Onset of type 1 diabetes mellitus in two patients with maturity onset diabetes of the young

The association between maturity onset diabetes of the young (MODY) and type 1 diabetes mellitus (T1DM) has been rarely described. We report two patients affected by MODY who developed T1DM. Case 1: a 4-yr-old girl referred for glycosuria presented hemoglobin A1c (HbA1c) of 6.6%. Islet cell antibodies (ICA) and anti-glutamic acid decarboxylase (GADA) were initially negative. As her father, uncle and grandmother showed mild hyperglycemia, they were screened for MODY 2. A novel mutation in glucokinase gene was found in the family. Few months later, her glycemic control worsened consistently and she required insulin treatment. A high titer of GADA and ICA was then detected. Six years afterwards insulin requirement is 0.8 U/kg and HbA1c 6.7%. Case 2: a 15-yr-old boy treated for growth hormone deficiency was found with a blood glucose level of 106 mg/dL. HbA1c was 7.2%, ICA and GADA were negative. Family history was positive for autoimmune diseases and type 2 diabetes mellitus. The patient was investigated for MODY 2 and MODY 3, and a mutation of hepatocyte nuclear factor-1 alpha gene was found. The same mutation was found in the mother who had never been referred for hyperglycemia. After 1 yr, due to an unjustified worsening of the metabolic control, autoimmunity was again investigated and a mild positivity was found. He then required insulin therapy and after 5 yr current HbA1c was 8.2%. The diagnosis of MODY does not exclude the risk of developing T1DM. Therefore autoimmunity should be investigated when ordinary treatments fail and metabolic control unexpectedly worsens

Comparison Between Sensor-Augmented Insulin Therapy with Continuous Subcutaneous Insulin Infusion or Multiple Daily Injections in Everyday Life: 3-Day Analysis of Glucose Patterns and Sensor Accuracy in Children

BACKGROUND: Sensor-augmented continuous subcutaneous insulin infusion (CSII) therapy is superior to CSII therapy alone, but little is known on the effectiveness of sensor-augmented multiple daily injections (MDI) therapy. METHODS: We compared during everyday life mean glucose control and several variability indexes recorded for 3 days by a real-time glucose sensor (Medtronic, Northridge, CA) in two groups of children treated with either CSII or MDI. Fifty-five consecutive subjects were examined: 17 receiving CSII and 38 receiving MDI basal-bolus therapy (age range, 7-22 years). All subjects wore the sensor for 4 days, and 3 days were used for statistical analysis. Mean glucose and SD, coefficient of variation (CV), mean amplitude of glucose excursion (MAGE), mean of daily differences (MODD), continuous overall net glycemic action (CONGA) at 2 and 4 h, blood glucose (BG) rate, area under the curve (AUC) above 180 mg/dL and below 70 mg/dL, Low BG Index (LBGI), and High BG Index (HBGI) were calculated. RESULTS: Patients receiving CSII administered more daily boluses than patients receiving MDI (5.2±1.5 vs. 3.2±0.3, respectively; P=0.001). Mean glucose was lower in the CSII group. AUC above 180 mg/dL and HBGI were higher in the MDI group. CV, CONGA at 2 h, CONGA at 2 h during the day, and HBGI were worse in the MDI group, whereas MODD, LBGI, BG rate, and MAGE were similar. A positive correlation (r=0.95; P<0.05) was found between the paired sensor-meter values. For the glucose values <70 mg/dL, sensitivity was 40%, and specificity was 99%. CONCLUSIONS: In our pediatric patients during everyday life sensor-augmented CSII therapy seemed more effective than sensor-augmented MDI therapy, in terms both of glucose mean values and of intraday variability. Mild hypoglycemic episodes and indexes of low BG values were similar in the two groups, although the latter results may be inaccurate because of low sensor sensitivity at low glucose value

