    Yksilöllisyyden huomioon ottaminen hoitotyössä on paitsi juridinen ihmisoikeus ja jokaisen potilaan oikeus, myös eettinen oikeus. Yksilöllisen kohtaamisen tärkeydestä ja yksilöllisyydestä hoidon tavoitteena olemme hoitotyössä yksimielisiä. Huomattavasti vaikeampi on hyväksyä sitä, että tutkimustulosten mukaan etenkin iäkkäiden kohdalla osa toteutumatta jäävää hoitotyötä on juuri yksilöllinen ja persoonakeskeinen hoito.</p

    Resursförstärkande mångprofessionell yrkespraxis; social- och hälsovårdsutbildning i utveckling

    Social- och hälsovården förändras och de nya utmaningarna förutsätter ett mångprofessionellt samarbete där yrkeskompetenser förenas för klientens bästa. Ett mångprofessionellt arbetssätt kräver också en mångprofessionellt inriktad utbildning, vilken förverkligats inom Yrkeshögskolan Novias avdelning för vård och det sociala området genom utvecklingsprojektet ”Mångprofessionell social- och hälsovård”. Projektet har resulterat i en ny utbildning där sjukskötare, hälsovårdare och socionomer redan inom utbildningen får erfarenhet av ett mångprofessionellt arbetssätt utgående från ett resursförstärkande perspektiv. Det utvecklingsinriktade arbetssättet innebär en ständig förnyelse och omvärdering av kunskap, strukturer och arbetssätt.Society as well as social and health care are changing, and in order to achieve a relevant response to these changes, inter-professional collaborative competencies are needed. Inter-professional collaboration, in turn, demands inter-professional education and training. During the development project “Inter-professional social and health care” the department for social and health care at Novia UAS in Turku developed an interprofessional pedagogical praxis. The result of this project is an education where Nurses, Public Health Nurses and Bachelors of Social Services learn interprofessional collaborative practice from a resource promoting perspective.     Huolimatta terveydenhuollon kustannus- ja vaikuttavuusmittareiden kansallisesta kehitystyöstä, Suomessa on niukasti vertailukelpoista tietoa sosiaali- ja terveydenhuollon palvelujen asiakas- ja potilasturvallisuudesta. Vuonna 2020 alkaneen valtioneuvoston kanslian selvitys- ja tutkimushankkeen ”Potilas- ja asiakasturvallisuuden kansallinen tilannekuva ja seurantamenettelyt” yhtenä tavoitteena oli selvittää asiakas- ja potilasturvallisuuden mittaamista ja menettelyjä Suomessa. Tässä artikkelissa raportoidaan kyselytutkimuksen havainnot sosiaali- ja kehitysvammaishuollon käyttämistä indikaattoreista. Asiantuntijatyönä selvitettiin kansainvälisiä ja Suomessa käytössä olevia mittareita ja menettelyjä, joita tunnistettiin kaikkiaan noin 200. Joukossa oli 79 RAI-järjestelmän mittaria. Indikaattorit koottiin Webropol-kyselyyn. Sosiaalihuollon kysely kohdennettiin ympärivuorokautisiin laitos- ja asumispalveluihin, kotiin vietäviin palveluihin sekä kehitysvammahuoltoon. Kysely lähetettiin 170 sosiaalipalvelujen organisaatiolle, joista 87 (51%) vastasi. Vastanneissa organisaatioissa oli käytössä keskimäärin 56 asiakas- ja potilasturvallisuusindikaattoria 94:stä; eniten maakunnallisilla hyvinvointikuntayhtymillä (69 kpl), yliopistosairaalakaupungeilla (64 kpl) ja keskussairaalakaupungeilla (61 kpl). Yleisimmin seuratut indikaattorit koskivat johtamista ja organisaatiota, vaaratapahtumien raportointia sekä asiakaspalautteita ja reklamaatioita, mutta seurattujen indikaattorien määrissä oli suurta vaihtelua organisaatioiden välillä. Lisäksi RAI-järjestelmän mittareita käytti noin puolet vastaajista laitoshoidossa ja asumispalveluissa sekä kotiin vietävissä palveluissa. Asiakas- ja potilasturvallisuusindikaattoreita oli käytössä kyselyyn vastanneissa sosiaalipalveluissa kohtalaisesti, omavalvontaohjeiden viitoittamana, mutta seuranta vaikutti epäyhtenäiseltä ja pääasiassa reaktiiviselta. Jatkotutkimusta tarvitaan siitä, kuinka järjestelmällisesti, avoimesti ja terveydenhuollon toimintaan integroituneena indikaattoreita käytetään, sekä siitä, mikä yhteys seurattavien indikaattorien määrällä on asiakasturvallisuuden tasoon

