    Factors influencing wolf Canis lupus roadkills in Northwest Spain

    International audienceRoadkill is one of the most prominent causes of wildlife mortality. Much research effort has focussed on collisions with ungulates because of traffic safety. However, studies about large carnivore roadkills are scarce despite vehicles being a main cause of mortality. The absence of studies can be explained in part because of difficulties in obtaining sufficient sample sizes. We collected data from locations of 82 wolf roadkill sites in the Castilla y León Region, northwest Spain. We evaluated different models to characterise collision localities using logistic regressions with corrections for rare events. The best models included traffic and human disturbance parameters. Landscape variables did not improve predictive power. Fencing was a decisive key predictor; roadkill was proportionally higher along fenced highways than on similar major roads that lacked fences. Wolf-vehicle collisions were more common in agricultural areas, although wolf densities were lower in these zones. Both the higher density of important roads and a greater proportion of roaming wolves on the plateau may explain this pattern

    Shear Wave Elastography for measuring the elastic properties of the Psoas Major muscle : An intra- and inter-examiner reliability study

Its evaluation considers clinical history, which can be confused with other lumbar and lower limb conditions, physical examination, with an inconsistent palpation, and imaging tests that provide inconclusive parameters in various studies. Therefore, developing reliable assessment procedures to evaluate PM elastic properties is necessary to improve diagnosis and follow-up. This study aimed to evaluate the intra and inter-examiner reliability of shear wave elastography (SWE) for calculating the PM stiffness in patients with LBP. Design Observational study. Methods Longitudinal views of the of the PM muscle using SWE were acquired bilaterally in 52 volunteers experiencing moderate LBP and disability. All measurements were performed twice, involving the assessment of shear wave speed and Young’s modulus as indicators of stiffness, with data collected by an experienced examiner and a novice examiner. Results Test-retest reliability showed strong consistency regardless of the examiner’s level of experience, with intraclass correlation coefficients (ICCs) exceeding 0.9 for both metrics. However, experienced examiners achieved smaller minimal detectable changes. Inter-examiner reliability was comparatively lower, with ICCs ranging from 0.854 to 0.925, and notable differences in mean values between examiners were observed (p<0.01). Conclusion Excellent test-retest reliability was observed for the measurement of PM muscle stiffness in chronic LBP patients using SWE by both experienced and novice evaluators, although statistically significant differences were found between the two trials for the novice examiner. Inter-examiner reliability was lower, highlighting variability between assessors. To minimize errors and improve consistency and accuracy, if multiple examiners are involved, averaging measurements is recommended.Peer reviewe

