Novel hybrid approach based on combination of textural features and clinical parameters for reliable prediction of thyroid malignancy
The dramatic increase in thyroid cancer, particularly among the younger population demands development of an automated decision support system for timely and reliable prognosis of the disease so as to facilitate improved chances of recovery in the subjects. While numerous methods are already reported by the researchers for the detection of Thyroid malignancy, the most crucial parameter of Thyroid Malignancy Index (TMI) has received very less attention. TMI is of paramount importance in diagnosis and treatment of the patients having malignant thyroid and its consideration is therefore inevitable. This research aims to develop an automated and a reliable decision support system for detection of thyroid malignancy. The proposed hybrid approach incorporates a novel combination of texture features and clinically observable parameters to initially identify a malignant thyroid tumor using support vector machines and further predicts its TMI value, thus exhibiting a performance like a trained radiologist. Publically available database comprising of 99 cases and 134 ultrasound images are used to validate the superiority of the proposed approach. Apt consideration and reliable prediction of the TMI in this research makes the designed approach novel and marks a mega leap towards its practical deployment in the clinical environment
Resistive index (RI) as a prognostic indicator of early morbidities in term neonates with hypoxic ischemic encephalopathy
Background: Hypoxemia and hypoxia sustained by neonates during birth process leads to Hypoxic-ischemic encephalopathy (HIE) which can give rise to immediate and/or long term sequelae of neonatal brain injury, increased neonatal morbidity and mortality worldwide. Material and methods: A prospective observational study done in tertiary neonatal intensive care unit of north Karnataka region. The study was conducted between September 2021 to May 2022. Results: A total of 44 neonates with HIE were studied. There were 26 (61%) male and 18 (39%) female babies and the male to female ratio was 1.4:1. There were 28 cases with grade I, 6 cases with grade II and 10 cases with grade III HIE. The Resistive index was normal in 30 (68.1%) cases and was abnormal in 14 (31.8%) cases. Common Doppler ultrasound findings were of increased echo density of cerebral parenchyma and increased periventricular density. There were 95% survivors and 5% deaths due to HIE. The most common complications were of acute kidney injury (34%) and seizures (11.4%).
Hyposmia impending neurodegeneration in rapid eye movement sleep behavior disorder
Background- Approximately 50% to 80% of people with isolated rapid eye movement (REM) sleep behavior disorder (RBD) will likely be diagnosed with an overt synucleinopathy within their lifetimes. Objective- To determine if hyposmia in isolated REM sleep behavior disorder (IRBD) predicts short-term conversion. Methods- Olfaction was tested using the University of Pennsylvania Smell Identification Test (UPSIT-40) in 100 consecutive patients with polysomnography-confirmed IRBD and in 100 matched controls. Patients were followed up for two years. The study was approved by the institutional ethics committee, and participants provided written informed consent. Results- The study was a male preponderance comprising 76% in cases and 70% in controls, and they were comparable to females and were not significant.(p>0.05). UPSIT-40 score was lower (poorer smell identification) in patients than in controls (20.3 ± 6.4 vs. 28.4 ± 4.9), and it was statistically significant (p<0.05). 60% of patients had hyposmia and showed 14.2 ± 2.8 scores. No differences occurred in age (p = 0.11), gender (p = 0.38) and follow-up duration (p = 0.65) between hyposmics and normosmics.
