    Junior doctors' experiences with interprofessional collaboration:Wandering the landscape

    Context: The transition from medical student to junior doctor is challenging. Junior doctors need to become part of the physician community of practice (CoP), while dealing with new responsibilities, tasks and expectations. At the same time, they need to learn how to navigate the frontiers and intersections with the other communities of practice that form the Landscape of Practice (LoP). This study aims to understand how junior doctors experience interprofessional collaboration (IPC) and what elements shape these experiences considering their transition to clinical practice.Methods: In this multicentre qualitative study, 13 junior doctors individually drew two rich pictures of IPC experiences, one positive and one negative. A rich picture is a visual representation, a drawing of a particular situation intended to capture the complex and non-verbal elements of an experience. We used semi-structured interviews to deepen the understanding of junior doctors' depicted IPC experiences. We analysed both visual materials and interview transcripts iteratively, for which we adopted an inductive constructivist thematic analysis.Results: While transitioning into a doctor, junior doctors become foremost members of the physician CoP and shape their professional identity based on perceived values in their physician community. Interprofessional learning occurs implicitly, without input from the interprofessional team. As a result, junior doctors struggle to bridge the gap between themselves and the interprofessional team, preventing IPC learning from developing into an integrative process. This professional isolation leaves junior doctors wandering the landscape of practice without understanding roles, attitudes and expectations of others.Conclusions: Learning IPC needs to become a collective endeavour and an explicit learning goal, based on multisource feedback to take advantage of the expertise already present in the LoP. Furthermore, junior doctors need a safe environment to embrace and reflect on the emotions aroused by interprofessional interactions, under the guidance of experienced facilitators.</p

    El Trabajo Social y la identidad gay : Una experiencia de intervención con grupos en condición de vulnerabilidad al VIH/ITS desde las acciones de Programa Compañeros, A.C.

    The following is a text that briefly introduces the author\u27s experience as a social worker from Programa Compañeros, A.C., a civil society organization in Juarez City, Mexico, which since 1986 has been working on HIV prevention through peer counselors. In this paper, the author tells us how his work was based on professional knowledge (training in Social Work) hand in hand with knowledge derived from the assumption of sexual identity and how these two elements helped to perform good interventions with men of gay identity, therefore, we talk about the different actions taken to prevent HIV transmission in this population by using the Face-to-Face methodologies and the Personalized Cognitive Counseling methodologies applied from the guidance and peer learning. Other aspects such as the Human Rights framework applied in Social Work and some reflections are also addressed.El siguiente es un texto que de manera breve da a conocer la experiencia que el autor tuvo como trabajador social desde Programa Compañeros, A.C., una organización de la sociedad civil en Ciudad Juárez, en México, la cual desde 1986 trabaja en la prevención del VIH por medio orientadores pares. En este artículo, el autor nos da a conocer cómo su trabajo se basó en conocimiento profesional (formación en Trabajo Social) de la mano con el conocimiento derivado de la asunción de la identidad sexual y cómo estos dos elementos ayudaron a realizar buenas intervenciones con hombres de identidad gay, por lo tanto, se habla sobre las distintas acciones emprendidas para prevenir la transmisión del VIH en esta población mediante el uso de las metodologías Cara a Cara y la de Consejería Cognitiva Personalizada aplicadas desde la orientación y el aprendizaje de pares. También se abordan otros aspectos como el marco de los Derechos Humanos aplicados en el Trabajo Social y algunas reflexiones finales

    Avaliação da adaptação passiva de próteses fixas aparafusadas sobre implantes: estudo in vitro

Programa de Pós-Graduação em OdontologiaO objetivo deste trabalho foi analisar in vitro o grau de desajuste da adaptação passiva de uma prótese fixa de três elementos aparafusada sobre dois implantes. O estudo baseou-se nos dados fornecidos por 18 corpos de prova. Estes corpos de prova foram divididos em três grupos: grupo 1 onde as estruturas foram fundidas em monobloco (sem solda), grupo 2 onde as estruturas foram unidas através do método convencional ou brasagem e grupo 3 onde as peças foram unidas através de solda a laser. A leitura dos desajustes foi realizada em um microscópio da marca Nikon, modelo Measurescope. Os resultados foram submetidos a tratamento estatístico de análise de variância (ANOVA). O grupo 1 (sem solda) apresentou os maiores valores de desajuste. Nos outros dois grupos, os desajustes foram semelhantes

    This article focus on how Varro (116-27 BC), along with the studies of Cleanthes(331-232 BC), from the of Pergamon School, those of Aristophanes of Byzantium (257-187 BC), from the Alexandria school, and departing from the grammatical definitions of Dionysius Thrax, which is divided into six parts with analogy being one of them –incorporated a popular Greek controversity between analogists and anomalists into the first Latin work on theory of grammar (43 BC), which consists of twenty five books. From the remaining books, V to X, the VIII focuses on the counterarguments for analogy; the IX focuses on the counterarguments for anomaly; and, in the book X, Varro exposes his personal opinion about analogy – this way, through varied linguistic facts, the author shows arguments that try to justify the side taken by one or another tendency in facing the discussion regarding the origin of languages.Este artigo visa a discutir como Varrão (116-27 a.C.), combinando os estudos de Cleantes (331-232 a.C.), da escola de Pérgamo, com os de Aristófanes de Bizâncio (257-187 a.C.), da escola de Alexandria, e partindo das definições da gramática de Dionísio Trácio – dividida em seis partes, sendo a analogia uma delas –, incorporou a famosa controvérsia grega entre analogistas e anomalistas ao primeiro trabalho latino de teoria gramatical (43 a.C.), constituído de vinte e cinco livros.     Patients' attitude towards residents' participation during gynaecological surgery

