Delayed response to maintenance therapy after first-line chemotherapy in metastatic intrahepatic cholangiocarcinoma: A case report
BACKGROUND:
Intrahepatic cholangiocarcinoma is an aggressive tumor originating in the epithelium of the bile duct, often associated with distant dissemination. The prognosis is poor and treatment is challenging due to low response rate to standard chemotherapy and lack of targeted therapies.
CASE PRESENTATION:
Here we report the case of a 74-year-old white woman affected by intrahepatic cholangiocarcinoma with metastatic involvement of spleen, lung, peritoneum, and intra-abdominal lymph nodes. As first-line chemotherapy, she was given cisplatin-gemcitabine chemotherapy. The treatment was well tolerated with the exception of grade 1 constipation and a single episode of grade 4 thrombocytopenia occurring after the fourth course. After the first three courses of chemotherapy a computed tomography scan evaluation demonstrated no change; her CA19-9 levels were slightly decreased. However, after the sixth course of chemotherapy a computed tomography scan revealed a dimensional enlargement of the lung metastases; her CA19-9 levels increased. She was then treated with gemcitabine alone. After 2 months of gemcitabine monotherapy a significant regression of lung and spleen metastases, as well a CA19-9 level reduction, occurred. Eight months after the start of gemcitabine monotherapy no signs of progression were reported.
CONCLUSIONS:
Treatment of metastatic intrahepatic cholangiocarcinoma with gemcitabine as maintenance therapy after first-line chemotherapy could be continued until clear evidence of disease progression since delayed responses are possible
“NUEVOS” INGRESANTES UNIVERSITARIOS CON “VIEJOS” PROBLEMAS
Desde el trabajo de investigación Integración de los estudiantes universitarios. Meta-análisis y propuestas1, desarrollada en la Universidad Nacional de San Juan, se planean algunas cuestiones que ponen la atención, nuevamente, en la formación previa de los ingresantes a la citada universidad. La investigación se propone un proceso de meta-análisis de estudios precedentes, concerniente al análisis de los mecanismos de incorporación de alumnos y las estrategias orientadas a la retención y progreso de los mismos. Metodológicamente, el recorte empírico se corresponde con los resultados de las distintas investigaciones que, sobre la temática, se han desarrollado en los últimos 15 años en la UNSJ. En la instancia de análisis se plantean dos etapas: una extensiva, con herramientas cuanti-cualitativas que abarca un estudio de todos los trabajos realizados en la institución universitaria y otra intensiva, mediante la aplicación de herramientas cualitativas-evaluativas para profundizar aspectos sustantivos del análisis. La instancia final consiste en elaborar una propuesta superadora para revertir la situación actual. En los primeros análisis, los datos estadísticos revelan altos índices de desgranamiento y reprobación paralelos a bajos egresos, a pesar de los múltiples cambios de planes de estudios, distintas formas de tutorías, nuevos formatos de Cursos de ingreso y convenios de Articulación con la mayoría de los establecimientos de nivel medio de la provincia. Como corolario, el tópico rendimiento académico vuelve a ser motivo de preocupación, según refleja la cantidad de trabajos en la última Convocatoria (2010-2013) y entre ellos, la mayoría concentran su atención en el ingreso, pues es donde se registran las mayores dificultades: el primer año es crucial para “convertirse en miembro”. Particularmente, en relación con el proceso de transformarse en alumno universitario, las reflexiones presentes transitan el “enlace” entre el nivel medio (los egresados y su formación) y la instancia de convertir un aspirante, en estudiante universitario
Blinded independent central review versus local investigator assessment of PFS in RCTs of immunotherapy in advanced cancers: A systematic review and meta-analysis
Background: Assessment of Progression-free survival (PFS) events by investigators might be inaccurate in randomized controlled trials (RCTs) with open-label design. We explored differences in PFS evaluated by blinded independent central review (BICR) or local investigator assessment (IA) in trials testing immunotherapy (IO) in advanced cancers. Methods: We systematically reviewed articles of RCTs investigating IO in advanced tumors, published in PubMed-indexed journals up to December 2023. For each RCT, we collected PFS results by BICR and by local IA. We calculated the discrepancy index (DI) as the ratio of BICR and IA Hazard Ratios (HRBICR/HRIA) for PFS. An overall DI and relative confidence interval (CI) were calculated using a fixed-effect model weighted for the inverse of variance. Findings: Only 32/140 (22.9 %) RCTs reported both BICR and local IA PFS data, including 17,054 patients. PFS was the sole primary endpoint in 19/32 (59.4 %) and a co-primary endpoint 9/32 (28.2 %) trials. The study design was open label or double-blind in 17/32 (53.1 %) and 15/32 (46.9 %) RCTs, respectively. The overall DI was 1.07 (95 % CI 1.01–1.13; I2 =0, p = 0.02), revealing a more optimistic analysis of results in favor of local IA. In the 17 open-label trials, the overall DI was 1.09 (95 % CI 1.02–1.17, I2 =0, p = 0.02), revealing a more favorable interpretation of PFS results by local investigators. Interpretation: We found a statistically significant difference between BICR and local IA of PFS in trials of IO in cancer. These results suggest that the double assessment is recommended in RCTs testing IO, especially in open-label trials. In the literary production of Alberto Cantoni, short story writer and novelist between the nineteenth and twentieth centuries, the novel L'Illustrissimo is highly important both because it is the last publication of the author, from Pomponesco, a small town a few kilometers south of Mantua, both because it summarizes in a single text the different nuances and different directions that his writing has taken over the course of his literary career, also due to a writing and processing time that embraces the entire span of years of his career itself. In the foreground, in addition to the numerous and brilliant characters, one of the protagonists is the Mantuan landscape which, not a simple background, becomes a true literary parameter which in different and significant ways affects the purposes and mechanisms of the novel

