Solitary Mandibular Lesion as the Presenting Sign of Multiple Myeloma: A Rare Case Report
Multiple myeloma (MM) is a rare hematological malignancy caused by monoclonal proliferation of plasma cells in the marrow of various bones. It is more common in men in the sixth and seventh decade of life. Patients usually present with bone pain, fatigue, recurrent infections, renal failure and nervous system dysfunction. Rarely, oral lesions may be the initial sign of multiple myeloma presenting with pain, jaw swelling, tooth mobility, multiple punched out radiolucencies and parasthesia. A case of multiple myeloma occurring in a 71 year old male patient who presented with a solitary lesion in the mandible is presented here. This paper highlights the importance of knowing oral manifestations of multiple myeloma and interdisciplinary approach required for early diagnosis
Dentigerous Cyst Associated With Supernumerary Tooth In Anterior Maxilla: A Case Report
ABSTRACT: Dentigerous cyst is a developmental odontogenic cyst which originates through alterations of the reduced enamel epithelium in a unerrupted tooth or a developing tooth bud. These cysts rarely associate with supernumerary teeth. The purpose of this article is to describe a rare case of dentigerous cyst associated with supernumerary tooth in anterior maxilla in a child. Unusual anatomy of a mandibular second molar with three separate roots: A case report
Anatomical variation in the number of roots and root canals can occur in any tooth. Aberrant morphology of the root canal system may hinder cleaning and shaping procedures, leading to failure of endodontic treatment. The aim of this study was to show one example of root canal anatomical variation and to emphasise the importance of knowledge of the internal anatomy of the tooth for the success of endodontic treatment. Mandibular second molars usually have two roots with three root canals; however, other anatomical configurations may also exist. In this case report, the author described the endodontic treatment of a mandibular second molar with three separate roots and discussed the importance of understanding, identification and treatment of this unusual anatomy
The Effect of Various Finishing and Polishing Systems on the Surface Roughness of Four Composite Resin Materials –An in Vitro Study
ABSTRACTAim: To compare the effect of three finishing and polishing systems having different abrasives and clinical steps on surface roughness of four types of composite resins i.e. microfilled composite, hybrid composite, packable composite and nanofilled composite resins.Materials and methods: The composite resins examined in this study include Microfilled composite Heliomolar® (Ivoclar Vivadent, Schaan, Liechtenstein), Hybrid composite Filtek™Z250 (3M ESPE, St Paul, MN, USA), Packable composite Filtek™P60 (3M ESPE, St Paul, MN, USA), Nanofilled composite Filtek™Z350 XT (3M ESPE, St Paul, MN, USA). Specimens (N=80 , Twenty for each composite material) measuring 10 mm in diameter x 5 mm in thickness were fabricated in split metal mould covered with a Mylar strip using four composite resins. A control group of five specimens of each material received no polishing after being cured under the Mylar strip. Fifteen specimens for each composite were randomly divided among three finishing and polishing systems (n=5) Sof- Lex™(3M ESPE, St Paul, MN, USA) OptraPol ( Ivoclar Vivadent , Schaan, Liechtenstein) Enhance®PoGo™ Complete kit ( Dentsply Caulk, Milford, DE, USA).The average surface roughness (Ra, μm) was measured with Profilometer Surfcom–130A Surface Roughness Tester (Carl Zeiss). Results were statistically analyzed by One-way analysis of variance (ANOVA) and the post-hoc Tukey’s Multiple comparison test at a p<0.05 significance level.Results: Significant differences were found for the surface roughness. The ranking order of surface roughness on the basis of the type of composite was as follows: Packable composite > Hybrid composite > Microfilled composite > Nanofilled composite; and the ranking for the finishing and polishing systems: Enhance/PoGo < Mylar strip< Sof-Lex < OptraPol. There were no statistically significant differences created between Enhance/PoGo, Mylar strip, Sof-lex, however smoother surfaces were obtained with Enhance/PoGo system Conclusion: Nanofilled Composites showed excellent polishability. Enhance/PoGo polisher produced smoothest surfaces Considering the reduced steps, application time and achievement of Mylar strip like surfaces two-step Enhance/PoGo polisher might be the preferred system for polishing composites
Adjunctive Orthodontic Treatment - A Case Report
Introduction: Adult orthodontics has gained popularity and is becoming a larger proportion of many treatments. A major reorientation of orthodontic thinking has occurred to provide the best possible occlusion, dentofacial esthetics and stability, to benefit the adult patients. Case description: A 40 year old male patient was diagnosed with a mutilated occusion and generalised spacing in both the arches. He had a convex profile with an unpleasant smile. Treatment objective was to correct spacing and prosthetic rehabilitation of the missing dentition. Discussion: Repositioning of teeth that have drifted and alignment of anteriors were the important parameters checked. Intrusion of both the arches during levelling and closing of all spaces, along with prosthodontics rehabilitation of the both the partial edentulous arches were challenging. Conclusion: Awareness and lifestyles have increased the demands for adult orthodontic treatment and interdisciplinary dental therapy has proven better management of the more complicated and unique requirements of adult patient population thus increasing the quality of care and treatment prognosis. Almost 75% of adult patients need treatment efficiency, convenience in appointment timings and good communication with other health care professionals with proper treatment execution
Bilateral Orthokeratinized Odontogenic Cyst: An Unusual Presentation and Review
Orthokeratinized odontogenic cyst (OOC) is an uncommon developmental cyst that occurs in the jaws. Because of differences in clinical behaviour and histological features, the parakeratinized variant of Odontogenic Keratocyst (OKC) is reclassified as neoplasm called ‘Keratocystic Odontogenic Tumour’ (KCOT) and orthokeratinized variant of OKC is designated as a distinct entity called OOC. The treatment for OOC is less aggressive and prognosis is much better with almost no recurrence, hence proper diagnosis is important. The incidence of OOC is approximately 5-10% of cases that had been previously diagnosed as OKC. Usually, OOC occurs as solitary lesion and KCOT presents as multiple lesions. Here, we present a rare case of bilateral OOC in the posterior mandible of a patient.  
