Infection and failure rates following total hip arthroplasty for septic arthritis: a case-controlled study
Introduction: Total hip arthroplasty (THA) is performed as a 2-stage or single stage procedure in patients with a history of septic arthritis of the native hip joint. The decision is based on whether the infection is active or quiescent in the joint. The aim of this study was to compare the outcomes of total hip arthroplasty for septic arthritis of the native hip to a control series of patients treated for primary osteoarthritis of the hip with standard THA. Methods: Between March 2000 and March 2013, 18 cases of septic arthritis of the hip treated with THA were retrospectively identified. During the same time period, 18 control cases of THA for degenerative osteoarthritis were identified. Cases and controls were comparable in age, gender, body mass index, type of anaesthesia and American Society of Anesthesiologists score. Results: 11 and 7 cases received 2-stage and single stage total hip arthroplasty respectively for septic arthritis. There was a mean interval of 4 months between 1st and 2nd-stage operations. Cases and controls were followed up for a mean of 70 and 72 months respectively. There was no reinfection or implant failure in the cases, and comparable functional outcomes between cases and controls. Conclusions: Two-stage and single-stage THA for active and quiescent native hip infection respectively, achieved similar outcomes to THA for primary osteoarthritis in controls. </jats:sec
A comparative study for hospitalization characteristics and predictors of Covid-19 inpatients in the United States
BACKGROUND:
In 2021, the world is still in the midst of the Covid-19 pandemic. Many aspects of Covid-19 related to transmission, infection, and treatment still remain unclear. Severe obesity increases the risk for negative outcomes in patients with Coronavirus Disease 2019 (COVID-19). The objectives of this study were to investigate the effect of BMI on in-hospital outcomes in the New York City Health and Hospitals ethnically diverse population, explore further this effect by age, sex, race/ethnicity, and timing of admission, and given the relationship between COVID-19 and hyperinflammation assess the concentrations of markers of systemic inflammation in different BMI groups.
METHOD:
This was a retrospective study of consecutive hospitalized patients with COVID-19 in the public health care system of New York City from March 1st, 2020 to October 31st, 2020. In total, 8833 patients were included in this analysis. Results of logistic regression are given as the odds ratio (OR) with the 95% confidence interval (CI). The threshold of statistical significance was p <0.05. All analyses were performed using SAS software (Release 9.4M6; SAS Institute, USA).
RESULTS:
8833 patients were included in this analysis (women: 3593, median age: 62 years). The median body mass index (BMI) was 27.9 kg/m2. Both overweight and obesity were independently associated with in-hospital death (BMI 25 to <30 kg/m2: OR 1.220, 95% CI: 1.068-1.393; BMI 30 to <35 kg/m2: OR 1.494, 95% CI: 1.278-1.746; BMI 35 to <40 kg/m2: OR 2.164, 95% CI: 1.780-2.632), and BMI ≥40 kg/m2: OR 2.242, 95% CI: 1.855-2.710). The association of overweight and obesity with death appeared to be stronger in men, older patients, and individuals of Hispanic ethnicity. In this study, higher concentrations of inflammatory markers in patients with obesity as compared to those without obesity were not observed.
CONCLUSION:
It was observed that overweight and obesity were associated with in-hospital death, admission to ICU, and invasive mechanical ventilation after adjusting for obesity-related potentially confounding factors. The association of overweight and obesity with death appeared to be stronger in men, older patients, and individuals of Hispanic ethnicity. Patients with obesity did not have higher concentrations of inflammatory markers on presentation. CRP, IL-6, ferritin, LDH, and D-dimer were positively associated with death, while albumin and monocytes were inversely associated with death after adjusting for age, sex, and BMI.Ph.D.Includes bibliographical reference
Considérations autour de l’expérience de la perte dans la production girodienne
The aim of this article is to reveal the ontological mutation undergone by the characters in three literary productions by Ryad Girod, an Algerian author of the extreme contemporary. Ravissements, Les yeux de Mansour and La fin qui nous attend are three texts revolving around the experience of loss: loss of speech in Ravissements, loss of life in Les yeux de Mansour and loss of humanity in La fin qui nous attend. The subjective experience of immaterial loss is examined in the light of a poetics of detachment capable of revealing to the characters the vital dimensions of their existence. Withdrawal reveals itself to be a vector of knowledge, founding a modified relationship with oneself and the world. The reconsideration of the spoken word is a decisive step in the discernment of characters to establish a new approach to the world, in resonance with the contingencies shaking their logical understanding of facts. The rationality governing the contemporary world is gradually replaced by an intuitive approach, leading to a new relationship with oneself, with others and with the world. This change in existential posture also engenders a new perception of time, understood in terms of duration rather than in terms of its decantation into divisible units. It also leads the characters to ontological and axiological reflections, revealing the intense philosophical potential of Ryad Girod\u27s works. The strictly cerebral approach is gradually giving way to a more immediate, intuitive knowledge, in an attempt to make a deep connection with the quintessence of inanimate and animate beings.La réflexion menée dans cet article a pour objectif de révéler la mutation ontologique opérée chez les personnages de trois productions littéraires de Ryad Girod, auteur algérien de l’extrême contemporain. Ravissements, Les yeux de Mansour et La fin qui nous attend, trois textes gravitant autour de l’expérience de la perte : de parole essentiellement dans Ravissements ; de vie dans Les yeux de Mansour surtout, et d’humanité dans La fin qui nous attend principalement. L’expérience subjective de la perte immatérielle est interrogée à la lumière d’une poétique de détachement à même de révéler aux personnages les dimensions vitales à leur existence. Ce retrait s’y révèle un vecteur de connaissance fondateur d’un rapport modifié à soi et au monde. La reconsidération de la parole est une étape décisive dans l’entreprise de discernement des personnages soucieux d’asseoir une nouvelle approche du monde, en résonnance avec les contingences ébranlant leur compréhension logique des faits. A la rationalité gouvernant le monde contemporain se substitue peu à peu une approche intuitive jusqu’à atteindre un nouveau rapport à soi, aux autres et au monde. Ce changement de posture existentielle engendre aussi une nouvelle perception du temps compris en termes de durée et non selon sa décantation en unités divisibles. Il mène également les personnages à des réflexions ontologiques et axiologiques révélant l’intense potentiel philosophique des ouvrages de Ryad Girod. La démarche strictement cérébrale se retranche peu à peu à la faveur d’une connaissance intuitive, plus immédiate, dans une tentative de connexion profonde avec la quintessence des êtres inanimés et animés.
