A CROSS SECTIONAL SURVEY ON THE PRACTICE OF RASAYANA DRUGS IN NEURO DEGENERATIVE CONDITIONS
Pre and Post Test Clinical Study to Assess the Combined Effect of Brihatyadi Ksheera Kashaya, Chandraprabhavati and Shweta Parpati in Urinary Tract Infection
Purpose: UTI is the broad term used to describe infection of any part of urinary tract and is a problem frequently encountered by health care providers today. E.coli is found to be the main causative agent for causing UTI (80%). The prevalence of UTI are increasing and the present standard of care includes the usage of antibiotics. The alarming and exponential use of nonspecific antibiotics, increased resistance and the high cost of prescribed medications for the management of the infective microorganisms will raise the need for alternate and safe medication. The signs and symptoms of Mutrakruchra and UTI are similar in nature. The combination of Brihatyadi Ksheera Kashaya, Chandraprabhavati and Shweta Parpati is widely practiced in Mutrakruchra. The present study was aimed at exploring the combined effect of the above combination in UTI.
Materials and Methods: The present study was a single group study with pre and post test study design. 20 diagnosed cases of UTI based on ICD10N39.0 diagnostic criteria and proper exclusion were selected from IPD and OPD of Amrita Ayurveda Hospital. They were administered Brihatyadi Ksheera Kashaya, Chadraprabhavati and Shweta Parpati for 15 days.
Result: The obtained data were analyzed statistically with symptoms of UTI, Urine analysis and urine culture by using Wilcoxon signed rank test done on the 0th and 16th day. There was significant change with 100% of improvement in clinical symptoms of UTI after treatment. Statistically significant result was obtained in reduction of WBC with p value 0.001, Epithelial cells with p value 0.005, and Nitrites, Leucocyte esterase, Bacteria with p value 0.0001. There was reduction in growth of E.coli in urine culture with statistically significant p value 0.0001. Conclusion: The combination of Brihatyadi Ksheera Kashaya, Chandraprabhavari and Shwetaparpati is effective in UTI
Narrative Journalism in Truman Capote’s In Cold Blood
This paper deals with how Narrative Journalism influences the novels in literature with special reference to the non-fiction In Cold Blood by Truman Capote. The author is forced to yield a serious new art form in Literature through Narrative Journalism. To demonstrate the literary techniques of the novel, Capote applied Literary Journalism. In a simple definition, Narrative Journalism is a technique and stylistic strategy traditionally associated with non-fiction, and also it is at times used interchangeably with creative nonfiction. Capote learned of the quadruple murder in The New York Times, before the killers were captured. Then he decided to travel to Kansas and write about the crime with his fellow author Harper lee who has also written a few non-fictions. Here Capote brings out the true crime story for which he interviewed local residents and investigators assigned to the case and took thousand pages of notes. He also took extensive detail and simultaneous triple narrative. And the story is told from two alternating perspectives; the thought of the murderers and the people affected by the crime. Capote’s purpose of writing this novel was to let the readers know about the prior planning, thoughts and purpose of the crime by the murderers that would not be shown in a typical news report. Unlike other authors his objective was in showing the mindset of the murderers while committing the crime. The highlight of the study is how Narrative Journalism is applied in Capote’s In Cold Blood
Integrative management of insomnia during cancer chemotherapy: A case report
Introduction: Insomnia is common among cancer patients, affecting about 50% undergoing cancer treatment. Insomnia can be due to various reasons, such as physical-pain, psychological-distress and medication side-effects. Insomnia has significant impact on quality of life of cancer patients. Even-though managed with hypnotics and antipsychotic drugs, they cause dependency with various short-term and long-term complications. Presenting a case throwing light on Ayurveda topical intervention as add-on to standard-of-care in insomnia during cancer chemotherapy. Method: A 51-year-old female patient with breast-cancer with extensive necrosis extending to subcutaneous areas was due for second cycle chemotherapy and was diagnosed with moderate-insomnia with a score of 21 as per Insomnia Severity Index in the Out-Patient-Department. Quality