"Disparities in Kidney Transplantation in the Immigrant Patient Population in Italy: Towards an Intervention to Improve Equity "
In Italy, many adult patients with end-stage kidney disease (ESKD) are of non-European Union (EU) origin.
Kidney transplant (KT) is the gold standard for treatment of ESKD. Italy guarantees universal access to KT
and post-transplant regimens for all clinically eligible migrants registered in the National Health System under
the same conditions as nationals. Yet, immigration is an important social determinant of health, with the
potential for disparities in accessibility, quality, and outcomes of care. Prior research has shown that non-EUborn individuals in Italy are more likely to experience unequal access to and quality of care because of potential
barriers associated with immigration. While disparities in KT for ethnic minority patients are well-known,
studies on disparities associated with immigration background remain scarce and none have ever been
performed in Italy. This study pursues three main objectives: (1) to assess whether disparities exist in KT
outcomes in the immigrant patient population in Italy, (2) to discuss the ethical dimensions with the potential
for disparities in this vulnerable group of patients, and (3) to explore the causal mechanisms linking
immigration with disparities in KT and identify areas for intervention. Three research phases were outlined to
pursue these objectives: (1) A study to assess disparities in KT outcomes in the immigrant patient population
was retrospectively performed based on national-level data of the Italian National Transplantation Center. (2)
Three clinical cases describing the challenges that might account for disparities in KT in this group of patients
were analyzed by the Stanford Integrated Psychosocial Assessment for Transplantation and by the Four-Boxes
Method for ethical decisions in clinical medicine. (3) A conceptual model of the causal mechanisms of
disparities was developed and the potential for intervention to reduce them was discussed. Study 1 revealed
that non-European immigration background is associated with long-term kidney graft function decline. Study
2 found that the Four-Boxes Method is a useful tool to gain a more comprehensive picture of the ethical
dimensions of single clinical cases and to fulfill the ethical obligation to provide whole-person care. Study 3
suggests that multiple modifiable factors may explain disparities in KT. While it would be necessary to act
directly upon the structural causes of disparities, it is more realistic to foster action on modifiable risk factors
by development of targeted interventions with the potential to enable the prevention/mitigation/elimination of
disparities in KT in this vulnerable group of patients. Prospective studies are needed to further elucidate the
causal mechanisms linking immigration with disparities in KT more rigorously. Further, research is necessary
to develop interventions and to assess their effectiveness in immigrant patients pursuing KT

Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Protocolo y ceremonial en tres obras del pintor Luis Paret y Alcázar
Art, and specifically painting, can represent scenes of protocol and ceremonial, its validity, for the student of these disciplines, will depend on whether what is represented is contemporary to its author or if it is only a representation of a legendary historical past. If the author witnessed these ceremonies or knew them first-hand, the scenes become a very valuable tool for the study of the history of protocol, which in this way passes from the printed letter to the graphic representation.
The work presented aims to review the protocol and ceremonial present in three court works by the painter Luis Paret y Alcázar (1746-1799): Royal Couples; Charles III dining before the Court and The Oath of Allegiance to Don Fernando as Prince of Asturias. Through the interpretation of the ceremonial reflected in the paintings and the review of the contemporary documentation in which these ceremonial acts are described, we will try to find out the fidelity they keep with reality and, therefore, their validity as a source for the scholar of protocol.
El arte, y en concreto la pintura, puede representar escenas de protocolo y ceremonial, su validez, para el estudioso de esas disciplinas, dependerá de si lo representado es contemporáneo a su autor o un pasado histórico legendario. Si el autor fue testigo de esas ceremonias o las o conoció de primera mano, las escenas se convierten en una herramienta muy valiosa para el estudio de la historia del protocolo, que pasa así de la letra impresa a la representación gráfica.
El trabajo que se presenta pretende hacer una revisión del protocolo y ceremonial presentes en tres obras de corte del pintor Luis Paret y Alcázar (1746-1799): “Las parejas reales”; “Carlos III comiendo ante su corte” y “La jura de don Fernando como príncipe de Asturias”. A través de la interpretación del ceremonial reflejado en los cuadros y la revisión de la documentación de la época en la que se describen esos actos ceremoniales, se intentará averiguar la fidelidad que guardan con la realidad y, por tanto, su validez como fuente para el estudioso del protocolo
Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more
sophisticated methods
