    Aggressive Periodontitis in a patient with Schizophrenia: Incidental or Accidental – A Case Report.

    Background: Schizophrenia is a psychiatric illness characterized by thought disturbances, bizarre behaviors and cognitive impairments that may diminish a person’s abilities in the area of social relations, school or work and self care. Periodontitis may be defined as an inflammatory disease of the supporting tissues of the teeth by specific microorganisms, resulting in progressive destruction of the periodontal ligament & alveolar bone, pocket formation, recession, or both. The present paper reports a case of generalized aggressive periodontitis in a schizophrenic patient and discusses about common immune mediated pathogenesis in schizophrenia and aggressive periodontitis.Methods: A 31 year-old male patient showed very negligible oral hygiene index score and generalized deep pockets and mobility. Patient was a schizophrenic since 11 years and is on Risperidone for 5 years.Results: Generalized aggressive periodontitis display increased levels of prostaglandin E2. Increased monocytes in schizophrenia patients may result in activation of IL-6 which causes bone resorption. There is altered helper or suppressor T cell function and polyclonal B cell activation in generalized aggressive periodontitis. Schizophrenic patients also display decreased T helper-1 related immune parameter. Polyclonal activation of B cells is also seen as a common immunological finding in generalized aggressive periodontitis and schizophrenic patients.Conclusion: The present paper illustrates the commonality in immune parameters in an generalized aggressive periodontitis patient with schizophrenia. It may be hypothesized that there may be a two way relation between aggressive periodontitis and schizophrenia

    Comparative assessment of efficacy of single application of dipotassium oxalate, Potassium nitrate and sodium fluoride with iontophoresis in the treatment of hypersensitive teeth: An in-vitro and in-vivo study

    The purpose of this study was to compare the efficacy of dipotassium oxalate, potassium nitrate and sodium fluoride with iontophoresis in treating hypersensitivity. Materials and Methods: This short term clinical study was carried out on 45 teeth from 15 patients from the out-patient section of Department of Periodontics. Three teeth were selected per patients. The selected patients were divided into three groups. Group I: Application of 30% dipotassium oxalate for 1 min with a cotton pallet. Group II: Application of 5% potassium nitrate for 1 min with cotton pallet. Group III: Application of 2% sodium fluoride with iontophoresis. Patients were randomly selected which include males and females between 18 to 55 years of age. Scaling was done one week prior to treatment. The entire patient’s were instructed not to use any other desensitizing agent during the study. Assessment of pre and post treatment hypersensitivity was done by objective (electric and mechanical stimulation) and subjective analysis (visual analog scale).Severity of sensitivity was assessed before application, immediately after application, and later on at weekly intervals for 6 weeks. As a part of an in vitro study, a total of 15 teeth, 5 teeth per group were selected from patients with sensitivity, whose teeth were indicated for extraction due to advanced periodontal disease. Data was analysed using student’s unpaired t-test (2 tailed), Results: It showed that comparison between group I and III, II and III were highly statistically significant throughout the 6 weeks (P< .01) , whereas group I and II were statistically significant. Conclusion: This study demonstrated that was a drastic reduction in dentinal hypersensitivity when iontophoresis with 2% sodium fluoride was applied and thus can be concluded that clinically, single application of iontophoresis with 2% sodium fluoride could prove to be a better therapeutic agent in comparision with 30% dipotassium oxalate and 5% potassium nitrate agents.KEY WORDS: sensitivity, dentine hypersensitivity, potassium nitrate, dipotassium oxalate, sodium fluoride, iontophoresis, dental pai

    Molar Uprighting simplified

    Impaction of the lower second molar is a common problem, and a very challenging one for the orthodontist . Here is a case report of uprighting of a mesioangular impacted mandibular right permanent second molar. Though various treatment options were available, uprighting with traction from RME in the opposite arch was chosen.This method assumes significance, because cases which require RME usually present with a need for uprighting of lower molars, where this technique can be employed simultaneously to upright the impacted or tilted lower molar and decrease the treatment time considerably. Since anchorage is derived from the RME appliance,the usual untoward reciprocal effects on the adjacent teeth associated with cases of molar uprighting are avoided.Key Words: Rapid maxillary expansion, Uprighting, impacted mola

    Sleep disordered breathing- An overview

    Sleep disordered breathing has been used to describe a spectrum conditions ranging from snoring as a mild form to Obstructive Sleep apnea (OSA) which is a more severe form. Upper airway resistance syndrome falls in between the two. Oral appliance therapy and therefore dentistry has become an important component of treating patients with upper airway sleep disorders.     Pyoderma gangrenosum with unusual periodontal manifestations.

    Pyoderma gangrenosum (PG) is a rare non infectious ulcerative dermatosis of uncertain aetiology, which is associated with systemic diseases in at least 50% of the patients. It is characterized by recurrent cutaneous ulcerations with mucopurulent or haemorrhagic exudates. The legs are most commonly affected but other parts of the skin and mucous membrane may also be involved.     Fibroma is a common benign tumor. It can occur at any age from almost any soft tissue site the tongue, gingiva, and buccal mucosa being the most common. Females are twice as likely  to develop this benign tumor than male. Typically the lesion is a rounded, well-demarcated, variably sized mass attached to tissue by a stalk.     main objective of pediatric dentistry is achieved when the restored tooth fulfills its role as a useful component of primary and young permanent dntition.Hence this study was conducted to evaluate the clinical efficacyfujiII,fuji VIII and compomer-DYRACT.Aim- To evaluate clinical behavior of fujiII,fuji VIII and compomer- DYRACTin early childhood caries in primary anterior teeth.Objective- To evaluate and compare the clinical efficacy regarding marginal integrity, anatomical form and recurrent caries in primary anterior teeth after restoration with fuji II, fuji VIII and compomer- DYRACT.Material and method- The total 130 class III and class V lesions on the maxillary and mandibular anterior teeth were restored in this study.After one, three and six months of the restorations,these restorations were evaluated using modifications of USPHS criteria (kalpatric et al 1995).Conclusions- Fuji VIII has got slight edge over compomer-DYRACT in marginal integrity but difference was not statistically significant.At the end of three months, fuji VIII has got maximum retention of anatomical formfolloed by compomr DYRACT and fuji II, which was statistically significant.Fuji II showed highest rate of recurrent carries followed by fuji VIII and compomer-DYRACT.     A cephalometric comparison of the dentoskeletal changes in class II malocclusion by using Jasper Jumper and Forsus- A clinical study

    Objective: A cephalometric study carried out to compare the dentoskeletal changes of class II malocclusion by using Jasper Jumper and Forsus Material and Methods: Our research was conducted on 12 adolescents, who had a normal or horizontal growth pattern and retrognathic mandible. The patients were divided into two equal groups randomly. First group was treated with Jasper Jumper  and second group was treated with Flat Nitinol Forsus Spring (FNFS). Lateral cephalograms were obtained after the leveling phase and at time of the removal of the appliances. Results: Cephalometric analysis revealed that both the appliances stimulated mandibular growth but have very minimum restraining effect on maxilla. Both Jasper Jumper group and Forsus group showed significant distal movement of upper incisors and molars. The skeletal and dental contribution together positioned mandibular incisors and molars mesially by a highly significant margin in both groups, and a Class I molar relationship and improvement in the Overjet and overbite attained in both treatment groups. Conclusions: Both the appliances were effective in the treatment of Class II malocclusion. The class II correction with Jasper Jumper and Forsus appliances mainly occurred by dentoalveolar changes than skeletal changes. . Key Words: Flat Nitinol Forsus Spring, Jasper Jumpe
