    Facing Challenges In Creating A Site for Osseointegrated Implants By Orthodontic Treatment: Case Report

    A 23 year-old patient with ankylosed upper first molar and congenitally missing lower second premolars requested prosthetic rehabilitation based on implant insertion in place of ankylosed and missing teeth. Orthodontic treatment was necessary to prepare sites for implants by opening spaces and correction of adjacent teeth tipping to these sites. Osseointegrated implants were inserted in place of extracted ankylosed first molar and missing teeth. During the orthodontic and surgical treatment of this patient several challenges were encountered and solved both surgically and orthodontically. &nbsp

    Comparison Of Plasma Lipid And Blood Glucose Level In Obese And Non Obese Patients With Moderate Periodontitis

    Background and objectives : Hyperlipidemia and  hyperglycemia are major risk factors associated with obesity. In recent years, strong evidence has been presented to support that periodontal disease is associated with increased incidence of obesity.In this study, plasma lipid and blood glucose levels are  assessed in obese and non obese patients with moderate periodontitis. Methodology : The total number of apparently healthy 80 subjects having Moderate Periodontitis were selected .Body Mass Index was calculated of all the participants and based on the BMI calculation, they are grouped as obese and non obese. The experimental group   consisted  of 40 obese with moderate periodontitis patients (group I)  and control group  consisted of 40 non obese with moderate periodontitis (group II). Gingival examination ( Modified Sulcular Bleeding Index by Lobene et al), Periodontal examination (CPI modified index approved by WHO), with probing pocket depth, gingival bleeding and attachment level were assessed . Patients with minimum periodontal pocket depth of 4- 5mm were selected for the study. Fasting lipid profiles and fasting blood sugar levels were measured in all the 80 subjects. Results: The results of the study showed very high statistical significance of gingival bleeding (p<0.001) and one of the  lipid profile variable triglycerides in  group  I.  Other variable such as total cholesterol (p=0.016) and LDL- cholesterol (p=0.023) was also statistically significant . Fasting blood glucose level was marginally high in group 1. Interpretations and Conclusions : The data from this study showed that there was a positive association of fasting lipid profile to obesity and inflammatory conditions  such as moderate periodontitis.  &nbsp

    Esthetic Crown Lengthening: Theoretical Concepts and Clinical Procedures

    Esthetic crown lengthening employs gingival tissue reduction with or without osseous surgery for tooth exposure. Central incisor is the dominant component in the anterior dental composition, with ideal height to width ratio of 10:8, and accordingly the gingival zenith for lateral and canine is adjusted. In this article we are showing three cases of crown lengthening done by calculating the height to width ratio. In first case gingivectomy is required for the crown height. In second case modified Widman flap is done to get desired result, and in third case apically displaced flap with osseous reduction is done.  &nbsp

    Root canal revascularisation-A novel biological treatment concept

    Pulp vitality is extremely important for the tooth viability, since it provides nutrition and acts as biosensor to detect pathogenic stimuli. In the dental clinic, most dental pulp infections are irreversible due to its anatomical position and organization. It is difficult for the body to eliminate the infection, which subsequently persists and worsens. The widely used strategy currently in the clinic is to partly or fully remove the contaminated pulp tissue, and fill and seal the void space with synthetic material. Over time, the pulpless tooth, now lacking proper blood supply and nervous system, becomes more vulnerable to injury. Recently, potential for successful pulp regeneration and revascularization therapies is increasing due to accumulated knowledge of stem cells, especially dental pulp stem cells.     Natural Tooth Pontic – A Case Series

    Sudden tooth loss in the esthetic zone of the maxillary or mandibular anterior region can be due to trauma, periodontal disease or endodontic failure. The treatment options for replacing the missing tooth can vary between removable prosthesis, tooth-supported prosthesis and implant-supported prosthesis. Irrespective of the final treatment, the first line of treatment would be to provisionally restore the patient’s esthetic appearance at the earliest, while functionally stabilizing the compromised arch. Using the patient’s own natural tooth as a pontic offers the benefits of being the right size, shape and colour. The abutment teeth can also be preserved with minimal or no preparation, thus keeping the technique reversible, and can be completed at the chair side thereby avoiding laboratory costs.     Coronal Microleakage in Endodontically Treated Teeth filled with Temporary Filling Materials –A Comparative study

