Conventional root canal therapy of C-shaped mandibular second molar- Case report
Aim: This clinical report presents the endodontic treatment of mandibular second molar with a C-shaped canal systems. Summary: According to the endodontic literature a C-shaped root canal is most frequently seen in mandibular second molar. Once recognised, the C-shaped canal is a challenge with respect to debridement and obturation. We observed this configuration in a case and successfully treated them using conventional methods of obturation. Key learning points: C-shaped root canal morphology occurs mostly in mandibular second molars in Asian population. Thorough knowledge of the root canal anatomy is necessary to perform a successful endodontic treatment
Nisin Vs Calcium Hydroxide – Antimicrobial Efficacy On Enterococcus Feacalis – An In-vitro Study
Objectives: This invitro study was carried out to evaluate the effect of Nisin against Enterococcus faecalis in solution and also to evaluate and compare the effect of Nisin and Calcium hydroxide against E.faecalis in root canal system. Methodology: Minimum Bactericidal Concentration and Minimum Inhibitory Concentration of Nisin against E.faecalis were determined. Root canal specimens were inoculated with E.faecalis and incubated for 14 days. Specimens were subjected to intracanal medicaments: Calcium hydroxide and Nisin and incubated for 7days. Bacterial samples were then obtained with paper points, inoculated onto BHI broth and incubated at 370C for 24hrs. Dentine fillings from each canal were obtained and placed in Eppindorf tubes containing BHI broth. After 24hrs incubation, bacterial samples were inoculated onto TSY-Blood agar plates to obtain bacterial CFU. The results were subjected to Mann-Whitney “U” statistical test. Results: Nisin group showed significantly lower bacterial Colony Forming Units obtained from dentinal shavings than Calcium hydroxide and control groups.Conclusion: that Nisin was effective at eradicating E.faecalis in pure culture and was more effective when compared to Calcium hydroxide in the elimination of this species with in the root canal system.Key words: E.faecalis, intracanal medicament, Calcium hydroxide, Nisi
Maxillofacial Applications of Alveolar Distraction Oseteogenesis: An Overview
The development of Distraction Osteogenesis required defining new concepts in regeneration of bone and soft tissue. To initiate and control the distraction process, surgical instruments specific to the biomechanical requirements of the technique needed to be developed. Rigorous analysis of the initial results was needed to define the biologic basis of the process. To reach the current state of this technology required many years. Today this technology holds immense potential in reshaping surgical endeavors in the future. Full Mouth Rehabilitation of a Patient with Severely Worn Molars: A Case Report
The severe wear of mandibular molar teeth with loss of few teeth, resulting in collapse of vertical dimension at occlusion and also, change in normal occlusal plane. This case report describes a 45yr old female patient who had severely attrited lower molars and loss of vertical dimension. Provisional crown and bridge was used for 4 months with increased vertical dimension, after, taking into consideration of anatomical landmark, facial and physiologic measurement. Once compatibility of stomatognathic system confirmed permanent restoration was initiated. This case reports that a satisfactory clinical result can be achieved by restoring vertical dimension to restore esthetics and function
Quantitative Digital Subtraction Radiography (DSR) as an approach for evaluating crestal alveolar bone density changes around teeth following orthodontic tooth movement.
Aim: The aim of the present study was to retrospectively evaluate changes in bone density using DSR at the crestal and subcrestal regions of interproximal bone around posterior teeth (premolars and molars) before and after orthodontic treatment using digital OPGs. Materials & Method : A total of 14 Pre and Post operative digital image pairs were obtained from the department of Orthodontics and 28 regions were sampled and analyzed. The selected patients were in the age group of 13-18 years. The mean duration of orthodontic treatment was 1.5 years. All pre and post operative radiographs were assessed at baseline and after completion of orthodontic treatment using DSR. All subtracted images were subsequently imported into The Image Tool® software to calculate the average density of Gray levels in the areas that showed changes in subtraction. Results: Out of 28 test regions 23 regions (82.14%) showed an increase in bone density whereas 5 regions (17.85%) showed a decrease in bone density. The mean bone density of the ROIs was 151.18 (gray level = 151.18 ± 19.97 SD). A one sample t test for statistical significance was carried out. The difference of the Mean values was found to be 23.18. The obtained p value was <0.001 at 95% confidence interval (15.44 to 30.92) Conclusion: In the present study, we have found that 23 out of 28 regions (82.14%) showed significant increase in bone density whereas 5 regions (17.85%) showed a decrease in bone density. DSR is a valuable tool to assess subtle radiographic changes that occur in the alveolar bone during and after orthodontic therapy and can be used to monitor the bony changes over treatment period
Antiplaque and Antigingivitis effect of a Dentifrice containing Bioactive Glass particulate-A Randomized Clinical Trial
