Stressor-induced alterations in serotonergic activity in an animal model of depression
Examined the effect of 2 neurogenic stressors (air puff and restraint) and a metabolic stressor (lipopolysaccharide; 100 ?g/kg) on accumbal serotonergic neurotransmission in the olfactory bulbectomized (OB) rat model of depression. Male Sprague-Dawley rats were used in 2 studies. Both puff and restraint stress caused greater increases in accumbal 5-hydroxyindoleacetic acid (5-HIAA) in OB vs sham-operated rats. In contrast, bulbectomy resulted in a blunted serotonergic response to a challenge with the metabolic stressor. In addition, OB rats displayed significantly basal levels of 5-HIAA than sham-operated Ss, a finding consistent with previous reports of the OB rat being a model of hyposerotonergic depression. The relevance of these findings to stressor-provoked depressive-like behaviors in the OB rat is discussed
Korelasi Rasio Neutrofil-Limfosit dengan Nilai Troponin sebagai Penanda Kerusakan Miokardium pada Pasien Sindroma Koroner Akut Non Elevasi Segmen ST di Rumah Sakit Umum Pusat Haji Adam Malik Medan
Background: Risk stratification in Non-ST elevation acute coronary syndrome (NSTEACS) patients is mandatory. Diagnosis of Non-ST elevation myocardial infarction (NSTEMI) confirmed by increased troponin level is one of the risk stratifications which then becomes a reference for early invasive management within 24 hours. The importance of this troponin test is not accompanied by the assurance of the availability of the test in remote regional hospitals. Other parameters are widely available and can be used to predict troponin elevation in the NSTEACS. The leukocyte component in the form of the neutrophil to lymphocyte ratio (NLR) is believed to be used because it has a relationship with troponin values in previous studies. Increased troponin in myocardial infarction occurs due to damage or necrosis of the myocardium then causes the release of troponin into the circulation, on the other hand, an increase in neutrophils and a decrease in lymphocytes can occur due to the inflammatory response in myocardial infarction.
Method: This cross-sectional study involved NSTEACS patients with 48 hours of onset, from January 2021 to December 2021 at H. Adam Malik Hospital, Medan. Recording the value of NLR and troponin I at the time of admission, then analyzing the correlation. After obtaining the correlation coefficient, a linear regression equation analysis was performed and continued with receiver operating curve (ROC) analysis to determine the area under the curve (AUC) value and NLR cut off in predicting the elevation of troponin I value.
Results: 80 patients were included in this study and 58 patients (72.5%) had myocardial infarction. The results of the analysis showed that there was a positive correlation between NLR and troponin I values (r= 0.516, p-value 0.000), a positive correlation between neutrophils and troponin I values (r= 0.419, p-value 0.000), and a negative correlation between lymphocytes and troponin I values (r =-0.312, p-value 0.005). NLR has the best correlation coefficient on troponin I values compared to other parameters. With a cut-off point of 2.79, NLR could predict an increase in troponin I value with a sensitivity of 70.7% and a specificity of 68.2%.
Conclusion: NLR has a moderate correlation with troponin I values, and can be used to predict troponin I elevation in the NSTEACS patients.
RNL memiliki koefisien korelasi paling baik terhadap nilai troponin I dibandingkan parameter lainnya.
Background : Elevated blood urea nitrogen (BUN), blood glucose, and alteration sodium
level are common among patients with acute myocardial infarction (AMI). These parameters
to be widely investigated to assess the prognosis in AMI patients. However, the combination
of these parameters (BUN, blood glucose, and sodium) calculated by a certain formula in the
form of plasma osmolality has not been widely studied to assess the prognosis of patients
with acute myocardial infarction. This study aims to assess plasma osmolality in predicting
hospital major adverse cardiovascular events (MACEs) among AMI patients.
Methods : Data were collected from 118 consecutive patients with AMI in Cardiac Centre
Haji Adam Malik General Hospital Medan. We measured admission plasma osmolality
[1,86 (Na+) + BUN/2,8 + Glucose/18+9 ]. Then we observed in hospital Major Adverse
Cardiovascular Events (MACEs) which consist of cardiovascular mortality, acute heart
failure, malignant arrhythmia, and cardiogenic shock. Statistical analysis was performed
using mean difference, logistic regression, and receiver operating curve (ROC).
Result : Among 118 patients, MACEs were observed in 49 (41.5%) patients with the most
common MACEs was acute heart failure (25.4%). Bivariate analysis showed a significant
relationship between the plasma osmolality and in hospital MACEs (p < 0.001). The plasma
osmolality AUC prediction value was 78.9%. The optimal cut-off value was 279.9 mosm/kg
(sensitivity 81.6%; specificity 75.4%). In multivariate logistic regression analysis, the
plasma osmolality was the strongest predictor with an OR value of 10.542 (95% CI 2.694-
41.255; p value <0.001).
Conclusions : Among AMI patients, High plasma osmolality value (≥280 mosm/kg) is a
better predictor of in-hospital MACEs than its components separately (BUN,Glucose level,
IMA.
Metode : Penelitian melibatkan 118 pasien rawat inap IMA di Pusat Jantung Terpadu RSUP
Haji Adam Malik Medan. Dilakukan penghitungan osmolalitas plasma admisi [1,86 (Na+) +
BUN/2,8 + Glukosa/18+9 ]. Selanjutnya diikuti selama perawatan apakah dijumpai Kejadian
jantung akut paling banyak dijumpai (25.4%). Analisis bivariat menunjukan hubungan
Nilai
prediksi (AUC) osmolalitas plasma adalah 78.9%. Nilai cut-off yang didapat adalah 279.9
mosm/kg (sensitivitas 81.6% ; spesifisitas 75.4%). Pada analisis multivariat regresi logistik,
osmolalitas plasma menjadi prediktor terkuat dengan nilai OR 10.542 (95% IK 2.694-41.255;
nilai p <0.001).

The design and fabrication of a free flowing water turbine for UiTM-PERHILITAN Research Center Taman Negara / Muhammad Zul Hilmi Asmad.
Universiti Teknologi MARA (UiTM) associated with PERHILITAN had established the UiTM-PERHILITAN Research Centre, located at Kuala Keniam, Taman Negara Pahang. The research centre located 25km upstream of Tembeling River, only accessible using boat. Although the loca-tion is strategic for geological, environment, and technical research and development, this research centre is located away from the National Power Grid. The generator set used to power-up the centre was not efficient, as it used 200 litres of gasoline for 2 hours operation. The gasoline price and its transportation might be costly and UiTM cannot bear the cost for generator's long run. Looking at this problem, the author had come up with a research thesis, supported by a full scale model of free flowing water turbine that can supply the centre with electricity up to 1kw of power. The design of the turbine can avoid environment damage such as land erosion, jungle flooding and loss of flora and fauna. No damp or weir is used in order to rotate the turbine. By finishing this thesis, the author hopes that the turbine can be further develop for commercial used in the future and a self-sustainable community can be achieved, without have to be dependent on the oil-based power generation
