Caracterización de la muerte encefálica y el estado vegetativo persistente
Se realizó un estudio longitudinal, prospectivo, observacional, para caracterizar la muerte encefálica y el estado vegetativo persistente como alteraciones de conciencia que presentan diferencias esenciales. La muestra quedó constituida por 357 sujetos en muerte encefálica y 37 en estado vegetativo persistente, egresados del Hospital Provincial “Saturnino Lora”, desde enero de 1994 a diciembre de 2007. El análisis estadístico inferencial se centró en la identificación de asociación estadísticamente significativa entre criterios de interés seleccionados. Fue aplicado el Test χ2 de homogeneidad. Se demostró que el diagnóstico de muerte encefálica se puede realizar con el examen clínico sin pruebas instrumentales. Se evidenciaron diferencias anatomopatológicas: para la muerte encefálica predominaron el cerebro de respirador y el edema cerebral difuso; mientras que para el estado vegetativo persistente, las necrosis del tálamo y laminar de la corteza. Se comprobó por Tomografía computarizada que el daño axonal difuso caracteriza al estado vegetativo persistente. Se demostró que los pacientes en muerte encefálica están muertos y la pérdida de las funciones del encéfalo se erigió como atributo esencial para identificar la muerte con fundamentaciones neurológicas. Los enfermos en estado vegetativo persistente están vivos porque no cumplen criterios de muerte desde el punto de vista neurológico ni cardiovascular
Evaluación del control autónomo vascular en las neuropatÃas periféricas
En esta investigación se evaluaron las funciones autónomas: parasimpática cardiaca y simpática α-adrenérgica vascular, en las etapas más tempranas de diversos tipos de polineuropatías: Diabetes, CIDP, NEC y CMT. Para comparar el estado de ambas funciones se monitorizaron simultáneamente las variaciones de la frecuencia cardiaca y la tensión arterial continua, la cual no se había estudiado previamente en Cuba. En todos los grupos de enfermos se evidenciaron alteraciones leves a moderadas de la inervación simpática vascular. Las latencias de recuperación de la TA durante la fase II de la maniobra de Valsalva se afectaron más intensamente que sus cocientes de amplitud. Los pacientes con Diabetes y CIDP también presentaron alteraciones de la inervación parasimpática cardiaca. En todas las polineuropatías, las alteraciones simpáticas vasculares fueron significativamente más frecuentes e intensas que las parasimpáticas cardiacas. Estos resultados demuestran que en las polineuropatías estudiadas, diabéticas y no diabéticas, las lesiones autónomas, vasculares periféricas y cardiacas, pueden presentarse de forma independiente o combinada. Por tanto, el empleo conjunto de instrumentos para evaluar de forma independiente las funciones parasimpática cardiaca y simpática vascular permitiría optimizar la identificación de las disautonomías, iniciar tempranamente su tratamiento y reducir la morbilidad y mortalidad cardiovascular de estos pacientes
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Estado vegetativo persistente: un reto para las Neurociencias contemporáneas
OBJECTIVES: To review literature about persistent vegetative state, to base criteria based on our experiences, to set out our arguments on the vital support and to contribute to the scientific spreading and the debate.DEVELOPMENT: The persistent vegetative state constitutes a controversial subject, in the chapter of contemporary alterations of it brings back to consciousness of the Neurosciences and was described in 1972 by Jennett and Plum. Epidemiology indicates to us that every time the number of patients is greater who after severe head injury or another injury of the nervous system happen to this state, of here the necessity to know this organization and to prepare us for their medical attention. This state is characterized by the complete absence of behavioural evidence for self or environmental awareness with total or partial preservation of the functions of the stem brain and hypothalamus. There is preserved capacity for spontaneous or stimulus–induced arousal, evidenced by sleep–wake cycles. These patients are not unconscious nor in comma in the usual sense of the word, are wide–awake without brings back to consciousness.CONCLUSIONS: It is an incorrect, arbitrary attitude, to describe the state as these patients like not deserved of being lived. From the ethical point of view, we considered that it constitutes an attack against the dignity of the human being to retire the hydration and the nutrition to a patient in persistent vegetative state. These patients are alive reason why we must offer the medical aid them who need.OBJETIVO: Revisar la literatura sobre el estado vegetativo persistente, fundamentar criterios basado en nuestras experiencias, exponer nuestros argumentos sobre el soporte vital y contribuir a la divulgación científica y al debate.DESARROLLO: El estado vegetativo persistente constituye un tema polémico en el capítulo de alteraciones de conciencia de las Neurociencias contemporáneas y fue descrito en 1972 por Jennett y Plum. La epidemiología nos indica que cada vez es mayor el número de enfermos que después de un traumatismo craneoencefálico grave u otra lesión del Sistema Nervioso pasan a este estado, de aquí la necesidad de conocer esta entidad y prepararnos para su atención médica. Dicha condición clínica esta caracterizada por inconsciencia, con preservación total o parcial de las funciones del tallo e hipotálamo. Estos pacientes se reponen de un sueño tipo coma, tienen ciclos sueño vigilia con despertares, apertura de los ojos, así como reactividad limitada a posturas primitivas además de movimientos reflejos de las extremidades, sin lograr verbalizar. Estos enfermos no están inconscientes ni en coma en el sentido usual de la palabra, están despiertos sin conciencia.CONCLUSIONES: Es una actitud incorrecta, arbitraria, calificar el estado de estos pacientes como no merecido de ser vivido. Desde el punto de vista ético, estimamos que constituye un atentado contra la dignidad del ser humano retirar la hidratación y la nutrición a un enfermo en estado vegetativo persistente. Estos enfermos están vivos por lo que debemos brindarles la asistencia médica que necesiten
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases.
