    Five-year longitudinal evaluation of quality of life in a cohort of patients with differentiated thyroid carcinoma

    Differentiated thyroid cancer (DTC) generally has a favorable outcome. Thyroid disease, treatments, stress and comorbidity can compromise health-related quality of life (QoL) and indirectly weigh upon the outcome. From 2004 to 2008, we evaluated QoL longitudinally in 128 DTC subjects. During scheduled examinations, subjects were asked to undergo a semi-structured psychiatric interview and five rated inventories. The same examination was conducted in 219 subjects after surgery for benign thyroid pathology. Low scores represent a better QoL. DTC and control group subjects were similar in terms of age, male/female ratio, concomitant psychopharmacological treatments and frequency of psychiatric diseases. In DTC subjects Billewicz Scale (BS) scores showed an increasing trend over time, especially among females. The ad hoc Thyroid Questionnaire (TQ) scores were similar in both groups and did not change over time, but at the end of the study ad hoc TQ and BS were significantly related. Ad hoc TQ scores were also related to age on entry to the study. In both male and female DTC subjects, Hamilton’s tests for anxiety (HAM-A), but not for depression (HAM-D), showed an improving trend. At the end of the study, HAM-A and HAM-D scores were comparable to those of the control group. HAM-A and HAM-D were both positively correlated with the stage of cancer and the time between diagnosis and treatment. Only HAM-D correlated with age on entry to the study. Kellner Symptoms Questionnaire item scores were higher in DTC subjects than in controls. The change over time in the items including anxiety, somatization, depression, and hostility was significant. Somatization and hostility were more significantly reduced in DTC females than males. Hostility scores were significantly lower in DTC subjects than in controls at the end of the study. Somatization and depression were significantly related to staging on diagnosis and age on entry to the study. Our study confirms a wide variation of illness perception in DTC subjects, which is generally unrelated to the favorable clinical follow-up of the disease. Psychological evaluation during long-term follow-up improved QoL scores, which reached the same levels noted in subjects with a history of thyroid surgery for benign thyroid pathology. Our data indicate that special attention should be paid to QoL in older DTC subjects and those with more severe staging on diagnosis

    A case-controlled study on the quality of life in a cohort of patients with history of differentiated thyroid carcinoma

    Although quality of life (QoL) has become an important aspect of cancer rehabilitation, psychometric studies on thyroid cancer patients are rare. We performed a case-controlled study on QoL in patients with differentiated thyroid carcinoma (DTC). QoL was evaluated in 61 patients with a history of DTC diagnosed from < 1 to 23 yr earlier. An undetectable thyroglobulin (Tg) level after recombinant human TSH (rhTSH) testing was considered the best predictor of cure. QoL was evaluated by means of a general psychiatric interview, the self-rating Kellner Symptoms Questionnaire (KSQ) and the Hamilton Depression Scale (HDS). QoL was also evaluated in a control group of subjects on L-T4 therapy with a non-toxic multinodular goiter diagnosed from < 1 to 25 yr earlier. DTC and control subjects were similar in age, male-female distribution and concomitant psychiatric therapies. Per-week dosage of L-T4 was higher in DTC patients than in controls (p < 0.01). In neither group of subjects was there any correlation between current TSH levels or interval from diagnosis and KSQ or HDS scores. Only in DTC patients was there a positive correlation between age and KSQ (p < 0.05) or HDS (p < 0.01) scores. There was a significant difference in overall KSQ scores between DTC (33.4 +/- 2.1) and control (24.5 +/- 1.9; p < 0.01) subjects. The subscales of KSQ showed a significant inter-group difference. HDS scores were higher in DTC subjects (35.8 +/- 1.0) than in controls (30.0 +/- 1.1; p < 0.01). HDS score was significantly (p = 0.02) higher in female than in male DTC patients. In patients with papillary carcinoma there was a positive correlation between the MACIS (metastases, age, completeness, invasiveness, size) score and KSQ (p = 0.01) or HDS (p < 0.01) scores. After rhTSH testing, detectable Tg levels were found in 13% of DTC patients. In Tg-positive patients, KSQ and HDS scores were not different from those of Tg-negative patients. After an 8-14 month period, a significant decrease in the KSQ scale somatization (p = 0.02) was found in a sub-set of 31 DTC patients. In conclusion, even in the age of rhTSH testing, DTC patients suffer an impairment of their QoL, as noted when short-term L-T4 withdrawal was the gold standard. Longitudinal evaluation seems to indicate a slight improvement in QoL when safe rhTSH testing is extensively used in the management of the disease

