    Empirical validation of the Horowitz Multiple Systemic Infectious Disease Syndrome Questionnaire for suspected Lyme disease

A validated Lyme disease screening questionnaire would be clinically useful for both providers and patients. Three studies evaluated such a screening tool, namely the Horowitz Multiple Systemic Infectious Disease Syndrome (MSIDS) Questionnaire. The purpose was to see if the questionnaire could accurately distinguish between Lyme patients and healthy individuals.Methods: Study 1 examined the construct validity of the scale examining its factor structure and reliability of the questionnaire among 537 individuals being treated for Lyme disease. Study 2 involved an online sample of 999 participants, who self-identified as either healthy (N=217) or suffering from Lyme now (N=782) who completed the Horowitz MSIDS Questionnaire (HMQ) along with an outdoor activity survey. We examined convergent validity among components of the scale and evaluated discriminant validity with the Big Five personality characteristics. The third study compared a sample of 236 patients with confirmed Lyme disease with an online sample of 568 healthy individuals.Results: Factor analysis results identified six underlying latent dimensions; four of these overlapped with critical symptoms identified by Horowitz – neuropathy, cognitive dysfunction, musculoskeletal pain, and fatigue. The HMQ showed acceptable levels of internal reliability using Cronbach’s coefficient alpha and exhibited evidence of convergent and divergent validity. Components of the HMQ correlated more highly with each other than with unrelated traits.Discussion: The results consistently demonstrated that the HMQ accurately differentiated those with Lyme disease from healthy individuals. Three migratory pain survey items (persistent muscular pain, arthritic pain, and nerve pain/paresthesias) robustly identified individuals with verified Lyme disease. The results support the use of the HMQ as a valid, efficient, and low-cost screening tool for medical practitioners to decide if additional testing is warranted to distinguish between Lyme disease and other illnesses. Direito.Com o objetivo de observar de que maneira a pergunta “quem é o sujeito de direitos na comunidade internacional” foi respondida, ressaltando as oposições entre Nação e Estado, o presente trabalho investigou o pensamento de Pasquale Stanislao Mancini, descrevendo como que, utilizando por fundamento do direito internacional o Princípio da Nacionalidade, o jurista italiano propõe a exclusividade da Nação como sujeito de direito internacional, em oposição ao Estado e de que maneira isto influenciou os autores da escola italiana de direito internacional, em especial Terenzio Mamiami e Pasquale Fiore. A seguir, adentra-se na doutrina brasileira da segunda metade do século XIX para verificar de que forma se deu a oposição entre os conceitos de Nação e Estado. Posteriormente, debruçando-se por sobre o pensamento de Lafayette Rodrigues Pereira e Clóvis Beviláqua, busca-se demonstrar como se tratou a problemática da subjetividade jurídica internacional no Brasil durante o início do século XX. Finalmente, verifica-se qual termo passou a ser adotado majoritariamente pela doutrina brasileira, até se encontrar um autor nacional que não mais defenda o exclusivismo de um ente político em específico, mas, consciente das diversas configurações históricas pelas quais passou a comunidade internacional, aceite uma pluralidade de sujeitos. Busca-se, ainda, ao longo de todo trabalho reforçar as continuidades e rupturas entre os diversos autores analisados, além de se proporem, quando cabíveis, críticas às concepções apresentadas.In order to observe how the question “who is the subject of rights in the international community” was answered, focusing on the contrasts between Nation and State, the present study investigated the thought of Pasquale Stanislao Mancini, describing how, using the Principle of Nationality as the basis of international law, the Italian jurist proposes the exclusivity of the Nation as a subject of international law in opposition to the State and how it influenced the authors of the Italian school of international law, especially Terenzio Mamiami and Pasquale Fiore. Hereafter, enter in the Brazilian doctrine of the second half of the nineteenth century to verify how occurred the opposition between the concepts of Nation and State. Posteriorly, looking at the thought of Lafayette Rodrigues Pereira and Clóvis Beviláqua, we seek to demonstrate how the problem of international juridical subjectivity was treated in Brazil during the early twentieth century. Finally, we verify which term was mostly adopted by Brazilian doctrine, until find a national author who no longer defends the exclusivity of a political entity in specific, but, aware of different historical settings of the international community, accepts a plurality of subjects. We also try throughout the study to highlight the continuities and changes between the various authors analyzed, and to propose, as appropriate, critical to the concepts presented