Management of Peri Implant deficiency with PRP and bone grafts – A Case Report
The objective of this paper is to report a case that has been successfully managed by incorporating Platelet Rich Plasma ( PRP) with a combination of autogenous bone graft and a commercial xenograft (BIO-OSS). Platelet rich plasma seems to enhance early bone healing (3 months) when used along with Auto grafts and Xeno graft mixture. A Substantial increase in the area of bone was observed post operatively. In our case, Clinically, no major complications were noted after 3months when assessed using parameters like implant mobility, pain, infection, implant exposure. Therefore, the use of Platelet rich plasma along with auto graft and xeno graft mixture in peri implants deficient regions appears to be beneficial in clinical situations.  
A New Concept Of Dental Arch of Children in Normal Occlusion
The development of human dentition from adolescence to adulthood has been the subject of extensive study by numerous dentists, orthodontists and other experts in the past. While prevention and cure of dental diseases, surgical reconstitution to address teeth anomalies and research studies on teeth and development of the dental arch during the growing up years has been the main concerns across the past decades, in recent years, substantial effort has been evident in the field of mathematical analysis of the dental arch curve, particularly of children from varied age groups and diverse ethnic and national origins. The proper care and development of the primary dentition into permanent dentition is of major importance and the dental arch curvature, whose study has been related intimately by a growing number of dentists and orthodontists to the prospective achievement of ideal occlusion and normal permanent dentition, has eluded a proper definition of form and shape. Many eminent authors have put forth mathematical models to describe the teeth arch curve in humans. Some have imagined it as a parabola, ellipse or conic while others have viewed the same as a cubic spline. Still others have viewed the beta function as best describing the actual shape of the dental arch curve. Both finite mathematical functions as also polynomials ranging from 2nd order to 6th order have been cited as appropriate definitions of the arch in various studies by eminent authors. Each such model had advantages and disadvantages, but none could exactly define the shape of the human dental arch curvature and factor in its features like shape, spacing and symmetry/asymmetry. Recent advances in imaging techniques and computer-aided simulation have added to the attempts to determine dental arch form in children in normal occlusion. Intentional replantation: A viable treatment option for specific Endodontic failure – A Case Report
For many years, intentional replantation has been a treatment option for pulpless teeth and an effective strategy for teeth that would be difficult, if not impossible, to treat using traditional root canal therapy. Intentional replantation consists of extracting the tooth, finding and correcting the defect, and replanting the tooth in its socket. Intentional replantation can be indicated correctly as an alternative treatment for cases in which conservative endodontic therapy or surgical technique cannot be performed. In this article, intentional reimplantation is described and discussed as a treatment approach for failed root canal treatment with a successful case report of broken instrument and middle one third root perforation of maxillary first premolar
Inferiorly Based Nasolabial Flap As A Backup Flap – A Surgical Case Report
Introduction: Nasolabial flaps have been recognised as versatile flaps for reconstruction of variety of defects in the face, nose, lip and the oral cavity. Random pattern inferiorly based nasolabial flaps (IBNLF) have been utilised for covering various small defects over the face and oral mucosa. This is also a reliable backup flap in partial failure of primary flaps in maxillofacial reconstruction. Aim:To evaluate the outcome of a secondary defect of commisure and buccal surface arising due to partial failure of bippadle PMMC flap with an inferiorly based nasolabial flap (IBNLF). The final functional and the esthetic result are evaluated, along with the need for subsequent secondary procedure. Case: A 73-year-old male presented with a rapidly growing Squamous cell carcinoma involving right cheek. En bloc resection of right oral commissure and buccal region was performed. Modified neck dissection was performed on the right side. Reconstruction of the defect was done using bipaddle PMMC flap. Partial failure of primary flap started from 5th post operative day. Complete defect of right commissure was evident by third week posteratively. Inferiorly based nasolabial flap (IBNLF) was used as a back up flap for correction of residual defect. Conclusion: An inferiorly based Nasolabial flap (IBNLF) is a reliable backup flap for the reconstruction of small and medium sized secondary defects in the face and oral cavity. This flap can be best utilized for elderly, high risk patients in whom neck dissection with facial artery ligation have been done