In ciascuno di essi la produzione scientifica di Albano Leoni ha tracciato un solco importante, che ha segnato gli sviluppi di ciascun settore: filologia germanica, linguistica storica, linguistique d'intervention, fonetica e fonologia, linguistica dei corpora, filosofia del linguaggio. Ognuno di questi ambiti di ricerca costituisce pertanto una sezione del volume, prefato da un/a suo/a allievo/a o da chi comunque come tale gli è stato/a scientificamente vicino/a (Simona Leonardi, Luisa Corona, Anna Rosa Guerriero, Mariapaola D'Imperio, Loredana Cerrato, Mauro Serra), mentre una bibliografia completa dei numerosi lavori prodotti sino ad oggi da Federico Albano Leoni è posta in chiusura, ordinata tematicamente e cronologicamente (un link segnala i lavori liberamente accessibili in rete). In each of these areas, Albano Leoni’s research has left a mark that contributed to further developments: Germanic philology, historical linguistics, linguistique d'intervention, phonetics and phonology, corpus linguistics, philosophy of language. Each of these research areas represents a section of this volume, with an introduction by one of his former students or scholars who shared his scientific interests (Simona Leonardi, Luisa Corona, Anna Rosa Guerriero, Mariapaola D'Imperio, Loredana Cerrato, Mauro Serra).     Contemporary theory of metaphor: an analysis of lakoffian echoes in Brazil

    Dissertação (mestrado) - Universidade Federal de Santa Catarina, Centro de Comunicação e Expressão, Programa de Pós-Graduação em Letras/Inglês e Literatura Correspondente, Florianópolis, 2010Nos últimos trinta anos, a metáfora tem sido observada sob aspectos mais psicológicos e cognitivos, tornando-se objeto de interesse central nas ciências humanas, principalmente na psicologia cognitiva que tem desenvolvido uma grande quantidade de pesquisas dedicadas ao processo de compreensão da metáfora. O objetivo deste estudo é contribuir com um mapeamento das diferentes perspectivas dos pesquisadores brasileiros dedicados à Teoria Contemporânea da Metáfora. Os dados consistem de trezentos e cinco estudos empíricos publicados entre 1998 e março, 2010. A análise mostra que (1) São Paulo foi o estado brasileiro mais frutífero a desenvolver pesquisas no tema e PUC-SP foi a universidade com o maior número de publicações, seguida pela UFJF e a UFSC. Também o GEIM - Grupo de Estudos da Indeterminação e da Metáfora - demonstrou ser o grupo de pesquisa com contribuições mais significantes para o campo; (2) a maioria dos estudos foi conduzida em português como língua materna - principalmente os baseados em Lingüística de Corpus; (3) a investigação brasileira tem sido conduzida de forma qualitativa e possui abundantes pesquisas etnográficas e de ação; (4) há uma tendência em favor do uso da metodologia de Lingüística de Corpus com ênfase no uso das ferramentas fornecidas pelos Laboratórios de língua brasileiros; (5) existe um leque de investigadores que têm observado a metáfora conceitual sob uma perspectiva mais filosófica e semântica; (6) a maioria dos pesquisadores selecionados neste estudo analisaram a indeterminação de significados na língua (real) em uso; (7) Metaphors we live by (Lakoff & Johnson 1980) e sua tradução ao português - pelo GEIM - demonstrou ser indispensável na literatura brasileira. Resultados gerais sugerem que Brasil é um campo fértil e tem explorado a abordagem Lakoffiana da metáfora nas mais diversas áreas de estudo