    Richard I Horowitz,1,2 Phyllis R Freeman2 1Health and Human Services, Tick-Borne Disease Working Group, Washington, DC 20201 USA; 2Hudson Valley Healing Arts Center, Hyde Park, NY 12538, USA Purpose: We collected data from an online survey of 200 of our patients, which evaluated the efficacy of dapsone (diaminodiphenyl sulfone, ie, DDS) combined with other antibiotics and agents that disrupt biofilms for the treatment of chronic Lyme disease/post-treatment Lyme disease syndrome (PTLDS). We also collected aggregate data from direct retrospective chart review, including laboratory testing for Lyme, other infections, and associated tick-borne coinfections. This helped us to determine the frequency of exposure to other infections/coinfections among a cohort of chronically ill Lyme patients, evaluate the efficacy of newer “persister” drug regimens like DDS, and determine how other infections and tick-borne coinfections may be contributing to the burden of chronic illness leading to resistant symptomatology. Patients and methods: A total of 200 adult patients recruited from a specialized Lyme disease medical practice had been ill for at least 1 year. We regularly monitored laboratory values and participants’ symptom severity, and the patients completed the online symptom questionnaire both before beginning treatment and after 6 months on DDS combination therapy (DDS CT). Paired-samples t-tests and Wilcoxon signed-rank nonparametric test were performed on each of eight major Lyme symptoms, both before DDS CT and after 6 months of therapy. Results: DDS CT statistically improved the eight major Lyme symptoms. We found multiple species of intracellular bacteria including rickettsia, Bartonella, Mycoplasma, Chlamydia, Tularemia, and Brucella contributing to the burden of illness and a high prevalence of Babesia complicating management with probable geographic spread of Babesia WA1/duncani to the Northeast. Borrelia, Bartonella, and Mycoplasma species, as well as Babesia microti had variable manifestations and diverse seroreactivity, with evidence of persistence despite commonly prescribed courses of anti-infective therapies. Occasional reactivation of viral infections including human herpes virus 6 was also seen in immunocompromised individuals. Conclusion: DDS CT decreased eight major Lyme symptoms severity and improved treatment outcomes among patients with chronic Lyme disease/PTLDS and associated coinfections.     Supervision of DPD RI in The Implementation of Regional Autonomy

    In a bicameral system, Regional Representative Council of Indonesia (Dewan Perwakilan Daerah Republik Indonesia/DPD RI) acts as a regional representative institution that fights for regional problems. The authority of DPD RI is to supervise implementation of the law. The supervisory functions of the DPD RI are to monitor and supervise the government and public institutions, including the implementation of laws, regulations and policies. In this study the authors used a qualitative approach. The purpose of this study is to see the extent of supervision carried out by DPD RI on the implementation of regional autonomy carried out by Committee I. The author used strategies, analysis and relevant and important sources related to the supervisory function of the DPD RI on the implementation of regional autonomy. The author found that the authority of supervision and follow-up of the results of DPD RI's supervision on the implementation of regional autonomy is still very limited, there are supervisory problems starting from the relationship between DPD RI and the central government and local governments, human resources and supporting resources are inadequate, and the level of public confidence in DPD RI decreases