Murottal Sura Al-Fatiha during pregnancy increased the number of neuron cells in the cerebrum and cerebellum of the newborn Rattus Norvegicus
Al-Qur'an verses that are read with correct recitation and tartil have frequencies and wavelengths that can affect the brain with the aim of analyzing differences in the number of neurons in the cerebrum and cerebellum of Rattus norvegicus newborns between exposure to Surah Al-Fatiha and the control group. The experimental study used a pure experimental design (Posttest Only Control Group Design), the treatment was carried out in a soundproof box for 1 hour at night on the 9th day of pregnancy with an intensity of 65 dB. The cerebrum and cerebellum were prepared, stained by HE, calculated by 400x magnification, and then analyzed using appropriate statistics. Number of neurons average in the cerebrum was 167.75 and for the control group was 126.62. In cerebellum was 80.50 and 63.00 in the control group. There were a statistically higher number of neurons in the Al Fatiha group, with p=0.046 in the cerebrum and p=0.030 in the cerebellum. Conclusion: Sura Al Fatiha, during pregnancy, gave a higher number of newborn brain neurons compared with the control group
Assessment of malondialdehyde level in broiler chickens suffering from chronic heat stress and fed on a diet supplemented with bioactive substances
Heat stress is one of the most important health dilemmas that faces poultry industry characterized by high degrees of economic losses. The presented work, here, was carried out to evaluate the effects of grape pomace (GP) on decreasing the levels of oxidative stress induced by heat stress in broilers. For that purpose, 150 chicks were randomly assigned into three groups (50 chicks/group). One group received no treatment, the control group (G1). The second group (G2) was provided with 25 gm (2.5%) of GP/kg of ration. The third group (G3) was given 50 gm (5%) of GP/kg of ration. The results indicated that the levels of MDA in fresh meat samples (G3) were significantly (p˂0.05) the lowest than those from other meat samples of the G2 and G1. Frozen meat samples (G2 and G3) had significantly (p˂0.05) lesser levels of MDA than those from G1. In the case of MDA from the plasmas of chickens, their levels were significantly (p˂0.05) the lowest in the G3 followed by G2 than those from other plasma samples of G1
Effects of moderate intensity treadmill exercise with gradual increase in speed and inclination on serum C-reactive protein in male with type 2 diabetes mellitus
The aim of this study is to look at the effect of moderate-intensity treadmill exercise for 30 minutes with an increase in speed and inclination of male with type 2 diabetes patients in reducing the chronic low-grade inflammation. A randomized two-group study was carried out between July 2019 and September 2019. Ten male patients with type 2 diabetes (30-55 years old) without complications, with blood sugar controlled from Endocrine Polyclinic Internal Medicine RSUD dr. Soetomo, was recruited and allocated randomly into two groups. In the treatment group, each patient received moderate intensity treadmill training with increased speed and inclination. The exercise is carried out for 30 minutes for 4 weeks, 2-3 times a week. In the control group, each patient received standard treatment along with physical activity education for 150 minutes a week. Measurement of serum CRP levels was done on the first and last day. In the treatment group, serum CRP levels were measured 30 minutes after aerobic exercise. This study showed that aerobic exercise for 4 weeks did not reduce serum CRP levels in type 2 DM patients.
Relation between asthma clinical presentation and diet patterns among adolescents living in Saudi Arabia: Evidence from national school-based study
Background: Dietary intervention based on evidence-based guidelines should be introduced into the clinical management of Asthmatic patients to promote overall health benefits and disease control. However, there is limited evidence in the Saudi medical literature. Objective of the study: To assess the association between diet patterns and asthma symptoms in Saudi adolescents. Subjects and Methods: In total, 4086 Saudi adolescents were recruited for the study. Data for this study were sourced from a national survey conducted by the Saudi Ministry of health. Results: 13.3% (n=543) of adolescents reported current wheeze in the past 12 months. The results showed that almost half of all adolescents reported unhealthy diet consumption on most or all days and a healthy diet was consumed at least twice per week in a third of them. The most important clinically relevant finding was a positive association between high weekly consumption of an unhealthy diet with asthma symptoms. Conclusion: Our findings suggest that the positive impact of diet on asthma control is encouraging. A healthy diet including fruits, vegetables, and fish, is implicated in the mitigation of asthma symptoms and severity
A comparative study of effectiveness of microneedling with and without topical corticosteroids in post-burn hypertrophic scars of face and neck
Background: The standard treatment of post-burns scars has unsatisfactory outcomes and required several treatments. Objective: To evaluate efficacy of the microneedling with or without topical steroids on treatment of post-burn hypertrophic scars in face and neck. Patients and Methods: We included patients with post-burn hypertrophic scar of face and neck, caused by burn within the 1st year after burn, we excluded patients with coagulation defects. Patients were divided into 3 groups; Group A: microneedling once/month for 5 months, Group B: microneedling with topical steroids once/month for 5 months, and Group C: control group for just conservative treatment. Histopathological study was used for evaluation. Results: we included 60 participants; the mean age was 20 ± 9 years. After 3 & 6 months microneedling significantly decrease the Vancouver scar scale (VSS), and adding steroids significantly improve the results. Microneedling group significantly decreased the VSS after 3 and 6 months. Moreover, adding steroids significantly improved the results. Histopathologicaly, after 6 months, there was statistically difference between the three groups in thickness (p= <0.001), Nodules (p= 0.02) and inflammation (p= 0.02) of the scar. Conclusion: Microneedling with or without topical steroids found to improve the outcomes of post-burn hypertrophic scars
