Acute Kidney Injury (AKI) in perinatal asphyxia
Background: Acute kidney injury (AKI) is a common consequence of perinatal asphyxia, occurring in upto 56% of these neonates. It is important to recognize AKI in asphyxiated neonates to facilitate administration of appropriate fluids and electrolytes in order to improve their outcome. Objectives: To determine the incidence of AKI in asphyxiated neonates and to correlate the severity and type of AKI with Apgar score and severity of hypoxic ischemic encephalopathy (HIE). Methods: 75 neonates were enrolled – 50 asphyxiated and 25 healthy neonates. Renal functions were assessed using urine output, urine microscopy, biochemical parameters and sonography. The values obtained were correlated with the severity of HIE. Neonates with AKI were managed as per unit protocol. Results: Of the 50 asphyxiated neonates, 32 (64%) had AKI: 25 (78.12 %) neonates with pre-renal AKI and the remaining 7 (21.88 %) with intrinsic AKI. Out of the 32 asphyxiated neonates with AKI, 12 (37.5%) had oliguric AKI, while the remaining 20 (62.5%) had non-oliguric AKI. Levels of blood urea and serum creatinine were significantly higher in asphyxiated neonates as compared to healthy controls (p 0.001). Biochemical derangements correlated well with the severity of HIE and Apgar scores. Serum sodium and creatinine clearance showed significantly different values in asphyxiated babies compared to controls. There was no significant difference in the urine ouput in the control and study group. Sonographic abnormalities were seen most often in oliguric babies, and indicated bad prognosis. Mortality was higher in babies with oliguric AKI. Conclusions: Perinatal asphyxia is an important cause of neonatal AKI. Majority of the babies had non-oliguric AKI and responded well to fluid challenge. Abnormalities in the renal function correlates well with the severity of HIE. Intrinsic AKI, oliguria, hyponatremia, reduced creatinine clearance and abnormal sonographic scan suggest bad prognosis in neonatal AKI secondary to perinatal asphyxia

Background: Published health promoting or health risk behaviors data in a young population is sparse for Saudi Arabia. Identifying health risk factors early in life and promoting primary prevention interventions among the youth is of utmost importance to move in a healthier direction Objectives: The aim of this study is to identify the socio-economic factors influencing BMI in female students in University of Hail. Methodology: A cross sectional survey was carried out to evaluate dietary behaviors in a random sample of 200 adolescent students, from UOH female campus during the winter semester. Self administered questionnaire for dietary behaviors was used for survey by following anonymity and privacy. Results: This study was conducted to measure BMI and determine the socioeconomic factors affecting the relative weight of young adult females living in the city of Ha’il, Saudi Arabia. A total of 200 female subjects (mean ± SD: age: 19.76 ± 1.98 years and BMI: 23.47 ± 5.58 kg/m2) participated in the present survey. 28.5 % percent of study sample were either overweight or obese. Age wise distribution indicates overweight and obesity significantly increases with age. Breakfast is frequently skipped in overweight and obese subjects while frequency of snacking less among them. Findings of the current study also suggest habit of frequent snacking behavior inverse association with BMI. Modulation of p21, DAPK1 and COX-2 during the DMBA/TPA-induced mouse skin tumorigenesis and its prevention by phytic acid
Chemoprevention by naturally occurring agents is gaining much attention as a newer dimension in the management of cancer. Many naturally occurring agents have shown cancer chemopreventive potential in a variety of bioassay systems and animal models, having relevance to human disease. Phytic acid or Inositol hexaphosphate (IP6), an antioxidant, is a naturally occurring polyphosphorylated carbohydrate that has shown a strong anticancer activity in several experimental models. We assessed the protective effects of Phytic acid against the 7, 12-dimethylbenz [a] anthracene (DMBA)/ 12-O-tetradecanoylphorbol-13- acetate (TPA) induced mouse skin tumorigenesis at 4 and 16 weeks, the time before and after the tumor development. At molecular level we studied expression and promoter CpG methylation status of p21, DAPK1 and COX-2. Our data suggests exposure of DMBA/TPA methylated the promoter region of p21 and DAPK1 genes in time dependent manner that could be the cause of down regulation of their expression with time, which were reversed by administration of phytic acid. But we did not observe methylation in COX-2 whereas upregulation of COX-2 was observed at protein level in mice treated with DMBA followed by TPA in time dependent manner. Administration of phytic acid prevented theses DMBA/TPA induced molecular changes. Study provides a rationale for cancer chemoprevention by natural occurring compounds like Phytic acid
Biochemical analysis, pharmacological activity, antifungal activity and mineral analysis in methanolic extracts of Myrica esculenta and Syzygium cumini: the Indian traditional fruits growing in Uttarakhand Himalaya
Myrica esculenta (Myricaceae) and Syzygium cumini(Myrtaceae), the Indian traditional fruits were studied for their biochemical assay, antifungal activity and mineral analysis. The biochemical assay in terms of total phenols, flavonols, ortho dihydroxy phenolic, ascorbic acid and phytic acid content and antioxidant activity revealed different quantitative make up in all the different samples. The in vitro antioxidant activity determined by reducing power assay, chelating power on Fe2+ ions and 2, 2-diphenylpicrylhydrazil (DPPH) radical scavenging activity were found to be good to moderate in a dose dependent manner in all the extracts. The extracts showed significant antinociceptive and antipyretic activity on mice. Methanolic extracts of both the fruits collected from different regions exhibited good antifungal activity against the three pathogenic fungi viz; Rhizoctonia solani, Sclerotium rolfsii, and Fusarium oxysporium. All the extracts studied for their mineral analysis through atomic absorption spectroscopy also revealed quantitative variation of N, P , K , S, Fe, Zn, Cu and Mn. Fe was found to be maximum in all the samples however Na was absent in all the samples
