Glucose metabolism outcomes after pituitary surgery in patients with acromegaly
[Abstract] Aim. To investigate the impact of pituitary surgery on glucose metabolism and to identify predictors of remission of diabetes after pituitary surgery in patients with acromegaly.
Methods. A national multicenter retrospective study of patients with acromegaly undergoing transsphenoidal surgery for the first time at 33 tertiary Spanish hospitals (ACRO-SPAIN study) was performed. Surgical remission of acromegaly was evaluated according to the 2000 and 2010 criteria.
Results. A total of 604 acromegaly patients were included in the study with a total median follow up of 91 months (interquartile range [IQR] 45–163). At the acromegaly diagnosis, 23.8% of the patients had diabetes mellitus (DM) with a median glycated hemoglobin (HbA1c) of 6.9% (IQR 6.4–7.9) [51.9 mmol/mol (IQR 46.4–62.8)]. In the multivariate analysis, older age (odds ratio [OR] 1.02, 95% CI 1.00-1.05), dyslipidemia (OR 5.25, 95% CI 2.81 to 9.79), arthropathy (OR 1.39, 95% CI 2.82 to 9.79), and higher IGF-I levels (OR 1.30, 95% CI 1.05 to 1.60) were associated with a greater prevalence of DM. At the last follow-up visit after surgery, 21.1% of the DM patients (56.7% of them with surgical remission of acromegaly) experienced diabetes remission. The cure rate of DM was more common in older patients (hazard ratio [HR] 1.77, 95% CI 1.31 to 2.43), when surgical cure was achieved (HR 2.10, 95% CI 1.01 to 4.37) and when anterior pituitary function was not affected after surgery (HR 3.38, 95% CI 1.17 to 9.75).
Conclusion. Glucose metabolism improved in patients with acromegaly after surgery and 21% of the diabetic patients experienced diabetes remission; being more frequent in patients of older age, and those who experienced surgical cure and those with preserved anterior pituitary function after surgery
Sensory Perception and Willingness to Pay for a Local Ancient Pear Variety: Evidence from In-Store Experiments in Italy
[Abstract] Product optimisation is one of the most crucial phases in the new product development or launch process. This work proposes applying penalty analysis to investigate the impact of not just-about-right (JAR) sensorial aspects on willingness to pay (WTP) and an overall liking for a local Italian ancient pear variety and to verify the mediating role of pleasantness in the relationship between not-JAR sensory attributes and consumers’ WTP using structural equation model (SEM). One hundred and twelve non-expert participants recruited during an in-store experiment evaluated overall liking and JAR attributes and were involved in an in-field experimental auction based on the non-hypothetical Becker–DeGroot–Marshak (BDM) mechanism. The participants’ average WTP for the sample was EUR 3.18 per kilogramme. Only juiciness and sourness significantly impact consumers’ overall liking but not on consumers’ WTP. Moreover, pleasantness did not mediate the relationship between non-balanced sensorial aspects and WTP. In conclusion, the penalty analysis for attributes not being JAR in monetary and hedonic terms is a beneficial research approach for a deep-inside evaluation of the potentiality of the product in the marketplace, providing helpful directions for product optimisation. Results show market potential for the local ancient pear variety ‘Angelica’.This research was funded by PSR 2014–2020—Misura 16.1.01—Focus Area 2A: Caratterizzazione e valorizzazione della biodiversità frutticola locale e verifica di genotipi innovativi di melo per l’agricoltura di montagna—VALORFRUIT, grant number 514956

An Integrated Planning Methodology for a Just Climatic Transition in Rural Settlements
[Abstract]The article presents the findings of a research project that focuses on the role of rural areas
as key players in addressing the current climate emergency. The article addresses the challenge
of a just energy transition by examining the obstacles to the implementation of renewable energy
infrastructure. The investigation is situated within the context of Galicia, a rural region in the northwest
of Spain. The study conducted an extensive review of the literature, surveys, and interviews,
which revealed a significant gap between local communities and planning decisions to be one of the
primary obstacles to a just transition. In light of these findings, the research puts forth an integrated
planning methodology founded on social and metabolic principles. This methodology investigates
the communal management of energy resources with the objective of improving local welfare and
integrating this into the planning process. This methodology proposes a series of steps and associated
tools for the analysis of the potential for local energy generation using biomass, hydropower, solar,
and wind infrastructures. Landscape and social considerations are articulated through continuous
community engagement. The energy generation capacity will be used as a catalyst to address the
most pressing issues and to improve living conditions in rural areas. The article confirms the need
for a holistic approach to energy infrastructures, paying particular attention to landscape integration
and endogenous developmentThis research was funded by MCIN/AEI/10.13039/501100011033/ and by ERDF “A way of making Europe”, grant PID2021-123961OA-I00
Its operation is based on technology that, through electromagnetic waves, allows us to detect and locate objects at a distance.
In this work, we will begin by exploring the origins of radar, starting from the early steps in the study of waves, which would later lead to the Daventry experiment in 1935, where for the first time an airplane was detected using radio waves. We will continue with the significant and rapid advances that radar experienced during World War II, driven by the obvious strategic advantages that this equipment provided on the battlefield. After the war, we will focus on the technological advancements that accompanied this equipment, with the most relevant being the ARPA system "Automatic Radar Plotting Aid."