    The aim of this research was to explore the client- and patient safety practices of home services (home help services, home nursing, hospital-at-home) with a Webropol-survey sent to the persons responsible for client- and patient safety. Data was analyzed with IBM program SPSS, mainly with Fisher´s test.  Of respondents (n = 114) organizations nearly half (45%) produced home services as collaboration between the public and the private sector. The personnel consisted of practical nurses, and RNs. The client/patient safety plan existed in 89% of organizations, and the content of it partially met the requirements of Acts. In plans, the responsibilities for different parts of client/patient safety were delineated - often in favor of the public sector (p = 0,001). The co-partnership of clients, their near-ones and personnel in safety management was enabled mainly with individual means. In this data the client- and patient safety structures in home care contexts fit to the requirements of Acts, at least in the public sector. However, the integration of social- and health care services anticipate unified client/patient safety processes: how to include all dimensions of safety and co-partnership of different instances into the safety management.Tutkimuksen tavoite oli kartoittaa kotihoidon (kotipalvelun, kotisairaanhoidon sekä kotisairaalahoidon) asiakas- ja potilasturvallisuuskäytänteitä julkisten ja yksityisten palveluntuottajien parissa Suomessa. Webropol-kysely toteutettiin vuodenvaihteessa 2021-2022. Tutkimusjoukko kartoitettiin Valverin ja THL:n rekistereistä ja kysely osoitettiin kotihoito-organisaatioiden asiakas- tai potilasturvallisuusvastaaville henkilöille. Aineisto analysoitiin IBM:n SPSS-ohjelmalla, pääasiassa Fisherin testillä. Vastanneiden edustamista organisaatioista (n = 114) 48% tuotti sekä terveyden- että sosiaalihuollon palveluja ja 45% edusti julkisen ja yksityisen palveluntarjoajan yhteistyötä. Kotihoidon henkilöstö koostui perus- tai lähihoitajista ja sairaanhoitajista (AMK). Asiakas-potilasturvallisuussuunnitelma oli suurimmassa osassa (89%) organisaatioista, ja suunnitelman sisältö vastasi pääosin annettuja säädöksiä. Myös asiakas- tai potilasturvallisuuden vastuualueet oli niissä tavallisesti määritelty, mutta monelta osin tilastollisesti merkitsevästi useammin (p ≤ 0,001) julkisissa kuin yksityisissä palveluntuottajaorganisaatioissa. Kotihoidon asiakkaiden tai potilaiden ja heidän omaistensa osallistuminen turvallisuustyöhön mahdollistettiin pääasiassa yksilötason keinoin, kollektiiviset menettelyt olivat harvinaisempia. Tässä aineistossa kotihoidon asiakas- ja potilasturvallisuuskäytänteet vaikuttivat rakenteiltaan noudattavan pääosin lakien vaatimuksia, etenkin julkisissa palveluntuottajaorganisaatioissa. Kotihoidon turvallisuusprosesseja on kuitenkin syytä pohtia valtakunnallisesti nyt, kun sote-integraatio edellyttää yhtenäistä asiakas- ja potilasturvallisuustyötä: miten asiakas- tai potilasturvallisuussuunnitelmissa huomioidaan turvallisuuden ulottuvuudet, palvelunsaajien osallisuus ja eri tahojen yhteistyö turvallisuuden varmentamisessa

    Advance care planning in dementia care: Wants, beliefs, and insight

    BACKGROUND: Advance care planning gives patients and their family members the possibility to consider and make decisions regarding future care and medical procedures. AIM: To explore the view of people in the early stage of dementia on planning for future care. RESEARCH DESIGN: The study is a qualitative interview study with a semistructured interview guide. The data were analyzed according to the Qualitative Analysis Guide of Leuven. PARTICIPANTS AND RESEARCH CONTEXT: Dementia nurses assisted in the recruiting of people with dementia for participation in the study. Study information was mailed to 95 people with early stage dementia. Ten people with dementia and eight caregiver spouses participated in the study. ETHICAL CONSIDERATIONS: People with dementia belong to a vulnerable patient group, and care was taken in the areas of informed consent and accessible information. FINDINGS: The views of people with dementia are characterized by a complex storyline involving tensions and movement within the themes of wants, beliefs, and levels of insight. Participants wanted to think about the future but also wanted to live in the here and now. DISCUSSION: High demands are placed on the advance care planning process for people with dementia and their family caregivers. A dignity-enhancing approach in dementia care emphasizes the dignity of and respect for this vulnerable and care-dependent patient group. CONCLUSION: The process of advance care planning in dementia care needs to go beyond person-centered care to a relationship-centered process. The illness trajectory and the impact on autonomy need to be taken into consideration

The interviewed nurses described 2 ways of assisting by touch: striving to be present and not being there. This study shows that touching and assisting by touch can constitute the core of the act of holistic caring in nursing, in which the most important content is a real encounter with the patient.</p

    Professional voice use in health and nursing care – Time for reconsideration? A scoping review

    Background: Communication is a key tool in the nursing profession. It is known that listeners are sensitive to the speaker's voice and interpret the speaker's intentions primarily from the non-verbal signal conveyed. Aim: To map and discuss the current state of knowledge and research evidence on professional voice use in health and nursing care. Design and Method: A scoping review adherent to the EQUATOR guidelines PRISMA-ScR. Data Sources: Searches in the EBSCO, CINAHL, Medline, PubMed, PsychInfo and PsycArticles databases were conducted between 1 and 28 February 2023. Results: According to the data (n = 23), health professionals use voice as a vital aspect of caring communication and caring relationships, and in leadership as well as inter-professional collaboration, to facilitate information exchange and understanding and promote shared decision-making. The data showed that there is also a rising tendency to explore vocal demands and problems among health professionals. No studies were found on the use of voice in digital health and nursing care services or by social robots. Most of the reviewed studies were cross-sectional, rather small, and focused primarily on the perspectives of healthcare professionals. Conclusion: The use of voice should be explored as a vital aspect of caring communication and caring relationships from the patients' perspective, and the effects of voice use on listeners' emotions and actions should be examined in different contexts with modern, voice-specific data collection methods. Studies focusing on voice use in digital health care and by social robots are also needed. By integrating training programmes, education and technological innovations, health care can leverage the full potential of voice-based communication to promote a more coordinated and patient-centred care environment, true inter-professional collaboration and effective leadership. Impact: Professional voice use is an essential part of all health and nursing care and an impressive method that should be used consciously. Therefore, the meaning and methods of voice use, including speech accommodation, should be systematically introduced into health and nursing care and included in nursing education
