    In this essay I have tried to present the ideas and thoughts of the Swedish author Rolf Edberg (1912-1997), journalist, ambassador and county governor. By many regarded as an environment philosopher and the introducer of deep ecology into Sweden. The essay emphasizes Edberg as a modern renaissance man of important knowledge with a rhetorical force to present and generalize the problems the planet Earth is meeting. The paper compares him with Henry Thoreau but also finds a close but perhaps unintended relationship with the German philosopher Ernst Haeckel although Edberg himself claimed to be a Darwinist at heart. The essay stresses on four characteristic features in his authorship namely 1) a holistic view of nature and man, rejecting dualism for monism; 2) a cosmic relationship within the whole universe and also between organic and inorganic matter; 3) the planet Earth is in a crisis due to an uneven and unjust distribution of the resources of the Earth, the nuclear weapon threat and the overpopulation; 4) the evolution and not the religion has the answer to our existential questions

Declining levels of estrogen at menopause could be a contribution factor. Isoflavones is a vegetable substance, with structural and functional similarities to the human estrogen. This is a potential reason for possible cholesterol-lowering effects. The substrate of studies on isoflavones and its effect on cholesterol found today is ambiguous and there are uncertainties about which statements can be made. Objective: To examine the scientific evidence on whether isoflavones in combination with isoflavones have a different impact on HDL- and LDL-cholesterol in hypercholesterolemic postmenopausal women in comparison with milk protein. Search strategy: Literature searches were made in the databases PubMed and Scopus. Search words used were of relevance for the research question of this systematic review. Selection criteria: Randomized controlled trials done on postmenopausal women were included. They compared isoflavones combined with soy protein with milk protein and were preformed on women with elevated levels of total- and /or LDL-cholesterol. Studies conducted on populations with hormone – and/or cholesterol -lowering therapy were excluded. Data collection and analysis: The literature search yielded three RCTs which were validated using SBUs Granskningsmall för randomiserade kontrollerade studier. The strength of evidence for the included studies was then weighed together with Göteborgs universitets sammanfattande evidensformulär. Main results: Two of the studies were estimated to have a mediate-high study quality and the third was judged to have a mediate-high to high study quality. The summarized grade of evidence for the selected outcome measures were considered mediate-high. Only one study found a significant difference in LDL-cholesterol between intervention- and control group. None of the studies showed a significant difference in HDL cholesterol. Conclusions: There is moderate evidence that isoflavones in combination with soy protein does not provide any significant difference in impact on HDL-cholesterol compared with milk protein, in hypercholesterolemic postmenopausal women. As the effect on LDL-cholesterol differs between the included studies no conclusions can be drawn. The scientific substrate with estimated moderate evidence shows an ambiguous result for the outcome measure LDL-cholesterol.     Improved Information Transfer in Elderly Care : - a case study of implementation of a modified SBAR tool

    Brister i kommunikation är den enskilt största orsaken till att avvikelser inträffar i hälso-och sjukvård. Genom att standardisera kommunikation och informationsöverföring minskar variationen, och logik och ömsesidig överenskommelse skapas.   Syftet med förbättringsarbetet var att öka patientsäkerheten genom att förenkla kommunikationen inom teamet på ett vård-och omsorgboende, genom att införa kommunikationsverktyget SBAR. Den efterföljande studien syftade till att undersöka hur SBAR upplevs inom äldreomsorgen, och vad som kan vara framgångsfaktorer och fallgropar vid införande.   Action research har tillämpats och en innehållsanalys har utförts efter fokusgruppsintervjuer.   Verksamheten såg en positiv utveckling av förbättrad informationsöverföring men nådde inte upp till de uppsatta målen. SBAR upplevdes användbart när de professionella hade fått praktisk träning. De professionella kände en ökad trygghet då den muntliga rapporteringen kombinerades med skriftlig rapportering. Projektet skapade gemensamt lärande genom deltagande observationer. Projektet gjorde att de omgivande strukturerna blev tydligare för personalgruppen och informationsöverföringens fokus lyftes från individnivå till systemnivå. SBAR-strukturen förenklades något för att passa bättre i den flerspråkiga kontexten. Nya områden för förbättring identifierades och en aktivitetstavla skapades. Att hålla jämn utbildningstakt och utbilda vikarier var en utmaning i projektet.  Lack of communication is the single biggest reason for errors in healthcare. By standardizing the communication, the variation is reduced and logic and mutual agreement are created. The purpose of the improvement work was to increase patient safety by simplifying communication by introducing the SBAR communication tool. The subsequent study aimed to investigate how SBAR was experienced in the elderly care team, and to describe success factors and difficulties at the time of implementation.   The author was inspired by action research. A content analysis has been applied on the data collected.   There was an improvement in the information transfer, but the unit did not reach the set targets. SBAR was found useful by the professionals after practical training. The professionals felt greater security with a combination of oral and written report. The project created common learning through participatory observations and by clarifying the surrounding structures for the professionals. The project contributed to changed focus from an individual to a system level concerning the information process. The SBAR structure was slightly simplified to fit the multilingual context. New areas for improvement were identified and an activity board for the elderly was created. Keeping consistent education and training temporary staff was a challenge