    Antecedentes: La leishmaniosis visceral canina es una de las enfermedades zoonosicas con mayor importancia en América latina. En esta enfermedad, la cual es producida por la leishmania infantum, el perro cumple el papel de reservorio y a su vez, el de infectado, es por esto que alarma la falta de reportes de caso realizados en las zonas urbanas, ya que el estudio epidemiológico exige un alto grado de atención que beneficiaria la prevención de infección humana, teniendo en cuenta el revuelo en los últimos años del perro como animal de compañía. Objetivo: Elaborar una revisión sistemática de literatura, con base en un caso clínico de un paciente canino (lupus familiaris) de raza pug, positivo a leishmaniosis canina visceral diagnosticado en la clínica veterinaria animales de compañía de la Universidad Cooperativa de Colombia, sede Bucaramanga. Diseño: Revisión sistemática de literatura. Fuentes Información: PubMed, Science Direct, Scielo, Redalyc, Scopus, con un protocolo acorde a la guía PRISMA. Selección de Estudios: Inicialmente 9.907 artículos científicos y revisiones bibliográficas, una segunda selección de 8.314 artículos, seguido de una selección de 5.350 y por último 209 estudios seleccionados de los cuales se usaron 47. Criterios de Exclusión: Artículos y estudios que correspondían a leishmaniosis cutánea y leishmaniosis en humanos, estudios en idiomas diferentes a español, inglés y portugués, aquellos estudios cuya fecha de publicación era inferior al año 2015 y todos aquellos que correspondían a literatura gris o que profundizaban en temas como inmunología y biología molecular. Resultados: De 9.907 artículos revisados en las seis bases de datos escogidas, se seleccionaron 5.350 que cumplieron con los criterios de inclusión. Posteriormente se realizó una tercera selección, en donde se obtuvieron un total de 47 artículos en los cuales se evidenció la problemática actual para el tratamiento y control de la leishmaniosis visceral en caninos siendo este un problema de tipo zoonosico. Conclusiones: Existen diversas presentaciones de leishmania las cuales pueden llegar a afectar a la especie canina, siendo estas de mayor relevancia en zonas selváticas o rurales donde su prevalencia se ve aumentada por la presencia del vector, considerando que por el contario, este mismo vector tiene una baja presentación en el área urbana. Aunque se ha reportado que es más común la presentación de leishmania cutánea en pacientes que llegan a la zona urbana los cuales informan haber tenido presencia en zonas de alta prevalencia del vector, podemos apreciar en este caso en particular que nuestro paciente a pesar de no haber tenido contacto con zona rural se encontró positivo a todas las pruebas establecidas a leishmania visceral. Podemos concluir que debido a diferentes factores posibles de comportamiento ambiental, el desplazamiento de dichos vectores se está haciendo más común en las zonas urbanas permitiendo así, un aumento de los casos de este tipo de parásito. Es importante tener en cuenta que este tipo de patologías son de carácter zoonosico, por tal motivo, se debería implementar planes que permitan un control y vigilancia del comportamiento epidemiológico de la enfermedad. El uso de los fármacos para este tratamiento está restringido a manejo intrahospitalario e institucional para las fuerzas armadas o militares lo cual deja una brecha que va encaminada hacia aquellos animales que viven en el ámbito rural y están expuestos tanto al vector como a la enfermedad, pero que no pueden ser tratados y terminan por convertirse así en un reservorio importante para la diseminación de la enfermedad.Background: Canine visceral leishmaniosis is one of the most important zoonosic diseases in Latin America. In this disease caused by leishmania infantum, the dog fulfills the role of reservoir and in turn that of infected, which is why the lack of case reports carried out in urban areas is alarming, since the epidemiological study requires a high degree of attention. It benefits the prevention of human infection taking into account the commotion in recent years of the dog as a pet. Objective: Prepare a systematic literature review based on a clinical case in a canine patient (lupus familiaris) of the pug breed, positive for visceral canine leishmaniosis diagnosed in the veterinary clinic of companion animals of the Cooperative University of Colombia, Bucaramanga headquarters Design: Systematic review of literature. Sources of Information: PubMed, Science Direct, Scielo, Redalyc and Scopus with a protocol according to the PRISMA guide. Exclusion Criteria: Articles and studies that corresponded to cutaneous leishmaniosis and human leishmaniosis, studies in languages other than Spanish, English and Portuguese, those studies whose publication date was less than 2015 and all those that corresponded to gray literature or that deepened in subjects such as immunology and molecular biology. Results: Of 9.907 articles reviewed in the six databases chosen, 5.350 were selected that met the inclusion criteria. Subsequently, a third selection was made, where a total of 47 articles were obtained in which the current problem for the treatment and control of visceral leishmaniosis in canines was evidenced, this being a zoonotic type problem. Conclusions: There are various presentations of leishmania which can affect the canine species, these being of greater relevance in jungle or rural areas where its prevalence is increased by the presence of the vector, considering that on the contrary, this same vector has a low presentation in the urban area. Although it has been reported that the presentation of cutaneous leishmania is more common in patients arriving in the urban area who report having had a presence in areas of high vector prevalence, we can appreciate in this particular case that our patient despite not having had contact with rural areas, all tests established for visceral leishmania were positive. We can conclude that due to different possible factors of environmental behavior, the displacement of these vectors is becoming more common in urban areas, thus allowing an increase in cases of this type of parasite. It is important to bear in mind that this type of pathology is zoonotic in nature, for this reason plans should be implemented that allow control and surveillance of the epidemiological behavior of the disease.The use of drugs for this treatment is restricted to in-hospital and institutional management for the armed forces or the military, which leaves a gap that is aimed at those animals that live in rural areas and are exposed to both the vector and the disease, but that they cannot be treated and thus end up becoming an important reservoir for the spread of the disease.1.     Humans with inherited MyD88 and IRAK-4 deficiencies are predisposed to hypoxemic COVID-19 pneumonia

