    Integrating three datasets of postpartum haemorrhage

    We harmonized four anonymized datasets. ANDERCAISM: the first dataset was obtained from CAISM Women’s Hospital, a tertiary referral maternity center in Brazil, and contains detailed, high-resolution clinical and hemodynamic data of 270 women, including measurements of blood loss and vital signs recorded every 5 to 15 minutes during the first two hours postpartum1 The additional datasets consisted of 2,390 postpartum cases pooled from three independent studies named after their abbreviations ADBRCT2, FAUOSMFT3, AND TRCTOM4 described below. Anonymized data from these studies were retrieved from the website https://dataverse.harvard.edu/ ADBRCT: A double-blind, randomized controlled trial conducted in Argentina comparing intravenous (IV) versus intramuscular (IM) oxytocin administration during the third stage of labor (n = 543), with blood loss measured using calibrated receptacles for a minimum of one hour postpartum.20 FAUOSMFT: Fever after use of sublingual misoprostol for treatment of postpartum hemorrhage: A prospective observational study evaluating the side effect profile of sublingual misoprostol for PPH treatment in Corrientes, Argentina (n = 49).21 TRCTOM: Two large, double-blind, randomized, placebo-controlled non-inferiority trials comparing sublingual misoprostol with intravenous oxytocin for PPH treatment across multiple LMIC settings, including Argentina and other countries (n = 1,787).22 Each study used standardized protocols for blood loss estimation and adhered to a consistent clinical definition of PPH as blood loss ≥500 mL. While not all external studies captured blood loss at high temporal resolution, they contained overlapping predictors with the internal CAISM dataset, which allowed for harmonization and external validation of predictive models using a reduced but comparable feature set. 1 - Pacagnella, R.     How corporate governance and globalization can run afoul of the law and good practices in business: The Enron's disgraceful affair.

    The purpose of this paper is to set out the Enron’s demise into the perspective of Corporate and Global Governance. To accomplish this target, the incremental cash flow model is expanded to give room for governance issues, while a functional introduction to information sets is developed, including bounded rationality, asymmetric information, opportunistic behavior, transaction costs and agency problems. Then, corporate governance is linked to globalization by means of some recent approaches that go beyond a narrow economic mindset to encompass a far-reaching dynamics. Taking advantage of such background, the Enron’s story is tracked down over a span of fifteen years since its starting day to its bankruptcy filing. Leading events are explained from corporate and global governance viewpoints, while an in-depth analysis is worked out on Enron’s complex game of deception and breach of contracts: the outrageous affiliated limited partnerships, the lavish pay package to its executives, the involvement with global governance through the Indian affair and the Taliban connection. It is for the incremental cash flow model to explain malfeasance with cash flows from assets, and how cash flows to creditors were actually contrived.     "A New Technique for Reconstructions of Large Metadiaphiseal Bone Defects"

    Tumors involving the metadiaphyseal region around the knee are often treated by intraarticular resection (and reconstructed by prosthesis or osteoarticular allografts) even when the subchondral bone could be saved and an intercalary reconstruction applied. This is due to problems associated with the reconstruction of the subarticular diaphyseal bone defects by conventional methods. Intercalary allografts (as well as cortical autografts) have significant complication rates in terms of delayed union or graft fracture: these would be significantly increased in those patients in whom an adequate osteosynthesis for support is not feasible. The use of a vascularized fibula alone is contraindicated because it cannot provide sufficient stability for the epiphysis and is too weak to allow early weight bearing. The Ilizarov technique (bone transportation) is extremely time consuming and it may be at risk in patients under aggressive chemotherapy (pin tract infection, possible interference with callus formation). Furthermore, the thin subarticular segment requires an additional ring across the joint, causing knee stiffness. The authors describe a new surgical technique to manage this surgical problem. The basic idea consists to bridge the bone defect using a massive allograft as a peripheral shell supporting a centrally placed micro-vascular fibular autograft.     Occurrence of sexual dysfunction among sterilized women in Ribeirão Preto (São Paulo - Brazil)

