    T2-mapping of the sacroiliac joints at 1.5 Tesla : A feasibility and reproducibility study

    Objective: To evaluate the reproducibility of T2 relaxation time measurements of the sacroiliac joints at 1.5 T. Materials and methods: Healthy volunteers underwent an oblique axial multislice multiecho spin-echo sequence of the sacroiliac joints at 1.5 T. Regions of interest were manually drawn using a dedicated software by two musculoskeletal radiologists to include the cartilaginous part of the sacroiliac joints. A senior radiologist performed the measurement twice, while a resident measured once. Intra- and inter-observer reproducibility was tested using the Bland-Altman method. Association between sex and T2 relaxation times was tested using the Mann-Whitney U test. Correlation between T2 relaxation times and body mass index (BMI) was tested using the Spearman’s rho. Results: Eighty sacroiliac joints of 40 subjects (mean age: 28 ± 4.8 years, range: 20–43; mean BMI: 23.3 ± 3.1, range: 18.9–30) were imaged. The mean T2 values obtained by the senior radiologist in the first series of measurements were 42 ± 4.4 ms, whereas in the second series were 40.7 ± 4.5 ms. The mean T2 values obtained by the radiology resident were 41.1 ± 4.2 ms. Intra-observer reproducibility was 88% (coefficient of repeatability = 3.8; bias = 1.28; p <.001), while inter-observer reproducibility was 86% (4.7; −.88; p <.001). There was significant association between sex and T2 relaxation times (p =.024) and significant inverse correlation between T2 relaxation times and BMI (r = −.340, p =.002). Conclusion: The assessment of T2 relaxation time measurements of sacroiliac joints seems to be highly reproducible at 1.5 T. Further studies could investigate the potential clinical application of this tool in the sacroiliac joints

    Nicorandil, a new vasodilator drug, in patients with essential hypertension

    In 12 mild to moderate hypertensive patients we investigated the acute antihypertensive efficacy of three different doses of nicorandil, a new vasodilating agent which probably acts by increasing the potassium efflux from smooth muscle cells and causing a cellular hyperpolarization. After a 3-day placebo period the patients were given, according to a double-blind Latin-square randomized design, 10, 20 and 30 mg nicorandil as a single acute dose every other day. Blood pressure and the heart rate were measured in both supine and upright positions at various times for 24 h after the dosing; fractional urine collections were obtained at the end of the placebo period and after each active dose. All doses of nicorandil similarly and significantly (P less than 0.01) reduced supine blood pressure, with a peak after 4-6 h (10 mg: -21/-8 mmHg; 20 mg: -20/-9 mmHg; 30 mg: -29/-17 mmHg), and the effect was still present, though reduced, after 24 h; no change in the heart rate was observed. The results from the upright position were similar. There were no significant changes in urine volume and electrolyte excretion during the nicorandil administration. The three different doses of nicorandil caused similar acute blood pressure reductions without change in the heart rate, nor in the urine volume and urinary sodium

    T2 mapping of the sacroiliac joints in patients with axial spondyloarthritis

    Purpose: To test whether T2 mapping of the sacro-iliac joints (SIJs) might help identifying patients with spondyloarthritis. Method: This study included 20 biologic-naive patients with axial spondyloarthritis (10 females; mean age: 38 ± 9years; range, 19–47) and 27 controls (16 males; mean age = 39 ± 13years; range = 28–71) who prospectively underwent SIJs MRI at 1.5 T, including a multislice multiecho spin-echo sequence. Standard MRIs were reviewed to assess the SIJs according to the Assessment of SpondyloArthritis International Society (ASAS) criteria and SPondyloArthritis Research Consortium of Canada (SPARCC) MRI index. T2 maps obtained from multiecho sequences were used to draw regions of interests in the cartilaginous part of the SIJs. Disease activity was assessed using BASDAI questionnaire. Bland-Altman method, ROC curve analysis, Chi square, Mann-Whitney U, Pearson's and Spearman's correlation coefficient were used for data analysis. Results: According to ASAS criteria, MRI was positive for sacroiliitis in 5/20 patients (25 %). Inter-observer reproducibility of T2 values was 87 % (coefficient of repeatability = 7.0; bias = 0.49; p &lt; .001). Mean T2 values of patients (58.5 ± 4.4 ms, range: 52.6–68.2 ms) were significantly higher (p &lt; .001) than those of controls (44.1 ± 6.6 ms, range: 33.6–67.2 ms). A T2 value of 52.51 ms yielded 100 % sensitivity and 91.7 % specificity to differentiate patients from controls. No statistically significant association/correlation was found between T2 values and BASDAI (r=˗.026, p = .827), disease duration (r = .024, p = .871), SPARCC (r=-.004, p = .981), ASAS criteria (p = .476), HLA-B27-positivity (p = .139), age (r=-.2.53, p = .891), and gender (p = .404). Conclusions: T2 relaxation times of the SIJs were significantly higher in patients than in healthy controls, making this tool potentially helpful to early identify patients with spondyloarthritis

    Happy Air (R): a school-based educational program to maximize detection of asthma in children

    Objective.To investigate whether an active partnership among school, parents, and pediatricians allows early identification and treatment of asthmatic children. Methods: An asthma educational program (Happy Air R ), based on a strong family-physician-school interrelationship, was performed in six primary schools (2,765 children) before administering a screening questionnaire to the parents. Results. A high response rate (96%) demonstrated 2,649 responders available for the asthma screening: 135 children (5%) received a diagnosis of asthma, of which 37 (27%) were recognized de novo. Conclusion. The active participation of school and parents is the determining factor for the success of an asthma screening program

    MonografiaA atual conjuntura política e climática demanda da comunidade internacional um esforço no sentido da redução da emissão de gases geradores de efeito estufa. Uma realidade que implica no esforço de substituir as contribuições dos combustíveis fósseis na matriz energética global por fontes renováveis. Dentro desse cenário, por se encontrar majoritariamente em uma região tropical, o Brasil tem disponível, em especial no Rio Grande do Norte, grandes áreas onde a incidência solar é intensa e que, portanto, apresentam um potencial para a geração de energia elétrica sem a necessidade da utilização de combustíveis fósseis. Adicionalmente essa potencialidade contribui para a diversificação da matriz energética com o consequente aumento da segurança energética do país. No presente trabalho foi realizada uma revisão bibliográfica através de artigos científicos, livros específicos da área, revistas, portais de informação e relatórios de entidades civis e governamentais. Sendo essa metodologia implementada com o objetivo de demonstrar as tecnologias disponíveis para a geração de energia elétrica utilizando fontes fototérmicas e fotovoltaicas, descrevendo o seu funcionamento e apresentando as suas principais vantagens e desvantagens. Através do presente estudo foram compreendidas as principais tecnologias e suas configurações de acordo com a disponibilidade de infraestrutura sendo detectado que a nível nacional e regional as tecnologias predominantes são os módulos fotovoltaicos do tipo ongrid. Durante a realização pesquisa detectou-se o interesse de empresas em implementar fábricas de módulos solares na Estado do Rio Grande do Norte resultando num investimento da ordem de R$ 112 milhões e com o potencial de gerar 1300 novos empregos, enquanto em termos nacionais há uma expectativa que em 2027 alcancemos um total de 1,35 milhões de pessoas sendo atendidas por sistemas de energia fotovoltaica e que o seja alcançada uma geração de 11,9 GW.Trabalho não financiado por agência de fomento, ou autofinanciad
