Lateral pelvic lymphadenectomy for low rectal cancer
Background: Uncertainty exists regarding importance of lateral pelvic lymph node metastases in advanced low rectal cancer. Our research identified LPLD-related oncological prognosis in studied cases with rectal cancer below peritoneal reflection. Methods: We examined studied cases at Al Haram Hospital who received LPLD between May 2015 and December 2020 & had clinical stage II-III low rectal cancer below peritoneal reflection. Results: there had been no correlation among pelvic LN & colonoscopy & there had been no correlation among pelvic LN & pathology. Conclusion: When compared to entire mesorectal excision alone, lateral pelvic lymph node dissection had not been significantly related with decrease in recurrence rates or enhancement in survival; nevertheless, LPLND had been linked to higher complication rate. The influence of market orientation and entrepreneurship orientation in achieving hospital performance: Study in Goenawan Partowidigdo Pulmonary Hospital Bogor
This study aims to deepen knowledge about the direct effects of entrepreneurial orientation (EO) and market orientation (MO) on hospital performance using the structural equation modeling partial least squares (PLS-SEM) model. This research is a quantitative, non-experimental, and descriptive study using the PLS-SEM statistical technique. The PLS-SEM results show that the first hypothesis of market orientation does not have a direct effect on hospital performance with a t-statistic value of 1.29 <1.96 with a p-value of 0.018 <0.05. The second hypothesis, entrepreneurial orientation has a direct effect on hospital performance t-statistic 10, 85> 1.96 with a p-value <0.005 so that the second hypothesis is declared accepted. The results show sufficient evidence that EO positively affects hospital performance. Likewise, the impact of EO on hospital performance is positive either directly. Regarding managerial implications, the study provides managers with insight into the process of strategy formation and decision-making, emphasizing the importance of both encouraging the development of EO capabilities and focusing efforts on being proactive and innovative to improve hospital performance
Association between Vitamin D, P1NP, and BMD as major indicators of bone health in local population of Pakistan
It is commonly known that the Pakistani population has inadequate vitamin D levels, but its effects on bone health and the level of N-terminal peptide of procollagen (PINP) have not yet determined. The aim of the research work was to check the vitamin D grade of exercisers and non-exercisers and examine how it related to P1NP concentration, a biochemical marker of bone production, and bone mineral density. A random sampling technique was used to select males and females who were staying in neighborhood. They were between the ages of 18 and 35 years. 92 people of Pakistani ancestry made up the total population. With 46 people, Group A was a non-exercising control group. Group B has 46 individuals in it. Both of these groups were gathered for the data analysis. BMD, procollagen type-1 amino-terminal pro-peptide, calciferol, and other biological indicator of bone remodeling were measured in serum. (P1NP). Using broadband ultrasound attenuation, the quality of the bones was evaluated. Before training, the total 25(OH)D serum were considerably less in non-exercising group than they were during or after training. (Mean: 0.4992, Standard Deviation: -1.6075; P 0001). 
Histo morphological changes in gall bladder mucosa associated with gall stones
Background and Aims: Cholelithiasis is known to produce diverse histopathological changes in the gallbladder mucosa. In the present study, we aimed to find the correlation between various gallstone characteristics (i.e., number, size, and morphological type) with the type of mucosal response in gallbladder mucosa (i.e., inflammation, hyperplasia, metaplasia, and carcinoma). Methods: The present study was conducted prospectively on 250 patients undergoing cholecystectomy for symptomatic cholecystitis. Gallstones were assessed for various parameters, i.e., number, size, and morphological type. Gallbladder mucosa was subjected to histopathological examination. Sections were taken from body, fundus, and neck of gallbladder. Results: Of 250 cases, maximum type was of mixed stones (54%) and was multiple in number (46%). However, gallstone type and number are non-significant variables to produce precancerous lesions (i.e., hyperplasia and metaplasia). Statistically significant results were obtained while comparing the mucosal response with gallstone size (P = 0.012). Conclusion: As the gallstone size increases, the response in gallbladder mucosa changes from cholecystitis, hyperplasia, and metaplasia to carcinoma. Gallstone type and number are nonsignificant variables to produce precancerous lesions. 
Prevalence of paraphilic interests among adults in Egypt
Background: The Paraphilia now around the world occupy an important position in medicine. Term “paraphilia” (from the Greek “para,” meaning “beside, aside,” and “philia,” meaning “love”) is currently used in psychiatry to define “anomalous” sexual interests. Objectives: Studying prevalence of paraphilic behaviors and interests in a sample of 200 males and 200 females in the population aged from (18 – 45). Methods: Communication was done with different online groups and sites to conduct this cross sectional study. These aimed to orient them about the aim and procedures of the study and explained its benefits. Admins of these groups were helpful but refused to put the name of the groups in the work. admins collected the answered questionnaire to protect identity and privacy of respondents. This study was approved by ethical committee of the Faculty of Medicine Menoufia University. Results: The overall prevalence of paraphilic behavior experience was found 21% with prevalence significantly higher in men than women. The most common paraphilic experience was frotteurism then voyeurism followed by fetishism then pedophilia. Conclusion: It was concluded that paraphilia is not an uncommon behavior in our community. It exists in both sexes but higher among men.