    Objective: To evaluate patients' attitude towards participation of residents in training in gynaecological surgery and patients understanding of resident position. Study design: An observational survey study was performed between July and February 2007-2008 in an outpatient clinic of a department of obstetrics and gynaecology of a teaching hospital. Patients planned for gynaecological surgery were concerned eligible. 247 questionnaires were handed out of which 204 were returned (response rate 82.6%). Primary outcome measure was level of confidence in a resident performing hysterectomy in various operative settings. Secondary, patients' understanding of the medical education system was tested. Results: There was no difference between confidence in a specialist performing a hysterectomy compared to an experienced resident considered competent and under supervision of a specialist. Respondents have significantly less confidence in less experienced residents operating under supervision and in all residents operating without supervision (P <0.001). From the respondents, 85.6% believes a resident in training has received less education than a resident not in training. 38.6% believes a resident received less training than a medical student. Statistical analysis was done in SPSS 16.0 (SSPS Inc., Chicago, IL, USA) using a Wilcoxon signed-rank test. Conclusions: Respondents have confidence in a resident in training performing an operation under supervision when it concerns an experienced resident that is considered competent. A majority of respondents does not understand the position of a resident in training. Patient education on resident participation should be improved and the certainty that an operating resident is competent should be emphasized. (C) 2010 Elsevier Ireland Ltd. All rights reserved

    Deconstructing conformal blocks in 4D CFT

    We show how conformal partial waves (or conformal blocks) of spinor/tensor correlators can be related to each other by means of differential operators in four dimensional conformal field theories. We explicitly construct such differential operators for all possible conformal partial waves associated to four-point functions of arbitrary traceless symmetric operators. Our method allows any conformal partial wave to be extracted from a few “seed” correlators, simplifying dramatically the computation needed to bootstrap tensor correlators. © 2015, The Author(s)

