Heart Failure Education in A VA Outpatient Clinic Delivered as Part of a Multidisciplinary Heart Failure Management Team
abstract: There is an estimated 6.2 million people Americans over the age of 20 suffering from Heart Failure (HF) (Bejamin et. al., 2019). It is essential that HF patients have sufficient knowledge about the disease and self-management (Abbasi, Ghezeljeh, & Farahani, 2018; Dinh, Bonner, Ramsbotham & Clark, 2018). Lack of self-management is largely to blame for many HF exacerbations. Current evidence supports utilizing both verbal and written education with an emphasis on self-care and education delivered in a group setting or individual setting showed equal impact on self-care and HF knowledge ( Hoover, et. al., 2017; Ross et. al., 2015; Tawalbeh, 2018). An outpatient VA clinic located in a suburb of the large metropolitan identified there was no consistency on how a HF patient was educated, managed, or tracked and the registered nurses (RNs) lacked knowledge of HF. As a results of these findings this Evidence Based Project (EBP) was implemented. RNs were educated on HF and completed a self-assessment questionnaire evaluating their knowledge pre and post education. The RNs, as part of a multidisciplinary team, educated HF patients on signs and symptoms of HF as well as on how to manage the disease. Patients completed, the Kansas City Cardiomyopathy Questionnaire (KCCQ) to assess quality of life and the Self Care Heart Failure Index (SCHFI) to assess knowledge of HF and self-management skills. These questionnaires were completed initially and at 30 and 60 day intervals. The RNs self-assessment of their knowledge and ability to educate patients increased in all areas. The patient’s KCCQ and SCHFI score improved at 30 days and 60 days when compared to their initial score. Larger EBPs are needed over a longer period of time to assess the impact on hospital readmissions and same day clinic visits for HF exhibitions
Imaging Predictors of 4q12 Amplified and RB1 Mutated Glioblastoma IDH-Wildtype
Introduction: Recent studies have identified that glioblastoma IDH-wildtype consists of different molecular subgroups with distinct prognoses. In order to accurately describe and classify gliomas, the Visually AcceSAble Rembrandt Images (VASARI) system was developed. The goal of this study was to evaluate the VASARI characteristics in molecular subgroups of IDH-wildtype glioblastoma.
Methods: A retrospective analysis of glioblastoma IDH- wildtype with comprehensive next-generation sequencing and pre-operative and post-operative MRI was performed. VASARI characteristics and 205 genes were evaluated. Multiple comparison adjustment by the Bejamin-Hochberg false discovery rate (BH-FDR) was performed. A 1:3 propensity score match (PSM) with a Caliper of 0.2 was done.
Results: 178 patients with GBM IDH-WT met the inclusion criteria. 4q12 amplified patients (n = 20) were associated with cyst presence (30% vs. 12%, p = 0.042), decreased hemorrhage (35% vs. 62%, p = 0.028), and non-restricting/mixed (35%/60%) rather than restricting diffusion pattern (5%), meanwhile, 4q12 non-amplified patients had mostly restricting (47.4%) rather than a non-restricting/mixed diffusion pattern (28.4%/23.4%). This remained statistically significant after BH-FDR adjustment (p = 0.002). PSM by 4q12 amplification showed that diffusion characteristics continued to be significantly different. Among RB1-mutant patients, 96% had well-defined enhancing margins vs. 70.6% of RB1-WT (p = 0.018), however, this was not significant after BH-FDR or PSM.
Conclusions: Patients with glioblastoma IDH-wildtype harboring 4q12 amplification rarely have restricting DWI patterns compared to their wildtype counterparts, in which this DWI pattern is present in ~ 50% of patients. This suggests that some phenotypic imaging characteristics can be identified among molecular subtypes of IDH-wildtype glioblastoma
In Colombia, the first people who used the cooperative association were the indigenous people who cultivated in community on the same land, ignoring the individual property.
This research presents a discussion on the jurisdictional protection of the abusive ecological risk. It supports the creation and development of actually Collective jurisdictional Processes that is, based on participatory democratic decision-making concerning the ecological risk tolerability as well as the practice of a procedural collective title on the environment as a public property. After offering a critical analysis of the current collective procedural paradigm of environmental protection, in order to identify some of its major structural deficiencies, this dissertation proposes the theorizing of a model more suited to face the challenges presented by a risk society. The conclusion argues for the possibility of characterizing the social and/or scientifically intolerable ecological risk as "abuse of right"; for the need of personifying the "Collective" into the holder of a procedural property over the ecological public property, and as such able to defend it in face of its private and state use; for the need of a procedural, participatory and inclusive making of the notion of abusive ecological risk in the real case, while seeking the best way of creating the scientific and axiological variables at issue
FINCH: A Blueprint for Accessible and Scientifically Valuable Remote Sensing Satellite Missions
Satellite remote sensing missions have grown in popularity over the past fifteen years due to their ability to cover large swaths of land at regular time intervals, making them suitable for monitoring environmental trends such as greenhouse gas emissions and agricultural practices. As environmental monitoring becomes central in global efforts to combat climate change, accessible platforms for contributing to this research are critical. Many remote sensing missions demand high performance of payloads, restricting research and development to organizations with sufficient resources to address these challenges. Atmospheric remote sensing missions, for example, require extremely high spatial and spectral resolutions to generate scientifically useful results. As an undergraduate-led design team, the University of Toronto Aerospace Team’s Space Systems Division has performed an extensive mission selection process to find a feasible and impactful mission focusing on crop residue mapping. This mission profile provides the data needed to improve crop residue retention practices and reduce greenhouse gas emissions from soil, while relaxing performance requirements relative to many active atmospheric sensing missions. This is accompanied by the design of FINCH, a 3U CubeSat with a hyperspectral camera composed of custom and commercial off-the-shelf components. The team’s custom composite payload, the FINCH Eye, strives to advance performance achieved at this form factor by leveraging novel technologies while keeping design feasibility for a student team a priority. Optical and mechanical design decisions and performance are detailed, as well as assembly, integration, and testing considerations. Beyond its design, the FINCH Eye is examined from operational, timeline, and financial perspectives, and a discussion of the supporting firmware, data processing, and attitude control systems is included. Insight is provided into open-source tools that the team has developed to aid in the design process, including a linear error analysis tool for assessing scientific performance, an optical system tradeoff analysis tool, and data processing algorithms. Ultimately, the team presents a comprehensive case study of an accessible and impactful satellite optical payload design process, in hopes of serving as a blueprint for future design teams seeking to contribute to remote sensing research
