Vitamina D – Perspetivas Atuais
The main biological effect of vitamin D is to promote bone mineralization and regulate calcium-phosphorus metabolism, but its extra-skeletal biological function has also been extensively studied. Vitamin D deficit has been associated with an increased risk for various diseases, including autoimmune, neoplastic, cardiovascular and neurological disorders. However, areas of controversy remain: a direct cause-effect relationship between vitamin D deficiency and non-skeletal pathology and the benefit of vitamin D supplementation with this goal still has to be confirmed. In individuals at risk for vitamin D deficiency vitamin D supplementation is safe, inexpensive and with demonstrated benefit in preventing fractures and falls. The authors present an overview of the synthesis, metabolism and action of vitamin D and perform a brief review of the pathophysiology of vitamin D in the skin, namely its influence on skin pigmentation and the effect of photoprotection on vitamin D synthesis and its relationship with skin diseases.A vitamina D tem como efeito biológico principal a promoção da mineralização óssea e a regulação da homeostasia do metabolismo cálcio-fósforo. A sua função biológica extra-esqueleto tem sido amplamente estudada e o seu défice associado a risco aumentado para diversas patologias, nomeadamente autoimunes, neoplásicas, cardiovasculares e neurológicas, entre outras. No entanto, esta mantém-se uma área de controvérsia e está por demonstrar a relação causa-efeito entre deficiência de vitamina D e patologia não esquelética, e o benefício da suplementação com esse objetivo. Nos indivíduos em risco de hipovitaminose a suplementação é segura, barata e com benefício demonstrado na prevenção de fraturas e quedas. É feita uma abordagem da síntese, metabolismo e modo de ação da vitamina D. Faz-se uma revisão sumária da fisiopatologia da vitamina D na pele - a influência da pigmentação cutânea e do uso de fotoproteção na sua síntese, e a sua relação com as dermatoses mais frequentes
Dermatologia na Ilha do Corvo Açores
Concerning the premium «dermatologist from the Heart» awarded by the Fondation La Roche Posay in 2014, we put into practice the winning project "Corvo Island Dermatology". The project’s main objective was to perform a screening of the skin in an area of the country with difficult geographical accessibility to specialized health care. The island has a single doctor, a general practitioner - Dr. José Pinto Faustino. The screening was performed for 5 days, between 13 and 18 July 2015 in the Health Unit of Corvo. There were attended 120 people living on the island that were observed by 2 participant dermatologists. No skin diseases with statistical significance were detected.No âmbito do prémio «dermatologist from the Heart» atribuído pela Fondation La Roche Posay no ano de 2014, pusemos em prática o projecto vencedor «Dermatologia na ilha do Corvo». Este projecto teve como objectivo principal efectuar um rastreio da pele numa zona do país com difícil acessibilidade geográfica a cuidados de saúde especializados. A ilha dispõe de um único médico em funções- Dr. José Pinto Faustino. O rastreio efectuou-se durante 5 dias, entre 13 e 18 de Julho de 2015 na Unidade de Saúde da Ilha do Corvo. Compareceram 120 pessoas residentes na ilha que foram observadas pelos 2 dermatologistas participantes. Não foram detectadas doenças da pele com relevância estatística
Spiny Keratoderma Palmar que precedeu o Diagnóstico de Micose Fungóide: Uma Nova Associação Paraneoplásica?
