    Frangkiskos Skoufos’ Techne Rhetorikes (1681): introduction, critical edition, commentary, glossary

    The aim of the present thesis is the critical edition of Techne Rhetorikes (1681) by Frangiskos Skoufos. The thesis is divided into four parts: Introduction, critical edition, commentary and glossary. In the introduction, after presenting the life and works of Skoufos, the printed edition and the manuscripts of the text are examined. Two 18th century manuscripts are described and studied; they seem not to play an important role for the text delivery. At the study of structure and content, the texts which Skoufos possibly used as basis for his dedicatory prologue and note to the readers are identified. The study of the rhetorical material shows that the author drew for its compilation from De arte Rhetorica (1562) of the Jesuit Suarez, while the several prooimia are taken from Pantheon argutae elocutionis (1670) of Verani and the Jesuit manual Tyrocinium (1647) of Paiot. Then, the exemplary stories − exempla are studied, while their classification is proposed according to the criteria of the handling of the sources by Skoufos and the narrative practice that he adopts. Searching for the real sources of the work it turns out that Skoufos draws mainly from intermediate Latin sources. The sources’ detection enriches the discussion on specifying the genre of Techne Rhetorikes with the elements of the emblematic standardization of speech, the encyclopedicity and the image rhetoric that are presented in the so-called “Grand Style” at epideictic speech. The language of the text is studied and its dominant stylistic elements are analyzed. At the investigation of the narrative technique the−probably pioneering for the literature of the 17th century− technique of the internal monologue of the heroes’ is identified. The critical edition is attempted in the monotonic system and is conservative in terms of any phonetic or spelling interventions. The thesis is completed by the critical notes, the glossary, as well as the indexes of the main names and subjects of the exempla.Σκοπός της παρούσας διατριβής είναι η κριτική έκδοση της Τέχνης Ρητορικής (1681) του Φραγκίσκου Σκούφου. Η εργασία χωρίζεται σε τέσσερα μέρη: Εισαγωγή, κριτική έκδοση, σχόλια και γλωσσάριο. Στην Εισαγωγή, αφού παρουσιάζονται τα εργο-βιογραφικά στοιχεία του Σκούφου, εξετάζεται η έντυπη και η χειρόγραφη παράδοση του κειμένου. Περιγράφονται δύο χειρόγραφα του 18ου αιώνα και αποδεικνύεται ότι δεν επηρεάζουν την παράδοση του έργου. Κατά την εξέταση της δομής και της θεματικής εντοπίζονται κείμενα από τα οποία αφορμάται ο Σκούφος για τη συγγραφή του αφιερωματικού προλόγου και του σημειώματος προς τον αναγνώστη. Από τη μελέτη της ρητορικής ύλης προκύπτει ότι για την κατάρτισή της ο συγγραφέας άντλησε από την De arte Rhetorica (1562) του ιησουίτη Suarez, ενώ οι τρόποι προοιμίων λαμβάνονται από το Pantheon argutae elocutionis (1670) του Verani και το ιησουιτικό εγχειρίδιο Tyrocinium (1647) του Paiot. Στη συνέχεια μελετώνται οι παραδειγματικές ιστορίες (exempla), ενώ προτείνεται η κατάταξή τους με κριτήρια τον χειρισμό των πηγών από τον Σκούφο και την αφηγηματική πρακτική που ακολουθεί. Από την αναζήτηση των πραγματικών πηγών του έργου αποδεικνύεται ότι ο Σκούφος αντλεί κυρίως από ενδιάμεσες λατινικές πηγές. Η ανίχνευση των πηγών εμπλουτίζει τη συζήτηση για την ειδολογική ένταξη της Τέχνης Ρητορικής με τα στοιχεία της εμβληματικής τυποποίησης του λόγου, της εγκυκλοπαιδικότητας και της ρητορικής της εικόνας που εκφέρονται με το υψηλό ύφος του επιδεικτικού λόγου. Κατά τη μελέτη της γλώσσας του κειμένου αναλύονται τα χαρακτηριστικά της καθώς και τα κυρίαρχα υφολογικά στοιχεία, ενώ στο πλαίσιο της διερεύνησης της αφηγηματικής τεχνικής εντοπίζεται η −πιθανώς πρωτοποριακή για τη λογοτεχνία του 17ου αιώνα− τεχνική του εσωτερικού μονόλογου των ηρώων. Η κριτική έκδοση επιχειρείται σε μονοτονικό σύστημα και είναι συντηρητική ως προς τις οποιεσδήποτε επεμβάσεις, φωνητικές και ορθογραφικές.     