The critically acclaimed and best-selling Mexican-born author of 13 books, Urrea has won numerous awards for his poetry, fiction and essays.

O duplo significado do sentir e do pensar nas obras de Alberto Caeiro e Descartes

Programa de Pós-Graduação em LiteraturaA pretensão desta dissertação intitulada O duplo significado do sentir e do pensar nas obras de Alberto Caeiro e Descartes é investigar a relação entre o sentir e o puro pensar, usando como referência a poesia O Guardador de Rebanhos, de Alberto Caeiro, que apresenta de forma evidente o tema; e como este assunto é tratado nas Meditações Metafísicas de René Descartes, também de forma claramente visível. Podemos perceber em ambos os autores, uma diplopia, uma espécie de duplo olhar sobre a relação sentir e puro pensar. No primeiro capítulo vou falar sobre a tese do duplo olhar de Descartes, a respeito da relação sentir e puro pensar na leitura de Merleau-Ponty, salientando a visão de natureza. No segundo capítulo, parto da leitura das três primeiras Meditações Metafísicas para caracterizar a tensão que se pode verificar entre Descartes e Alberto Caeiro quando se referem ao puro pensar. O foco principal de Descartes, nas três primeiras Meditações, é o pensar puro, e neste ponto Alberto Caeiro critica o pensar puro. No terceiro capítulo, analisando as três últimas Meditações, percebe-se que há uma virada no pensamento de Descartes, no qual o autor inverte seu ponto de vista, aproximando-se da idéia de Alberto Caeiro. No quarto capítulo, mostrarei a leitura que outros autores fazem acerca do sentir e do pensar na obra de Alberto Caeiro, mostrando que assim como Descartes, este também ostenta uma diplopia, não do sentir, mas do pensar. The pretension of this work entitled O duplo significado do sentir e do pensar nas obras de Alberto Caeiro e Descartes is to investigate the relationship between the feeling and the pure thinking, using as reference the poetry O Guardador de Rebanhos, by Alberto Caeiro, that presents in an evident way this theme; and as this subject is treated in René Descartes' Metaphysical Meditations, also in a clearly visible way. It is possible to notice in both authors, the diplopy, a sort of double look about the relationship between feeling and pure thinking. The thesis about Descartes'double look regarding the relation of feeling and pure thinking, in Merleau-Ponty reading, pointing out the nature vision, will be treated in the first chapter. In the second chapter, I start from the reading of the first three Metaphysical Meditations to characterize the tension that can be verified between Descartes and Alberto Caeiro when they refer to the pure thinking. Descartes' main focus in the first three Meditations is pure thinking and, in this point, Alberto Caeiro criticizes pure thinking. In the third chapter, analyzing the last three Meditations, it is possible to notice that there is an alteration in Descartes' thought, in which the author inverts his point of view, approaching of Alberto Caeiro's idea. In the fourth chapter, I will show the reading that other authors do concerning feeling and thinking in Alberto Caeiro's work, showing that as well as Descartes, this also shows a diplopy, not of feeling, but of thinking
