    Focusing in on use of pharmacokinetic profiles in routine hemophilia care

    Emergence of population pharmacokinetic models for prediction of individual pharmacokinetic (PK) profiles facilitates individualization of prescribed prophylactic therapy for patients with hemophilia A and B and may have a favorable impact on clinical outcomes and annual factor utilization. How providers approach the integration and application of these data into routine clinical practice is not clear. To explore the potential application of and barriers to incorporating PK profiles into current hemophilia prophylaxis decision making. A facilitated group discussion of hematologists practicing within the federally-supported United States Hemophilia Treatment Center Network was conducted. Separately, a group of parents of patients with severe hemophilia less than 18 years of age participated in a focus group on individualizing prophylactic factor regimens with the use of PK data. Physician participants constructed a conceptual model for factors that determined their selection of hemophilia prophylaxis. These factors clustered in five groupings. When charged with creating a prophylaxis regimen for a specific clinical case including PK data, eight of nine providers generated a unique regimen. Parent focus group supported PK data use as they preferred data driven treatment decisions. Clinician application of PK data for prophylaxis decision making is heterogeneous. Prospective evaluation of the use of PK-tailored prophylaxis in routine care and its impact on patient outcomes is needed. Parents perceived that, while obtaining blood draws could be challenging, images of factor activity decay informed their decisions about physical activity timing and provided an opportunity for partnership and shared decision making with their provider

    Geodynamic implications derived from Numidian-like distal turbidites deposited along the Internal–External Domain Boundary of the Betic Cordillera (S Spain)

    New data reveal Early Burdigalian ‘Numidian-like lithofacies’ in successions of the internal (southernmost) part of the South Iberian Margin (SIM) and the south-western margin of the Mesomediterranean Microplate (MM). The well-known Numidian Formation was deposited in the external (Massylian) sub-domain of the Maghrebian Flysch Basin (a south-western branch of the Tethys Ocean). The anomalous occurrence of ‘Numidian-like lithofacies’ is induced by the particular Early Miocene palaeogeographical and geodynamic complexity of the sector. This consisted of a ‘triple point’ with a dextral transform fault between the SIM and the MM-Maghrebian Flysch Basin system. In this framework, the ageing of Iberian reliefs and the MM collapse, coupled with an African Margin upbulging, and a shortening of the Maghrebian Flysch Basin (both related to the subduction), could have resulted in the arrival of the Numidian depositional system from so far away.This research was supported by Urbino University ‘Carlo Bo’ grant (responsible F.     Este artículo se propone el estudio de los motivos caballerescos en El Vasauro de Pedro de Oña como una perspectiva de análisis. Así, es posible ubicar claramente esta obra del siglo XVII dentro de lo que consideramos literatura caballeresca hispánica. Focaliza su atención en la figura de la doncella guerrera y las variantes del motivo asociadas a ella. Concluye la importancia de las funciones que tiene este motivo en la obra y destaca cómo pervive y continúa la tradición caballeresca, adaptándose a las necesidades e ideología de ese momento. Asimismo, se indica que el poeta lleva a cabo la adaptación del motivo sobre una base de hechos históricos que recrea literariamente.This article proposes the study of the chivalric motives in El Vasauro by Pedro de Oña, as a way to approach the literary analysis of the work. In this perspective, it is possible to understand this poem from Seventeenth Century within the Hispanic literature of chivalry. The article focuses on the character of the warrior lady and its variations. It shows the significant functions of this motif in Oña’s work, and how he used the motives adapting them to the current needs and ideology of the poem. The author makes an adaptation of the motif over a basis of historical events besides its artistic recreation

    Laser anemometry and pressure measurements in the endwall region of an annular turbine cascade utilizing a pressurized aerodynamic window

    The purpose of this research was to compare previous laser anemometry measurements obtained through an unpressurized laser window with the results from a pressurized laser window and to validate this innovative measuring technique in the endwall region of a confined annulus. Two dimensional velocity, flow angle, and turbulence intensity measurements were obtained with a fiber optics laser Doppler velocimeter. The measurements were performed through a 1.09 mm opening in the endwall region of an annular turbine cascade at depths ranging from 0.01 mm to 0.89 mm with varying pressure applied to the chamber of the modified window. Cobra probe measurements were performed to validate the flow angles obtained by the laser anemometer.
