Atypical presentation of pulmonary embolism: A case report
In spite of high incidence of pulmonary embolism (PE), its diagnosis can be challenging due to vagaries of symptoms. Conversely, right shoulder pain is a relatively easy clinical presentation which is mostly musculoskeletal in nature. But right shoulder pain only in a case of PE, has proven to be a difficult correlation to make. We here describe a patient with no prior history of DVT or PE, no history of prolonged bed rest or recent surgery but admitted to emergency department with right shoulder pain for two days. CTPA revealed pulmonary thromboembolism. Her symptoms improved with subcutaneous Clexane followed by Rivaroxaban oral therapy. 
CRISPR/Cas9-based manipulation of oncogenic chromosomal changes in vivo and drug impact on blood pressure
Human malignancies develop in large part as a result of chromosomal alterations. These modifications might result in the production of gene fusions, which are aberrant combination of two distinct genes. Gene fusions are important cancer-causing factors because they can generate aberrant proteins that encourage unchecked cell proliferation and result in tumor development. Chromosome 2 inversions result in a specific gene fusion known as EML4-ALK, which is found in certain non-small cell lung cancer patients. The EML4-ALK gene fusion has clinical significance because it renders cancer cells susceptible to an exclusive class of medications known as ALK inhibitors. Targeted treatments called ALK inhibitors can selectively reduce the action of the aberrant ALK protein generated by the fusion gene. These drugs, which work by blocking ALK, can delay or stop the growth of cancer cells that have undergone the EML4-ALK fusion, providing a viable therapeutic option for individuals with this particular genetic change. The clinical significance of this gene fusion lies in its ability to render the cancer cells receptive to ALK inhibitors, enhancing their sensitivity to treatment
Consultation liaison psychiatry (CLP) approach to esophagus stenosis patients with depression
Background. Esophageal stenosis/stricture is a delayed condition, with significant morbidity and mortality depending on the cause. With the main symptoms of dysphagia and or odynophagia, this condition causes impairment in individual activities because it is very disturbing, especially if it is felt in the long term. Conditions like this can lead to a sense of hopelessness if not handled optimally. Method. This journal is a case report. Case Report. A 59-year-old woman, married and has two sons, works as a building painter, diagnosed with depression with stupor feature. The patient did not speak at all and did not move his body or limbs spontaneously during the examination, had time to maintain the position of the hand raised by the examiner. Discussion. Esophageal stenosis/stricture causes impairment in individual activities because it is felt to be very disturbing, especially if it is felt in the long term and can cause a sense of hopelessness if not handled optimally. People who have depression and other medical illnesses tend to have more severe symptoms than suffer from either one. 
Effect of extracorporeal shock waves on neuromuscular junctions versus carpal tunnel syndrome: A narrative review
Objective:to determine effectiveness of using extracorporeal shockwave therapy (ESWT) directly on the NMJ versus using it on injuries that exert pressure on the nerve such as (CTS). Background: The nervous system can be affected by many conditions and disorders that can disrupt its normal functioning and transmission of impulses, such as the destruction of neuromuscular junctions or pressure on nerves, such as carpal tunnel syndrome. Methods: The review was carried out by searching scientifically recognized medical databases, including PubMed, Scopus, Biomed, National Library of Medicine, Science direct library, Sage Journals and Spinger Link. Date restrictions were not applied. Conclusion: Preliminary research on extracorporeal shockwave therapy for carpal tunnel syndrome has shown encouraging results, leading to recommendations for its potential use as a treatment modality. However, the current literature examining shockwave therapy for neuromuscular junction dysfunction is limited. Further studies through high-quality human trials are still needed
Endovascular arteriovenous fistula creation
Functioning vascular access is an essential element for life-saving hemodialysis therapy. A surgically-created arteriovenous fistula has been considered the best option for many years. Recently, two manufacturers developed systems for percutaneous/endovascular creation of an arteriovenous fistula. Shahverdyan et al. looked back at the records of 100 people who had 4-Fr WavelinQ and Ellipsys endoAVF implants. During the index operation, the WavelinQ cohort had brachial vein ablation, and the Ellipsys group had balloon angioplasty of the junction. There were no big changes between the two groups at the start (6). This study showed that compared to WavelinQ (19), Ellipsys has higher total patency rates but the same maturity rates and dialysis rates
Antibacterial activity of Rhizopus specie isolated from rhizosphere of Mentha Piperita
Soil is loaded by microbes which are capable of producing antibiotics. These compounds were used therapeutically and sometimes prophylactically in the control of infectious disease. Soil microbes can be grouped into Bacteria, Actinomycetes, Fungi, Algae, Protozoa and Nematodes. Fungi are unique eukaryotes which ingest food from outside and take nutrients through its cell walls and consists of tiny tubular mycelia known as hyphae. The fungi are well known in medical field due to the production of a wide variety of secondary metabolites. The current research study was focused on isolation, identification and antibacterial activity of fungi isolated from rhizosphere of Mentha Piperita (Mint), Abasyn University, Peshawar. The research study was conducted in the Microbiology Research Laboratory (MRL) of Abasyn University Peshawar. The soil sample was collected from rhizosphere by using sterile techniques. Serial dilution method was used to dilute the concentration of microbes present in soil sample. The Potato Dextrose Agar media was prepared and by pour plate method the sample was streaked on the plate. The plates were incubated at 280C for 72hours. The fungus was isolated by sub culturing technique and then identified by morphological and microscopic examination using Lactophenol cotton blue stain (LPCB)