Pierce Ph.D.African American women are amongst the fastest growing population of new HIV cases in the United States. Washington, D.C. represents the highest rates of HIV in the United States. There are many contributing factors as to why African American women are at such risk. Poverty, violence, illegal drug use and histories of childhood sexual abuse are some of these factors. This dissertation examines the interplay between violence, social networks, drug abuse, sexual behaviors and HIV risk among African American women in Washington, D.C. who have had lifelong histories as victims of violence and sexual abuse. Thirty-two months of ethnographic research was conducted with five selected women who have histories of drug abuse, sexual abuse and other forms of abuse and violence in their lives. Research was also conducted with harm reduction organizations, such as HIV risk reduction outreach and education organizations to assist in contextualizing the participating women's lives. Neighborhood community data collected on issues of violence and social beliefs provided further social contexts in which to frame the lives of the participating women. A multi-methodological approach using participant observation, life history interviews, social network plotting and event recall interviews was utilized to illustrate and examine the effects of psychological and physical trauma brought on by experiences of violence and abuse, and how said trauma impact individual social and sexual practices. This research also examined ways in which cyclical violence and abuse within the participating women's family and social network histories have influenced the women's decision-making abilities with regards to HIV risk behaviors. This research illustrates the interplay between the self and social networks in relation to drug addiction and violence, and demonstrates how violence affects the self and limits human agency, especially in regard to HIV risk behaviors, and identifies the need to include childhood sexual abuse as a key indicator for risk of HIV and other diseases and social ailments.Made available in DSpace on 2011-06-24T17:13:08Z (GMT).     abstract: Introduction: Individuals with osteoarthritis (OA) show increased morbidity and mortality. Telomere length, a measure of cellular aging, predicts increased morbidity and mortality. Telomeres shorten with persisting biological and psychosocial stress. Living with chronic OA pain is stressful. Previous research exploring telomere length in people with OA has produced inconsistent results. Considering pain severity may clarify the relationship between OA and telomeres. Objectives: We hypothesized that individuals with high OA chronic pain severity would have shorter telomeres than those with no or low chronic pain severity. Methods: One hundred thirty-six adults, ages 45 to 85 years old, with and without symptomatic knee OA were included in the analysis. Peripheral blood leukocyte telomere length was measured, and demographic, clinical, and functional data were collected. Participants were categorized into 5 pain severity groups based on an additive index of frequency, intensity, time or duration, and total number of pain sites (FITT). Covariates included age, sex, race or ethnicity, study site, and knee pain status. Results: The no or low chronic pain severity group had significantly longer telomeres compared with the high pain severity group, P50.025. A significant chronic pain severity dose response emerged for telomere length, P50.034. The FITT chronic pain severity index was highly correlated with the clinical and functional OA pain measures. However, individual clinical and functional measures were not associated with telomere length. Conclusion: Results demonstrate accelerated cellular aging with high knee OA chronic pain severity and provide evidence for the potential utility of the FITT chronic pain severity index in capturing the biological burden of chronic pain.The final version of this article, as published in PAIN Reports, can be viewed online at: https://insights.ovid.com/crossref?an=01938936-201706000-0000

    Vikingaskeppet Ormen Friskes undergång : Ett drama i det kalla krigets skugga

    This dissertation consists of three separately published parts, all dealing with problems involving reconstructions of Viking Age ships, travelling experiments with such ships and people’s relations and attitudes to them. The Ormen Friske, a Swedish replica of the Norwegian Gokstad ship, sank in the North Sea in June 1950, drowning all 15 on board. The ship was caught in ugly weather and steered for Helgoland, but at the same time U.S. aircraft carried out a high-altitude practice bombing run, targeting the island, unintentionally preventing the ship from reaching sheltered waters. Swedish officials suppressed the role of the bombing in the accident and rejected proposals for an inspection of the wreck. The disaster was instead blamed on the ship’s alleged deficiencies and the crew’s inexperience. The reason for the cover-up was to avoid annoying the Western allies. Most of the ship’s wreckage was then cleared away. But a few parts and artifacts were safeguarded, some of which still survive in museums, archives and private homes. They are discussed in terms of the varying meaning attributed to them by different authorities and people involved. Also, the remaining artifacts have their sentimental value thanks to their accompanying narratives. Together with graves and memorial sites these objects continue to mark people’s memories and conception of the disaster. The archaeologist studying the contemporary past must move beyond Archaeology’s traditional sources. Documents and files in government and private archives, letters, photos, newspaper articles, interviews, ethno-archaeological experiments and participating observation may be used. He shall feel free to intersect borders to other disciplines, fitting and interpreting evidence into a narrative context. Archaeology of the contemporary past deals with issues inseparable from the present and is therefore unevitably controversial. It is argued that the scholar should apply a reflexive approach, also scrutinizing his own background, social position and research incentives. During the 1990’s the author co-organized a number of experimental river journeys in Russia, Ukraine, Belarus and Latvia, analyzing the results in an archaeological, etnographical and historical context. The ship formed the required experimental platform but the essential feature was the voyage itself. Rowing vessels of the Scandinavian type upstream, through rapids and long portages, proved to be a strong, often impossible, challenge even for a very well trained and singe-minded crew. It is improbable that travellers ever brought ships all the way from the Baltic Sea to the Black Sea. It is suggested that Viking Age travellers instead traversed the part of the route ”From the Varangians to the Greeks” between the Novgorod area and the upper Dnepr area by foot or horse and sledge during winter. Russian archaeological material supports this explanatory model. The popular image of Viking ships crossing Russia on rivers seems, at least in part, to derive from folklore and legends in an international tradition.</p