Spiny keratoderma is a rare dermatosis of unknown etiology that has been described with both hereditary and acquired variants. The acquired form has been associated with underlying malignancy and systemic diseases. We report a case of a 65-year- -old caucasian male presenting with multiple filiform hyperkeratotic papules in the palmar aspect of his hands which were present for the past year. Skin biopsy revealed aspects compatible with spiny keratoderma. After 10 months of follow-up, the patient developed mycosis fungoides, staged as IB according to the European Organization for Research and Treatment of Cancer and treated with PUVAtherapy with good control of both dermatosis. The authors raise the hypothesis of a new paraneoplastic association of spiny keratoderma.Os quadros de spiny keratoderma são raros, de etiologia desconhecida e ocorrem de forma esporádica ou hereditária. A variante esporádica ocorre frequentemente em associação a neoplasias ou doenças sistémicas. Os autores relatam o caso de um doente caucasiano que recorreu à consulta de Dermatologia com multiplas pápulas hiperqueratósicas filiformes palmares presentes desde há um ano. Foi realizada uma biópsia cutânea que permitiu efectuar o diagnóstico de spiny keratoderma. Após dez meses de follow-up, o doente desenvolveu lesões cutâneas de micose fungóide, tendo sido classificado como estadio IB de acordo com a European Organization for Research and Treatment of Cancer e tratado com PUVAterapia com controlo satisfatório da dermatose. Os autores discutem a hipótese de este caso se tratar de uma nova associação paraneoplásica de um quadro de spiny keratoderma
ENTOMODERMATOSCOPIA - UM NOVO MÉTODO DE DIAGNÓSTICO EM DERMATOLOGIA GERAL
Among the new applications of dermoscopy, five main fields can be distinguished: dermoscopy of skin infections and infestations (entomodermoscopy), inflammatory skin diseases (inflammoscopy), hair disorders (trichoscopy), nailfold cappilaries (capillaroscopy), and dermoscopy for treatment decisions or treatment monitoring. The term entomodermoscopy, from the words dermoscopy and entomology, has been introduced to describe the use of dermoscopy in the diagnosis of infectious skin disorders. Herein, we report several cases concerning its applicability in the diagnosis of molluscum contagiosum, scabies, lice infestations, viral warts and early cutaneous leishmaniasis.Entre as diversas aplicações da dermatoscopia, distinguem-se cinco campos: dermatoscopia nas infeções e infestações (entomodermatoscopia), nas doenças inflamatórias da pele (inflamoscopia), na patologia do cabelo (tricoscopia), na avaliação da prega ungueal (capilaroscopia), e a sua aplicação na monitorização da resposta à terapêutica não cirúrgica no cancro cutâneo. O termo entomodermatoscopia deriva das palavras dermatoscopia e entomologia, tendo sido introduzido para descrever a aplicação da primeira no diagnóstico das patologias infeciosas da pele. Assim, descrevemos casos clínicos que demonstram a sua utilidade reconhecida no diagnóstico de molusco contagioso, escabiose, pediculose, verrugas virais e leishmaniose cutânea
LEISHMANIOSE TEGUMENTAR AMERICANA - APRESENTAÇÃO ATÍPICA DIAGNOSTICADA COM TÉCNICA DE BIOLOGIA MOLECULAR
American cutaneous leishmaniasis is an infectious, not contagious disease, caused by a protozoa from genus Leishmania and affects skin and mucosal tissue. The clinical symptoms is varied and it is related with the Leishmania species and the immune response of the host; Most common symptoms are single or multiples ulcerated lesions, however, we will present an atypical case of the disease. The patient presented mainly verrucous lesions in lower limbs, the polymerase chain reaction confirmed American cutaneous leishmaniasis diagnosis. After meglumine antimoniate treatment, the patient evolved with clinical improvement of the lesions, which supports the diagnosis. This case relevance is the need of knowledge of the disease atypical forms, in order to establish an early diagnosis and also an appropriate therapy.Leishmaniose tegumentar Americana é uma doença infecciosa, não contagiosa, causada por protozoários do gênero Leishmania, que acomete pele e mucosa. A clínica é variada e relaciona-se com a espécie de Leishmania e a resposta imune do hospedeiro; o quadro clássico são lesões ulceradas únicas ou múltiplas, porém mostraremos um caso atípico da doença. Paciente apresentava lesões predominantemente verrucosas em membros inferiores, a reação em cadeia da polimerase confirmou o diagnóstico de leishmaniose tegumentar Americana. Este paciente, após inicio do antimoniato de meglumina, evoluiu com melhora clínica das lesões, corroborando para o diagnóstico. A importância se deve a necessidade do conhecimento das formas atípicas da doença, a fim de se estabelecer um diagnóstico precoce e uma terapêutica correta