    Migrações internacionais e mundos do trabalho

Programa de Pós-graduação em HistóriaA presente tese aborda o processo de migrações internacionais e de transformações do mundo do trabalho entre o final do século XX e início do século XXI. Seu foco são os brasileiros e brasileiras, documentados e indocumentados, que, em 2007 e 2008, viviam em Portugal e na Espanha. O trabalho tem como objetivos investigar a história das migrações internacionais entre Brasil, Portugal e Espanha, mostrando a chegada de espanhóis e portugueses no Brasil e o contexto de ampliação das migrações de brasileiros para esses dois últimos países a partir de 1986; mostrar as narrativas, memórias, territórios e o cotidiano dos imigrantes brasileiros em Portugal e Espanha; e identificar as novas faces do trabalho imigrante entre o final do século XX e início do século XXI tendo como referência o trabalho de imigrantes brasileiros na construção civil, na limpeza e no cuidado de pessoas, na agricultura e na hotelaria. O período estudado vai de 1986 até 2008. Dentre as fontes documentais utilizadas estão entrevistas orais, notícias de jornais, documentos oficiais e dados estatísticos. O principal argumento defendido é o de que as migrações internacionais e a dinâmica atual do mundo do trabalho não são algo novo na história, mas são parte de uma estrutura que se mantém há mais de 100 anos. Contudo, a conjuntura do final do século XX e início do século XXI é distinta, principalmente em aspectos relacionados ao trabalho imigrante. Tal trabalho vem sofrendo interferências diretas de alterações mais amplas nos padrões produtivos, nas tecnologias e nos fluxos internacionais do capital e força de trabalho. Ao mesmo tempo, ainda que os imigrantes enfrentem piores condições de trabalho, que tenham lugares reservados para atuar e que recebam menores salários que os trabalhadores nacionais, estes não formam um mundo do trabalho à parte, uma vez que estão contabilizados, como precarizados, entre aqueles que devem produzir a riqueza mundial. Outro argumento defendido é o de que no contexto atual também são distintas para os imigrantes as formas de morar e de viver, bem como as noções de família, comunidade, profissão, emprego, sociabilidade, pertencimento e a maneira como estes lidam com o passado, o presente e o futuro. This dissertation investigates the international migration process and the transformations in the world of work between 1986 and 2008 so as to contribute to deepen the research on international migrations and the contemporary world of work. It focuses on Brazilian documented and non-documented immigrants who were living in Portugal or Spain in 2007 and 2008. The specific objectives of this study are (1) to investigate the history of international migrations among Brazil, Portugal, and Spain focusing on the arrival of the Portuguese and Spanish immigrants in Brazil and the context of expansion of the Brazilian migrations to Portugal and Spain; (2) to identify the narratives, memories, territories, and the everyday life of Brazilian immigrants in Portugal and Spain; (3) to uncover the new features of the immigrant work in the end of the twentieth century and beginning of the present century having as reference the work of Brazilian immigrants in areas like civil construction, cleaning, in the care of people, agriculture, and hotel management. The data analyzed in this study consist of recorded interviews in which Brazilian immigrants tell their life stories, newspaper news, official documents, pictures, statistics, among others. The main argument defended in this study is that the international migrations and the present dynamics of the world of work are not something new in the world history, but they are part of a structure that has been in existence for more than a century. However, the conjuncture of the end of the twentieth century and beginning of the present century is different, especially in aspects related to immigrant work which has been suffering direct interferences from wide changes in the productive patterns, in the technology, international flux of capital and work force. Despite the fact that the immigrants have worse working conditions, specific working areas, and get lower salaries that the national workers, they do not form an apart world of work, since they are counted as precarious workers between those who have to generate the worldwide wealth. Another argument defended in this dissertation is that in the present context the ways of living as well as the notions of family, community, profession, employment, sociability, belonging and the way the migrants deal with the past, the present, and the future are also different
