    The initial management of mild and moderate burned patients in the emergency department: an expert consensus statement

     On behalf of Italian Society of Emergency Medicine (SIMEU), Association of Italian Hospital Dermatologists Venereologists and Public Health (ADOI) and Italian Skin Ulcer Association (AIUC).     Early diagnosis and treatment of burn patients are a key issue in the emergency setting. With the aim of assisting physicians in the diagnosis and subsequent management of burns in emergency departments (EDs), a Nominal Group Technique (NGT) was designated. A group of 6 Italian experts (2 emergency physicians, 2 emergency medicine nurses, and 2 dermatologists chief of a burn center) developed a consensus document based on the available evidence and their clinical experience on the fundamental steps to be followed in the ED in the management of patients with a mild to moderate degree burn. The expert consensus statement proposed can assist emergency physicians in applying the results of clinical studies and clinical experience to routine care settings, providing guidance on all aspects of 7 steps: prehospital treatment, centralization criteria, transport (equipment and composition of the healthcare team), management of ED phase, clinical evaluation, treatment of pain and disposition

    The essay aims to explore the potentialities of collective writing for the deconstruction of the authorship in a cultural and political sense. Some historical experiences of collective writing from the avant-gardes are in fact cited in Roland Barthes’essay which, in a dialogue with Michel Foucault, opened in 1968 the debate on “The Death of the Author”; and even if the two scholars came to divergent outcomes as to the diagnosis of the author’s health, both reflections were based on the assumption of the political implications inherent in the figure of the Author as it was built and mythologized by the bourgeois society. It will be seen how, from the end of the twentieth century, some collective writing experiences, in particular the Italian collectives Wu Ming and Scrittura Industriale Collettiva, have attempted to force and deconstruct the authorship by transforming collective writing into a tool of political and cultural struggle. However, the authorship remains unavoidable both for the aesthetic needs of literary criticism and for the commercial exploitation of the publishing market, which transforms this name into a brand to sell; the essay therefore intends to show that collective writing can not in itself be an antidote to the marketing of the author unless it is accompanied by paratextual strategies and extra-literary activities that are truly capable of deconstructing the mythology denounced by Barthes

    This article analyses the economic structure of Sardinia which up to the end of the fifties was dominated by a subsistence economy. This equilibrium broke up as a consequence of the expansion of the national market during the sixties. The article studies the industrial development based on chemicals, mining and metallurgy in detail. Both sectors have experienced an internal expansion before the crisis which went parallel to the recession of the seventies. The present situation is defined as a dualist economy. The author argues that it is impossible to go back to traditional activities while defending, on the contrary, the promotion of the regional economy whithin the existing national and international conditions

In this paper a semantic- based representation of Application Patterns and Cloud Services is presented, with an example of its use in a typical distributed application, which shows how the proposed approach can be successfully employed for the discovery and composition of Cloud Services.

    The article aims to demonstrate that Umberto Eco’s author name is a reference point for successful Italian novels on a global scale. Starting from the debate about Eco’s name associated to an apocryphal text such as Q (Luther Blissett, 1999) in international reviews, the essay attempts to reconstruct an Eco model, in order to test its consistency: first of all, questioning whether this model really refers to the historical novel, as the reviews suggest; and, more generally, whether the reference to the author is really due to the literary genre. This reference seems to have relevance linked on the one hand to The Name of the Rose as an example of the «Italian way to postmodernism», and on the other as the first Italian novel to be highly successful on a transnational editorial market