of sleep was assessed using Pittsburgh-Sleep-Quality-Index. Treated for 14 days during the break between cycle two and cycle three with Shirothalam (applied on the vertex) using Kachuradi Churnam with Kshirabala 101 and Padabhyangam (foot massage) using Kshirabala thailam as add-on to Tab Zolpidem5mg. Assessment was conducted on baseline and after 14 days of intervention. Results: Assessment for insomnia before and after intervention was conducted with Insomnia-Severity-Index. The score improved from 21 to 2. Quality of sleep before and after intervention was assessed using global PSQI. It improved from 20 to 8. Discussion: In Ayurveda, Nidranasam (loss of sleep) results from aggravation of Vata-Pitta (body humors responsible for movement and cognition and digestion, metabolism and heat of body), depletion of Kapha (body humor responsible for structural cohesion of body), derangement of Manasika-Dosa (mental constituents) and other diseases. All these etiological factors are attributed by Tikshna(sharp)- Uṣhna(hot potency) and Ruksha(dry) chemotherapy regimens. Vata-Pitta-hara (normalising Vata and Pitta) and Indriyaprasadaka (clearing senses) action of medicines used could induce sleep and effectively improve quality of sleep. Conclusion: Integrative-intervention was found to be beneficial in improving insomnia and quality of sleep without any reported complications or dependency in this case. After 14 days of ayurvedic intervention, the patient could get sleep even without taking zolpidem 5mg and external therapies. Same protocol could be considered for generalization so that it could modify or reduce usage of hypnotics and antipsychotic-drugs
An Open Label, Randomized, Parallel Efficacy, Active Controlled Clinical Study to Compare the Efficacy of Nirgundi Patra and Erandamuladi Niruha Basti in Yoga Basti Pattern in Gridhrasi (Sciatica)
It is estimated that Low back pain afflicts nearly two thirds of the Indian population at some point in their life. Erandamuladi Niruha Basti is a widely practiced and proven formulation that is used in Gridhrasi (Sciatica). It contains more than 20 ingredients, which can be a daunting task for an ayurvedic practitioner in a clinic to procure. Hence the literary search was done with a view to find out other effective formulations which were preferably single drug formulations. Nirgundi patra Kashaya was mentioned by acharya Chakrapanidatta in Vatavyadhi Adhikarana of Chakradatta. Nirgundi itself has anti-inflammatory and analgesic properties, so this Kashaya was selected as a Basti to be compared with Erandamuladi Niruha Basti in Yoga Basti pattern. 30 patients were randomized into two groups, Group A and Group B based on a set of inclusion and exclusion criteria. Group A was given Nirgundi Patra Kashaya Niruha and Murchita Tila Taila Anuvasana and Group B patients were given Erandamuladi Niruha and Murchita Tila Taila Anuvasana. Objective and Subjective parameters were evaluated before the start of the treatment and at the end of the treatment. The parameters were evaluated using relevant statistical tests. The result showed that even though all the patients had relief after the treatment, he percentage of relief was more across all parameters in case of Group B. This showed that, Nirgundi patra Kashaya was effective in the treatment of Gridhrasi in Basti formulation, but, Erandamuladi Niruha Basti produced much significant relief
Pre and Post test Clinical study to evaluate the Immunomodulatory effect (IgE) of Virecana followed by Citrakaharitaki Leha in Allergic Asthma
Allergic Asthma is one among the diseases which imposes a great burden on the subjects by hampering the quality of life of patients, reducing productivity, and causing work absence. The literary search hinted that there are no published works reporting both clinical and immunological(IgE) outcomes in Allergic Asthma. So the present study was a pre and post test clinical study to evaluate the effect of Ayurvedic line of management in inducing both immunological(IgE) and clinical outcomes in Allergic Asthma. In this study 30 patients were administered classical Virecana Karma (drug induced purgation) followed by Shamanoushadi (internal medicine) for 30 days. Statistical Analysis was done using SPSS VER. 20. Wilcoxon signed rank test was used to evaluate the Subjective parameters, in order to interpret the time of significant change. For objective parameters Paired Samples t- Test was used to evaluate the difference of significant change. The results showed improvement in the subjective criteria, objective criteria and the overall effect of the therapy with the exception of no statistically significant results in immunomodulatory (IgE) effect which proves that the particular management protocol adopted is found to be clinically efficant but not effective in inducing changes at the immune level