BACKGROUND Guidelines recommend nonstatin lipid-lowering agents in patients at very high risk for major adverse cardiovascular events (MACE) if low-density lipoprotein cholesterol (LDL-C) remains >= 70 mg/dL on maximum tolerated statin treatment. It is uncertain if this approach benefits patients with LDL-C near 70 mg/dL. Lipoprotein(a) levels may influence residual risk.OBJECTIVES In a post hoc analysis of the ODYSSEY Outcomes (Evaluation of Cardiovascular Outcomes After an Acute Coronary Syndrome During Treatment With Alirocumab) trial, the authors evaluated the benefit of adding the proprotein subtilisin/kexin type 9 inhibitor alirocumab to optimized statin treatment in patients with LDL-C levels near 70 mg/dL. Effects were evaluated according to concurrent lipoprotein(a) levels.METHODS ODYSSEY Outcomes compared alirocumab with placebo in 18,924 patients with recent acute coronary syndromes receiving optimized statin treatment. In 4,351 patients (23.0%), screening or randomization LDL-C was = 70 mg/dL (median 94.0 mg/dL; interquartile range: 83.2-111.0 mg/dL).RESULTS In the lower LDL-C subgroup, MACE rates were 4.2 and 3.1 per 100 patient-years among placebo-treated patients with baseline lipoprotein(a) greater than or less than or equal to the median (13.7 mg/dL). Corresponding adjusted treatment hazard ratios were 0.68 (95% confidence interval [CI]: 0.52-0.90) and 1.11 (95% CI: 0.83-1.49), with treatment-lipoprotein(a) interaction on MACE (P-interaction = 0.017). In the higher LDL-C subgroup, MACE rates were 4.7 and 3.8 per 100 patient-years among placebo-treated patients with lipoprotein(a) >13.7 mg/dL or <= 13.7 mg/dL; corresponding adjusted treatment hazard ratios were 0.82 (95% CI: 0.72-0.92) and 0.89 (95% CI: 0.75-1.06), with P-interaction = 0.43.CONCLUSIONS In patients with recent acute coronary syndromes and LDL-C near 70 mg/dL on optimized statin therapy, proprotein subtilisin/kexin type 9 inhibition provides incremental clinical benefit only when lipoprotein(a) concentration is at least mildly elevated.     Apolipoprotein B (apoB) provides an integrated measure of atherogenic risk. Whether apoB levels and apoB lowering hold incremental predictive information on residual risk after acute coronary syndrome beyond that provided by low-density lipoprotein cholesterol is uncertain.The ODYSSEY OUTCOMES trial (Evaluation of Cardiovascular Outcomes After an Acute Coronary Syndrome During Treatment With Alirocumab) compared the proprotein convertase subtilisin/kexin type 9 inhibitor alirocumab with placebo in 18 924 patients with recent acute coronary syndrome and elevated atherogenic lipoproteins despite optimized statin therapy. Primary outcome was major adverse cardiovascular events (MACE; coronary heart disease death, nonfatal myocardial infarction, fatal/nonfatal ischemic stroke, hospitalization for unstable angina). Associations between baseline apoB or apoB at 4 months and MACE were assessed in adjusted Cox proportional hazards and propensity score-matched models.Median follow-up was 2.8 years. In proportional hazards analysis in the placebo group, MACE incidence increased across increasing baseline apoB strata (3.2 [95% CI, 2.9-3.6], 4.0 [95% CI, 3.6-4.5], and 5.5 [95% CI, 5.0-6.1] events per 100 patient-years in strata &lt;75, 75-&lt;90, ≥90 mg/dL, respectively; Ptrend&lt;0.0001) and after adjustment for low-density lipoprotein cholesterol (Ptrend=0.035). Higher baseline apoB stratum was associated with greater relative (Ptrend&lt;0.0001) and absolute reduction in MACE with alirocumab versus placebo. In the alirocumab group, the incidence of MACE after month 4 decreased monotonically across decreasing achieved apoB strata (4.26 [95% CI, 3.78-4.79], 3.09 [95% CI, 2.69-3.54], and 2.41 [95% CI, 2.11-2.76] events per 100 patient-years in strata ≥50, &gt;35-&lt;50, and ≤35 mg/dL, respectively). Compared with propensity score-matched patients from the placebo group, treatment hazard ratios for alirocumab also decreased monotonically across achieved apoB strata. Achieved apoB was predictive of MACE after adjustment for achieved low-density lipoprotein cholesterol or non-high-density lipoprotein cholesterol but not vice versa.In patients with recent acute coronary syndrome and elevated atherogenic lipoproteins, MACE increased across baseline apoB strata.     Safety of the PCSK9 inhibitor alirocumab: insights from 47 296 patient-years of observation /

    The ODYSSEY OUTCOMES trial, comprising over 47 000 patient-years of placebo-controlled observation, demonstrated important reductions in the risk of recurrent ischaemic cardiovascular events with the monoclonal antibody to proprotein convertase subtilisin/kexin type 9 alirocumab, as well as lower all-cause death. These benefits were observed in the context of substantial and persistent lowering of low-density lipoprotein cholesterol with alirocumab compared with that achieved with placebo. The safety profile of alirocumab was indistinguishable from matching placebo except for a ∼1.7% absolute increase in local injection site reactions. Further, the safety of alirocumab compared with placebo was evident in vulnerable groups identified before randomization, such as the elderly and those with diabetes mellitus, previous ischaemic stroke, or chronic kidney disease. The frequency of adverse events and laboratory-based abnormalities was generally similar to that in placebo-treated patients. Thus, alirocumab appears to be a safe and effective lipid-modifying treatment over a duration of at least 5 years

    Relation of Lipoprotein(a) Levels to Incident Type 2 Diabetes and Modification by Alirocumab Treatment

    OBJECTIVE: In observational data, lower levels of lipoprotein(a) have been associated with greater prevalence of type 2 diabetes. Whether pharmacologic lowering of lipoprotein(a) influences incident type 2 diabetes is unknown. We determined the relationship of lipoprotein(a) concentration with incident type 2 diabetes and effects of treatment with alirocumab, a PCSK9 inhibitor. RESEARCH DESIGN AND METHODS: In the ODYSSEY OUTCOMES trial alirocumab was compared with placebo in patients with acute coronary syndrome. Incident diabetes was determined from laboratory, medication, and adverse event data. RESULTS: Among 13,480 patients without diabetes at baseline, 1,324 developed type 2 diabetes over a median 2.7 years. Median baseline lipoprotein(a) was 21.9 mg/dL. With placebo, 10 mg/dL lower baseline lipoprotein(a) was associated with hazard ratio 1.04 (95% CI 1.02-1.06, P < 0.001) for incident type 2 diabetes. Alirocumab reduced lipoprotein(a) by a median 23.2% with greater absolute reductions from higher baseline levels and no overall effect on incident type 2 diabetes (hazard ratio 0.95, 95% CI 0.85-1.05). At low baseline lipoprotein(a) levels, alirocumab tended to reduce incident type 2 diabetes, while at high baseline lipoprotein(a) alirocumab tended to increase incident type 2 diabetes compared with placebo (treatment-baseline lipoprotein(a) interaction P = 0.006). In the alirocumab group, a 10 mg/dL decrease in lipoprotein(a) from baseline was associated with hazard ratio 1.07 (95% CI 1.03-1.12; P = 0.0002) for incident type 2 diabetes. CONCLUSIONS: In patients with acute coronary syndrome, baseline lipoprotein(a) concentration associated inversely with incident type 2 diabetes. Alirocumab had neutral overall effect on incident type 2 diabetes. However, treatment-related reductions in lipoprotein(a), more pronounced from high baseline levels, were associated with increased risk of incident type 2 diabetes.
