    SEM Evaluation of Shear Bond Strength Of Composite Resin to Dentin After Application Of Cavity Disinfectants

    Abstract:Context: Aims: To evaluate the effect of different cavity disinfectants on dentin bond strengths of composite resin applied with two different adhesive systems.Settings and Design: Methods and Material: Two hundred mandibular molars were sectioned parallel to the occlusal surface to expose dentin in the midcoronal one third. The dentinal surfaces were polished with waterproof-polishing papers. The specimens were randomly divided into 5 groups of 40 teeth each as follows: Group 1(Control) - specimens were not treated with any cavity disinfectants. Groups 2 to 5 (Experimental Groups) - dentin surfaces were treated with the following cavity disinfectants, respectively; 2% chlorhexidine solution, 0.1% benzalkonium chloride based disinfectant, 1% chlorhexidine gel,  and an iodine potassium iodide / copper sulfate based disinfectant. The specimens were then randomly divided into 2 subgroups including twenty teeth each to evaluate the effect of different bonding systems. Dentin bonding systems were applied to the dentin surfaces and the composite buildups were done. After the specimens were stored in an incubator for 24 h, the shear bond strength was measured at a crosshead speed of 1 mm/min. The specimens were then statistically analysed. Statistical analysis used: One way analysis of variance and Tukey-HSD tests.   Results: There was no significant difference between chlorhexidine gel and control groups regardless of the type of the bonding agent used (p > 0.05). On the other hand, pretreatment with benzalkonium chloride based, iodine potassium iodide / copper sulfate based disinfectants or chlorhexidine solutions had a negative effect on the shear bond strength of self-etching bonding systems.    Conclusions: The findings of this study suggest that when benzalkonium chloride based, iodine potassium iodide / copper sulfate based disinfectants or chlorhexidine solutions are used as a cavity disinfectant, an etch-and-rinse bonding system should be preferred.  &nbsp

    Effect of Office Bleaching Agents on the Shear Bond Strength of Metallic Brackets Bonded Using Self-Etching Primer System at Different Time Intervals – An In-Vitro Study

    A common method of treating discoloured teeth is to bleach them using hydrogen peroxide. Many studies in the literature have concluded that bleaching reduces shear bond  strength of composite resin significantly and thus, to achieve clinically adequate shear bond strength of composite for bonding brackets to bleached enamel, it is important to find out the various methods of reducing adverse effects of bleaching. It has been proposed by some authors that a delay in bonding effectively reverses the effects of  bleaching on bond strength. OBJECTIVES- To test the null hypothesis that there is no statistical significance in (1) bond strength and (2) failure site location with bleached and unbleached enamel prepared with TransbondPlus Self-etching Primer between  different time intervals. MATERIALS AND METHODS- Sixty freshly extracted human premolar teeth were randomly divided divided into three groups of 20 teeth each Bleaching treatment was performed at two different time intervals (bleaching immediately before bonding and bleaching 30 days before bonding). All brackets were bonded with a self-etching primer system. The shear bond strength of these brackets was measured and recorded in MPa. Adhesive remnant index (ARI) scores were determined after the brackets failed. RESULTS- The mean shear bond strength of Group A (control) is found to be 20.4315 MPa (SD± 6.81041) which was reduced to 11.1955 MPa (SD ± 4.58041) when bonding was done immediately after bleaching (group B). The shear bond strength increased to 14.9910 MPa (SD ± 4.74545) when enamel was 14.9910 MPa (SD ± 4.74545) when enamel was Group C) Pair wise comparison of shear bond strength between the three groups  using Post hoc Tukey test showed that there is a very high statistically significant difference between shear bond strength of all three groups. CONCLUSION This study shows that bonding immediately after bleaching significantly reduces the shear bond strength , it can be reversed by a delay in the bonding by 30 days which shear bond strength , it can be reversed by a delay in the bonding by 30 days which improves the shear bond strength as reflected  in the results of the study ,So it is advisable to delay the orthodontic bonding procedure by an interval of 30 days to improve survival rates and prevent frequent breakages of brackets due to bond failure

    AIMS AND OBJECTIVES A CONSERVATIVE SOLUTION FOR AN ESTHETIC CHALLANGE PROVIDING SINGLE VISIT AND A CHAIR SIDE PROCEDURE TO IMPROVE A SMILE   CASE DISCRIPTION Conservative solutions  for the restoration of a single edentulous space in the anterior maxilla present an esthetic challenge to the clinician.A 24-year-old female patient whose left upper central tooth was planned to be extracted was referred to our department for a conservative, rapid, and economic treatment.After radiographic and clinical examinations, it was decided that the tooth which was to be extracted should be used for the restoration of its own extracted area. The extracted tooth was splinted using Ribbond fiber (Ribbond Inc., Seattle WA) to adjacent teeth with the aid of the surface modifications on extracted tooth and adjacent teeth . CONCLUSION  The conservative bridge with natural tooth pontic satisfied the patient with a good mechanical behavior, and long-term durability. &nbsp

    Horizontal transmigration of mandibular canines with bilateral enostosis between the premolars - A case report.

