    Study of clinical profiles of tubercular meningitis patients with seizures

    Background: Tuberculosis remains a serious global health problem, with an estimated 10 million new cases occurring globally during 2019. CNS involvement one of the most devastating clinical manifestations of tuberculosis (TB) is noted in 5-10% of extrapulmonary TB cases, and accounts for approximately 1% of all TB cases and it also carries significant higher mortality and neurological disability among infected individuals especially in low income countries. Materials & Methods: It’s a hospital based prospective study which will include 80 patients admitted in department of Medicine and Neurology diagnosed as tubercular meningitis with a duration of 3 years. All Subjects were subjected to anthropometric measurements and routine biochemical tests – Hemogram, Urea, Serum Creatinine, LFT, Lipid Profile. Results: In patients who developed seizures before hospital admission, most common finding in neuroimaging is hydrocephalus, inflammatory edema and ventriculomegaly in patient who developed seizure within first week of hospital admission, majority had normal CT finding (27.27%) followed by metabolic cause like hyponatremia and hepatitis(18.18).In patients who developed seizure after 1st week of hospital admission, commonest cause found is Hydrocephalus(25.69%) and tuberculoma (17.64%)  followed by calcified granuloma (15.68%)

    Breast cancer is known to be one of the most cancer affecting women around the globe and the second most common cancer in general. In third worlds countries, breast cancer is the most cause of cancer death. Early diagnosis and accurate follow-up of these patients affect the management. There are multiple prognostic factors most important one is the immunohistochemical molecular markers in the specimens including human epidermal growth factor, progesterone, and estrogen receptors (HER 2, PR, ER). In breast cancer, the HER2 positive molecular subtype is associated with bad prognosis and aggressive histological features, yet while following neoadjuvant chemotherapy, it achieves an increased pathological complete response rate. 18F-fluoro-2-deoxy-d-glucose positron emission tomography (FDG PET) has proved to be an effective and accurate imaging technique for lymph node and distant metastasis assessment, tumor staging, restaging of recurrence, treatment response, and follow-up. In breast cancer, tumor molecular subtype, tumor size, proliferation index, and histological grade correlated with 18F-fluoro-2-deoxy-d-glucose (FDG) uptake. This study evaluates the possible correlation between tumor SUV max ,tumor to liver and tumor to spleen (standardized uptake value) SUV max ratio and the four different molecular subtypes in patients with pathologically proven breast cancer.&nbsp

    Rehabilitation of the surgical defect secondary to mucormycosis: A case report

    Aim: To report a case of Mucormycosis of right maxilla due to uncontrolled Diabetes Mellitus along with its surgical management and immediate Prosthetic rehabilitation. Background: Phycomycetes is the causative agent leading to the deadly fungal infection called Mucormycosis. Predisposing factors involve patients with uncontrolled diabetes mellitus, immunosuppressive therapy, organ transplant. The survival rate of the patients depends upon how quickly the disease is diagnosed and treated. Case Description: This article presents a case report of a 54-year-old female patient reported with the complaint of pain on the right side of the face along with headache and difficulty in breathing. Patient was diabetic and not under medication. After routine investigations, patient was diagnosed for Mucormycosis. The treatment involved excision of the necrotic tissues present in the right maxilla. Patient was referred to Department of Prosthodontics before surgery. Following hemi-maxillectomy, closure of the defect was done to prevent regurgitation due to oro-antral communication using surgical obturator and inserted immediately following surgery. To reduce the weight, a hollow bulb surgical obturator was the prosthetic choice in this case report. Conclusion: Surgical obturator can be a treatment of choice for immediate closure of defect secondary to Mucormycosis

    Antioxidant and hepatoprotective activities of Pongamia pinnata (PP) Linn. leaves on anti-tubercular medicines (isoniazid & rifampin) induced hepatotoxicity in Wistar rats

    Objectives: -The goal of this study was to see if an aqueous extract of Pongamia Pinnata Linn. (APP) leaves could protect against the hepatotoxicity caused by anti-TB medications (ATM). Methods: -In order to cause hepatotoxicity in rats, anti-tubercular medications were utilised. Silymarin (100 mg/kg p.o.) be present utilised by means of the control medication. For 21 days, an aqueous extract of Pongamia Pinnata Linn. leaves (200 & 400 mg/kg p.o.) was given with anti-TB medicines given one hour before to the aqueous extract. Results: -Biomarker enzymes found in the liver are SGPT, SGOT, ALP, Total Bilirubin & total cholesterol remained raised on anti-TB medicines management. The management of aqueous extract of Pongamia Pinnata Linn.leaves200 mg/kg & 400 mg/kg with anti - TB medicines were considerably decreased biomarker enzymes found in the liver. Antioxidant factors like Super Oxide Dismutase (SOD), Glutathione(GSH), Catalase(CAT), Glutathione Peroxidase (GPx)& Glutathione Reductase(GRx) were inhibited and improved ThioBarbituric Acid Reactive Substances (TBARs)points in anti-TB medicines administration then returned these antioxidant points in the management of aqueous extract of Pongamia Pinnata Linn. Leavesatadosageof 200mg/kg& 400mg/kg. Conclusion: - According to the findings of this research, Pongamia Pinnata Linn. leaves have a protective consequence against hepatotoxicity caused through anti-TB medicines

