    No full text
AbstractIn the paper the Mamdani-Type Fuzzy inference method is applied for the expert evaluation of the impact of the tax legislation reforms on the tax potential. The model being investigated includes 4 inputs, 8 IF-THEN rules and a single-output. Triangle, trapezoid, Gaussian and Bell-shaped membership functions are used for the fuzzification of input data and output of the system is evaluated by Gaussian curve membership function. For the investigated year the laws made reforms in the Azerbaijan tax legislation are classified and evaluated on the base of fuzzy inference system

Author Institution: Samarkand State UniversityWe observed a number of simultaneous vibrational transitions in the absorption spectra of gas and liquid mixtures with HCl, CHCl3, CDCl3, H2O and some alcohols. Frequencies, widths and integral band intensities of the IR spectra have been measured. In approximation of pair electrostatic induction mechanism the absolute intensities of observed IR absorption band were calculated. In these calculations the models of intermolecular potentials of the Lennard-Johns (12:6) and Kihara were used. Important role of complex formation on intensity of simultaneous transitions bands was demonstrated

Electrophysiological investigation of the contribution of attention to altered pain inhibition processes in patients with irritable bowel syndrome

Irritable bowel syndrome (IBS) is a functional gastrointestinal disorder associated with chronic abdominal pain and altered pain processing. The aim of this study was to examine whether attentional processes contribute to altered pain inhibition processes in patients with IBS. Nine female patients with IBS and nine age-/sex-matched controls were included in a pain inhibition paradigm using counter-stimulation and distraction with electroencephalography. Patients with IBS showed no inhibition of pain-related brain activity by heterotopic noxious counter-stimulation (HNCS) or selective attention. In the control group, HNCS and selective attention decreased the N100, P260 and high-gamma oscillation power. In addition, pain-related high-gamma power in sensorimotor, anterior cingulate and left dorsolateral prefrontal cortex was decreased by HNCS and selective attention in the control group, but not in patients with IBS. These results indicate that the central pain inhibition deficit in IBS reflects interactions between several brain processes related to pain and attention. © 2020 The Author(s)

Integration of bilateral nociceptive inputs tunes spinal and cerebral responses

    No full text
Together with the nociceptive system, pain protects the body from tissue damage. For instance, when the RIII-reflex is evoked by sural nerve stimulation, nociceptive inputs activate flexor muscles and inhibit extensor muscles of the affected lower limb while producing the opposite effects on the contralateral muscles. But how do the spinal cord and brain integrate concurrent sensorimotor information originating from both limbs? This is critical for evoking coordinated responses to nociceptive stimuli, but has been overlooked. Here we show that the spinal cord integrates spinal inhibitory and descending facilitatory inputs during concurrent bilateral foot stimulation, resulting in facilitation of the RIII-reflex and bilateral flexion. In these conditions, high-gamma oscillation power was also increased in the dorsolateral prefrontal, anterior cingulate and sensorimotor cortex, in accordance with the involvement of these regions in cognitive, motor and pain regulation. We propose that the brain and spinal cord can fine-tune nociceptive and pain responses when nociceptive inputs arise from both lower limbs concurrently, in order to allow adaptable behavioural responses. © 2019, The Author(s)

Objective: To introduce into clinical practice a new minimally invasive technique of endoscopic third ventriculocisternostomy (ETV) in children with occlusive hydrocephalus with using a semi-rigid needle-shaped miniature neuroendoscope. Methods: Reviewed 131 cases (73 boys, 58 girls) occlusive hydrocephalus of various genesis in children aged 1 month to 5 years, which was performed by ETV proposed by the first author of the methodology. The period of observation of children after treatment was 24.2±3.8 months. Results: Applying a minimally invasive technique of ETV with using a semi-rigid needle-shaped miniature neuroendoscope which significantly reduced the length of the soft tissue incision in the access areas, and the use of a more advanced toolkit – to reduce the size of the twist drill hole in the skull by more than 2 times. Access to the lateral ventricle is done by means of blunt trephination of the dura without the need for significant corticectomy or coagulation and is accompanied by minimal damage to the parencima of the brain. This is very important, given the operation in children with occlusive hydrocephalus, in whom the brain is already damaged by organic changes. Constant endoscopic control when accessing the lateral ventricle increases safety and reduces the risk of damage to vascular and nerve structures. It can be performed in cases of pathologies affecting the anatomical relationships of the lateral and third ventricle such as severe narrowing or complete occlusion of the interpeduncular cistern, narrow (slit) ventricles of the brain (width of the third ventricle is less than 8 mm in the anterior sections), narrow foramen of Monro, anteroposterior size of prepontine cistern less than 6 mm. Lethal outcomes was not, and the effectiveness of surgical treatment was 77%. Postoperative neurological, endocrine and infectious complications have not been observed Conclusion: A minimally invasive technique of performing ETV using a semi-rigid needle-shaped miniature neuroendoscope is an effective and safe method of surgical treatment occlusive hydrocephalus, which minimizes surgical trauma at all stages, reduces intra- and postoperative complications and can be recommended for widespread clinical use

Anti-Mllerian hormone-tailored stimulation protocols improve outcomes whilst reducing adverse effects and costs of IVF

Background Anti-Mllerian hormone (AMH) is increasingly used to quantify ovarian reserve, but it has not yet realized its full clinical potential in assisted reproduction technology. We investigated the possible benefits of using novel, stratified ovarian hyperstimulation protocols, tailored to individual AMH levels, compared with conventional stimulation. Methods Retrospective data were collected from 769 women (first cycle of IVF, using fresh embryos), in a UK tertiary care unit: 346 women using conventional stimulation protocols; 423 women treated under new AMH-tailored protocols. ResultsEmbryo transfer rates increased significantly (7987: P = 0.002) after the introduction of AMH-tailored stimulation protocols. Pregnancy rate per cycle started and live birth rate also increased significantly compared with conventionally treated women (17.927.7, P = 0.002 and 15.923.9, P = 0.007, respectively). Moreover, in the AMH group, the incidence of the ovarian hyperstimulation syndrome (OHSS) fell significantly (6.92.3, P = 0.002) and failed fertilization fell from 7.8 to 4.5. The cost of fertility drug treatment fell by 29 per patient and the overall cost of clinical management of OHSS fell by 43 in the AMH group. GnRH antagonist protocols, introduced as part of AMH-tailored treatment, may have contributed to the observed improvements: however, within the AMH-tailored group, the live birth rate was not significantly different between agonist and antagonist-treated groups. Conclusions Although large, prospective, multicentre studies are indicated, we have clearly demonstrated that individualized, AMH-guided, controlled ovarian hyperstimulation protocols significantly improved positive clinical outcomes, reduced the incidence of complications and reduced the financial burden associated with assisted reproduction. © 2011 The Author