Recent trends and advances in hemophilia – its management and new therapeutic outcomes
Hemophilia unfortunately a less attracted disease for researchers compared to other life threatening diseases. The prevalence of hemophilia is estimated to be about 1:10,000 birth and that of the severe form of the disease to be about 6% per 1,00,000 population. The most pathetic part of this disease is that even medical personnel are sometimes not familiar with its diagnosis and management. There is obviously a need to establish facilities and treatment options that will help the patient with hemophilia to manage their life with ease. As this is a genetic disorder no complete cure is possible as of now. The only available treatment option is the infusion of factors and some adjuvant therapies depending upon the bleeding conditions .The initiative for the development of new dosage forms, new delivery systems of the existing therapies or new treatment options has to be driven by pharmacy professionals. This article present an overview of hemophilia, in order to drag the attention of medical as well as pharmacy professionals for the benefit of millions of hemophilic patients
Antinociceptive and cytotoxic potential of ethanolic extract of Delonix regia (Leaves)
This study investigated the antinociceptive and cytotoxic effects of alcoholic extract of Delonix regia Leaves in rodents. Antinociceptive activities were done using paw licking and writhing tests. The extract was used at dose of 200 and 400 mg/kg per orally as compared to Standard drug Diclofenac-Na 100 mg/kg bw in Swiss albino mice of body weight 25-30 g. The cytotoxic activity was determined by the Brine Shrimp lethality bioassay. The alcoholic extract showed very highly significant effect in formalin test (p 0.01). Also had the highest analgesia (p 0.001) in the acetic acid-induced writhing model as well. The extract possessed a good in vitro cytotoxic activity (LC50=4.06 μg/ml). The results suggested that Delonix regia Leaves extract has a suitable antinociceptive effect as well as cytotoxic potential
Antibacterial activity of Crotalaria pallida Aiton. (Fabaceae)
Crotalaria pallida Aiton. Fabaceae has been used for the treatment of various diseases like. The aim of the present study is to assess antibacterial activity of methanolic leaves extract of Crotalaria pallida Aiton. Escherichia coli and Klebsilla pneumonieae showed maximum susceptibility 19±1 and 18.66±0.57 at 25 mg/ml concentration respectively while Pseudomonas aeruginosa, Bacillus sp. and Staphylococcus aureus showed less susceptibility 11±1, 11.33±1.5 and 15±1 at 25 mg/ml concentration respectively. Overall results have proved that C. pallida leaves posses significant antibacterial activity
Evaluation of phytoconstituents and assessment of adaptogenic activity in vivo in various extracts of Rhododendron arboreum (leaves)
Natural products particularly of plant origin with antistress property and nutraceutical value have become the focus of scores of scientific investigation in recent times. Against this background the present study was carried out to analyse the phytochemical components and adaptogenic activity of Rhododendron arboreum which grow in high altitudes. Adaptogenic activity of the plant extract was evaluated in vivo in rat and mice animal models. Methanol, hydroethanol and aqueous extract of the leaves were prepared and preliminary screening of the phytochemical constiteunts was carried out. The important phytoconstituents viz. flavonoids and phenolics were estimated and amount of gallic acid and quercetin were evaluated by HPTLC. Adaptogenic activity of the extract was studied in vivo using rat and mice as test animals. The criteria taken were forced swimming test in rat and mice and tail suspension test in mice. Methanol extract showed the presence of diterpenes, triterpenes, flavonoids, steroids, tannin, phenolics. Hydroethanol extract showed the presence of diterpenes, triterpenes, saponin, glycosides, alkaloids, tannin and aqueous extract showed the presence of tannin, glycosides, triterpenes, flavonoids, diterpenes. Quantitative analysis exhibited that the methanol extract have the highest amount of flavonoid and phenolics. The HPTLC profile of methanol extract showed the presence of highest amount of quercetin. Gallic acid was detected only in methanol extract. In vivo study in test animals revealed that the methanol extract of R. arboreum exhibited potent anti stress activity. Hence, presence of these compounds which are known to have strong anti-oxidant activity in high quantity might be responsible for adaptogenic activity
VACTERL association in the newborn
VACTERL association is a non-random association of birth (congenital) defects that affects multiple median and para-median structures. VACTERL association is a useful acronym to denote vertebral abnormalities (V), anal atresia (A), cardiac defects (C), tracheo-esophageal fistula (TE), renal or radial abnormalities (R), and limb abnormalities (L). Atleast 3 or more defects must be present to make a diagnosis of this condition. Most of these cases occur sporadically, although few cases with chromosomal abnormalities have been reported. Herein, we report a newborn who had most of the defects seen in VACTERL association
