An empirical evaluation of recovery transformation at a large community behavioral healthcare organization

    No full text
In recent decades, the concept of “recovery” from severe mental illness (SMI) has gained increased prominence among organizations providing behavioral health services. Organizations in many states are currently developing plans to transform their mental health systems in accordance with recovery-oriented care. Even though efforts to bring the principles of recovery to mental health agencies have been well documented in the United States and abroad, there is little empirical evidence to suggest that recovery-oriented services are advantageous. The purpose of this longitudinal study was to evaluate the effectiveness of a recovery-oriented transformation carried out by a large private not-for-profit behavioral healthcare organization serving individuals with SMI. This transformation targeted the philosophy, and specific procedures involved in the provision of care to consumers within the organization. The outcome variables selected to evaluate the impact of the transformation were grouped into the following categories: 1) organizational performance indicators; 2) consumer recovery indicators; 3) staff competencies; and 4) processes that promote recovery. Six hundred and twenty seven consumers and 490 staff participated in the evaluation. The findings suggest that recovery-oriented services had a positive impact on rates of overnight hospitalization, consumers’ ability to function in the community, professional competencies of direct care employees, and working alliance between direct care providers and consumers. This indicates that comprehensive and well structured recovery-oriented behavioral healthcare may offer a cost-efficient and effective alternative to the deficit approach in mental health.Psy.DIncludes bibliographical referencesby Igor Malinovsk

Previously, we have shown that the weekly load of rats with water-salt solutions of different chemical compositions causes both general and specific reactions of the parameters of metabolism. The purpose of this study is to identify under these conditions specific neuroendocrine and immune responses. Materials and methods. Experiment was performed on 58 healthy female Wistar rats 240-290 g divided into 6 groups. Animals of the first group remained intact, using tap water from drinking ad libitum. Instead, the other rats received the same tap water as well as waters Sophiya, Naftussya, Gertsa and its artificial salt analogue through the probe at a dose of 1,5 mL/100 g of body mass for 6 days. The day after the completion of the drinking course in all rats some neuroendocrine and immune parameters were registered. Results. The method of discriminant analysis revealed 29 parameters of the neuroendocrine-immune complex (10 of them reflect the neuroendocrine regulation, 4 thymus mass and thymocytogram elements, 5 elements of splenocytogram, 10 elements of immunocytogram and leukocytogram of blood and parameters of phagocytosis), according to which the reaction on various water-salt loads are identified with an accuracy of 98.3%. Conclusion. The peculiarities of the reactions of the parameters of the neuroendocrine-immune complex are due to the content of water in sulfate, bicarbonate and magnesium, as well as organic carbon and nitrogen. There is a hypothesis that urate is not only the end product of the metabolism of purines like adenosine, but even more a novel target for neuroprotection. The purpose of this study, conducted in line with this hypothesis, is to clarify the relationship between the level of uricemia and EEG/HRV parameters both in men and women. Materials and Methods. The object of observation were almost healthy volunteers: 30 females (30÷76; 49±13 y) and 31 males (24÷69; 47±12 y). In basal conditions we estimated the state of the autonomous regulation by the method HRV, recorded EEG, determined level of uric acid and testosterone as well as estimated level of the trait and reactive anxiety. After 4 or 7 days, repeated testing was performed. Results. The method of canonical correlation analysis revealed a strong urato-neural relationships in both men (R=0,709; R2=0,503; Adjusted R2=0,393) and women (R=0,812; R2=0,659; Adjusted R2=0,542). Moreover, in women, the factor structure of the neural root is almost completely different from that in men, and also includes reactive anxiety and HRV parameters, which are absent in the factor structure of men. In addition, significant differences were found between the profiles of urato-neural correlations of women of reproductive age and postmenopausal women. Conclusion. The level of uricemia is significantly related to a number of parameters of EEG, HRV and anxiety. The strength and even the direction of the correlations are determined by factors linked to gender. Keywords: uricemia, EEG, HRV, anxiety, testosteroneemia, men, women

Genetic association study of QT interval highlights role for calcium signaling pathways in myocardial repolarization.

The QT interval, an electrocardiographic measure reflecting myocardial repolarization, is a heritable trait. QT prolongation is a risk factor for ventricular arrhythmias and sudden cardiac death (SCD) and could indicate the presence of the potentially lethal mendelian long-QT syndrome (LQTS). Using a genome-wide association and replication study in up to 100,000 individuals, we identified 35 common variant loci associated with QT interval that collectively explain ∼8-10% of QT-interval variation and highlight the importance of calcium regulation in myocardial repolarization. Rare variant analysis of 6 new QT interval-associated loci in 298 unrelated probands with LQTS identified coding variants not found in controls but of uncertain causality and therefore requiring validation. Several newly identified loci encode proteins that physically interact with other recognized repolarization proteins. Our integration of common variant association, expression and orthogonal protein-protein interaction screens provides new insights into cardiac electrophysiology and identifies new candidate genes for ventricular arrhythmias, LQTS and SCD