    Incorrect beliefs about traditional medicine for malignant and chronic diseases and disabilities among members of saudi society

    The study aimed to explore incorrect beliefs about traditional medicine for malignant and chronic diseases and disabilities among members of the Saudi society and their relationship to some demographic variables. The study used the descriptive approach. The study sample consisted of (560) persons who were chosen following purposefully from members of the Saudi society in Najran region in the Kingdom of Saudi Arabia in February of the year 2022. A questionnaire was used to collect the data. The results showed that 65.5% of the study sample did not resort to traditional medicine in treating malignant and chronic diseases and disabilities, while 34.5% of the study sample turned to traditional medicine. Also, it was shown that the total degree of wrong beliefs about traditional medicine for malignant and chronic diseases and disabilities among members of the Saudi society came with a mean of (1.14) rating low level. In addition, the results revealed statistically significant differences in the incorrect beliefs about the traditional medical treatment of malignant and chronic diseases and disabilities among members of the Saudi society due to the variables of gender in favor of females

    Evaluation of lingual fracture pattern of the mandible after bilateral sagittal split osteotomy with and without inferior border osteotomy

    The Purpose of the study was to evaluate the pattern of lingual split when performing a bilateral sagittal split osteotomy (BSSO) with different osteotomy methods. A total of 15 dry human cadaveric mandibles was taken for the study. The classical Epker technique of BSSO was performed on the left side. The same was done on the right side along with an-additional inferior border osteotomy. There resultant split was assessed based on modified lingual split scale. The maximum torque force that was needed to split the mandible was recorded and the fracture pattern was observed. Similar osteotomies were performed in 15 fresh goat mandibles which acted as control group. The human cadaveric dry mandible recorded an average torque of 12.6 +2.4 Nm (SD: 0.32) with a maximum of 16.0 Nm and a minimum of 8.0 Nm on left side.80% of the mandible were Type I fracture pattern and 20% had Type III fracture pattern. In contrast with the modified BSSO technique with an additional inferior border osteotomy required a maximal torque of 12.0 N and a minimal torque of 5.0 with an average required torque of 8.7 + 2.1 N on the right side of the mandible

    Background and Objective: A new pathogen of high contagious abilities named Corona virus was seen to target pregnant women due to physiologic changes in their immune system, cardiopulmonary and coagulation system leading to poor fetus-maternal outcome. Thus, to determine maternal and fetal outcome in pregnant women affected with COVID-19 and its frequency of vertical transmission is the main objective of the study. Methods: This is a prospective study of 50 pregnant women with RTPCR confirmed COVID-19 infection between 1st March 2021 and 1st August 2021. The foetal and maternal outcome, frequency of vertical transmission, mode of delivery, presenting symptoms and Level of serum markers such as LDH and CRP were noted. Results: Among 50 patients, 68% delivered through Caesarean section 34% delivered vaginally. The most common presenting symptom was fever in 50% patients. Other symptoms were running nose, headache, myalgia, loss of smell and taste.68% patients had raised Serum LDH level and 26% patient had raised Serum CRP level. The maternal condition was taken into account based on source of oxygen saturation by monitoring Spo2 level

    The community always supervises the implementation of development programs and always focuses on a hope, namely the creation of economic justice and social justice. The welfare development strategy is certainly very useful, but the community as the beneficiaries of the assistance also responds in the form of support and criticism. The people who became the research informants were the recipients of the Prosperous Card assistance. Primary data obtained by observation, in-depth interviews (in-depth interviews). Secondary data comes from books and research journals published online and in print. Data analysis by transcribing the data, reading the entire data, analyzing it in detail and describing it holistically-integratively. The results of the study indicate that the Prosperous Family Card can be interpreted as a program aimed at meeting all forms of life needs, getting a positive response from the community, especially basic ones such as food, clothing, education and health care, taking into account the components in community welfare, including improvement progressively from the conditions of human life, human resource development, human-oriented, social change and adjustment
