Over-the-balloon placement of the Gore Hybrid Vascular Graft in challenging clinical conditions
This report describes a useful technique for deployment of the Gore Hybrid Vascular Graft (W. L. Gore and Associates, Flagstaff, Ariz) in difficult clinical situations that create challenges in obtaining vascular control before introducing the nitinol-reinforced section of the graft. In both arterial and venous applications, introducing the Gore Hybrid Vascular Graft over a balloon allows concurrent vascular control by inflation of the balloon and easier introduction of the device into the recipient vessel by creating a smoother transition between the leading edge of the graft and the guidewire. Examples of both arterial and venous applications of this novel technique are presented

Successful off-label use of the GORE EXCLUDER Iliac Branch Endoprosthesis to preserve gluteal perfusion during staged endovascular repair of bilateral isolated hypogastric aneurysms
Endovascular repair of iliac artery aneurysms has emerged as an alternative to traditional open surgical repair. Although there is little consensus on indications to preserve hypogastric blood flow during aneurysm repair, it is well understood that complications from bilateral hypogastric occlusion may be significant. The GORE EXCLUDER Iliac Branch Endoprosthesis (W. L. Gore and Associates, Flagstaff, Ariz) received United States Food and Drug Administration approval in March 2016 for treatment of common iliac artery and aortoiliac aneurysms. Ein schneller Ortswechsel steht für viele Geschäftsleute oft an der Tagesordnung. Ein Leben aus dem Koffer stellt mehr den je unterschiedlichste Anforderungen an die Bekleidung, unserer zweiten Haut. Damit kompensieren wir den unzureichenden Schutz unseres Körpers vor äußeren Einflüssen wie Wind und Temperaturschwankungen. Im Gegensatz dazu haben Tiere im Laufe der Evolution erstaunliche Mechanismen entwickelt, sich an verschiedene Lebensräume und Situationen anzupassen. Das Chamäleon, eines der flexibelsten und wandlungsfähigsten Tiere, wurde zum Gegenstand und Inspirationsquelle dieser Diplomarbeit. Tarnen und Auffallen, Dresscode und Individualität, Klassik und Innovation stehen im Kontrast zueinander und vereinen sich in einer Kollektion. Das Konzept wurzelt in der Verhaltensanalyse von Mensch und Tier, nutzt diese Erfahrungen und interpretiert sie neu. Das von dem Diplompartner gestellte Thema eine Business- Travel Kollektion zu entwerfen, setzte zudem eine intensive Auseinandersetzung mit den Phänomenen Dresscode und Etikette voraus. Klassische, stilvolle Merkmale fließen in die Formensprache ein und befinden sich in einem spannungsvollen Kontrast zu dynamischen, asymmetrischen und organischen Linien des Chamäleons. Die Kleidung und Accessoires sollen dabei nicht nur funktionale Bekleidung sein, sondern gleichzeitig zu einem Teil seines Trägers und seines Lebens werden. This way, we also compensate the insufficient protection of our body against outer influences such as wind and variations in temperature. As a contrast, animals have developed sensational mechanisms in the course of evolution to adjust to different habitats and situations. The chameleon, one of the most flexible and versatile animals became the topic and source of inspiration of this master thesis. Disguising and attracting attention, dress code and individuality, classical concepts and innovation are in contrast to each other and are combined in one collection. The concept has its roots in the behaviour analysis of humans and animals, takes advantage of the experiences made and interprets them anew. The topic suggested by the diploma partner, the company W. L. Gore & Ass. GmbH, to design a business travel collection, required a close examination of the dress code phenomenon and of the aspects that come along with it. Classical, stylish characteristics flow into the style elements and are in a fascinating contrast to dynamic, asymmetric and organic lines of the chameleon. Early and midterm results of covered balloon-expandable stents (VBX-Gore) for endovascular treatment of chronic aorto-iliac occlusion
BACKGROUND: Endovascular treatment is nowadays accepted as first-line treatment for most patients with aorto-iliac obstructive disease (AIOD), including those with Trans-Atlantic Inter-Society Consensus II (TASC-II) lesion types C and D. Aim of the study was to evaluate the role of Viabahn VBX (W. L. Gore and Associates, Flagstaff, AZ, USA), in patients with chronic occlusive aorto-iliac disease (ChO). METHODS: A retrospective review of patients undergone elective endovascular repair with VBX (W. L. Gore and Associates) for ChO at five referral vascular institutions in Italy from 2018 to 2022 was conducted. Primary endpoints were technical success and the incidence of any early and midterm procedure-related reintervention. Secondary endpoints were clinical success, primary and secondary patency. RESULTS: Among 89 included patients, technical success was obtained in 87 patients (97.8%). Postoperative complication requiring early surgical reintervention was necessary in 5 (6%), all but one for arterial bleeding. Clinical improvement of at least 3 classes was observed in 49 (55%). After a mean follow-up of 24.1 months ±14.1, primary patency and freedom from any procedure-related reintervention at 12, 24 and 36 months was 83.0% and 94.8%, 80.0% and 91.5%, 77.9% and 89.3%, respectively. Secondary patency was 100%. TASC-II D lesion (OR=3.67, 95% CI: 1.1289-11.9823, P=0.03) and Grade III iliac calcification (OR=3.41, 95% CI: 1.0944-10.6428, P=0.03) were identified as independent predictors for procedure-related reintervention. CONCLUSIONS: Use of VBX (W. L. Gore and Associates) in ChO was safe and effective with low rate of stenosis/occlusion in the early and mid-term follow-up. U.S.A., 28 April 1981, Research Submersible Johnson Sea Link 1, R/V Johnson cruise 121, dive 1023, 80 m, within Oculina rubble. Distribution. Known only from the type locality, Sebastian Pinnacles, off Sebastian Inlet, Indian River County, 27 °50.2’N, 78 °58.0’W, Florida, U.S. A Measurements. Carapace length 2.2 mm, carapace width 1.9 mm, front-orbital width 1.08 mm. Diagnosis (modified from Gore 1986: 143) Carapace longer than broad, smooth, subpentagonal, front noticeably produced, margin broadly truncate, oblique, with heavy fringe of anteriorly-directed setae extending backwards along anterolateral margin. MXP 3 with ischium-merus indistinguishably fused, merus truncated; carpus longer than wide; propodus longer than carpus, subtrapezoidal, outer surface lacking transverse ridge as described and figured in original account, dactylus small, shorter than propodus, digitiform, inserted in angular notch in middle third of ventral margin of propodus, falling short of end of propodus. P 2 -P 5 symmetrical, margins dorsally, ventrally hirsute, surface of merus, carpus, propodus of P 3 -P 4 with additional long swimming setae. Abdomen with somites 2, 3, 6 –telson fused, clear suture present between somite 6, telson. Remarks. Gore (1986) was precise in his generic assignment of Fabia felderi, although his description and illustration of the third maxilliped and abdomen of his very small specimen was incorrect. The original description of some characters was for this reason inconsistent with the male diagnosis of Fabia provided by Campos (1996) and Campos & Manning (1998), particularly in the supposed structures of the third maxilliped and abdomen. A reexamination of the holotype male confirms that F. f e l d e r i agrees with our current understanding of Fabia. Fabia felderi is actually morphologically close to F. emiliai and might be the northern counterpart of this South Atlantic species, but differing in the fusion of the abdominal somites and shape of the first gonopods. In F. felderi, male abdominal somites 2, 3 and somite 6 –telson are fused, instead of only somite 6 -telson fused as in F. emiliai (Fig. 2 B, 4 C) and the first gonopods are relatively less curved in F. felderi (Fig. 2 C, 4 E).