Pre and post test clinical study to assess the combined effect of Brahmi Gritha-Nasya, Ksheerabala Taila-Abhyanga and Saraswatha-Churna in reducing the symptoms of Generalized Anxiety Disorder on Hamilton Anxiety Rating Scale
63-67Generalized Anxiety Disorder (GAD) is the most frequently occurring anxiety disorder. It is characterized by tension, persistent worry about a variety of everyday problems and other anxiety symptoms for at least 6 months. Although there have been significant improvements over the years in its treatment, GAD remains the least successfully treated among anxiety disorders. This paper aims to explore the efficacy of Ayurvedic treatment protocol in providing a remedy for GAD. A clinical trial was conducted with 20 patients in the proposed age group of 20 years to 60 years, diagnosed with GAD according to ICD 10 F 41.1 Criteria, and having a score between 17-24 (mild to moderate) on Hamilton Anxiety Rating Scale. The patients were administered 7 days of Sarvanga-Abhyanga (full body massage) with Ksheerabalataila and Brahmigritha-Nasya, followed by internal intake of Saraswatha-Churna for 21 days. The assessment was done using Hamilton Anxiety Rating Scale at baseline and after treatment. Patients’ age ranged from 27 to 60 years. Maximum incidence of the disease was seen in the age-wise classification group 20-29 years. There were 8 male subjects and 12 female subjects. After 28 days of treatment, the mean scores of Hamilton Anxiety Rating Scale, which was 21 before treatment, reduced to 9.8 with a mean difference of 11.2 and a statistically significant p-value of 0.01. Thus the present protocol was effective in reducing the symptoms of GAD within a short time-span and without producing any side effects

An exploratory single-arm open-label clinical study to evaluate the combined effect of Vamana and Sarasvata Churna in reducing excessive daytime sleepiness
Background: Excessive Daytime Sleepiness (EDS) is inferred as persistent sleepiness and often a general lack of energy during the day even after apparently adequate or even prolonged nighttime sleep. The subjects may not be identifying excessive daytime sleepiness but may present as feeling tired, lack of energy etc. They may seem apparently healthy and without any other systemic illness. In the current health scenario there is a need for alternative interventions to manage this disease. Objective: This study was aimed to assess the combined effect of Vamana (therapeutic vomiting) and Sarasvata Churna in reducing Excessive Daytime Sleepiness. Materials and methods: This was a single-arm exploratory open-label clinical trial. 10 subjects having EDS diagnosed using Modified Epworth Sleepiness Scale (MESS) with a score of 10–24 with proper exclusion were recruited from OPD and IPD, Department of Kāyacikitsa, Amrita School of Ayurveda. All the participants were clinically examined, sought for medical history and further checked biochemically to exclude any other systemic illness that may lead to this condition. Only healthy subjects with quality night sleep (assessed by PSQI) and without any other systemic illness were selected. The treatment protocol adopted was Vamana Karma followed by 30 days of Sarasvata Churna as Shamana Aushadha (medicine for pacification). Assessment was done using MESS on 0th day and after 30 days of Shamana Aushadha Sevana. After completion of treatment, the results were statistically analysed using Wilcoxon Signed Rank Test and final conclusion was drawn. Results: On analysis of the symptoms using Modified Epworth Sleepiness Scale, the mean total score reduced from 14.8 to 2.8 with 81.08 % relief in these 10 subjects and a significant p value < 0.05 after Vamana followed by 30 days of Shamana Aushadha Sevana. Discussion: Sarasvata Churna which was administered as Shamana Aushadha possesses Medhya (Nootropic), Buddhi Smriti Dhriti Vardhaka (intellect, memory and patience promoting) attributes and is being indicated in Vicetas. Most of the drugs in Sarasvata Churna are Rasayana (rejuvenation) that imparts Indriya Bala (strengthening of senses) and Priti (contempt) to Manas by Prinana (nourishing) of Rasa Dhatu. Conclusion: From this study, we could conclude that the combination of Vamana Karma followed by Sarasvata Churna administration is effective in reducing EDS
