Rehabilitation Counsellors, Disabled Clients and Interaction 2006
Tutkimuksessa tutkittiin kuntoutustyöntekijöiden näkemyksiä vammaisuudesta sekä kuntoutusohjaajien työn merkityksiä ja vaikutuksia. Tutkimuksen kohderyhmänä olivat kaikki sairaanhoitopiireille töitä tekevät kuntoutusohjaajat: keskussairaaloiden omat työntekijät ja ostopalveluna työtään tekevät. Kysymyspaperin ensimmäisessä osassa pyrittiin selvittämään työtehtävien luonnetta: miten keskimääräinen työviikko jakautuu, millaisten asiakkaiden kanssa kuntoutusohjaajat työskentelevät sekä minkä mittaisia yhteistyöprosessit ovat. Aluksi tiedusteltiin välitöntä kuntoutusohjauksen asiakastyötä, kuten asiakkaiden tapaamisen tyypillistä kestoa ja asiakastapaamisten määrää kuukaudessa. Puolestaan välillisen asiakastyön laatua selvitettiin kysymällä yhteistyötahojen kanssa neuvottelemiseen, matkoihin, kurssien järjestelyyn ja koulutuksiin kuluvaa aikaa. Kyselyn toisessa osassa keskityttiin asiakkaiden vuorovaikutussuhteisiin. Vastaajia pyydettiin kuvaamaan numeroasteikon avulla työssä näkemiään ja kokemiaan vuorovaikutussuhteiden laatua. Vastaajia pyydettiin arvioimaan (1) omaa ja asiakkaan, (2) asiakkaan ja perheenjäsenien sekä (3) asiakkaan ja palvelujärjestelmän vuorovaikutussuhdetta. Taustamuuttujina olivat mm. sukupuoli, ikä (luokiteltuna), työkokemus ja koulutus.The study surveyed Finnish rehabilitation counsellors' views on the impact of their work for clients with disabilities or long-term illnesses as well as the position of their clients when they interact with their families and the service system. The target group included all rehabilitation counsellors working for joint municipal authorities for specialised health care in Finland (i.e. both the staff of central hospitals as well as outsourced employees). The first part of the questionnaire charted the nature of the respondents' work and how the average working week was distributed between different types of tasks. The first questions examined matters relating to direct patient work in rehabilitation counselling, e.g. the typical duration of a client meeting, the number of client meetings per month, and the number of times per year the respondents met with long-time clients. Questions also covered indirect patient work, e.g. the time spent on participating in negotiations and trainings, travelling, and arranging courses. The second part of the survey focused on the respondents' client relationships. The respondents were asked to evaluate a variety of different aspects in the interaction between (1) the respondent and his/her clients, (2) clients and members of their family, and (3) clients and other employees in the service process. Vuorovaikutussuhteet ja valtaistuminen : kuntoutustyöntekijän ja vammaisen ihmisen näkemyksiä vammaisuudesta
Tutkimus paneutui vammaisten ja pitkäaikaissairaiden ihmisten vuorovaikutukseen muiden ihmisten kanssa. Vammainen ihminen rakentaa identiteettejään ja käsitystä itsestään ja rooleistaan koko elämänsä ajan vuorovaikutuskokemustensa vaikutuksessa. Kokemukset voivat olla täysivaltaisen itsenäisen ihmisen identiteettiä vahvistavia tai estäviä. Kuitenkin se, miten ihminen reagoi ympäristön erilaisiin viesteihin, on yksilöllistä.
Tutkimuksen tavoitteena oli tehdä näkyväksi vuorovaikutusilmiöitä, jotka vaikuttavat vammaisen ihmisen identiteettien kehittymiseen, hänen valtaistumiseensa ja hänen osallistumiseensa yhteiskunnassa ja yhteisöissä. Vuorovaikutusta tarkasteltiin vammaisen ihmisen kokemuksina arkielämän tilanteista, kuntoutusohjaajien kokemuksina asiakasyhteistyöstä sekä kuntoutusohjaajien käsityksinä heidän asiakkaidensa vuorovaikutustilanteista perheessä ja palvelujärjestelmän asiakkaina. Tarkoituksena oli löytää keinoja, joilla vuorovaikutuksessa voidaan vahvistaa vammaisen ihmisen valtaistumista, toimintavoimaa oman elämän merkityksellisillä alueilla.
