Middle part of main portion (MMP) of temporal muscle as interpositional material in the treatment of temporomandibular joint ankylosis
Introduction: This study was conducted to determine the efficacy of interpositional arthroplasty using the middle part of main portion (MMP) of temporal muscle as interpositional material in the treatment of temporomandibular joint (TMJ) ankylosis. Materials and Methods: This study of ten cases evaluated the postoperative results of interpositional arthroplasty with MMP of temporal muscle in treatment of unilateral and bilateral TMJ ankylosis. Resection of ankylotic mass with coronoidectomy when deemed necessary was done. Interpositional transfer of the MMP of temporal muscle flap was done followed by early mobilization and aggressive physiotherapy. Results: The study evaluated ten patients with follow-up checks from 03 to 31 months (mean 17 months). Patients had a preoperative maximal interincisal opening of 0–19 mm (mean 9.5 mm). During the last follow-up observation after surgery, the patients had a maximal interincisal opening of 25–40 mm (mean 32.5 mm).The results of this protocol were encouraging; the functional results of interpositional arthroplasty with MMP of temporal muscle were also satisfactory. Conclusion: The findings of this study support the use of MMP of temporal muscle flap in patients with TMJ ankylosis. Early postoperative initial exercise, physiotherapy and strict follow-up play an important role in preventing postoperative recurrences.  
Oral Piercing : A Deleterious vogue
Marked propensity towards “Ornamental oral piercing” has pervaded in youth today. Albeit the role of western culture can not be ignored in proliferating this trend, history reveals that it has been an archaic tradition in our country as a part of religious, cultural, tribal’s and sexual symbolism. On the other hand it is considered as “truly ostentatious” in developed countries, which has influenced our youth to great extent. Intraoral jewelry or other oral accessories may lead to increased plaque level, gingival inflammation, recession, caries, diminished articulation and metal allergy and it is associated with pathological conditions including pain, infection, scar formation, tooth fractures, localized periodontal disease, speech impediment and nerve damage. Reckoning the fact that there is dearth of adequate literature in context to ‘oral piercing’ in our country, we perused more than hundred foreign articles including clinical studies, case reports and review articles to consummate knowledge on various aspects of this topic. This article is an wholehearted attempt to make youth cognizant to various aspects of oral piercing, risks and complications associated with it and their prevention and management.  
C-Reactive Protein as a Monitoring Tool for Facial Space Infections of Odontogenic Origin: A Prospective Study
Objectives: To study the efficacy of the C – Reactive Protein (CRP) level as a monitoring tool for facial space infection of odontogenic origin. Materials and Methods: A prospective study was conducted on 25 patients diagnosed with facial space infection of odontogenic origin. Using CRP- turbilatex, quantitative analysis of CRP were done on 1st, 3rd and 7th day respectively. The normal serum CRP value is 6 mg/L. Parameters like swelling size, grades of dysphagia, hoarseness of voice, trismus, number of spaces involved and discharge were recorded on 1st, 3rd and 7th day and a correlation was established with the value of CRP using Pearson’s correlation test. Results: The mean change in the number of the spaces involved, swelling size & CRP value showed decrease with the time with high significance (p<0.01).The improvement in the mean change in mouth opening were seen on subsequent days with high significance (p<0.01).CRP value has direct and linear relationship with swelling size, number of spaces and inverse relationship with the mouth opening. Conclusion: This study showed that CRP is a significant predictor of severity of infection and effectiveness of the treatment regime. Thus, serum CRP level can be an important tool in managing patients with facial space infections of odontogenic origin. Evaluation of vertical and sagittal position of maxillary first molar in various skeletal types.
Aim : To evaluate factors affecting the sagittal and vertical position of maxillary first permanent molar in Class I and Class II division 1 malocclusions . Method : The pre-treatment lateral cephalograms of sixty patients were divided into 2 study groups i.e. Group 1 (Class I subjects) – Angle’s Class I molar relation, with ANB =0-40 with normal overjet and Group 2 (Class II division 1 subjects) – Angle’s Class II molar relation, ANB > 40 with overjet > 4mm. Linear & angular measurements e.g. Cranial base length, Maxillary length , Maxillary location, Upper anterior face height, Posterior face height, Sagittal and Vertical position of maxillary first molar, Palatal plane angle , Mandibular plane angle, Interjaw angle and Cant of occlusal plane were made on tracings. Statistical analyses included calculation of linear correlation coefficients and student’s t-test to correlate these parameters. Results and Conclusion: The sagittal & vertical position of maxillary first molar remained same in Angle’s Class I and Class II division 1 malocclusion. In Angle’s Class I malocclusion, maxillary first molar was located more forward with increased maxillary length and its height increased with increase in upper anterior face height and posterior face height. In Angle’s Class II division 1 malocclusion, maxillary first molar was located more forward with increase in maxillary length as well as posterior face height while it was located more posteriorly with a steeper cant of occlusal plane and maxillary first molar height increased with increase in maxillary length as well as posterior face height and decreased with steeper cant of occlusal plane
Comparison of the dimensional accuracy and surface detail reproduction of different impression materials under dry and moist conditions - An Invitro study.
