    The Impact of Virtual Discharge Interventions Compared to Traditional Methods on Adult Patient Population Readmission Rates: An Integrative Review

    Hospital readmissions are a persistent quality and cost burden to the healthcare system, prompting evaluation of modern strategies to strengthen transitional care. This integrative review synthesized findings from thirteen studies published between 2011 and 2025 to assess the effectiveness of virtual discharge interventions in reducing readmission rates among adult patients. Virtual discharge interventions included programs utilizing technology, such as phone calls, video visits, apps, or remote monitoring, to help patients understand their care and improve positive outcomes. Many of these studies showed that virtual support can reduce hospital readmissions, especially for people with heart failure. Some programs also improved patients’ understanding of their medications and deterioration warning signs. Despite promising findings, generalizability was constrained by some intervention designs, inconsistent outcome measures, and limited sample sizes. High-quality, multi-site RCTs are needed to clarify the comparative effectiveness and scalability of virtual discharge models. Overall, virtual discharge support appears to be a promising way to help patients feel more confident, supported, and prepared after leaving the hospital

    Effect of Nonpharmacological Interventions on Agitation among Persons Living with Alzheimer’s Disease and Related Dementias

    Alzheimer’s disease and related dementias (ADRD) are devastating and debilitating disorders that continue to increase prevalence over time. According to the Alzheimer\u27s Association (2024), nearly seven million Americans are living with Alzheimer’s disease, and this number is projected to rise to about 13 million by 2050. Alzheimer\u27s disease is the most usual form of dementia accounting for about 60-70% of cases (Pinyopornpanish et al., 2022). Persons living with ADRD (Pw-ADRD) experience behavioral and psychological symptoms of dementia (BPSD) that include disruptions in perception, thought processes, mood, or behavior (Richler et al., 2023). Agitation is one of the most prevalent and challenging dementia-related behavioral symptoms, characterized by inappropriate verbal, vocal, or physical actions that impact the life of Pw-ADRD and increases family and caregivers’ burden (Cohen-Mansfield & Billig, 1986; Givens et al., 2015; Wetzels et al., 2010). This integrative review aimed to evaluate and synthesize evidence from published literature on the effectiveness of nonpharmacological interventions (NPIs) in reducing dementia-related agitation. The review was guided by the following questions: (1) Do NPIs decrease agitation in Pw-ADRD? (2) What are the most effective NPIs for reducing agitation episodes in persons living with ADRD

    Asthma Integrative Review: Effects of Aerobic Exercise on Asthma Exacerbation

    Asthma is a chronic respiratory illness defined by airway inflammation and episodes of exacerbation. Pharmacological intervention remains the treatment of choice; however, there is growing evidence to support moderate intensity aerobic exercise as adjunct therapy to promote asthma control. The integrative review evaluates current evidence supporting the PICO question: In adults with mild to moderate asthma, what is the effect of prescribed aerobic exercise on reducing exacerbations? A systematic search of CINAHL, Health Source: Nursing, MEDLINE, and PubMed identified eligible studies. The inclusion criteria required peer-reviewed quantitative articles written in English and published within the past five years. In addition, adults older than 18 years, mild to moderate asthma, and the effects of aerobic exercise on asthma exacerbations were included in the search. Pediatric studies and articles unrelated to exercise were excluded. An evaluation table was used to collect data which was used to organize, code, and categorize themes common across the selected articles. The four themes include: quality of life and asthma control, moderate to high intensity exercise, pulmonary rehabilitation, and weight reduction coinciding with exercise. The findings suggest that moderate intensity aerobic exercise programs decreased the frequency and severity of exacerbations. Pulmonary rehabilitation programs integrating aerobic exercise supported symptom management, quality of life, and weight reduction. Collectively, the research supports prescribed aerobic exercise and rehabilitation-based training may provide a valuable treatment for adults with mild to moderate asthma. Standardized exercise protocols and long-term studies are essential to determine optimal training practices and strengthen clinical guidance

    Breast Milk Versus Formula Feeding in the Development of Necrotizing Enterocolitis Among NICU Infants: An Integrative Review

