    Thyroid nodule recurrence following lobo-isthmectomy: incidence, patient’s characteristics, and risk factors

    Purpose: This study was aimed at assessing the incidence and timing of thyroid nodules recurrence, defined as appearance of new benign or malignant nodules in contralateral lobe in patients with benign thyroid nodules or thyroid microcarcinoma treated with lobo-isthmectomy. Patient’s characteristics and risk factors associated with this phenomenon were also investigated. Methods: A retrospective study was performed by evaluating 413 patients undergoing lobo-isthmectomy with a minimum follow-up of 1 year. Clinical characteristics, surgical interventions and complications, histological diagnosis, and thyroid function at last follow-up were collected. Results: Single or multiple thyroid nodule recurrence equally occurred in 80 patients (23%) with a median time to relapse of ~ 5 years (range 0.3–34.5 years) after lobo-isthmectomy. Recurrence was significantly associated with younger age ( 2.43 mIU/L was associated with the need of l-thyroxin replacement therapy after surgery. The most frequent surgical complication was transient hypoparathyroidism (4.6%), while the rate of permanent hypoparathyroidism significantly increased in patients submitted to completion thyroidectomy (5.3%). Conclusions: Thyroid nodules recurrence following lobo-isthmectomy is not a rare event and occurs within 5 years after surgery, more frequently in younger patients with family history of nodular goiter and in women with multiple pregnancies. Pre-surgical TSH levels may predict the development of post-surgical hypothyroidism, possibly improving the management of patients addressed to surgery

    Dissertação (mestrado) - Universidade Federal de Santa Catarina, Centro de Ciências da Saúde, Programa de Pós-Graduação em Ciências Médicas, Florianópolis, 2012Introdução: A escolaridade é positivamente associada com a performance cognitiva de pessoas saudáveis e pacientes com doenças neurológicas, incluindo epilepsia. Particularidades socioculturais podem interagir com as variáveis relacionadas à epilepsia e levar a um baixo nível educacional. Esse estudo pretende investigar se os motivos da precoce interrupção da educação estão associados ao desempenho cognitivo de pacientes adultos com epilepsia do lobo temporal refratária com esclerose no hipocampo (ELTM-EH). Métodos: 61 pacientes consecutivos com ELTM-EH e com menos de nove anos de educação foram classificados de acordo com a causa da interrupção da educação, como: 1) limitações devido às crises (n = 23); 2) começar a trabalhar cedo (n = 16); 3) limitações financeiras (n = 11); 4) dificuldade de aprendizagem (n = 11). A associação independente entre esses quatro grupos de pacientes e a performance cognitiva em 25 testes neuropsicológicos foram investigadas por meio de uma análise de regressão linear múltipla. Resultados: Depois de controlar as variáveis demográficas, clínicas, de neuroimagem, eletrofisiológicas e anos de educação, os grupos da dificuldade de aprendizagem e de limitações devido às crises continuam independentemente associados com baixos escores nos testes de Vocabulário e Memória Lógica. Conclusão: Os motivos da precoce interrupção do estudo são preditores independentes do prejuízo de domínios cognitivos específicos em pacientes adultos com epilepsia com ELTM-EH. Uma intervenção precoce dos fatores responsáveis pela baixa escolaridade dos pacientes pode ajudar a sanar os déficits cognitivos na vida adulta.Abstract : Introduction: Schooling is positively associated with cognitive performance of healthy persons and patients with neurologic diseases including epilepsy. Socio-cultural particularities may interact with epilepsy related variables leading low schooling. This study investigated if the causes of early interruption of education were associated with cognitive performance in adults patients with refractory mesial temporal lobe epilepsy related to hippocampus sclerosis (MTEL-HS). Methods: Sixty one consecutive patients with refractory MTLE-HS and lower than 9 years of education were classified according to the cause of interruption of education as: 1) limitations due to seizures itself (n = 23); 2) start to work young (n = 16); 3) financial limitations (n = 11); 4) learning difficulties (n = 11). The independent association between these four patients groups and cognitive performance in 25 neuropsychological tests were investigated by linear multiple logistic regression analysis. Results: After controlling the demographic, clinical, neuroimaging, electrophysiological variables and years of education, the cause of earlier study interruption remain independently associated with lower scores in Vocabulary and Logical Memory tests. Conclusion: Causes of early study discontinuation are independent predictors of specific cognitive domains impairments in adult patients with refractory MTLE-SH. Early intervention on factors responsible for lower schooling of patients may help to remedy their cognitive deficits in adult life

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