    X-linked recessive deficiency of TLR7, a MyD88- and IRAK-4-dependent endosomal ssRNA sensor, impairs SARS-CoV-2 recognition and type I IFN production in plasmacytoid dendritic cells (pDCs), thereby underlying hypoxemic COVID-19 pneumonia with high penetrance. We report 22 unvaccinated patients with autosomal recessive MyD88 or IRAK-4 deficiency infected with SARS-CoV-2 (mean age: 10.9 yr; 2 mo to 24 yr), originating from 17 kindreds from eight countries on three continents. 16 patients were hospitalized: six with moderate, four with severe, and six with critical pneumonia, one of whom died. The risk of hypoxemic pneumonia increased with age. The risk of invasive mechanical ventilation was also much greater than in age-matched controls from the general population (OR: 74.7, 95% CI: 26.8-207.8, P < 0.001). The patients' susceptibility to SARS-CoV-2 can be attributed to impaired TLR7-dependent type I IFN production by pDCs, which do not sense SARS-CoV-2 correctly. Patients with inherited MyD88 or IRAK-4 deficiency were long thought to be selectively vulnerable to pyogenic bacteria, but also have a high risk of hypoxemic COVID-19 pneumonia.sponsorship: We thank the patients and their families for placing their trust in us.     Prognostic Factors of Late-onset Hearing Loss in Infants With Congenital Cytomegalovirus and Normal Audiologic Assessment at Birth

    Children with congenital cytomegalovirus (cCMV) can develop late-onset sensorineural hearing loss (LO-SNHL). In this study, we aim to assess the characteristics and predictors of LO-SNHL in infants with cCMV having normal hearing at the first neonatal assessment. A retrospective study within the European Registry of Children with cCMV ( www.ccmvnet.org ) was performed. Included children had cCMV and a normal first audiological assessment by Auditory Brainstem Response (ABR). Late-onset hearing loss (LO-SNHL) is defined as the presence of sensorineural hearing loss after an initial normal hearing test. Hearing evaluation was performed at birth, at 6 months of age, and at least annually up to 6 years of age. Seven hundred twenty-one children with normal audiological tests at birth were included, and 47/721 (6.5%) developed LO-SNHL. LO-SNHL was diagnosed at a range of 4-65 months of age [median (IQR) age: 34.3 (15.1-48.7) months]. Children with LO-SNHL had a higher proportion of abnormalities on physical examination at birth (45.7% vs. 20.8%; P < 0.001): petechiae (17.4% vs. 6.0%; P = 0.008), splenomegaly (8.7% vs. 2.3%; P = 0.031), hepatomegaly (13% vs. 2.9%; P = 0.001), microcephaly (15.2% vs. 4.5%; P = 0.005) and small for gestational age (21.7% vs. 8.3% P = 0.005). Children with LO-SNHL showed lower platelet count at birth [177500.0 (88750.0-261250.0)/μL vs. 243500.0 (173000.0-304000.0)/μL; P = 0.012], and higher blood viral load at birth [3.7 log (3.3-4.4) vs. 3.4 log (2.7-3.9) IU/mL; P = 0.013] and had more frequent white matter involvement (27.7% vs. 14.7%; P = 0.03) and ventriculomegaly (20.7% vs. 4.6%; P = 0.001) on birth magnetic resonance imaging. Overall, symptomatic children at birth showed a higher risk of developing LO-SNHL than asymptomatic children (32/317, 10.1%, vs. 15/404, 3.7%; P < 0.0001). Among asymptomatic children, only 0.3% developed severe or profound LO-SNHL in the best ear.In multivariate logistic regression analysis, ventriculomegaly [odds ratio (OR): 7.503 (1.78-27.9)], white matter abnormalities [OR: 3.19 (1.010-9.01)], and splenomegaly [OR: 3.679 (1.56-8.506)] at birth were associated with the development of LO-SNHL ( Fig. 1 ). Among this large cohort of children with cCMV and a first normal audiological assessment, the risk of LO-SNHL was 6.5%. Asymptomatic children developed LO-SNHL in 3.7% of the cases versus 10.1% in symptomatic cases.     SARS-CoV-2 infection and venous thromboembolism after surgery: an international prospective cohort study

