    A novel estetrol-containing combined oral contraceptive: European expert panel review

Purpose: Despite considerable advances in recently developed combined oral contraceptives (COCs), resulting in lower rates of adverse events while maintaining contraceptive efficacy, there is interest in further innovation. Materials and Methods: Estetrol (E4), a native oestrogen, and progestin drospirenone (DRSP) were combined in a new COC. A European expert panel reviewed the pharmacology, efficacy, and safety and tolerability of this combination. Their findings are presented as a narrative review. Results: E4 15 mg/DRSP 3 mg in a 24/4 regimen provided effective contraception with good cycle control, characterised by a predictable regular bleeding pattern and minimal unscheduled bleeding, together with a good safety profile. The combination was associated with high user satisfaction, well-being, and minimal changes in body weight. The effects on endocrine and metabolic parameters were limited, and the combination was found to have a limited impact on liver function and lipid and carbohydrate metabolism. Moreover, its effect on several haemostatic parameters was lower than that of comparators containing ethinyl oestradiol (EE) 20 \ub5g/DRSP 3 mg and EE 30 \ub5g/levonorgestrel 150 \ub5g. Conclusion: E4 15 mg/DRSP 3 mg provides safe and effective contraception, with high user satisfaction and predictable bleeding. Further research will evaluate the long-term safety of the COC

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods

    International audienceObjectivesTo study the methods and strategies of fertility preservation in young women with stage I epithelial ovarian cancer (EOC), in order to provide recommendations for clinical practice.MethodsThe PubMed database was searched for english and french language articles, between 2005 and 2001, according to predefined search equations.ResultsYoung patients with stage IA EOC willing to conceive should be informed that conservative treatment (contralateral ovary and salpinx, uterus) is possible (Grade C), associated with a 6 % to 13 % recurrence risk (Grade C) on the remaining ovary. This conservative surgical treatment includes adnexectomy, peritoneal and lymph node staging for all subtypes, and additional endometrial curettage for endometriosis and mucinous subtypes (Grade C). In case of positive staging conservative treatment is not possible. In case of mucinous EOC with an infiltrative pattern, lymph node staging is not necessary. Multidisciplinary analysis (including oncologists and reproductive medicine specialists) of the risk-benefit balance for a conservative surgery is recommended and must rely on a complete final pathology report (Grade C). No recommendation on bilateral adnexectomy and uterine conservation to allow pregnancy using egg donation can be provided in case of low-grade stage IA EOC, in the absence of data. Bilateral adnexectomy and uterine conservation to allow pregnancy using egg donation can be offered in case of serous, mucinous or endometrioid high-grade FIGO stage IA or low-grade FIGO stage IC1 or IC2 EOC (Grade C). Preservation of the uterus and contralateral ovary and Fallopian tube can be discussed with a specialized rare ovarian tumors multidiciplinary staff in case of clear cell stage I EOC.ObjectifsÉtudier les modalités et stratégies de préservation de la fertilité pour une femme jeune en cas de cancer de l’ovaire stade I en vue d’émettre des recommandations à l’usage des praticiens.MéthodesRecherche bibliographique en langue française et anglaise effectuée par consultation de la base de données PubMed entre 2005 et 2017, selon les équations de recherche prédéfinies.RésultatsIl est recommandé d’informer les patientes atteintes d’un cancer de l’ovaire de stade IA en âge de procréer sur les possibilités d’un traitement conservateur (Grade C), cette stratégie étant associée à un risque de récidive compris entre 6 % et 13 % au niveau de l’ovaire controlatéral. Un traitement chirurgical conservateur de l’utérus et de l’annexe controlatérale après annexectomie unilatérale peut être proposé pour un cancer de l’ovaire stade IA de bas grade chez une femme en âge de procréer, à la condition d’une stadification négative (péritonéale et ganglionnaire complète pour toutes les histologies et associée à un curetage utérin pour les sous-types histologiques endométrioïde et mucineux) (Grade C). Pour le cas particulier du cancer ovarien mucineux expansif, la stadification ganglionnaire n’est pas nécessaire. Une information sur le risque de récidive compris entre 6 % et 13 % au niveau de l’ovaire controlatéral est recommandée en cas de souhait de préservation de la fertilité (Grade C). Une évaluation de la balance bénéfices/risques d’une chirurgie conservatrice est recommandée sur une anatomopathologie définitive de façon multidisciplinaire entre oncologues (médical et/ou chirurgical) et un médecin de la reproduction (Grade C). En l’absence de données, aucune recommandation ne peut être formulée sur la stratégie d’annexectomie bilatérale avec préservation de l’utérus en vue d’un don d’ovocytes dans le cadre des cancers épithéliaux de l’ovaire de stade 1A de bas grade. Une annexectomie bilatérale avec préservation utérine peut être proposée afin d’envisager ultérieurement une grossesse par don d’ovocyte pour un cancer séreux, mucineux ou endométrioïde de l’ovaire de stades FIGO IA de haut grade ou FIGO IC1 ou IC2 de bas grade (Grade C). La préservation utérine et annexielle controlatérale peut être discutée au cas par cas en réunion de concertation pluridisciplinaire de tumeurs rares pour un cancer à cellules claires de stade I

    We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
