Duration of untreated illness in panic disorder : a poor outcome risk factor?
Objective: The aim of this naturalistic study was to evaluate the impact of the duration of untreated illness (DUI) on the outcome and treatment response of panic disorder (PD).
Methods: Ninety-six outpatients with PD who underwent an 8-week open-label treatment with serotonergic antidepressants were subdivided into two subgroups: those with DUI ≤ 1 year and those with DUI > 1 year. The main baseline demographic and clinical variables were calculated and compared between the two subgroups of patients (chi-square test or t-test for independent samples). The effect of the antipanic medication was evaluated by analysis of variance with repeated measures considering Hamilton Rating Scale for Anxiety, Clinical Global Impression rating scores, and the number of panic attacks/week as the dependent variables (outcome measures), while the subgroups were the independent ones. Comorbidity with onset later than PD was also considered.
Results: There were no differences between patients with DUI ≤ 1 year and patients with DUI > 1 year with respect to the outcome measures considered. However, patients with DUI > 1 year (N = 64) had a higher frequency of comorbid major depressive disorder (MDD) with onset later than PD (p = 0.006).
Conclusions: Results from this study suggest that the DUI may be a predictor of the development of comorbid MDD in PD. Further investigations on larger samples and with longer follow-up are warranted

Numerical analysis of a 3-D printed porous trailing edge for broadband noise reduction
Lattice Boltzmann simulations were carried out to investigate the noise mitigation mechanisms of a 3-D printed porous trailing-edge insert, elucidating the link between noise reduction and material permeability. The porous insert is based on a unit cell resembling a lattice of diamond atoms. It replaces the last 20 % chord of a NACA 0018 at zero angle-of-attack. A partially blocked insert is considered by adding a solid partition between 84 % and 96 % of the aerofoil chord. The regular porous insert achieves a substantial noise reduction at low frequencies, although a slight noise increase is found at high frequencies. The partially blocked porous insert exhibits a lower noise reduction level, but the noise emission at mid-to-high frequency is slightly affected. The segment of the porous insert near the tip plays a dominant role in promoting noise mitigation, whereas the solid-porous junction contributes, in addition to the rough surface, towards the high-frequency excess noise. The current study demonstrates the existence of an entrance length associated with the porous material geometry, which is linked to the pressure release process that is responsible for promoting noise mitigation. This process is characterised by the aerodynamic interaction between pressure fluctuations across the porous medium, which is found at locations where the porous insert thickness is less than twice the entrance length. Present results also suggest that the noise attenuation level is related to both the chordwise extent of the porous insert and the streamwise turbulent length scale. Does comorbid subthreshold anxiety affect clinical presentation and treatment response in depression? A preliminary 12-month naturalistic study
The aim of this study was to evaluate the effects of comorbid subthreshold anxiety on the course and the treatment of Depressive Disorders. The sample studied comprised four groups defined by the DSM-IV Axis I diagnosis: (1) Patients with a Major Depressive Disorder (MDD) and an Anxiety Disorder (DA); (2) patients with MDD and a subthreshold Anxiety Disorder (Da); (3) patients with subthreshold depression and an Anxiety Disorder (dA); (4) patients with subthreshold depression and subthreshold anxiety (da). HAMD, HAMA and CGI rating scales were administered monthly for 12 months while patients were treated with different antidepressants. Significant differences were found among the four groups with respect to the baseline depressive symptoms: Da presented more frequently suicidal ideation (chi2=9.568, d.f.=3, p=0.023), psychomotor retardation (chi2=12.568, d.f.=3, p=0.006), sexual dysfunctions (chi2=7.761, d.f.=3, p=0.05), hypochondriacal ideation (chi2=13.633, d.f.=3, p=0.003), weight loss (chi2=9.520, d.f.=3, p=0.023), and diurnal variation of symptoms (chi2=13.258, d.f.=3, p=0.004). With respect to the treatment response Da patients showed an overall worse response to antidepressants, having a longer latency and a lower reduction of symptoms. These results suggest that patients with Major Depression and subthreshold anxiety present with a more severe baseline clinical picture and seem to have a less efficient response to antidepressants
