Drug-resistant TB treatment: concerns over absconding patients

    No full text
A study in Khayelitsha in the Western Cape illustrated the difficulty of retaining patients on drug-resistant TB treatment, a tendency that poses a significant threat to containing the spread of multidrug-resistant TB in the country, writes Sizulu Moyo. That drug-resistant TB is increasing year-on-year is clear when analysing the number of multidrug-resistant TB cases.

This case study was developed to inform subsequent research and analysis of local government leadership and prioritisation of sanitation and hygiene (S&H) in East Africa. Consolidated learning from across the three countries involved can be found in the Sanitation Learning Hub (SLH) Learning Brief: Strengthening sub-national systems for area-wide sanitation and hygiene. From late 2020 to early 2021, the SLH collaborated with local government actors and development partners from three subnational areas to explore ways of increasing local government leadership and prioritisation of S&H to drive progress towards area-wide S&H. For some time, local government leadership has been recognised as key to ensuring sustainability and scale and it is an important component of the emerging use of systems strengthening approaches in the sanitation sector. It is hoped that this work will provide practical experiences to contribute to this thinking. Three case studies were developed to capture local government and development partners’ experiences supporting sub-national governments increase their leadership and prioritisation of S&H in Siaya County (Kenya), Nyamagabe District (Rwanda), and Moyo District (Uganda), all of which have seen progress in recent years. The development partners involved were UNICEF in Kenya, WaterAid in Rwanda, and WSSCC/ Uganda Sanitation Fund in Uganda. The cases were then analysed through three online workshops facilitated with staff from the local governments, central government ministries and development partners involved to explore them in further detail, review experiences and identify levers and blockages to change. Lessons from the workshops are documented in the SLH learning brief mentioned above. This is the case study developed by Moyo District and WSSCC/Uganda Sanitation Fund documenting their experiences and reflections from working together to increase prioritisation of S&H in Moyo District, Uganda. This is the case study developed by Moyo District and WSSCC/Uganda Sanitation Fund documenting their experiences and reflections from working together to increase prioritisation of S&H in Moyo District, Uganda

Realising the maximum health benefits from water quality improvements in the home: a case from Zaka district, Zimbabwe

The provision of good quality household drinking water is often regarded as an important means of improving health. There are however indications that the microbiological quality of water deteriorates between source and point of use in the home. The notion that providing clean water sources could be the solution to the rural water supply problem might therefore not be correct. This paper presents the findings from a survey that sought to investigate the quality of water in the home post-collection and any factors that were contributing to the quality. The 24 households that participated in the study were selected randomly from two villages. Water samples were collected from the households and from the drinking water sources. These were tested for total coliforms and Escherichia coli.Greater contamination was detected in the water drawn from unprotected wells than in that drawn from protected ones. For protected sources, there was an increase in the numbers of pathogens in the stored water relative to the source water. In a few instances there was a decrease in indicator bacteria numbers following water collection. This could have been due to a natural die off of the pathogens. There however did not seem to be a significant correlation between the microbiological quality of water in the home and the length of time the water has been stored. These findings indicate that there is some deterioration in the microbiological quality of water post-collection and it appears the phenomenon may be attributable to water handling and hygiene behaviours in the home. Water was collected frequently and rarely treated after collection. Only one household out of the 24 reported treating their water after collecting it. Home water treatment or safe storage may be an appropriate intervention in this community to reduce diarrhoeal disease.<br/
