    A Patriotic Internationalism: The Tunisian Communist Party’s Commitment to the Liberation of Peoples

    At its onset, internationalism has been the horizon of struggle of communism and the spearhead of its strategy. In the Third World, however, it has been remoulded due to the spread of the nationalist wave and the challenge of underdevelopment. By focusing on the Tunisian Communist Party (PCT), this chapter examines key features of its commitment to the external front, notably in the ‘tricontinental’ arena, subsequent to the nationalist turn of the mid-1950s. After analysing the PCT’s stance towards the Arab movement of national liberation, special attention will be devoted to the Palestinian and Sahrawi causes. Th e different stance adopted by the PCT – activism in the first case and taciturnity in the second one – sheds light on the party’s priorities and networks, where pride of place was given to the Soviet Union and the Maghrebi communist parties

    Tra le tante tematiche relative allo spazio privato in questo volume verranno affrontati nella loro complessità aspetti personali che riguardano l'individuo quali nascita, alimentazione, sessualità (comprese le questioni d identità), maternità, malattia, morte, e la stretta connessione con il contesto culturale e sociale più ampio che condiziona pratiche e credenze condivisi come i rituali di guarigione e di passaggio. Per quanto riguarda lo spazio pubblico saranno oggetto di studio i rapporti tra la società civile e la politica (nascita di movimenti, sindacati, partiti ecc..), tra la collettività e l'amministrazione, i mezzi di comunicazione e le autorità, l'individuo e le normative che ne regolano la vita privata come il diritto di famiglia, la fecondazione assistita e i diritti e libertà individuali. Lo spazio pubblico è inteso anche alla lettera come luogo d incontro e organizzazione della collettività in un intersecarsi ed evolversi della realtà urbana e architettonica. Non mancheranno studi relativi ai rapporti politici ed economici tra l Europa o più in particolare tra l'Italia e i Paesi in oggetto. Inoltre sarà dato ampio spazio alla scrittura letteraria e autobiografica sia come processo di esternizzazione del privato sia come re-interpretazione e evocazione di luoghi pubblici e politici

    To investigate the safety and efficacy of a synthetic prostacyclin analogue (epoprostenol) for circuit maintenance during continuous veno-venous haemofiltration (CVVH) in patients with acute renal failure (ARF).Observational case study.University-affiliated six-bed intermediate renal care unit in a nephrology and internal medicine department of a 1300-bed teaching hospital.A consecutive series of critically ill ARF patients in whom prostacyclin was the sole anti-haemostatic agent used for CVVH.Bicarbonate-based CVVH in pre-dilution (1.5 l/h); blood flow rate at 200 ml/min; prostacyclin at 4 ng/kg per min infusion in the extracorporeal circuit before the haemofilter.Fifty-one ARF patients (mean APACHE II 27.2, SD 7.8; acute tubular necrosis in 44/51, 83\%; mechanical ventilation 14/51, 21\%; in-hospital mortality 28/51, 54\%) underwent CVVH for a total of 4040 h (230 circuits, median number 4 circuits per patient, range 1-13). Four patients out of 51 (7.8\%) experienced major bleeding during CVVH (1.0 episode per 1000 patient-hours of treatment; 95\%CI, 0.4-2.6); no death could be attributed to haemorrhage. Therapeutic intervention for hypotension (fluids and/or vasopressors) was required in 15.5\% of the CVVH sessions monitored. The median duration of the circuit was 15.0 h (95\% CI, 13.0-16.5).The use of prostacyclin as the sole anti-haemostatic agent for CVVH entails a low risk of haemorrhagic complications, while maintaining the patency of the circuit long enough to allow the delivery of an adequate dose of renal replacement therapy. Further studies are needed to compare this technique to other anti-haemostatic strategies for CVVH

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

    Renal involvement in secondary amyloidosis of Muckle-Wells syndrome: marked improvement of renal function and reduction of proteinuria after therapy with human anti-interleukin-1β monoclonal antibody canakinumab

    Muckle-Wells syndrome (MWS) is a rare hereditary autoinflammatory disorder characterized by recurrent urticaria-like skin rashes, arthralgias, conjunctivitis, hypoacusia, and risk of reactive AA amyloidosis due to the progressive accumulation of amyloid fibrils in different organs. Its genetic defect lies in mutations in the NLRP3 gene, encoding the cryopyrin protein, and resulting in interleukin (IL)-1β oversecretion. Renal involvement, in terms of proteinuria or renal insufficiency, can be observed in up to 25 % of patients. Herein, we describe our experience with two Caucasian patients, father and son, aged 52 and 26 years, respectively, heterozygous for both V198M and R260W NLRP3 mutations who had AA amyloid deposits on renal biopsy. The fully human monoclonal antibody canakinumab, providing selective and prolonged IL-1β blockade, was administered in both patients every 60 days over a period of 18 months. This treatment allowed to obtain amazing results: a rapid disappearance of any clinical symptoms, the stable normalization of serum amyloid-A and, furthermore, a marked improvement of glomerular filtration rate and proteinuria with no adverse events. Our data, though limited to only two patients, emphasize that therapeutic intervention with canakinumab, suppressing both inflammation and IL-1β-mediated manifestations, can contribute to improve kidney function in MWS with overt renal amyloidosis

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.