    none10020SARS-CoV-2 has been associated with an increased rate of venous thromboembolism in critically ill patients. Since surgical patients are already at higher risk of venous thromboembolism than general populations, this study aimed to determine if patients with peri-operative or prior SARS-CoV-2 were at further increased risk of venous thromboembolism. We conducted a planned sub-study and analysis from an international, multicentre, prospective cohort study of elective and emergency patients undergoing surgery during October 2020. Patients from all surgical specialties were included. The primary outcome measure was venous thromboembolism (pulmonary embolism or deep vein thrombosis) within 30 days of surgery. SARS-CoV-2 diagnosis was defined as peri-operative (7 days before to 30 days after surgery); recent (1–6 weeks before surgery); previous (≥7 weeks before surgery); or none. Information on prophylaxis regimens or pre-operative anti-coagulation for baseline comorbidities was not available. Postoperative venous thromboembolism rate was 0.5% (666/123,591) in patients without SARS-CoV-2; 2.2% (50/2317) in patients with peri-operative SARS-CoV-2; 1.6% (15/953) in patients with recent SARS-CoV-2; and 1.0% (11/1148) in patients with previous SARS-CoV-2. After adjustment for confounding factors, patients with peri-operative (adjusted odds ratio 1.5 (95%CI 1.1–2.0)) and recent SARS-CoV-2 (1.9 (95%CI 1.2–3.3)) remained at higher risk of venous thromboembolism, with a borderline finding in previous SARS-CoV-2 (1.7 (95%CI 0.9–3.0)). Overall, venous thromboembolism was independently associated with 30-day mortality (5.4 (95%CI 4.3–6.7)). In patients with SARS-CoV-2, mortality without venous thromboembolism was 7.4% (319/4342) and with venous thromboembolism was 40.8% (31/76). Patients undergoing surgery with peri-operative or recent SARS-CoV-2 appear to be at increased risk of postoperative venous thromboembolism compared with patients with no history of SARS-CoV-2 infection.     The aim of this article is to show, using Belén Gopegui’s novel "Quédate este día y esta noche conmigo" (2017), some links between literature and socio-political engagement in our hypertechnological environment, since in it the author focuses her attention on Google as the epitome of contemporary society. In order to highlight her criticisms and proposals, the text will be brought into dialogue with different essays on the above-mentioned enterprise. In this way, it will become clear which are the most prominent aspects considered by Gopegui, in narrating the experiences of a university student and an ill sexagenarian, to realize her claim and to provide alternatives. Indeed, if on the one hand she reveals the connections between Google and capitalism and the consequences that this entails for ordinary citizens, on the other she suggests some strategies that can be used against a “common sense” that nowadays is increasingly determined by algorithms.El objetivo del presente artículo es mostrar a través de la novela "Quédate este día y esta noche conmigo" (2017), de Belén Gopegui, algunas imbricaciones entre literatura y compromiso políticosocial en el contexto hipertecnológico contemporáneo, ya que en ella la autora centra su atención en Google como epítome de nuestra sociedad. Para resaltar sus críticas y propuestas, se hará dialogar el texto con diferentes ensayos elaborados en el ámbito de los estudios culturales sobre dicha empresa. Se pondrán así de manifiesto cuáles son los aspectos en que Gopegui, aprovechando las vivencias de un joven universitario y de una sexagenaria enferma, insiste para realizar su denuncia y brindar alternativas disidentes. En efecto, si por un lado desvela las conexiones con el capital de Google y sus consecuencias en los ciudadanos de a pie, por el otro, sugiere formas de resistencia contra un «sentido común» cada vez más determinado por los algoritmos