COVID-19 vaccinations and menstrual cycle changes in Egyptian females
Background: To date, a number of clinical studies have investigated the impact of different COVID‐19 vaccines on menses, while the results have been varied and inconsistent. Bearing in mind the strong relationship between regular normal menses and the general health of females, it is judicious to accomplish menstrual health research in the context of the COVID‐19 pandemic and vaccination. Objective: To determine if there are any possible menstrual irregularities following the first and second doses (at least) of the COVID-19 vaccine and correlate it to the type of vaccine administered, and evaluate how long these effects will last. Subjects and methods: This study was a prospective observational study that included 347 questionnaires fulfilled by females who attended to receive COVID-19 vaccine at Menoufia University and Behera health directorate during the period of study from January 2022 till February 2023. Data were collected in two parts from cases who received any past doses; first part is past history about the previous doses, then prospective follow up for 3 months after the current second or booster dose
Simulation’s usage in training for dental trauma: An analysis of four splinting methods
Background and Aim: Majority of the dentists gain hands on experience in managing dental traumas after entering into the field practically leading to an overall limited exposure to dental trauma injuries before their graduation time. The present study aimed to assess the dental student’s perception regarding simulation based dental trauma training. Materials and Methods: This comparative cross-sectional study involved 80 dentistry students at a Tertiary Care teaching institute of Lahore, Pakistan. A questionnaire based survey was done on final year dentistry students to answer the questions regarding their experience of simulated trauma splinting. Prior to this simulation-based practice, all participants were given a 60-minute lecture on oral trauma damage, followed by post-training questions. All the students completed simulated treatment using orthodontic wire, twist flex wire, nylon fishing line, and powermesh as splints. The glass ionomer cement and composite resin were bonding materials used. A 5-point Likert scale was used for post-training questions and answers. Results: Out of the 80 dental students, there were 46 (57.5%) male and 34 (42.5%) female students. The incidence of students who agreed and strongly agreed to their learning assisted by simulated trauma practice were 46.3% (n=37) and 33.8% (n=27) respectively. 
Evaluation of hospital management information systems using the human organization and technology fit model in Goenawan Partowidigdo Pulmonary Hospital Bogor
The purpose of this study was to determine the effect of the dimensions in the HOT-Fit framework on net profit and to determine the factors that support ERM information systems. The research method is quantitative with a cross-sectional research design. The number of samples was 87 respondents, namely medical record officers, doctors, nurses, and support unit officers. Research using a questionnaire in the form of Google Forms. The results showed that there is a strong positive relationship between aspects of organizational structure and aspects of organizational environment, there is a strong positive relationship between aspects of service quality, there is a moderate positive relationship between aspects of information quality and aspects of user satisfaction, there is a strong positive relationship between aspects of system quality and aspects of real benefits, there is a moderate/quite strong positive relationship between tangible benefits and organizational structure aspects, there is a strong positive relationship between tangible benefits and organizational environmental aspects with a Sig value of 0.000 (p<0.05). It is suggested that the ERM usage guidelines, training on the use of ERM for each individual, and ease of access to ERM services can be further developed
Prevalence of lupus nephritis in diagnosed patients of systemic lupus erythematosis (SLE) presenting to nephrology division, Khyber teaching hospital Peshawar
Objective: To determine the prevalence of lupus nephritis in patients diagnosed with systemic lupus erythematosis (SLE). Settings: This observational study was conducted at the department of nephrology, Khyber Teaching Hospital, Peshawar Pakistan. Participants were enrolled from 1st January 2022 to 31st March 2022. Materials and methods: Patients diagnosed with SLE were enrolled. SLE was confirmed based American college of rheumatology criteria for SLE (positive ANA ≥1:80 and clinical score 10 or more). Lupus nephritis was confirmed when serum creatinine was more than 1.5mg/dl and urinary protein loss more than 0.5gm/day. Results: A total of 109 patients were enrolled. Mean age of the patients was 29.80 ± 5.245 years. 68 participants (62.4%) were female. Lupus nephritis was observed in 56 patients (51.4%). Correlation of lupus nephritis showed significant association with age and gender of the patient. Conclusion: Lupus nephritis is fairly common in patients with SLE in our population. Young female patients are more likely having it