Tutkimuksen keskeisiä teoreettisia käsitteitä olivat kasvokkaisen vuorovaikutuksen ohella identiteetin rakentuminen, valta ja valtaistuminen, vammaistaminen sekä valtaistumisen mahdollistaminen ja tukeminen. Vammaisuutta käsiteltiin sosiaalisena konstruktiona; sosiaalisrelationaalisen mallin mukaan vammaisuutta tuottavat fyysiset, sosiaaliset ja asenteelliset esteet vammaisen ihmisen ympäristössä. Näin tuotettu vammaisuus ilmenee sekä siinä, mitä vammainen ihminen voi tehdä, että siinä, mitä vammainen ihminen voi olla.
Neljästä osatutkimuksesta ensimmäinen perustui kolmen liikuntavammaisen äidin kertomiin kokemuksiin vuorovaikutussuhteistaan. Tekijä analysoi toisen kirjoittajan Anja Dobler-Mikolan kanssa äitien haastatteluaineiston teemoittelun avulla. Vammaisen naisen äitiyden rakentuminen kuvautui osin merkittävien toisten tukemana, osin taisteluna vammaistavien vuorovaikutuskokemusten vaikutusta vastaan.
Kolme muuta osatutkimusta perustuivat kuntoutusohjaajien käsityksiin ja näkemyksiin vammaisten tai pitkäaikaissairaiden asiakkaidensa asemasta vuorovaikutussuhteissa perheessä ja palvelujärjestelmässä. Toisen osatutkimuksen aineisto kerättiin kahden mielenterveyskuntoutuksen kuntoutusohjaajan haastatteluista ja heidän työssään tekemistä kirjauksista. Asiakkaina oli iäkkäitä ihmisiä, jotka olivat sairastaneet pitkään psyykkistä sairautta. Edelleen teemoitellen aineistosta löytyi asiakkaiden vuorovaikutuskokemuksia, jotka vahvistivat heidän asemaansa potilaina ja muiden holhouksesta riippuvaisina. Erityisesti teemoiteltiin niitä keinoja, joilla kuntoutusohjaajat pyrkivät tukemaan asiakkaan oman mielipiteen ilmaisua ja oman elämänhistorian arvostamista. Näistä keinoista hahmottui toimintamalli heikossa asemassa olevien mielenterveyskuntoutujien tukemiseen.
Kolmas ja neljäs osatutkimus perustuivat Suomen erikoissairaanhoidossa toimiville kuntoutusohjaajille tehtyyn lomakekyselyyn (n = 148). Kolmas osatutkimus kuvasi kuntoutusohjaajien työn sisältöä ja tavoitteita. Työ näyttäytyi asiakaslähtöisenä ja paljolti asiakkaiden omissa ympäristöissä toteutuvana työnä, jossa pyrittiin paitsi tukemaan vammaisten tai pitkäaikaissairaiden ihmisten omaa toimintavoimaa myös tukemaan heidän omaistensa asemaa vammaisen ihmisen täysivaltaisuuden mahdollistajana.
Neljännessä osatutkimuksessa, joka on yhteisartikkeli Marianne Silénin kanssa, analysoitiin kuntoutusohjaajien näkemyksiä asiakkaiden vuorovaikutuksesta kuntoutusohjaajien itsensä, muiden palvelujärjestelmän työntekijöiden ja perheenjäsenten kanssa. Asiakkaiden vuorovaikutus näyttäytyi kuntoutusohjaajille pääasiassa myönteisenä ja vammaisen ihmisen asemaa vahvistavana. Kuitenkin palvelujärjestelmässä havaittiin toisinaan myös vammaista asiakasta mitätöivää asennetta. Vammaisten ihmisten perheissä tapahtuva vuorovaikutus näyttäytyi kuntoutusohjaajille useimmiten myönteisenä ja arvostavana. Toisinaan tunnistettiin myös alistamista tai taloudellista hyväksikäyttöä.
Vammaiset ja pitkäaikaissairaat ihmiset saavat sekä valtaistumista vahvistavia että estäviä viestejä sekä lähiyhteisöltään että palvelujärjestelmän työntekijöiltä. Tutkimuksen yhtenä loppupäätelmänä eritellään sitä, mitä valtaistumista tukeva tai estävä toiminta merkitsee käytännössä yksilön kokemuksena, läheisten toimintana ja palveluissa. Myönteisiä merkityksiä kuvaavat esimerkit yksilön arvostamisesta, osallistumisen mahdollistumisesta sekä itsemääräämisen ja pystyvyyden vahvistumisesta. Yksilön valtaistumisen estäminen vuorovaikutuksessa ilmenee muun muassa mitätöintinä, vammaistamisena tai alistamisena.