    Introdução: Embora a contracepção seja bastante prevalente no Brasil (77%), apenas dois métodos predominam: o contraceptivo hormonal oral e a esterilização cirúrgica (LT). No entanto a LT não é inócua e pode trazer diversas conseqüências para a vida dessas mulheres que se submetem a ela. Dentre estas pode estar a deterioração da função sexual o que seria contraditório visto que a LT objetiva uma vida sexual melhor, menos atemorizada pelo medo da gravidez. O presente estudo teve como objetivo avaliar a função sexual de mulheres submetidas à LT. Métodos: Estudo transversal de 235 casos, representativos de 1826, com dados obtidos através de inquérito da função sexual entre as mulheres laqueadas pelo SUS em Ribeirão Preto(SP) entre 2000 e 2004, utilizando-se o Female Sexual Function Index (FSFI) adaptado para o contexto brasileiro. Resultados: As entrevistadas tinham em média 35,9 anos e foram esterilizadas em média aos 33,3 anos; 89,8% estavam em união marital, 57,9% declararam-se brancas e 66,8%, católicas, tinham em média 6,1 anos de estudo e 76,6% pertenciam às classes C e D; 93,4% referiram ter um bom relacionamento conjugal e 59,5% declararam que o relacionamento não mudou após o procedimento. A média de filhos vivos foi 3,2, resultaram aborto 8,8% das gestações, 71,2% resultaram partos vaginais e 28,8%, cesáreas; 52,3% usaram pílula 6 meses antes da cirurgia; 98,7% responderam estar satisfeita com a cirurgia e 6,8% referiram dor pélvica. Em geral, 32,5% das mulheres apresentaram escores de índice com risco para disfunção sexual medido pelo FSFI. Foi observada associação entre a variável disfunção sexual e categoria de escolaridade, renda per capita, dor pélvica, número de gravidezes, número de partos vaginais e de cesáreas. Observou-se correlação negativa entre o escore de função sexual e o número de filhos vivos e correlação positiva entre o escore e renda familiar, renda per capita e os valores de classificação econômica. Conclusão: A partir dos dados obtidos, pôde-se observar que dentre as mulheres laqueadas do estudo a presença de disfunção sexual estava associada à dor pélvica e maior número de cesarianas, assim como a situações ligadas à vulnerabilidade social (baixa renda e escolaridade e maior número de filhos).Introduction: Although contraception is high prevalent in Brazil (77%), only two methods prevail: the hormonal pills and the surgical sterilization. However female sterilization is not innocuous and it can bring several consequences for those women\'s life. Among these consequences it can be to deterioration of the sexual function what is contradictory sees that the use of contraceptional methods aims at a better sexual life, less frightened by the fear of pregnancy. The present study had as objective evaluates the women\'s sexual function submitted to LT. Methods: prevalence study of 235 cases, representative of 1826, with data obtained through inquiry of the sexual function among the women sterilized by the public health system in Ribeirão Preto(SP) between 2000 and 2004. There has been used the Sexual Female Function Index (FSFI) adapted for the Brazilian context. Conclusion: Starting from the obtained data, it could be observed that among the sterilized women the presence of sexual dysfunction was associated to the pelvic pain and larger number of cesarean operations, as well as linked situations to the social vulnerability (low income and education and larger number of children)

    RECENT DEVELOPMENTS IN LAND TENURE LAW IN ERITREA, HORN OF AFRICA

    After a historical sketch of the evolution of land tenure systems in the Eritrean highlands, this paper describes the main features of the new Eritrean land law and its operative assumption that the legislation is meant to extend state control over land. The legal devices employed by the law are widely used in sub-Saharan Africa (and were largely inspired by colonial policies). The State of Eritrea frequently asserts that its recent independence gives it the opportunity to learn from other developing countries' mistakes and to avoid them. The basic patterns of the new land law, however, are common to the rest of Africa, notwithstanding the evident poor results. The central government wants its control to be widespread and pervasive. The fight against traditional social groups controlling land, at least in the highlands, is severe. Apart from a formal repeal of customary law, the state's acquisition of the power to modify village boundaries according to a scheme already completed at higher administrative levels and to introduce equal rights on land for women entails a disruption of the villages' social identity. Mandatory state control over landed property in Eritrea is, as usual, motivated by the necessity to address higher social needs. The ultimate intent, of course, is that the evolution from communal property to state property will eventually result in the widespread introduction of individual property once a sufficient level of economic development is achieved.     Assessing and managing hypovolemic shock in puerperal women

    The major cause of maternal death worldwide is postpartum hemorrhage (PPH). Early identification is the basis for adequate treatment. In addition to the visual estimation of blood loss, clinical signs could offer a more reliable representation of the cardiovascular system of the bleeding woman. However, in postpartum women, recognition of hypovolemic shock through vital signs is impaired owing to physiological cardiovascular changes in pregnancy. The Shock Index [SI] is one composite vital sign that may help in the identification of women with hypovolemic shock. Values of SI >= 1 in the first hour postpartum indicate cardiac decompensation, and treatment should be implemented immediately. From the diagnosis of PPH, first-line measures should ensure coordinated care actions including the availability of blood derivatives, the establishment of conditions for volume replacement, oxygen therapy, and identification and timely treatment causes of bleeding. Importance of Ge in photonics has grown and through techniques developed in our group we demonstrated low density of dislocations (1x1019cm-3. Both devices showed improved performance compared to pin Ge LED. Furthermore, heavy doped Ge diodes exhibit evidence of bleaching or transparency. The techniques described permitted the development of Ge-on-Si laser with a concentration ~1-2x1019cm-3. It is the first demonstration of a Ge laser optically pumped working under the direct band gap assumption like other semiconductors. It represents the evidence of carrier inversion on an indirect band gap semiconductor. With 50cm-1 gain, the material shows Fabry-Perot cavity behavior. Finally, we demonstrated a fully functioning laser diode monolithically integrated on Si. Ge pnn lasers were made exhibiting a gain >1000cm-1 and exhibiting a spectrum range of over 200nm, making Ge the ideal candidate for Si photonics.by Rodolfo E. Camacho-Aguilera.Ph.D

    Simple and enlarged separation portfolios. On their Use when Arbitraging and Synthesizing Securities.

    This paper seeks to provide a framework for separation portfolios when they are used not only as synthetics of a matching security but also as building blocks of arbitrage portfolios, in a background provided by the CAPM world. Firstly, synthetics are defined by means of a vectorial framework that maps portfolios onto their risk-return profiles. Separation portfolios are extensively analyzed afterwards, establishing three propositions that lay the groundwork for using them as synthetics. Next, a distinction is brought about between plain separation portfolios (which are located on the Capital Market Line) and enlarged separation portfolios (which lie outside the CML). Furthermore, it is shown that simple separation portfolios become synthetics in few cases only, whereas enlarged portfolios allow for synthetics in much wider contexts. Later, arbitrage portfolios are designed by means of simple separation portfolios in the context of the Security Market Line, and also by resorting to the enlarged ones without requiring the SML as a benchmark.