Next, we will discuss the regulations designed by the IMO “International Maritime Organization,” which govern the technical characteristics that radar and ARPA must have depending on the tonnage of the ship and its intended use. We will also detail the knowledge that operators must possess according to the STCW “Standards of Training, Certification, and Watchkeeping” regulations. This international standard establishes the minimum requirements for the training, certification, and working conditions of seafarers, aiming to ensure safety in navigation and protect human life at sea.
Once the above is understood, we will describe its operation, based on the emission of electromagnetic pulses. To do this, we will explain what these pulses are and how they move through space. This will allow us to optimize the use of the equipment and understand its limitations, while also understanding the advantages and disadvantages of an X-band radar versus an S-band radar. Subsequently, we will discuss the integration of radar with the ship’s sensors, along with the most relevant functionalities of the equipment. In this way, we will be able to interpret all the information provided by the radar with good judgment, which is essential in critical situations, where reaction time and familiarity with the equipment play a very significant roleTraballo fin de grao (UDC.ETSNEM). Agent-based model to assess public acceptance of an energy sustainability project in the island of El Hierro
Presented at the 28th International Conference on Knowledge Based and Intelligent information and Engineering Systems, KES 2024, Seville11-13 September, 2024.[Abstract]: This paper presents an agent-based model to address citizen acceptance of energy sustainability projects, using the 100% Renewable El Hierro Project as a case study. The island of El Hierro is a pioneer in the search for sustainable energy sources, and this project seeks to understand how the individual psychosocial needs of El Hierro’s citizens influence their perception and adoption of energy policies.
The HUMAT architecture, inspired by psychological and sociological theories, is adapted to model the behaviour of the citizens of El Hierro in relation to the energy sustainability project. Each agent in the model represents an individual citizen and their behaviour is determined by their psychosocial needs, which include factors such as the island’s energy independence, economic sustainability or environmental quality.
Simulations are used to assess the impact of different communication strategies by project stakeholders, such as the press or local government, on the evolution of public acceptance of a project extension.
This study highlights the importance of integrating multidisciplinary approaches combining psychology, sociology and engineering to address energy sustainability challenges in specific contexts such as El Hierro. It also offers new perspectives on how to design and implement energy policy communication campaigns in a way that is more acceptable to the public. The work on this document has been supported by Horizon 2020 of the European Commission SMARTEES project (grant agreement no. 7639). This inquiry aims to assess the current state of research on the application of VR in inclusive urban design for people with intellectual disabilities (ID). The literature was synthesized, and three primary research questions were addressed using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. A total of 18 original research articles were identified from reputable sources, including Cyberpsychology & Behavior, Urban Studies and Virtual Reality. The inquiry found that VR interventions hold promise for promoting independent living skills, enhancing cognitive performance, and improving social skills. Additionally, it highlighted the importance of urban encounters for the well-being and autonomy of individuals with ID. However, this investigation also identified limitations, such as potentially excluding relevant studies published in non-English languages or non-peer-reviewed sources. In conclusion, this inquiry presents a comprehensive synthesis of the current literature on the use of VR in inclusive urban design for individuals with ID. The analysis presents findings that contribute to the understanding of the potential benefits and challenges associated with VR technology in this field
español canario; el II, que incluye una comparativa de Panza de burro y Leche condensada;
Curso 2023/202
Epidemiology, resistance genomics and susceptibility of acinetobacter species: results from the 2020 spanish nationwide surveillance study
[Abstract] BackgroundAs increasing antibiotic resistance in Acinetobacter baumannii poses a global healthcare challenge, understanding its evolution is crucial for effective control strategies.AimWe aimed to evaluate the epidemiology, antimicrobial susceptibility and main resistance mechanisms of Acinetobacter spp. in Spain in 2020, and to explore temporal trends of A. baumannii.MethodsWe collected 199 single-patient Acinetobacter spp. clinical isolates in 2020 from 18 Spanish tertiary hospitals. Minimum inhibitory concentrations (MICs) for nine antimicrobials were determined. Short-read sequencing was performed for all isolates, and targeted long-read sequencing for A. baumannii. Resistance mechanisms, phylogenetics and clonality were assessed. Findings on resistance rates and infection types were compared with data from 2000 and 2010.ResultsCefiderocol and colistin exhibited the highest activity against A. baumannii, although colistin susceptibility has significantly declined over 2 decades. A. non-baumannii strains were highly susceptible to most tested antibiotics. Of the A. baumannii isolates, 47.5% (56/118) were multidrug-resistant (MDR). Phylogeny and clonal relationship analysis of A. baumannii revealed five prevalent international clones, notably IC2 (ST2, n = 52; ST745, n = 4) and IC1 (ST1, n = 14), and some episodes of clonal dissemination. Genes bla OXA-23, bla OXA-58 and bla OXA-24/40 were identified in 49 (41.5%), eight (6.8%) and one (0.8%) A. baumannii isolates, respectively. ISAba1 was found upstream of the gene (a bla OXA-51-like) in 10 isolates.ConclusionsThe emergence of OXA-23-producing ST1 and ST2, the predominant MDR lineages, shows a pivotal shift in carbapenem-resistant A. baumannii (CRAB) epidemiology in Spain. Coupled with increased colistin resistance, these changes underscore notable alterations in regional antimicrobial resistance dynamics. This work was supported by Projects PI20/01212 and PI23/00851 awarded to AB, Project PI21/00704 awarded to GB, and Project PI22/01212 awarded to JA-S, funded by the Instituto de Salud Carlos III (ISCIII) and co-funded by the European Union.