Switching from serotonin reuptake inhibitors to duloxetine in patients with resistant obsessive compulsive disorder : a case series
Obsessive compulsive disorder (OCD) is a chronic disorder, currently recognized as one of the most common psychiatric disorder as well as one of the most disabling of all medical disorders. OCD is characterized by high rates of partial and/or absent response to standard, recommended treatments (serotonin reuptake inhibitors and psychotherapy). Recent investigation showed that Venlafaxine, a dual serotonin and norepinephrine reuptake inhibitor (SNRI), may be a valid alternative for some treatment-refractory patients. We present the cases of four OCD patients with comorbid mood or anxiety disorders, who were treated with serotonin reuptake inhibitors (SRIs) at adequate doses for at least 12 weeks, showing partial/no response. Patients were then switched to Duloxetine up to 120 mg/day and followed up for 12 weeks. Three out of four patients showed a Yale-Brown Obsessive Compulsive Scale(Y-BOCS) score reduction >or=35%. Duloxetine may be helpful in patients with treatment-resistant OCD, although larger and controlled studies are warranted to confirm this preliminary observation

A investigação teve como referencial teórico-metodológico a perspectiva do materialismo histórico. Foram entrevistados nove sujeitos. Foi possível resgatar nas falas dos sujeitos a evidencia de um clima geral de incertezas e de uma cultura de sobrevivencia que se articula a uma adesão ao paradigma do conhecimento porém, distintivamente de acordo a suas perspectivas de mundo, de homem e de sociedade sejam estas social ou individual. Poder-se-ia dizer que as novas formas do trabalho trazem junto consigo novos desafios que exigem a reflexão histórica do sujeito e seu trabalho a partir da política e da ética
A 2 h periodic variation in the low-mass X-ray binary Ser X-1
Spectroscopy of the low-mass X-ray binary Ser X-1 using the Gran Telescopio Canarias have revealed a ?2 h periodic variability that is present in the three strongest emission lines. We tentatively interpret this variability as due to orbital motion, making it the first indication of the orbital period of Ser X-1. Together with the fact that the emission lines are remarkably narrow, but still resolved, we show that a main-sequence K dwarf together with a canonical 1.4 M? neutron star gives a good description of the system. In this scenario, the most likely place for the emission lines to arise is the accretion disc, instead of a localized region in the binary (such as the irradiated surface or the stream-impact point), and their narrowness is due instead to the low inclination (?10°) of Ser X-1
El problema de la eternidad del mundo en Siger de Brabante
Resumen: El segundo ingreso de Aristóteles en Occidente trajo consigo una verdadera revolución intelectual. Junto con las obras de Aristóteles, fueron introducidas asimismo las traducciones latinas de Averroes, Comentador por excelencia del Filósofo. Fue así como se incorporaron al ámbito universitario parisino y al quehacer cotidiano de los pensadores, nuevos temas, algunos de ellos, de naturaleza controvertida respecto de la verdad revelada; entre ellos el tema de la eternidad del mundo. Nuestro trabajo está centrado en el tratado De Aeternitate Mundi de Siger de Brabante en el que el autor se refiere a la eternidad de las especies más que a la eternidad del mundo. Allí discute si puede o no haber creación de la especie humana en un primer hombre Adán, o si, en cambio la especie humana debe ser considerada eterna. A pesar de ello, el texto ha sido leído en una clave interpretativa que afecta el dogma de la creación.Abstract: The second entry of Aristotle in the Western world brought about a true intellectual revolution. Along with the works of Aristotle, were also introduced Latin translations of Averroes, Philosopher Commentator per excellence. In this way, new themes were incorporated to the university Parisian environment and daily work of the thinkers, some of them of controversial nature on revealed truth: among them the issue of the eternity of the world. Our work is focused on the treatise De Aeternitate Mundi from Siger of Brabant in which the author refers to the eternity of the species rather than the eternity of the world. He discusses whether or not there may be creation of the human species in a first human being Adam, or whether the human species must be considered eternal. However, the text has been read in an interpretative key that affects at the dogma of creation