    Introduzione. Le cadute in ambito ospedaliero rappresentano un problema di notevole impatto sulla qualità della vita dei pazienti, potendo determinare un incremento della morbilità e della mortalità e allungamento dei tempi di ospedalizzazione . Obiettivo. Analizzare, attraverso la descrizione di un campione i fattori di rischio estrinseci dell’evento caduta correlandoli ad eventuali responsabilità medico-legali. Metodo. È stato condotto uno studio retrospettiva su un campione di 131 pazienti (60% maschi, 30% femmine) afferenti alla Unità Operativa Complessa (U.O.C.) Pronto Soccorso e Osservazione Breve Intensiva (O.B.I.) del Policlinico Di Tor Vergata. È stata creata una scheda che consentisse di raccogliere i dati relativi alle caratteristiche dei pazienti e le circostanze dell’evento caduta; inoltre è stata effettuata dal personale infermieristico una stratificazione del rischio- caduta al momento del ricovero, mediante la Scala di Conley. Risultati. Le cadute sono avvenute perlopiù nella stanza del paziente (60%), nel 21% dei casi in presenza del personale sanitario e nel 64% dei casi il letto era in posizione di sicurezza. Discussione. Nel nostro campione le sponde di sicurezza sono state apposte nel 58% dei casi ed è significativo osservare che nel 36% i pazienti sono caduti nel tentativo di alzarsi, di scendere dalla barella e di scavalcare tali sponde. Le sponde di sicurezza rappresentano una misura di prevenzione del rischio ma da adottare solo dopo un’attenta valutazione preliminare del paziente. Esse infatti potrebbero esporre i pazienti ad ulteriori rischi nel tentativo di scavalcarle.Introduction. Falls within the hospital environment constitute a severe problem and induce a notable negative impact upon the patients. They may lead to an increased morbidity and/or mortality with an extended period at the hospital. Aims. Analyze, through the description of a sample of extrinsic risk factors correlating event falling to eventual medico-legal responsibility. Methods and Materials. The retrospective study used data derived from a total of 131 patients (60% males and 40%, females) who were obtained from the Unit of Complex Operations (U.O.C.), and Emergency Unit and Short-term Intensive Observation ( O.B.I.), from the Polyclinic of the Tor Vergata . A questionnaire/record was designed that registered the patients’ characteristics and falling-accident circumstances. The questionnaire-register was completed by nursing staff who performed a falls risk assessment using the Conley scale. Results. Falling-accidents occurred in the patients room in 60% of the cases, in 21% of the hospital staff were present, and the patient’s bed was locked in the safety position in 64% . Disscussion. The present study revealed that the use of the safety rail occurred in 58% of the falling-accident cases and it important to observe that in 36% of the cases the patients fell during his/her attempt to leave the bed, or stepping down from the stretcher or climbing up the bedsides. The safety rails are useful measures for risk prevention but only acted upon after careful each patient’s preliminary assessment . Indeed, they could expose patients to additional risks in an attempt to step over

    «Ovviamente», il Medioevo? Note per una lettura metastorica de «Il nome della rosa» di Umberto Eco

    The essay aims to reflect on the choice of the Middle Ages by Umberto Eco for the setting of his first novel Il nome della rosa (1980). While in the Postille to the novel published by the author in 1983 this choice is described as obvious, it is instead worth focusing on it in order to place it in its historical and cultural context – a wide-ranging medievalist revival in the 1970s – and thus understand its significance as a node in a wider network. Considering specifically what kind of Middle Ages is represented in the text, and relating it to the historical period during which Eco claims to have conceived the book, we see how what seems to interest him is the story of the eternal return of the dynamics of power, the definition of archetypes of power that could constitute a passepartout for the interpretation of reality: historical depth seems here to yield in a certain way to a 'meta-historical' hypothesis aimed at formulating a model of the clash between power and counter-power in the midst of which those who seek to lucidly investigate its mechanisms are bound to succumb.Il saggio si propone di riflettere sulla scelta del Medioevo da parte di Umberto Eco per l’ambientazione de Il nome della rosa (1980). Se nelle Postille al romanzo pubblicate dall’autore nel 1983 questa scelta è raccontata come ovvia, vale invece la pena di concentrarvisi per inserirla nel suo contesto storico e culturale – quello di un ampio revival medievalista degli anni Settanta – e comprenderne dunque il significato in quanto nodo di una rete più ampia. Ragionando poi nello specifico su che genere di Medioevo venga rappresentato nel testo, e mettendolo in relazione con il periodo storico durante il quale Eco afferma di aver concepito il libro, si vede come ciò che sembra interessargli è il racconto dell’eterno ritorno delle dinamiche di potere, la definizione di archetipi del potere che costituiscano dei passepartout per l’interpretazione della realtà: la profondità storica pare cedere in una certa misura a un’ipotesi ‘metastorica’ volta a formulare un modello di scontro tra potere e contropotere nel cui mezzo soccombe chi cerca di indagarne lucidamente i meccanismi