ERITRODERMIA – ESTUDO RETROSPECTIVO DE 5 ANOS
Background: Erythroderma is a diffuse erythema that affects more than 90% of the body surface area that may be caused by a variety of underlying clinical entities.Material and Methods: We reviewed clinical, laboratory and histopathological data of all patients diagnosed with erythroderma admitted to our inpatient clinic over a 5-year period (2008-2012), identified on a digital database.Results: Nineteen cases were identified over the 5-year period, diagnosed on 15 patients, most being male (n = 13; 68.4%). Psoriasis was the most common etiology (n=10; 52,6%), followed by mycosis fungoides/cutaneous T-cell lymphoma (n=4; 21.0%) and drug reaction (n=3; 15.8%).Introdução: O termo eritrodermia designa um eritema difuso e/ou descamação cutânea superior a 90% da superfície corporal, sendo que várias entidades clínicas poderão estar implicadas na sua etiologia.Material e Métodos: Identificaram-se todos os doentes com diagnóstico de eritrodermia internados no Serviço de Dermatologia do Hospital Curry Cabral num período de 5 anos (2008-2012) por meio de uma base de dados informatizada, tendo sido avaliados os parâmetros clínicos, laboratoriais e histopatológicos em cada caso.Resultados: No período de 5 anos foram identificados 19 casos, diagnosticados em 15 doentes, dos quais a maioria foi do sexo masculino (n=13; 68,4%). A etiologia mais frequente foi a psoríase (n=10; 52,6%) seguida de micose fungóide/ linfoma cutâneo de células T (n=4; 21,0%) e toxidermia (n=3; 15,8%)
INFECÇÃO GENITAL POR CHLAMYDIA TRACHOMATIS E NEISSERIA GONORRHOEAE EM HOMENS ASSINTOMÁTICOS
Introduction: Genital infection by Chlamydia trachomatis (CT) and Neisseria gonorrhoeae (NG) is common and may be asymptomatic, particularly for CT. The availability of nucleic acid amplification tests with high diagnostic sensibility and specificity allowed implementing routine screening in Sexually Transmitted Infections (STI) Clinics. We aimed to determine the prevalence of genital infection by these bacteria in asymptomatic men attending an STI Clinic in a Dermatology Department and to characterize this subpopulation.Methods: A retrospective 51-month-period study on male patients attending a STI Clinic in Lisbon and screened for asymptomatic genital infection by CT and NG was conducted. Data on demographics, sexual behavior, CT and NG screening results and other co-existing STIs were retrieved.Results: A total of 199 patients were analyzed. The prevalence of asymptomatic genital infection by CT and NG was 5% (10 patients) and 0.5% (1 patient), respectively. Asymptomatic genital infection by CT was higher in heterosexual men (7.8% vs 1.3%) and median age of the infected patients group was lower, although not statistically significant.Conclusion: Our study showed a prevalence of asymptomatic genital infection of 5.5% in male patients, which is in line with global epidemiologic data, thus further supporting the current recommendations on routine screening of these infections in the STI Clinic.Introdução: A infecção genital por Chlamydia trachomatis (CT) e Neisseria gonorrhoeae (NG) é frequente, sendo a infecção assintomática importante sobretudo no caso da CT. A disponibilidade de técnicas de amplificação de ácidos nucleicos de elevada sensibilidade e especificidade para o diagnóstico de infecção genital por estas bactérias levou à implementação do seu rastreio sistemático em Consultas de Infecções Sexualmente Transmissíveis (ISTs). Os autores pretenderam determinar a prevalência de infecção genital por CT e NG em homens assintomáticos numa Consulta de Venereologia e caracterizar a população testada.Métodos: Foi realizado um estudo retrospectivo em indivíduos do sexo masculino observados pela primeira vez na Consulta de Venereologia de um Serviço de Dermatologia em Lisboa, durante um período de 51 meses, rastreados para infecção genital assintomática por CT e NG.Resultados: Na população estudada (n = 199), a prevalência de infecção genital assintomática por CT e NG foi de 5% (10 doentes) e 0,5% (1 doente), respectivamente. Verificou-se uma maior prevalência de infecção por CT nos heterossexuais (7,7% vs 1,3%) e a idade mediana do grupo dos infectados mostrou-se inferior, embora não estatisticamente significativa.Conclusão: A prevalência de 5,5% de infecção genital assintomática em homens encontrada no nosso estudo está de acordo com dados epidemiológicos de outros estudos e apoia as actuais recomendações em rastrear estas infecções na Consulta de ISTs