    Tese (doutorado) - Universidade Federal de Santa Catarina, Centro de Ciências da Saúde, Programa de Pós-Graduação em Enfermagem, Florianópolis, 2015.A mudança do perfil epidemiológico do HIV/aids a partir da implementação da terapia antirretroviral vem modificando o acompanhamento das pessoas que vivem com HIV/aids, suscitando novas demandas e preocupações, como a transição do cuidado aos adolescentes soropositivos para o HIV do serviço de saúde infantil ao adulto. Diante da cronicidade da aids e necessidade de um acompanhamento contínuo, a transição do adolescente com HIV/aids por transmissão vertical, do serviço de saúde infantil ao adulto torna-se inevitável, trazendo consigo preocupação aos profissionais que acompanham esse público. Diferentes fatores, durante o processo de transição, interferem na aceitação pelo adolescente desse processo de mudança como a capacidade do adolescente em se adaptar. O adolescente que vive com HIV/aids representa um grupo que demanda desafios e necessidades especiais de cuidado, tais como suporte emocional, acompanhamento por diferentes profissionais, educação contínua e novos saberes e práticas de cuidar. Assim, o objetivo do estudo foi compreender o significado que os adolescentes com HIV/aids atribuem ao processo de transição de cuidado de saúde do serviço de referência infantil para o serviço de referência adulto. Estudo de natureza qualitativa, descritivo, teve como referencial teórico o Interacionismo Simbólico. Foi realizado em um serviço de Referência estadual no controle e tratamento de pessoas com HIV/aids, localizado em Florianópolis/SC, vinculado a Secretaria do Estado da Saúde de Santa Catarina (SES/SC) e em um Serviço Especializado em DST/aids na Secretaria Municipal de Saúde do município de São José/SC. A coleta dos dados ocorreu no período de agosto a novembro de 2014 através de entrevista dialogada e individual, utilizando um roteiro semiestruturado. Participaram da pesquisa 17 adolescentes, com diagnóstico de infecção pelo HIV por transmissão vertical, com conhecimento prévio de seu diagnóstico, que vivenciaram a transição do serviço de saúde infantil ao serviço de saúde adulto e em acompanhamento nestes serviços de saúde. Os dados foram analisados a partir da Análise de Conteúdo, que levou a formação das categorias e temas. Os resultados foram organizados e apresentados em dois manuscritos, o primeiro intitulado ?O significado de viver com HIV/aids na adolescência: limitações e expectativas?, que descreve as repercussões de viver com HIV/aids na adolescência frente a orfandade e proteção familiar, influências sob os relacionamentos, os limites e expectativas quanto ao tratamento e as dicotomias entre o ser12adolescente igual e diferente dos outros no viver com a doença. O segundo manuscrito aborda ?Os significados da transição entre os serviços de saúde para o adolescente com HIV/aids?, discute a repercussão da revelação do diagnóstico no tratamento e transição do adolescente, o processo de amadurecimento e o impacto da transição no seguimento dos cuidados e adesão ao tratamento, demonstrando a importância do acolhimento e da construção e reconstrução dos vínculos com a equipe de saúde. No processo de vida do adolescente com HIV/aids, pode ser determinante na maneira como a doença é vista por eles, como também as relações de preconceito e estigma, sendo as relações interpessoais símbolo da dificuldade de aceitação de sua condição crônica. A medida que o adolescente busca se parecer com os demais adolescentes, a necessidade de assumir uma rotina de cuidados e tratamentos imposta pelo viver com HIV/aids torna o adolescente diferente dos demais, sendo a busca pela auto-aceitação uma constante para esses adolescentes, fato que deve ser considerado pelos profissionais de saúde. Dessa maneira, a transição de serviço pode ser muito dolorosa para alguns adolescentes, sobretudo pelo desconhecimento do novo serviço e pela mudança de abordagem adotada pelos profissionais, ou, bem sucedida quando encontra acolhimento adequado, se corresponsabiliza e ao representar o seu crescimento e a inserção na vida adulta. O estudo permitiu compreender os significados atribuídos pelos adolescentes no viver com HIV/aids na fase da adolescência e no processo de transição, e nos levou a compreender que tais significações precisam ser consideradas na construção de um processo de transição positivo e no cuidado integral a esse paciente. Somente um novo olhar sobre o processo de transição, fundamentado nos significados de viver com HIV/aids na adolescência é que pode nos permitir desenvolver ações que façam diferença no processo de saúde-doença desses adolescentes, servindo de auxílio para diminuir o sofrimento e promover sua saúde.Abstract : The change in the epidemiological profile of HIV / AIDS from the implementation of Antiretroviral Therapy is changing the monitoring of people living with HIV / AIDS, raising new demands and concerns, such as care from transition to HIV positive adolescents to HIV health service child to adult. Given the chronic nature of AIDS and the need for continuous monitoring, the transition of adolescents with HIV / AIDS by vertical transmission of child health to adult service is inevitable, bringing concern to professionals who assist the public. Different factors during the transition process, interfere acceptance by adolescents this process of change as the adolescent's ability to adapt. The teenager living with HIV / AIDS is a group that demand challenges and special care needs, such as emotional support, monitoring by different professionals, continuing education and new knowledge and practice of care. The objective of the study is to understand the meaning that adolescents with HIV / AIDS attribute to the child reference service health care transition to the adult reference service. Study of qualitative, descriptive nature, the theoretical reference the Symbolic Interaction. It was held in a state reference service in the control and treatment of people with HIV / AIDS, located in Florianópolis / SC, linked to the State Secretariat of Health of Santa Catarina (SES / SC) and a specialized service for STD / AIDS in Municipal county Health St. Joseph / SC. Data collection took place between August-November 2014 through dialogue-based and individual interviews, using a semi-structured script. The participants were 17 adolescents with diagnoses of HIV infection by vertical transmission, with prior knowledge of their diagnosis, they experienced the transition from child health service to adult health services and in monitoring these health services. Data were analyzed from the content analysis, which led to the formation of the categories and themes. The results were organized and presented in two manuscripts, the first entitled "The meaning of living with HIV / AIDS in adolescence: limitations and expectations", which describes the effects of living with HIV / AIDS in adolescence front orphanhood and family protection, influences in relationships, the limit and expectation to treatment and dichotomies between the teenager be equal and different from others in living with the disease. The second, entitled "The meaning of the transition between health services for adolescents with HIV / AIDS", discusses the impact of the disclosure of diagnosis in the treatment and adolescent transition, the process of maturation and the impact of transition following the care and adherence to treatment,14demonstrating the importance of the host and the construction and reconstruction of links with the health team. In adolescent life process with HIV / AIDS can be decisive in how the disease is seen by them as well as the relations of prejudice and stigma, so that interpersonal relationships are symbol of the difficulty of acceptance of their chronic condition. As the adolescent search resemble the other adolescents, the need to take routine care and treatment imposed by living with HIV / AIDS makes it different from other teenagers, and the search for self acceptance a constant for these teenagers, a fact that should be considered by health professionals. Thus, the service transition can be very painful for some adolescents, especially by ignorance of the new service and the approach of change adopted by professionals, or successful when it finds suitable host, if corresponsabiliza and represent its growth and integration in adulthood. The study allows us to understand the meanings attributed by adolescents in living with HIV / AIDS in adolescence phase and the transition process, and led us to understand that such meanings need to be considered in building a positive transition process and comprehensive care to this patient. Only a new perspective on the transition process, based on the meanings of living with HIV / AIDS in adolescence is that it can allow us to develop actions that not only provide, but they do different in the health-disease process of these adolescents, serving as aid to reduce suffering and promote their health