    Author Institution: Northeast Parallel Architectures Center, Syracuse UniversityThe so-called ``resolution of the identity'' integral approximation applied to second-order many-body perturbation theory (RI-MP2) is a method well-suited to the treatment of large chemical systems at a much lower computational cost than conventional MP2 calculations. In addition, features of the RI-MP2 method make it more efficient to implement on parallel computers than the conventional MP2 approach, an important advantage in pushing in limits of large-scale correlated calculations. I will describe the RI-MP2 method itself and the implementation of both energies and gradients, emphasizing the important differences from the conventional MP2 as well as the parallelism. I will discuss fitting basis sets, introduced by the RI approximation, and present results from several RI-MP2 applications, including energy calculations on systems of more than 2400 AO basis functions, which run an estimated 30 times faster than the equivalent conventional MP2 calculation

    Turbulent stratified shear flow experiments: Length scale comparison

    Stratified shear flows are ubiquitous in geophysical systems such as oceanic overflows, wind-driven thermoclines, and atmo- spheric inversion layers. The stability of such flows is governed by the Richardson Number Ri which represents a balance between the stabilizing influence of stratification and the destabilizing influence of shear. For a shear flow with velocity difference U, density difference ∆ρ and characteristic length H, one has Ri = g(∆ρ/ρ)H/U^2 which is often used when detailed information about the flow is not available. A more precise definition is the gradient Richardson Number Rig = N^2/S^2 where the buoyancy frequency N = ((g/ρ)∂ρ/∂z)^{1/2}, the mean strain S = ∂U/∂z in which z is parallel to gravity and suitable ensemble or time averages define the gradients. We explore the stability and mixing properties of a wall-bounded shear flow over a range 0.1< Rig <1 using simultaneous planar measurements of density and velocity fields using Planar Laser-Induced Fluorescence (PLIF) and Particle Image Velocimetry (PIV), respectively. The flow, confined from the top by glass horizontal boundary, is a lighter alcohol-water mixture injected from a nozzle into quiescent heavier salt-water fluid with velocity between 5 and 10 cm/s and with a relative fractional density difference of 0.0026 or 0.0052. The injected flow is turbulent with Taylor Reynolds number between 50 and 100. We compare a set of length scales that characterize the mixing properties of our turbulent stratified shear flow including the Thorpe Length L_T, the Ozmidov Length L_o, the Ellison Length L_E, and turbulent mixing lengths L_m and L_ρ

    Many street vendors are still using a wood stove / brazier for cooking. The reason is more efficient and the food tastes better. however, the conditions emissions that are toxic produced because incomplete combustion not at all considered by traders even by the buyer. This condition is exacerbated because the traders who sell at night are also burdened with electric lighting. As a shortcut, many traders who use illegal electricity directly from the flow of electricity distribution. Besides harming PLN is also threatening condition due to illegal electricity usage does not meet basic safety standards. Because of the above problems, the author designed the concept of alternative energy harnesses the heat from the gas stove RI-511A and RI-300HP used as electrical energy by using a thermoelectric generator TEG type 126-40A. The working principle of thermoelectric generators based on the Seebeck effect, if heat is applied to each series of the two different characters will result in electric current. Of the testing and the analysis of data generated stove RI-511A the highest voltage is 5.51 V and the highest current is 0.13 A, while the highest voltage generated stove RI-300HP is 7.78 V and the electrical highest current was 0.23 A.     Reverse Innovation in Retrospect and Prospect: Quo Vadis?

    Reverse innovation (RI) is considered as an innovation originally designed and developed for low-income customers living in severely resource-constrained environments in emerging and developing countries’ markets, with the potential to be diffused in developed markets. After more than a decade of academic studies, the potential role of RI in creating higher impact global innovations has progressively advanced. With the upsurge in research on RI, there is a need for scholars and business practitioners to retrospectively reflect on existing/current research state and prospect for future research directions. In this article, we examine the existing conceptualization and research landscape of RI to further identify and map future research directions. First, through a bibliometric review of a decade of research (2009–2019), we provide insights into the evolution of research topics in the field of RI including the identification of main research streams, influential scholars and works, important scholarly associations, and collaborative networks.
