    The Single Complete Denture – A Case Report.

    The single complete denture is a complex prosthesis that requires a complete understanding of the basics of prosthetic rehabilitation of lost natural dentition. Several difficulties are encountered in providing a successful single complete denture treatment, the most common being repeated fracture of the prosthesis. An ideal solution to strengthen the single complete denture bases is to provide metal reinforcement by fabrication of metal based single complete denture. Another common problem in single complete denture is attrition of denture teeth while opposing natural dentition which can be taken care of by providing metal occlusals in single complete dentures. Metal occlusal surfaces preserve the established occlusion and prevent loss of vertical dimension. This case report describes the clinical management and step by step fabrication of single complete denture with metal denture base and metal occlusals in a patient with completely edentulous maxillary jaw and partially edentulous mandibular jaw.&nbsp

      ABSTRACT   A multimodal and multistage surgical protocol that immensely improved the facial appearance and the functional demands of a   patient is described in this article. Our aim was to bring back the normal oral functions and improve the facial appearance by performing various surgeries. A 20 year old girl with bilateral bony ankylosis, hypoplastic mandible, and the compensated maxilla was treated in several sittings by  gap arthroplasty and temporal   muscle interposition, bilateral  distraction osteogenesis for the hypoplastic mandible, anterior maxillary osteotamy with 5 mm set backing and augmentation of an alloplastic material, Medpor . All the surgeries were conducted one by one in four sittings with adequate time interval, under general anesthesia, excepting the 4th surgery which was conducted under local anesthesia and the result obtained was excellent. The struggle experienced by the surgeon for obtaining the consent from the patient for performing the surgeries is being compared with the tedious journey

    The advancement in scientific research is providing us with the tools to prevent, diagnose, and treat the infection or disease before it causes damage. Probiotics are health-beneficial bacteria which have only recently been introduced in dentistry after years of successful use in mainly gastro-intestinal disorders. It comprises knowledge of the relationship between diet and health and the effects of food ingredients on physiological functions and health. Probiotics are commonly consumed as part of fermented foods with specially added active live cultures; such as in yogurt or as dietary supplements. The potential application of probiotics for oral health has recently been the focus of attention for various health researchers. The number of products containing probiotics entering the market is increasing. These products usually contain streptococci, lactobacilli or bifidobacteria. The application of probiotic strategies may, in near future provide an end to many infections occurring in oral cavity.     Association Between Stress Hormone And Chronic Periodontitis

    ABSTRACT Aim: The aim of the study was to assess the association between Stress, Serum Cortisol level and Chronic Periodontitis in Police personnel of Cuddalore District, Tamilnadu, India Methodology: One hundred and ten police personnel were assigned for the study and grouped into Test (Group 1 and Group 2 ) and Controls depending on their probing pocket depth. Control group (Probing pocket depth ≤ 3 mm, n = 30), Testgroup 1 (at least four sites with probing pocket depth ≥4mm and ≤6 mm, n = 40) and Test group 2 (at least four sites with probing pocket depth> 6 mm, n = 40). The Clinical parameters such as Silness Löe plaque index, Sulcus Bleeding Index, Probing Pocket Depth and Clinical attachment level were recorded. Stress was measured usinga stress questionnaire.  Blood sample was collected and serum cortisol level was  evaluated  using ELISA. The clinical, psychological and biochemical data were  subjected to F- test (ANOVA) and Pearson’s Correlation for interpretation  of the results. Results: The mean probing pocket depth, clinical attachment level, occupational stress index score and serum cortisol level in Control was 3.02 (SD ± 0.01), 2.63 (SD ± 1.05), 79.53 (SD ± 23.57) and 125.67(SD ± 50.6) respectively; in Test group 1 it was 4.13 (SD ± 0.54), 4.68 (SD ± 0.63), 133.68(SD ± 33.23) and 187.75 (SD ± 97.84); similarly in Test group 2 it was 5.19 (SD ± 0.57), 5.92 (SD ± 0.69),158.13 (SD ± 32.44), and  212.4 (SD ± 91.8). Conclusion: The present study showed a positive correlation between stress, serum cortisol level and chronic periodontitis in police personnel

    Prosthodontic management of iatrogenic palatal defect

    Abstract Iatrogenic maxillary defects are often created following surgical resection of neoplasms or congenital malformation, but can also result following trauma. These defects cause functional disability and psychological trauma to the patient. Patients with acquired maxillary defects differ from those with congenital defects because of the abrupt alteration in physiologic processes associated with surgical resection. Subsequently they create oro-nasal and oro-antral communication leading to difficulties in mastication, deglutition, speech and cosmetic deformity. Prosthodontic rehabilitation in these patients not only addresses the functional disability, but also offers aesthetic corrections thereby improving the self confidence and the overall quality of life. A successful prosthodontic rehabilitation should recreate the partition between the oral and nasal cavity, restore facial contour, and improve mastication and speech. Authors describe the prosthetic rehabilitation following hemi maxillectomy in two patients, which showed significant improvement in function and facial esthetics.   &nbsp

