Get PDFRésumé Objectifs : Décrire le phénomène des problèmes et erreurs reliés à la prescription des médicaments à l’Hôpital Laval ainsi que le rôle du pharmacien dans leur détection et leur prévention. Méthodologie : Durant quatre semaines, les pharmaciens de l’Hôpital Laval ont répertorié les ordonnances problématiques acheminées à la pharmacie centrale. Résultats : Durant la période à l’étude, 30 009 ordonnances ont été traitées à la pharmacie centrale, dont 740 se sont avérées problématiques. Dans 60,4 % des cas, une imprécision est à l’origine du problème de prescription. Que ce soit pour clarifier une ordonnance ou pour la faire modifier, les pharmaciens réfèrent peu à leurs collègues pharmaciens, préférant contacter l’infirmière du patient. Les interventions effectuées par les pharmaciens sont acceptées dans 88,0 % des cas. Finalement, les activités cliniques d’un pharmacien à l’unité de soins apparaissent avoir un impact positif sur la prévalence des problèmes reliés à la prescription de médicaments. Conclusion : Le pharmacien joue un rôle important dans la détection et la prévention des problèmes et erreurs reliés à la prescription des médicaments. Abstract Objective: To describe the problems and errors related to the prescription of medication at the Hôpital Laval, as well as the role of the pharmacist in their detection and prevention. Methods: For four weeks, the Hôpital Laval pharmacists tracked problematic prescriptions that were sent to the hospital’s main pharmacy. Results: During the study period, 740 of the 30,009 prescriptions evaluated at the main pharmacy were deemed problematic. In 60.4% of cases, imprecision was at the root of the problem. Whether to clarify a prescription or to have a prescription modified, pharmacists preferred to contact the patient’s nurse as opposed to referring to colleagues. The interventions done by pharmacists were accepted in 88% of cases. Finally, the work of the clinical pharmacist on the ward appears to have had a positive impact on the prevalence of problems related to the prescription of drugs. Conclusion: The pharmacist plays an important role in the detection and prevention of problems and errors related to the prescription of drugs. Get PDFAbstract Part II: Perspectives and challenges facing healthcare establishments Natural healthcare products have existed for a long time. In 2002–2003 the sales estimates in Canada of approximately 50,000 natural health care products approached 15.9 billion. Today more than 240,000 products are included in the drug product database.³ Consequently, after January 2004, Canada adopted the Natural Health Products Regulations, falling under the Food and Drugs Act.The purpose of this report, a two-part series, is to provide a perspective on the role of natural health products in Canada and to identify potential challenges for the hospital pharmacist. The first part will provide a review of regulations governing natural healthcare products, it will describe natural healthcare product utilization, and it will discuss some of the ethical concerns surrounding their use. The second part of the report will discuss the pharmaceutical issues related to natural health product use in community and hospital pharmacy as well as recommendations for the use of these products in healthcare establishments

Get PDFRésumé Objectif : Cet article décrit l’expérience faite dans un milieu où l’on a intégré le recensement des interventions cliniques des pharmaciens dans les activités quotidiennes du Département de pharmacie. Contexte : L’orientation par programme de soins dans un établissement a fourni l’occasion aux pharmaciens de réorienter leurs activités cliniques sous forme de soins pharmaceutiques prodigués aux patients. Afin d’être en mesure d’évaluer le type et la quantité des interventions ainsi que le degré d’acceptation des recommandations, nous avons effectué un recensement des interventions portant sur les problèmes relatifs à la pharmacothérapie. Nous avons également évalué l’information dispensée aux patients, les relances, les histoires médicamenteuses et les transferts de patients. Conclusion : Les résultats obtenus démontrent clairement l’importance du rôle que joue le pharmacien, en tant qu’acteur de premier plan, dans le processus de soins prodigués aux patients et son impact sur la qualité de la pharmacothérapie. Abstract Objective: A documentation tool for pharmacist collection of data on drug-related problems is described. Summary: A tool for documenting drug-related problems and pharmacist interventions was developed. Medication histories, patient counselling and transfer notes were also evaluated. Conclusion: The results clearly demonstrate the importance for the pharmacist to document their interventions
Hiérarchisation des services cliniques de pharmacie offerts à la clientèle du CSSS de Rivière-du-Loup
Get PDFRésumé Objectif: Cet article a comme objectif de décrire une façon de hiérarchiser les soins pharmaceutiques dans un centre hospitalier. Mise en contexte: Le nombre de postes de pharmaciens octroyés au CSSS de Rivière-du-Loup est en deçà des recommandations émises par le Comité d’inspection professionnelle de l’Ordre des Pharmaciens du Québec. L’équipe de pharmaciens en place est donc appelée à faire des choix quant aux services pharmaceutiques offerts à la clientèle du Centre hospitalier. Conclusion: Il existe beaucoup de documentation décrivant l’impact du pharmacien dans certains secteurs cliniques. Cet article propose une démarche simple, basée sur la description des services offerts actuellement et leur impact sur la clientèle visée, qui permet de hiérarchiser les soins pharmaceutiques dans le but de proposer à la direction du centre hospitalier un plan de gestion des effectifs réaliste. Abstract Objective: The purpose of this article is to describe the development of a hierarchy with regard to pharmaceutical care in a hospital. Context: The number of designated positions for pharmacists at the CSSS of Rivière-du-Loup is less than the number recommended by the professional inspection committee of the Quebec Order of Pharmacists. Pharmacists of this centre are asked to decide which pharmaceutical services to offer to the hospital clientele. Conclusion: Many data exist that describe the impact a pharmacist has in various clinical sectors. This article proposes a simple procedure to allow for the hierarchy of pharmaceutical services, one that is based on the description of services that are actually offered and their impact on the targeted clientele with the goal of developing a managerial plan to be used for the hospital.