 
Develop a risk stratification model and clinical analysis to predict the impact of primary percutaneous coronary intervention on complete AV block in acute inferior STEMI patients
Background: Acute inferior ST-elevation myocardial infarction (STEMI) is medical emergency entailing immediate intervention to restore blood flow and mitigate myocardial damage. Primary percutaneous coronary intervention (PCI) has replaced thrombolytic therapy as preferred treatment strategy for having superior outcomes. Objectives: To develop a risk stratification model incorporating the identified predictors to estimate the individual risk of complete AV block in patients undergoing primary PCI for acute inferior STEMI. Methods: This study enrolled 139 patients diagnosed with acute inferior STEMI who underwent primary PCI at the hospital. They were divided into two groups, i.e. pre-operative and post-operative carrying 54 and 85 patients, respectively. On admission, demographic, clinical data and peak values of cardiac biomarkers were obtained and the complications associated with PCI were elaborated. Results: Mean age of patients was 59.81+8.19 years, and comprised significantly more male (97) than female (42) patients (p<0.05). Proportion of patients with a history of complete AV block was 10.79%. Comorbidities included diabetes mellitus (33.09%), hypertension (49.64%), and dyslipidemia (64.74%).Coronary angiography findings for predicting AV block in patients with STEMI were categorized by location of culprit lesion in coronary vessels. 
Effect of PRF on extraction socket healing
Extraction socket healing is a critical process in dental care that determines the success of subsequent dental treatments, such as implant placement. Platelet-rich fibrin (PRF) has emerged as a promising biomaterial for enhancing wound healing in various medical and dental applications. This abstract aims to provide a comprehensive overview of the effect of PRF on extraction socket healing, with a particular focus on the sample size of studies conducted in this area. PRF is an autologous blood-derived product rich in growth factors, cytokines, and platelets, which play key roles in tissue repair and regeneration. When applied to extraction sockets, PRF promotes accelerated wound healing by stimulating angiogenesis, enhancing cell proliferation, and modulating the inflammatory response. These biological effects contribute to improved soft tissue healing and osseous regeneration. A thorough review of the literature reveals that several studies have investigated the effect of PRF on extraction socket healing, with varying sample sizes. Sample sizes ranged from small-scale studies with fewer than 20 participants to larger-scale investigations involving over 100 subjects. The inclusion of sufficient sample size is crucial for obtaining statistically significant results and ensuring the generalizability of findings
Work-related quality of life of Pakistani Doctors
About millions of doctors suffer from occupational health disease in Pakistan. WRQoL of doctor affected due to many factors existing in working conditions. Proportion of perceived quality of working life of doctor is important for understanding actual environment and for taking some potential intervention to refine quality of working lives of doctor in society The aim behind the review was to utilize this instrument (WRQoL) to uncover reality of doctor’s life style and deeper understanding of the impact of stress on doctors’ health as well as evaluate perceived WRQoL, using a large nationally representative sample in the Pakistan (Van Laar etal.,2007). In this study instrument (WRQoL) use to evaluate the perceived WRQoL of general practitioner. As secondary objective, it is used to offers the opportunity of identification of the strengths and weaknesses of doctors’ professional life. These studies concluded some results that stress has great influence in female doctors than male doctors due to home and work interference. Thus, female doctors perceived low quality of work life in profession. Job career satisfaction rate is high among private sector doctors than public. Evaluating the efficacy of chatGPT in near-peer simulation for resident doctors in the emergency department
Introduction: Near-peer teaching is gaining popularity as a newer teaching tool, as it improves the learner’s comprehension, targets the right audience and promotes familiarity with the clinical situation and enhances critical thinking.(1) This study was initiated to evaluate the efficacy of chatGPT in near-peer simulation using AI-enabled scenarios in the residency training programme of an emergency department. (2) Methods: ChatGPT an LLM was asked to generate clinical scenarios as per prompts given to evaluate its efficacy in generating real-time and realistic scenarios with information on stepwise approach and critical treatment decisions for the patient. Results: In our study, ChatGPT was able to successfully generate real-time and realistic scenarios based on the prompts given with detailed treatment approaches and critical decisions for all patients/scenarios given, serving as a successful tool to consider in near-peer education using simulation enabled by AI. CONCLUSION: Near-peer simulation training was found to be a valuable method of teaching residents for increasing hands-on experience, skill assessment, confidence in diagnosis and practical thinking. Integration of AI into near-peer simulations aids in creating a wider range of scenarios with prompt treatment decisions.