      Introduction: Morphologic variations  of the root canal system, instrumentation techniques, obturation materials as well as temporary filling materials can influence the coronal microleakage. The aim of this study was to assess in vitro coronal microleakage of temporary filling materials in endodontically treated teeth by the method of colored solutions. Material and Methods Forty extracted human teeth (single-rooted and multi-rooted) divided into two groups of (intact and caries affected) were used. The root canals were instrumented by step-back preparation technique and 2.5% NaOCl as irigant solution. Obturation was performed by lateral condensation technique with Apexit paste and gutta-percha. Cavit and Glass Ionomer Cement were used as temporary filling materials. The solution of 50% AgNO3 was used for microleakage assessment. The penetration of the color was evaluated using Light Microscope. Results Among the study groups, 60% temporarily filled by Cavit showed the color penetration, while only 25% of the teeth temporarily filled by combination of GIC and Cavit. The difference was statistically significant (p<0.05). In caries affected teeth, the penetration of the color was observed in 55% of the samples when Cavit was used and in 32% when combination of GIC and Cavit was used as temporary filling material. The difference was statistically significant (p<0.05). Less color penetration occured in single-rooted than in multi-rooted teeth in both groups, but the differences were not statistically significant. Conclusion Glass Ionomer Cements in combination with Cavit provide better sealing of endodontically treated teeth. The least color penetration was observed in intact single-rooted teeth

    Epidemiological and intervention studies have suggested that infections are risk factors for coronary heart disease (CHD). Dental infections have appeared as CHD risk factors in cross sectional and follow up studies, and the association has been independent of the classic “coronary risk factors”. Periodontal disease , caused mainly by bacteria, is characterized by inflammation, strong immune response and loss of connective tissue attachment and bone. Periodontal disease may initiate pathological changes in blood vessel wall and act as precursor of atherosclerosis in susceptible host. Periodontal disease caused as a low grade inflammation may represent one of several possible causal factors for heart diseases to occur. C-reactive protein (CRP) has been assessed as marker of inflammation in cardiovascular diseases. This finding was subsequently reproduced by other vascular disorders.     Metal Occlusal Surface Dentures: A Case Report

    The wearing out of the occlusal surfaces of the acrylic teeth of a complete denture with its use over a period of time is a common phenomenon. Other factors which lead to the wearing of denture teeth are some unconditional forces & eating habits of the edentulous patients. Following a successful insertion of the complete denture with acrylic teeth the dentures were taken back from the patient & the occlusal surfaces have been modified with cast metal using the lost wax technique. The final restorations were cemented on the prepared plastic teeth. This article describes a simplified method for making cast metal occlusal surfaces & to improve the retention of the same with the underlying plastic structure

    Role of Interleukin-8 in Periodontal Disease

    AIM & OBJECTIVE: a) To estimate the concentration of IL-8 in health, in plaque associated gingivitis and in chronic periodontitis.b) To correlate the concentration of IL-8 with other clinical parameters of gingival and periodontal status.c)To determine if IL-8 can act as a marker/indicator in chronic periodontal disease destruction. STUDY DESIGN: A total of 75 systemically healthy patients, aged 25-50 years were included in the study. Based on the clinical evaluation and radiographic examination, the patients were divided into 3 groups - healthy, plaque associated gingivitis and chronic periodontitis. Gingival Crevicular fluid from these patients was collected using micro-capillary pipettes and the concentrations of IL-8 in the samples were determined using ELISA. RESULTS: IL-8 was detected in all three groups. In periodontally diseased subjects, the concentration of IL-8 was significantly elevated when compared to healthy subjects. CONCLUSION: The results suggested that gingival crevicular IL-8 levels increases with the increase in periodontal destruction. Based on the results of this study and previous clinical trials this article discusses the relevance of IL-8 in chronic periodontitis and its role as a biological marker

    An Overview in Management of Intraoral Bleeding

    The clinical significance of hemorrhage depends on the volume and rate of bleeding. Rapid loss of up to 20% of the blood volume or slow losses of even larger amounts may have little impact in healthy adults. Bleeding that is confined within the body cavity and is not apparent on the surface is known as internal or concealed bleeding whereas, blood escaping through a wound in the skin is known as external bleeding.