Background: Maintenance of gingival health is critical in preventing gingivitis and its progression into periodontal disease. Bioactive glass has recently been formulated into a dentifrice and has demonstrated strong antimicrobial property. Objectives: The objective of this clinical trial was to evaluate the antigingivitis and antiplaque effect of a dentifrice containing bioactive glass compared with a placebo control dentifrice in a four weeks clinical study. Methodology: The Study design was a randomized triple blind parallel clinical trial. Sixty volunteers took part in the study and were matched for age and gender. Plaque index (Turesky et al modified Quigley Hein index, 1970) and Gingival bleeding index (Ainamo and Bay, 1975) was recorded at baseline for all selected subjects. Following the baseline assessments, all subjects received a supragingival prophylaxis and polishing to remove plaque, calculus and extrinsic stain. After prophylaxis the plaque levels were brought to uniform levels, ideally close to zero, which was assessed by plaque index. After which the subjects were instructed on the proper brushing technique with their assigned dentifrice and toothbrush. Gingiva and plaque was evaluated at the end of 4 weeks using same indices. Results: A total of 60 subjects completed the study. Total number of participant (n) = 60 males (n) =50 females (n) =10. The results showed that the PLI (baseline 1.12, 4 weeks 0.65) and GBI (baseline 0.16, 4 weeks 0.08) were significantly reduced, respectively, over the 4 weeks period in the test group (p<0.001 for each measure). There was a 50% reduction in gingival bleeding and a 40%reduction in plaque growth. There was no difference of the PLI (baseline 1.34, 4 weeks 1.30) and GBI (baseline 0.13, 4 week 0.15) over the 4 week period in the control group. Conclusion: The study demonstrated that the dentifrice containing Bioactive glass significantly improves gingival health as measured by a reduction in gingival bleeding and reduction in supragingival plaque compared with Placebo dentifrice over the 4 weeks study period. Atraumatic restorative treatment (ART) in Pediatric Dentistry
Atraumatic restorative treatment (ART) is an alternative treatment for dental caries and is an easy, low cost and painless restorative technique where soft infected dentin is removed with sharp hand instruments retaining the reversible affected dentin and the resultant cavity and adjoining fissures are restored and sealed with Glassionomer cement. This technique involves minimal intervention and invasion and can be used for both treatment and prevention of dental caries. Several Studies have shown good results with this procedure because of which this simple but innovative technique is gaining broader acceptance among dentists and ART approach is increasingly being advocated for use in pediatric and general clinical practice, and field use. More recently, ART has become increasingly accepted in developed countries because of its “atraumatic” approach in relation to the stress and pain experienced by patients
Assessment of dye penetration at composite – repair interface: Effect of application of bonding agent and comparison of two different bonding systems: An in vitro study
Introduction: Reparability of defects / fractures restricted to the bulk of the composite restoration is a highly desirable property, as being less time consuming and cost effective. This study was aimed to evaluate the microleakage at the composite repair interface following surface treatment and application of the bonding agent and compared the microleakage following application of two different bonding systems at the composite repair interface. Material and Methods: Sixty composite resin specimens (Filtek Z350, 3M Dental Products) aged in distilled water for 1 week, were divided into four groups (n = 15) based on surface treatment and application of bonding agent. All the groups then received new resin applications using same composite resin. The samples were then thermocycled (200 cycles/50C to 550C{±2}) and immersed overnight in aqueous methylene blue dye solution. Single examiner carried out dye penetration analysis using stereomicroscope. Results: Mann-Whitney U-Test (p<0.05) for group one and two showed no significant difference in repair microleakage. Group 3 and 4 showed statistically significant difference when compared with group 1 & 2. However, both group 3 and 4 did not significantly differ in microleakage scores with each other.Conclusion: Irrespective of the type of bonding systems used, surface treatment with 50 micron size aluminum oxide particles followed by application of adhesive system are the essential steps to significantly reduce the microleakage at repair interface
Molarization of the mandibular second premolars with concurrent dentin dysplasia: A rare case report
This paper presents a rare case of bilateral macrodontia with dentin dysplasia. The case presented as molar like appearance of the second mandibular premolars which appeared radiographically as rootless tooth on one side and with a short, conical root on the other. Appropriate preventive therapy is recommanded to increase the life of such teeth in the oral cavity.KEY WORDS: Macrodontia, molarization, rootless teeth, dentin dysplasia
Polymorphous Low Grade Adeno Carcinoma- A Rare Case Report
Polymorphous low grade adenocarcinoma of the salivary glands (PLGA) is a low grade neoplasm that predominantly occurs in the minor salivary glands. It is usually located on the hard or soft palate. This report describes the case of a patient that was diagnosed and treated of some unknown swelling 10 years back and patient again came back with same complaint on same side that turning out to be PGLA with maximum size reporting in the literature in our notice. The case was unique in saying that this might be reoccurrence of the original swelling that occurred 10 years back.