HISTIOCITOSE DE CÉLULAS NÃO LANGERHANS PEDIÁTRICA - UM CASO ATÍPICO
Introduction: Histiocytoses correspond to a heterogeneous group of disorders characterized by the proliferation and infiltration of histiocytes in tissues. For years, many of the histiocytoses where known by numerous names, reflecting the lack of understanding regarding their origin.Case report: A previously healthy, 4-month-old infant presented a cutaneous lesion of progressive growth on his nose since 1 month ago. The mother has a diagnosis of Sjögren syndrome. On examination, there was a firm well- -defined erythematous plaque with 30 mm of diameter and raised edges, on the bridge of the nose. At the left cheek, left thoracic region and right arm there were also small erythematous papules with 5 days of evolution. The analytical study showed elevated antinuclear antibodies and anti-SS-A. Histopathological examination confirmed the diagnosis of non-Langerhans cells histiocytosis. Progressively, more annular erythematous papules and plaques appeared scattered over the face, trunk and limbs with spontaneous resolution of the first lesion on the nose. About 2 months after the initial evaluation, all lesions had regressed spontaneously, with normalization of analytical study at one year-old.Conclusion: Many authors believe that the different subtypes of histiocytosis represent only different stages of the same disease. Immunophenotyping of histiocytosis does not always identify the subtype involved, making the occurrence of atypical cases frequent.Introdução: As histiocitoses são um grupo heterogéneo de doenças caracterizadas pela proliferação e infiltração dos tecidos por histiocitos. Durante muito tempo as histiocitoses foram conhecidas por diferentes nomes, reflectindo a falta de conhecimentos em relação à sua origem.Caso clínico: Uma criança aparentemente saudável de 4 meses de idade foi observada por lesão cutânea de crescimento progressivo com 1 mês de evolução. Mãe com diagnóstico de síndrome de Sjögren. Observava-se uma placa bem definida, firme, eritematosa, com 30mm diâmetro, de bordos elevados, no dorso do nariz, além de pequenas pápulas eritematosas, com 5 dias de evolução, na região malar esquerda, região torácica esquerda e braço direito. O estudo analítico mostrou anticorpos antinucleares elevados e anti-SS-A positivo. O exame histopatológico confirmou o diagnóstico de histiocitose de células não Langerhans. Surgiram progressivamente novas pápulas e placas anulares, eritematosas, dispersas pela face, tronco e membros, com resolução espontânea da lesão inicial do dorso do nariz. Cerca de 2 meses após a observação inicial, todas as lesões regrediram espontaneamente, com normalização laboratorial ao 1 ano de idade.Conclusão: Muitos autores acreditam que os diferentes subtipos de histiocitose representam apenas diferentes estadios da mesma doença. A imunofenotipagem da histiocitose nem sempre permite identificar o subtipo envolvido, pelo que são frequentes os casos atípicos
Tumor de Colisão de Odd – Dermatofibroma e Nevo
A cutaneous collision or contiguous tumor occurs when two or more cutaneous tumors coexist in a single lesion. These tumors usually have misleading clinical and histological presentations, making them a diagnostic challenge. Although several different combinations of collisions have been described we report an unusual case of a cutaneous collision tumour on the back of the patient involving an intradermal naevus and a dermatofibroma. To the best of our knowledge, this is the first time such combination is reported.Um tumor de colisão ou tumor contiguo ocorre quando dois ou mais tumores cutâneos coexistem numa lesão única. Estes tumores têm habitualmente características clinicas e histológicas atípicas, dificultando o diagnóstico. Apesar de terem sido descritas diferentes combinações de tumores de colisão, descrevemos um caso raro de um tumor de colisão localizado no dorso com associação de um nevo intradérmico a um dermatofibroma. Julgamos que se trata da primeira publicação desta combinação