AIMS AND OBJECTIVES The purpose of this study was to compare the ability of different impression materials to record surface detail under dry and moist conditions and to also compare the accuracy of gypsum casts obtained from these impression materials. METHODS Impressions were made of stainless steel dies as described in ANSI/ADA Specification No.19, with two vertical and three horizontal lines inscribed on the die superior surface. Impressions were made under dry and moist conditions. Impressions were poured with Type IV die stone. Dimensional accuracy was measured by comparing the average length of the middle horizontal line in each impression and each gypsum cast with the same line on the metal die using a measuring microscope. Surface detail reproduction was evaluated: continuous replication of at least two of the three horizontal lines. RESULTS The percent change of Polyether, Polyvinylsiloxane, Irreversible Hydrocolloid impression materials under dry condition were +0.05, -0.14, -0.27 and under moist condition were +0.04, -0,15, -0.27 respectively. The percent change of Polyether, Polyvinylsiloxane and Irreversible Hydrocolloid poured gypsum samples under dry condition were +0.06, -0.13, -0.26 and under moist condition were +0.06, -0.13, -0.26 respectively. CONCLUSIONS Within the limitations of the study, the following conclusions were drawn: 1. Moisture did not cause a significant adverse effect on the dimensional accuracy of the three impression materials. 2. There was no statistically significant difference in dimensional accuracy or surface detail reproduction among the gypsum casts produced from the three different impression materials when compared between dry and moist conditions.  
An insight into dental implant fixture fracture - A rare but important complication
Dental implants are a functional and esthetic solution to partial and total edentulism. The initial success rate of this treatment modality is 90-95%. But, that treatment modality is not free of complications. One of the rare complications yet, with severe clinical results is fracture of dental implants. Implant fracture is an infrequent and late biomechanical complication with a serious clinical outcome. In effect, such fractures pose important problems for both the patient and the dental surgeon. A search of English language peer-reviewed literature was completed using Medline and Pubmed between the period of 1981 to 2010, using dental implant, fracture as keywords. This was supplemented with a manual search of selected journals. The present literature review describes the management options and discusses the possible causal mechanisms underlying such failures, as well as the factors believed to contribute to implant fracture

Periodontal Probes - A Review
A periodontal probe till today remains the most important diagnostic tool in periodontal diseases. A probe is used to make initial assessment, followed by detailed evaluation to determine the extent and degree of severity of disease and tissue destruction for specific treatment planning. During treatment the probe is applied to assess the progress. Desquamative Gingivitis as a Sign in Mucous Membrane Pemphigoid- A Case Report and Review of Literature
Mucous membrane pemphigoid (MMP) is a chronic, sub-epithelial autoimmune disease, which predominantly involves mucosal surfaces and results in mucosal blistering, ulceration, and subsequent scarring. The autoantibodies are directed against epithelial basement membrane zone proteins. Oral lesions, in the form of Desquamative gingivitis, oral vesicles, erosions, and positive Nikolsky sign usually predominate. Diagnosis is made on a thorough history, general and oral examination, coupled with biopsy and immunoflourescence studies. The corticosteroids form the mainstay of treatment, although the disease also shows remission with tetracyclines, dapsone, azathioprine, cyclosporine, tacrolimus, and other immunosuppressive agents. An Interesting Case of Hodgkin’s lymphoma
Introduction: Lymphomas are a group of malignant solid tumors involving cells of the lymphoreticular or immune system. Lymphomas are divided into two major categories: Hodgkin’s disease (HD) and non-Hodgkin’s lymphoma (NHL). The incidence is bimodal with the first peak in the third decade of life, and a second smaller peek after the age of 50 years. The etiology of HL is still unknown, but some factors include a positive family history, higher socioeconomic status, and prior EB virus infections. Case Description: We present a case of Hodgkin’s lymphoma in a 52 year old male patient presenting with a swelling involving bilateral sub-mandibular region. Following incisional biopsy, Hodgkin’s lymphoma was diagnosed.Conclusion: Combination of chemotherapy and radiotherapy is the treatment of choice in Hodgkin’ s lymphoma