    Assessment of apical leakage of different endodontic sealers -In vitro study

    Aim: The aim of this prospective study was to assess in vitro the comparison between apical leakage of Apexit Plus (calcium hydroxide based), Roth 801 (Zinc oxide eugenol based) and Perma Evolution (epoxy resin based) sealers in lateral condensation technique using linear dye leakage penetration method.Material and methods: In this study 70 freshly extracted maxillary central incisors and canines with straight root canals were used. Teeth were decoronated and step back root canal preparation using Gates Glidden drill and stainless steel hand K files was performed with size 45 being the master apical cone. After preparation, the teeth were randomly divided into 5 groups. Three groups with 20 teeth for experimental and two groups with 5 teeth as positive and negative control. Obturation was done with lateral condensation in three groups with Roth 801, Apexit plus and Perma Evolution sealers. The obturated specimens were then stored in 2% Basic fuchsin dye for 72 hours. After splitting the teeth longitudinally, each tooth has been placed on a standardized scaled paper under the microscope and digital images have been captured by this microscope. Then the digital images were processed by Analyzing Digital Image software.Results: The results showed that Roth 801 sealer leaked significantly higher than both Apexit plus and Perma Evolution sealers (p < 0.05) and no statically significant difference has been found between Apexit plus and Perma Evolution sealers (p > 0.05).Conclusion: According to the methodology proposed and based on the results of this study, it may be concluded that Roth 801 showed the most leakage but Perma Evolution and Apexit plus showed similar sealing ability. However, further in vivo studies requirement should be done to find the best root canal filling material

    Shear Bond Strength of Composite to Dentin after Various Drying Techniques and Its Micro Morphological Analysis under SEM - An In-vitro Study

    Aim: The present in-vitro study compared the shear bond strength of composite to dentin after various methods to remove excess water from the dentin. Materials and Methods: Sixty extracted human molars were used for the study that were randomly divided into 6 groups with 4 experimental (three-way syringe, cotton pellet, mini sponge, tissue paper)  and 2 control groups (over dry, over wet). The exposed dentin surfaces were acid etched with 37% phosphoric acid for 20 seconds followed by thorough rinsing with water for 15 seconds. The teeth were kept visibly moist in all the experimental groups. Adhesive agent was applied on all specimens and restored with composite resin. The specimens were then subjected to shear bond strength tests and one specimen from each group was randomly selected for micro-morphological analysis of dentin. Results and Conclusion: Statistical analysis was done and data were tabulated to determine average shear bond strength for each group.  The results indicated that the mean values in the group using tissue paper were significantly higher than the mean values in all the other experimental groups and control groups

    Application of Platelet Rich Fibrin and Osseomold Bone Graft in Different Intrabony Defects – 2 Case Reports

    ABSTRACT:    Aims & objectives: The motto behind any periodontal treatment is arrest of periodontal disease and regeneration of lost periodontium. Various treatment strategies have been employed in treatment of intrabony defects, but the best way to obtain regeneration is probably by mimicking the actual occurring events that takes place in the formation of the periodontal tissues at embryonic stage. Conventional open flap debridement falls short of regenerating tissues destroyed by the disease and current regenerative procedures offer a limited potential towards attaining complete periodontal regeneration.Platelet rich fibrin (PRF), a second generation platelet concentrate is widely used in osseous regeneration.Case description: The present study aimed to explore the clinical and radiographical effectiveness of autologous PRF along with the osseomold bone graft in treatment of 2 different cases of intrabony defects in chronic periodontics subjects.Conclusion: Among the 2 subjects, case-1 had 2-wall defect and case-2 patient had 3-wall defect. Both the subjects reported to the department with a complaint of food impaction and with clinically accessible >7-8mm pocket.  Pocket depth was assessed at 1st week, 6months and 9months respectively and radio graphically bone gain was accessed at 3 month and 6 months

    Peripheral Ameloblastoma – Controversies and Recent Concepts about Origin & Behaviour

    The peripheral ameloblastoma is a soft tissue variant of the more common intraosseous ameloblastoma. The precise nature of peripheral ameloblastoma (PA) continues to be controversial (neoplastic vs hamartomatous). Some authors believe that peripheral ameloblastoma is an innocuous lesion of hamartomatous nature without actual infiltration into the underlying bone. However over the years, rare malignant varieties of peripheral ameloblastomas have been documented. Also the etiology of peripheral ameloblastoma is unclear.  It is believed to arise directly from the overlying epithelium or from the remnants of the dental lamina located in the extraosseous soft tissue. This article attempts to review the features of peripheral ameloblastoma along with  the controversies and recent concepts regarding its origin and behavior. &nbsp
