensive therapy what needs additional new drugs for correction of endothelial dysfunction.     Protein expression and gene mutation status of KIT and PDGFRA in renal cell carcinoma

    The author investigated protein expression and gene mutations of KIT and PDGFRA in 61 consecutive surgical cases of renal cell carcinoma (RCC). The cases of RCC consisted of 43 clear cell RCC (CCRCC), 9 chromophobe RCC (ChrRCC), or 9 papillary RCC (PaRCC). Normal distribution of KIT and PDGFRA protein was also examined in non-tumorous normal parenchyma (n=10). In normal kidneys, KIT was expressed in distal convoluted tubules and collecting ducts, and PDGFRA in distal and proximal convoluted tubules and collecting ducts. KIT expression was recognized in 9 ChrRCC (100%, 9/9), but not in 43 CCRCC (0%, 0/43) and 9 PaRCC (0%, 0/9). PDGFRA expression was recognized in 7 CCRCC (16%, 7/43) and 2 PaRCC (28%, 2/9), but not in ChrRCC (0%, 0/9). A molecular genetic analysis using PCR-direct sequencing was performed in selected 30 cases (ChrRCC=9, CCRCC=12, PaRCC=9): it revealed no mutations in KIT (exons 9, 11, 13, and 17) or PDGFRA (exons 12 and 18) genes in any cases examined. These results suggest that in normal renal parenchyma KIT is expressed in distal convoluted tubules and collecting ducts, and PDGFRA in proximal and distal convoluted tubules and collecting ducts, that KIT is expressed exclusively in ChrRCC and its incidence is 100%, that KIT-positive ChrRCC was negative for PDGFRA, that PDGFRA is expressed in a small percentage in CCRCC and PaRCC, and that mutations of KIT (exons 9, 11, 13, and 17) and PDGFRA (exons 12 and 18) are absent in RCC