    Teatro e psicanalisi

    No full text
The author describes her first journey through the territories that may appear to be common between theatre and psychoanalysis: first of all the “theatre-that-heals”, but also the nomenclature that psychoanalysis derives from theatre; usages such as those by Moreno (psychodrama), or by the Actors Studio; theatre as a research on yourself. Despite the affinities between the two fields, the author is above all interested in theatre as a “third way” compared to illness and healing, a way parallel and independent from psychoanalytical approaches, and investigates it through two symbolic performances: the Marat-Sade, by Peter Brook and Marco Cavallo, by Giuliano Scabia

The article focuses on situating Jean-Claude Grumberg's Le Petit Chaperon Uf into the wider context of the Francophone drama repertoire for young people, as well as into the author's work, from the perspective of the hermeneutic dialogue the translator is expected to engage with the original text. The analysis stresses the importance of diffusing the Francophone drama repertoire for young audiences, with particular regard to such texts as Le Petit Chaperon Uf, which alerts "today's little ridinf hoods" by means of a powerful, free writing, where words are 'juicy', 'crispy' and author relies on their power to lead young readers and spectators towards discovering and (re)inventing "the story, the stories, the real History"

Ms Ambr. Trotti 330 is the only witness of a hitherto unknow poem on the conquest of the Spanish city of Almeria by the fleet of Genoa in 1147. The poem is fragmentary, less than a half of the text (1337 lines) survives; the manuscript is autograph by the author, who was a member of the expedition. A description of the manuscript is given, as well as a summary of the text content and comments on the relevant historical information

Mirella Napodano, il romanzo e la memoria delle emozioni

The author reads Mirella Napodano's novel as a singular case of writing in autobiographical terms that goes beyond the blurred boundaries of the genre to establish itself as an experiential path for the search for the self. The researcher identifies in the "novel", built around the rich paternal correspondence, the wonderful script of a family story during the war years, to the point of capturing in the plot of the book the intersection of two different writing registers: the first consists of the letters, with the direct testimony of the father Carmine, the other given by the 'narration' of the author, with large descriptive inserts, placed in commentary on the letters, which return, between history, traditions and folklore, the cross-section of an era

    Don Giovanni Trionfante

During the first half of the nineteenth century, the theme of Dom Juan was transformed into one bearing literary qualities (giving rise to an array of poems, plays and novels). It was like a ‘buzz word’, or the obsession of an élite. The author explores this literary explosion by following up three clues from the theatre: a) investigating the period which marked a turning point, through Mozart’s opera as well as through the convergence of his opera and Hoffmann’s tale (1813), as these were to have interesting consequences on the development of relations between Dom Juan and the theatre; b) examining the variants of the “second Don Juan” in order to highlight the tiny anomalies that are also present in the purely theatrical “first” version; c) and from these anomalous presences trying to infer what theatre craft at the beginnings of professional theatre, might have been

    Gender Diversity and Financial Intermediaries

The paper analyzes some recent rules (Directive 2013/36/EU, called CRD IV and Directive 2014/65/EU, called MIFID 2) oriented towards a modern financial management focused on people with great expertise and managing abilities, but at the same time diversified by age, sex, geographic provenience and experience. The goal is to identify ways to produce a strategic plan filled of the most different contributions to provide innovative solutions. The introduction of more diversified administrative organs is intended to contrast the “group mentality phenomenon”, through the representation of “a variety of points of view and experiences”. This research highlights the fact that women move in an under-confidence context (unlike men, who are usually overconfident) that often causes them to accept decisions even when not fully in agreement. Moving on to the identification of the reasons that justify a more appropriate placement of women in the banking and finance enterprises, the Author emphasizes the need for a particular technical qualification of the activity performed by the board of administration and control of the institutions belonging to this sector. Indeed, the various options (from organization to investment) for decisions oriented towards sound and prudent management require the ability to assess a balanced evaluation of information data remitted to the boards of directors and control in the reports that the structure periodically prepares