Vammaisten ihmisten asemaan yhteisöissä ja yhteiskunnassa voidaan vaikuttaa sosiaali- ja terveydenhuollon henkilöstön koulutuksessa. Ammattilaiset puolestaan voivat vaikuttaa ihmisten asenteisiin työskennellessään paitsi asiakkaidensa, myös heidän läheistensä ja heidän palveluverkostonsa kanssa. Kuntoutusohjaajat ovat ammattiryhmä, joka voi erityisesti paneutua vuorovaikutukseen, asiakaslähtöisyyteen ja laaja-alaisuuteen ohjaustyössään. Heillä on myös vaikutusvaltaa palvelujärjestelmässä, koska he tekevät laajasti yhteistyötä järjestelmän toimijoiden kanssa eri tasoilla ja alueilla.This research concentrates on people with disabilities and long-term illnesses interacting with other people. Throughout life, a disabled person constructs identities, a self-consciousness, and an awareness of one’s roles through experiences of interaction. The experiences may either support or inhibit the identity of an empowered and independent person. However, everyone reacts differently to different messages from the surroundings. One does not necessarily link the received message with one’s identity. Nevertheless, receiving dismissive or patronising messages repeatedly and over a long period of time is apt to impede empowerment.
The purpose of the study was to point out features of interaction that either strengthen or weaken the position of a person with disabilities and the effects of these features on identity. The parties of interaction were people with disabilities, their families, and social and health care workers. Rehabilitation counsellors had an important role in this study as they typically strive to learn about the client's view and to support the client's own power to act. The aim was to find out ways of interaction that enhance empowerment in areas of life that people with disabilities find important.
The core theoretical concepts are face-to-face interaction, identity construction, power and empowerment, disablism and enabling empowerment. Disability is regarded as a social construct. The social relational model of disability maintains that disability is produced by both physical barriers and negative social interaction and stigmatizing attitudes. This is manifested in what a person with disabilities can do and what that person can be.
People with disabilities and long-term illnesses may receive messages from their community and service system personnel that both support and impede empowerment. The first substudy, written jointly with Anja Dobler-Mikola, is based on open interviews with three mothers with a mobility impairment. A thematic analysis was conducted of the experiences described by the mothers. The construction of a woman’s motherhood was described partly as a process supported by significant others, partly as a fight against the strain of disabling experiences of interaction.
The other three substudies were based on the conceptions and views of rehabilitation counsellors about the position of their clients with disabilities or longterm illnesses when they interact with their families and the service system. The material of the second substudy was based on interviews with two mental health rehabilitation counsellors and on data they had recorded in their daily work. Their clients were aged people with a long history of mental illness. A thematic analysis revealed experiences of interaction that reinforced the clients’ position as patients and as people who depend on others. As for positive factors affecting the position of clients, I thematised the methods used by rehabilitation counsellors when en couraging their clients to express their opinions and to appreciate their life history. This resulted in an operating model to support dependent and powerless clients in psychosocial rehabilitation.
The third and fourth substudies were based on a questionnaire survey among rehabilitation counsellors (148 respondents) working in Finnish special health care units. The third substudy dealt with the contents and objectives of rehabilitation counsellors’ work. Their work was client-oriented and took place mostly in the client environment, where they supported not only the client in gaining personal power to act but also their close relatives in enabling the empowerment of a person with a disability or long-term illness.
The fourth substudy, an article written jointly with Marianne Silén, was an analysis of the rehabilitation counsellors’ views on how clients interact with (1) rehabilitation counsellors themselves, (2) other workers within the service system, and (3) family members. Employing e.g. factor analysis we classified features of interaction into those that weaken the position of people with disabilities and to those that support their empowerment. In general, the counsellors perceived interaction as something positive that reinforced the position of a person with disabilities. Sometimes they, however, perceived dismissive or disablist attitudes and acts in both the service system and families. In rare cases, oppression and abuse was perceived in the families of people with disabilities.
Summarizing, persons with disabilities or long-term illnesses receive supportive as well as dismissive and patronising messages during their interactions with their close ones and the service system. In the last chapter, a synthesis is presented of how supporting empowerment or hindering empowerment are reflected as a person’s experience, actions by close ones and in the service system. Positive meanings consist of esteem, supporting participation, and strengthening self-determination and self-efficacy. Diminishing acts, oppressive acts and disablist behaviours are examples of activities hindering empowerment.
Social and health care professionals can influence people’s attitudes when working with clients, their families, and their service networks. They have an influence on the way in which people with disabilities are encountered in communities and society. This emphasizes the importance of social and health care workers' education. As an occupational group, rehabilitation counsellors are in a good position to invest in an interactive, client-oriented, and comprehensive service. Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases.