    ABSTRACT:  Aim & Objective: To report a case of transmigrated mandibular canine associated with enostosis between the premolars bilaterally. Case description: A 21 yr old female patient reported with a chief complaint of missing lower teeth. On examination, the patient had bilaterally missing permanent canines with no history of extraction and showed no evidence of deciduous canines. Radiographs revealed horizontal impaction of bilateral mandibular canines with transmigration of right canine to the apex of left lateral incisor associated with enostosis between the premolars bilaterally.  Conclusion: Intraosseous tooth migration is a very rare dental ectopia that involves lateral incisors, second premolars and canines in the mandible but canine is the only tooth that migrates across the midline which could result in resorption or displacement of other teeth. Many etiologic factors have been put forward to explain the causative of transmigration including cysts and tumors. Identifying this is important to prevent damage to oral structures and to avoid complications during surgical procedures. &nbsp

    Mandibular first molar with multiple canals- Report of three cases

    Aim: To present clinical cases report of three mandibular first molar with middle mesial canal, middle distal canal and radix paramolaris. Introduction:  Morphological variations in root canal system anatomy should always be considered at the beginning of treatment. Once endodontic treatment has been initiated, proper access cavity preparation is a basic prerequisite for the investigation and successful detection of all root canal orifices. Case Description: Three patients with chief complaint in mandibular first molars were referred for endodontic treatment. Two patients had extra canal in their mandibular first molar and third patient had additional canal with supernumerary root. With aid of proper diagnostic and radiographic techniques the endodontic treatment was performed. Conclusion: Good knowledge of the potential aberrant canal morphology in mandibular molar will help clinician to successfully recognize and treat these difficult cases.The incidence of extra buccal root and canal variations are rare, every effort should be made to find and treat all canals for successful clinical results.     Full mouth crown lengthening: a case report

    Aims and objectives: Crown lengthening is a surgical procedure which aims at removal of periodontal tissue to increase the clinical crown height and reestablishing the biologic width. It is procedure which helps to meet both restorative and aesthetic demands of the patient.Case description: A female patient presented with a chief complaint of generalized worn out teeth. On examination collapsed bite with reduced vertical dimensions was observed, for which prosthetic rehabilitation was not possible so full moth crown lengthening was selected as the treatment modality.Conclusion: This case report demonstrated that surgical crown lengthening technique improved the vertical dimension of the patient  and also helped to reestablish the biologic width

    A Systematic Approach to Management & Prevention of Dental Trauma in Children

    Trauma to teeth and supporting structures is one the most challenging situations dentists face in their day to day practice. World wide studies have shown an increased prevalence of dental trauma and resultant fractured, displaced, or avulsed teeth can have functional, esthetic, and psycho­logical effects on children. For successful management of patients with dental trauma it is therefore very important to know the history, circumstances of the injury, type of trauma, and initial reaction of the child and/or guardian. Dentists should therefore be able to recognize, differentiate, and either treat or refer patients based on the type of dental trauma. To efficiently determine the extent of injury and correctly diagnose injuries to the teeth, periodontium, and associated structures, a systematic approach to diagnosis and treatment is essential. The article gives details of the treatment modalities for dental trauma and recommendations for prevention of Dento-facial injuries. &nbsp

    Aims & Objectives: Fabrication of fixed dental prosthesis using gingival porcelain to mask the ridge defect for a patient with cleft lip and palate.Case description:  This paper describes the prosthetic rehabilitation of a 19 year old girl patient with gingival porcelain after the surgical correction of cleft lip and palate. This patient was treated with a four unit metal-ceramic fixed dental prosthesis for replacement of missing maxillary left lateral incisor and the ridge defect was masked with gingival porcelain. In this case, a well planned periodontal surgery was performed and followed by prosthetic rehabilitation therapy.     Piezosurgery And Its Applications In Periodontology And Implantology

    ABSTRACT:  The piezosurgery instrument, developed in 1988, uses a modulated ultrasonic frequency that permits highly precise and safe cutting of hard tissue. Nerves, vessels, and soft tissue are not injured by the microvibrations (60 to 200 mm/sec), which are optimally adjusted to target only mineralized tissue. The selective and thermally harmless nature of the piezosurgery instrument results in a low bleeding tendency. The precise nature of the instrument allows exact, clean, and smooth cut geometries during surgery. Postoperatively, excellent wound healing, with no nerve and soft tissue injuries, is observed. Because of its highly selective and accurate nature, with its cutting effect exclusively targeting hard tissue, its use may be extended to more complex oral surgery cases, as well as to other interdisciplinary problems.
