Repair of Cerebrospinal Fluid Rhinorrhea in a Patient Presenting Empty Sella Syndrome: Case Report and Review of Literature
Introduction: The cause of Empty Sella syndrome is unknown, however it occurs when intracranial contents herniate through the sellar diaphragm, filling the sella turcica with cerebrospinal fluid (CSF) and giving the appearance of an absent pituitary gland on radiography.
Case Presentation: Author presents a rare case of a 67 years-old woman who was admitted into our department with a complaint of dripping clear fluid from the left side of her nose. Clinical examination and biological, radiological investigations found a CSF leakage through a minute bony defect in the floor of the sella associated with empty Stella syndrome. Treatment consisted of the reparation of the leak using a fascia lata graft, and the sphenoidal sinus was obliterated with fat.
Conclusion: When evaluating patients with non-traumatic CSF rhinorrhea, the otolaryngologist must consider empty sella syndrome as a diagnostic possibility
PENGARUH KONSENTRASI KITOSAN SEBAGAI PENGAWET ALAMI TAHU DENGAN METODE EDIBLE COATING

Normal Variations of the Sella Turcica on Thin-Section Tomography : Concerning the Sellar Floor and the Sphenoid Sinus
Normal variations of the sella turcica on thin-section tomography were studied in 40 normal clinical cases who had no abnormal lesions in the sella turcica and the sphenoid sinus, and in 50 normal dry skulls. Tomography of the sella turcica was performed with hypocycloidal motion at 2 mm intervals in anteroposterior and lateral projections. The author investigated the shape of the sellar floor and the sphenoid sinus and its septa. In the relationship between the sellar floor and the pneumatization of the sphenoid sinus, the sellar height of the major and extensive sellar type was smaller than the height of the conchal and presellar type of sphenoidal pneumatization. In the relationship between the sellar floor and the sphenoid septum, 34 of 68 cuts which had lateral septum or double septa had an asymmetric sellar floor. Twenty-three of 48 cases on frontal tomograms and 24 of 44 cases on lateral tomograms had a downward V-shaped projection of the sellar floor at the site of the attachment of the septum. The sellar floor was concave in 57.0%, flat in 41.1% and convex in 1.9% on the frontal tomograms. Fourteen of 90 cases had different sellar floor shapes on every tomographic cut of the frontal tomograms and eight of 14 cases had asymmetric sellar floors. Fifty-eight cases had concave sellar floors, of which the maximum measurement was 2.4 mm. Twenty-five cases had lateral depression of the sellar floor, the maximum measurement of which was below 2.5mm in ordinary cases and the maximum degree of the angle of the sellar floor was below 10 degrees in those cases who had the same degree of pneumatization in the sphenoid sinus. The thickness of the lamina dura on the anterior and posterior sellar floor was thinner than that on the inferior sellar floor. Localized expansion of the sellar floor was seen in 29 cases and localized thinning was seen in 22 cases. On the other hand, localized expansion associated with thinning of the sellar floor was seen in only four cases and this finding never exceeded one tomographic cut. Therefore, the presence of pituitary microadenoma might be suspected when localized expansion associated with thinning of the sellar floor is seen more than two tomographic cuts.departmental bulletin pape
Identifying and analyzing extremely productive authors in intensive care medicine: A scientometric analysis
Introduction: Clinical progress relies heavily on research, however, recent years have seen distortions in this process due to the “publish or perish” model. This model is further amplified by team science, leading to inflated author counts and metrics. Recently the rise of hyperprolific (HA) and almost hyperprolific (AHA) authors has been highlighted in the global literature scenario, but data on intensive care medicine (ICM) is lacking. This study aims to investigate HA and AHA authors in ICM and the impact of COVID-19 pandemic on publication rates. Material and methods: We identified authors publishing in ICM journals indexed by Scopus from 2019 to 2023, retrieving their Scopus IDs, publication details, and gender. HA were defined as authors who published at least 73 articles per year, while AHA as authors who published more than 60. The effect of COVID-19 literature was assessed by excluding COVID-related articles from the dataset. Results: We identified 42860 articles in ICM journals, involving 186150 unique authors with a median of 5 publications per author. Only 248 (0.1 %) were extremely productive, with 131 being hyperprolific (HA). Removing COVID-19 papers significantly reduced HA and AHA counts by up to 40 %. Extremely productive authors were predominantly male (91.5 %) and globally distributed, primarily from Europe, Asia, and the Americas. Conclusions: Hyperprolific authors in ICM represent a very small minority. These authors are typically related to ICM, male, senior researchers with a global distribution, who publish high-quality research through a significant research network

A STUDY OF PSYCHOLOGICAL CONFLICT AS A LESBIAN IN ANNIE ON MY MIND NOVEL BY NANCY GARDEN
In life, there are so many problems. Problems also happen in novel. We find various kinds of problems in novel which reflect the main characters’ daily life. The problems are sometimes in the form of psychological conflict. this kind of conflict is harder to solve especially for minority people such as lesbian. It is interesting to study about psychological conflict in novel, especially in Nancy Garden novel entitled Annie On My Mind. The novel is about Liza Winthrop as the main character faces her problem as a lesbian. This novel also was banned and burnt because Nancy garden was the first author for young adults to create a lesbian love story. Therefore, the researcher has been interested in studying this problem by formulating three research problems such as: (1) How is psychological background of Liza Winthrop as the main character in Annie On My Mind novel by Nancy Garden? (2) What are psychological conflicts that Liza Winthrop got when she realized that she fall in love with Annie Kenyon in Annie On My Mind novel by Nancy Garden? (3) How Liza Winthrop character in the Annie On My Mind novel by Nancy Garden overcomes her psychological conflict ? The research design which used in this research is descriptive qualitative research since the researcher intends to describe psychological backgorund, psychological conflict, and the way overcome the conflict. The data in this research analyzed into six steps, i.e data collection, data analysis, data reduction, data display, conclusion drawing, and data reportation. The researcher found there are five of Liza’s Winthrop character or psychological background that is formed based on her life environment. Those are introvert, masculin, responsible, idealis, and kind-heart person. Those Liza Winthrop’s psychological background influence Liza in overcome her psychological conflict which there are two of the most of psychological conflict faced by her, they are approach-approach conflict and double approach-avoidance conflict. Finally, the researcher really hopes for the student to learn fiction deeply and enjoyed analyzing literary works. And for the lecturer in English Literature Department, lecturer can give explanation about analyzing another aspect in the literary works in order students will not bored in learning or studying fiction. Last, but not least, for the further researcher, they could take different way in analyzing the literary works