    Introduction/Question: NEC is a serious condition characterized by the death of intestinal tissue, primarily affecting premature infants or those with low birth weight. The incidence of NEC poses a significant concern in neonatal healthcare due to its substantial mortality risk, particularly among vulnerable populations. This condition can lead to various complications, including sepsis, nutritional deficiencies, delays in growth and development, and, in severe cases, death. NEC not only impacts the health of the infant but also affects the emotional well-being and bonding experiences of parents. Despite advancements in neonatal care, the rate of NEC has increased, highlighting the need to assess the associated risk factors and potential interventions. The objective of this integrative review is to investigate how different types of feeding, specifically breast milk compared to formula, affect the development of NEC in infants in the United States. Findings: Research revealed the protective role of breast milk composition against NEC. Specific substances, such as bioactive lipid dioleoyl phosphatidylcholine (DOPE), Immunoglobulin A (IgA), and Human Milk Oligosaccharides (HMOs), make breast milk unique and beneficial for newborns. Studies have also found that Exclusive Human Milk Diet (EHMD) and fortified maternal milk decrease the risk of NEC compared to bovine-based fortifiers. In contrast, endotoxins and lipopolysaccharides, which are found in formulas, are known to contribute to mortality from NEC in premature infants. Conclusion: This review highlights the advantages of breast milk over formula feeding in preventing NEC in premature infants. Key findings suggest that human milk’s protective properties, including oligosaccharides and IgA, contribute to its superiority. The use of EHMD with fortifiers improves growth and lowers NEC incidence. Further research is needed on educational programs for healthcare professionals to standardize feeding practices and understand the effects of combining breast milk and formula. Education for caregivers about the benefits of breast milk is essential for the health of premature infants

    A Review of Routing Attacks in Routing Protocol Over Low-Power and Lossy-based IoT Networks

    Low-power and Lossy IoT Networks (LLNs) comprise physical sensors, processing capability, power, and other technologies to exchange information between systems and devices over the internet. However, these networks are susceptible to routing attacks affecting resources, traffic flow, and topology formation. In the related work, it has been discovered that many previous studies do not consider algorithms and implementation approaches for routing attacks. This research study provides a comprehensive in-depth synthesis insight into the description, effects, and algorithms & implementation of four routing attacks in LLNs i.e., rank, sinkhole, DIS-flooding, and worst parent attacks. The findings of this research study highlight the most used and suitable simulation tool to implement routing attacks for RPL-based networks, moreover, it presents most of the network parameters mostly targeted by routing attacks. The study also presents a breakdown of effective attacking percentages to measure the impact of routing attacks. Lastly, we present a guide to implementing routing attacks in RPL-based IoT using the Cooja Contiki simulation tool

    Ivy Proteins as Guardians of Bacterial Cell Wall Integrity: Regulation of Lytic Transglycosylases Explored

    Lytic transglycosylases (LTs) are bacterial enzymes involved in biosynthesis and maintenance of the peptidoglycan cell wall. LTs are reported to be regulated by two paralogs of Inhibitor of Vertebrate Lysozymes: Ivyp1 and Ivyp2. This study investigated the regulatory dynamics between the soluble LT (SltB1), and Ivy paralogs as putative regulators in Pseudomonas aeruginosa (PA). The secondary objective was to delineate the critical roles played by a glutamic acid at position 135 in SltB1 and a histidine at position 62 in Ivyp1 in governing their molecular interaction through site directed mutagenesis. Isolated SltB1 shows no lytic activity in our assay, in spite of the fact that it was correctly folded and pure, suggesting a potential dependency on a cofactor or coenzyme for activation. In addition, Biolayer Interferometry revealed that isolated SltB1 is not subject to regulation by either Ivyp1 or Ivyp2. These results challenge Ivy\u27s characterization as a general inhibitor for all LTs suggesting that SltB1 is regulated by other factors

    One of the greatest curses and pleasures of my life is being a geographer. The curse? Do you have any idea how many times I am asked to name the capitol of Kentucky (I don\u27t even care) or how many people pick me as their partner in Trivial Pursuit or Jeopardy? The pleasure? Practicing real geography, that is, using and teaching geographical skills to solve real problems and understanding the world around me. More of this later. It always amazes me how a discipline as old and distinguished as geography came to be so misunderstood through the centuries. Granted, there are some who have not succumbed to the terrible simplification of geography which has, for too many, reduced it to a boring recitation of names of places and products and lists of the longest, biggest, highest, etc., ad nauseum