 
Autonomous Flying With Neuromorphic Sensing
Autonomous flight for large aircraft appears to be within our reach. However, launching autonomous systems for everyday missions still requires an immense interdisciplinary research effort supported by pointed policies and funding. We believe that concerted endeavors in the fields of neuroscience, mathematics, sensor physics, robotics, and computer science are needed to address remaining crucial scientific challenges. In this paper, we argue for a bio-inspired approach to solve autonomous flying challenges, outline the frontier of sensing, data processing, and flight control within a neuromorphic paradigm, and chart directions of research needed to achieve operational capabilities comparable to those we observe in nature. One central problem of neuromorphic computing is learning. In biological systems, learning is achieved by adaptive and relativistic information acquisition characterized by near-continuous information retrieval with variable rates and sparsity. This results in both energy and computational resource savings being an inspiration for autonomous systems. We consider pertinent features of insect, bat and bird flight behavior as examples to address various vital aspects of autonomous flight. Insects exhibit sophisticated flight dynamics with comparatively reduced complexity of the brain. They represent excellent objects for the study of navigation and flight control. Bats and birds enable more complex models of attention and point to the importance of active sensing for conducting more complex missions. The implementation of neuromorphic paradigms for autonomous flight will require fundamental changes in both traditional hardware and software. We provide recommendations for sensor hardware and processing algorithm development to enable energy efficient and computationally effective flight control.</p

These drafts aim to develop legal regulation of land relations in gardening, vegetable gardening and cottage economy. The author focuses on improving legislative regulation of land relations in gardening set in the Russian Federation President State of the Nation address of December 12/2013, instructions of the Russian Federation President, Conception of the Development of Civil Legislation prepared under the Russian Federation President Decree On Improving the Russian Federation Civil Code, the Russian Federation Constitutional Court judicial acts and recommendations developed by the State Duma. The paper examines the problems of implementing the judicial acts of the Russian Federation Constitutional Court recognized contradicting the Russian Federation Constitution provisions of the Federal law On Gardening and Cottage noncommercial Associations of Citizens as to the ban on registering citizens in their location in the dwelling facilities situated on gardening plots including in terms of the Russian Federation Government bill On Amending the Federal Law On On Gardening and Cottage noncommercial Associations of Citizens. Special attention is given to the bill of federal law no. 465407-6 On amending the Russian Federation Land Code as to classifying lands into the categories under territorial zoning aimed to eliminate one of the key institutions of land legislation, i.e. dividing lands according to the purpose into categories. The author has conducted a detailed analysis of consequences of this elimination for farmers and cottage owners. An extensive part of the article is devoted to the analysis of provisions no. 465407-6 On amending the Russian Federation Land Code and the Russian Federation legislative acts as to improving the order of providing land plots belong- ing to state or municipalities which will reconsider the order of allocating land plots for gardening. В статье подробно исследованы проблемы реализации судебных актов Конституционного Суда Российской Федерации, которыми были признаны противоречащими Конституции Российской Федерации положения Федерального закона «О садоводческих, огороднических и дачных некоммерческих объединениях граждан» в части запрета на регистрацию граждан по месту жительства в жилых строениях, расположенных на садовых земельных участках, в том числе, в свете подхода, предлагаемого Правительством Российской Федерации в проекте федерального закона «О внесении изменений в Федеральный закон «О садоводческих, огороднических и дачных некоммерческих объединениях граждан». Отдельное внимание уделяется проекту федерального закона № 465407-6 «О внесении изменений в Земель- ный кодекс Российской Федерации и отдельные законодательные акты Российской Федерации в части перехода от деления земель на категории к территориальному зонированию», которым пред- лагается упразднить один из центральных институтов земельного законодательства — деление земель по целевому назначению на категории. Автором проведен подробный анализ последствий отмены деления земель на категории для объединений садоводов, огородников и дачников, деятельность которых имеет широкое распространение на территории Российской Федерации. Значительная часть статьи посвящена планируемым изменениям законодательства, которые предусматривают серьезный пересмотр порядка предоставления и образования земельных участков, предназначенных для ведения садоводства, огородничества и дачного хозяйства. Автором обозначены как недостатки существующего правового регулирования в данной сфере, так и обращено внимание на основные пути совершенствования Земельного кодекса Российской Федерации, предлагаемые Правительством Российской Федерации
